[Antibodies against HIV-2 (LAV-II) in Africa and West Germany].

Antibodies against HIV-2 were tested by indirect immunofluorescence in over 2000 sera from Africa and the Federal Republic of Germany. The specificity of the test for HIV-2 was established by absorption of the sera with HIV-1 infected cells. Antibodies against HIV-2 were found in sera from two of 51 Africans who are living in the FRG. In addition, antibodies against HIV-2 were occasionally found in two of 1396 sera from Central Africa and two of 322 from West Africa. Cross-reacting antibodies were found in four of 189 high risk patients from the FRG who had HIV-1 infection, these antibodies no longer being present after absorption. It would therefore seem that HIV-2 infections are still rare among German risk groups.

Evidence for different immune responses to HIV in CSF and serum?

A case of acquired immune deficiency syndrome (AIDS) and a case of AIDS-related complex (ARC) are described. In both instances comparative Western blot analysis of cerebrospinal fluid (CSF) and serum samples show evidence of qualitative differences in antibody-binding patterns to viral polypeptides.

[Primary isolated amyloidosis--a rare corneal degeneration].

Report on a 47-year-old woman patient in whom an eccentric tumorlike corneal change was found in the left eye. Only after penetrating keratoplasty was the exact diagnosis--a primary isolated corneal amyloidosis--established, by light microscopic examination. The graft remained clear. Familial and systemic amyloidosis were ruled out by further examinations. The prognosis is favorable.

[Rectal administration of midazolam as an adjuvant in the premedication of infants].

In a randomized, double-blind study of premedication in 69 infants aged between 13 and 48 months the effects of 0.82 mg/kg midazolam or diazepam rectally plus 2.0 mg/kg ketamine i.m., or the administration of 2.4 mg/kg ketamine i.m. alone was studied. A satisfying result of 94.1% following the premedication with midazolam/ketamine, of 82.9% with diazepam/ketamine and of 81.3% with ketamine alone was observed. Premedication with midazolam/ketamine was the best one in the review of vigilance, agitation, and behaviour of defence against the mask at the beginning of anaesthesia. The amnestic action of midazolam extinguished the infants' memory of the i.m.-injection. The dose of midazolam/ketamine is suitable as an effective and positive method for premedication of infants within 20 min.

[Effect of prior operations on fear of anesthesia and surgery in young children].

From the standpoint of behavior therapists three different prognoses are possible for the behavior of preschool-children with prior operations when they undergo a new operation: 1. The pain of the prior operation has led to classical conditioned surgical phobias (conditioning paradigm); 2. during prior operations the pain was suppressed by anaesthetics, therefore a classical conditioning of surgical phobias cannot have happened, and as a consequence the behavior before a new operation is not predictable; 3. prior operations have led to habituation and extinction of phobias (habituation paradigm). By comparing two groups of children all aged between 2 and 6 years, one group with at least one prior operation, the other undergoing the first operation in their lives, an empirical decision between the proposals is sought. N = 66 children (none with an acute indication for surgery) were observed immediately before premedication and again immediately before anaesthetization; the anxiety of the children was scaled on 8 rating-scales. Posthoc the children were classified as prior operated or first operated according to their medical history. Statistical analysis of the data reveals that children with prior operations in their lives behave significantly more anxiously immediately before premedication compared with first operated children. After premedication immediately before anaesthesia the difference between the two groups is at random. The authors conclude that prior operations must have been associated with pain, which led to the conditioning of surgical phobias; they cannot exclude the possibility that conditioning could have happened both before and after surgery. Implications for post-surgery-care are discussed.

3'-Deoxy-3'-fluorothymidinetriphosphate: inhibitor and terminator of DNA synthesis catalysed by DNA polymerase beta, terminal deoxynucleotidyl transferase and DNA polymerase I.

3'-deoxy-3'-fluorothymidine-5'-triphosphate (dFTTP) was examined with respect to its capacity to inhibit and to terminate DNA-synthesis catalysed by DNA polymerases alpha and beta, terminal deoxynucleotidyl transferase (TdT) and E. coli DNA polymerase I (Klenow fragment). In analogy with 2', 3'-dideoxythymidine-5'-triphosphate (ddTTP) it was found that DNA polymerase beta was much more sensitive to dFTTP inhibition than DNA polymerase alpha. Moreover, the activity of TdT is strongly inhibited by both dFTTP and ddTTP, whereas the two compounds displayed a lower inhibitory potency on DNA polymerase I. Except for DNA polymerase alpha, the other investigated DNA polymerases are able to incorporate dFTTP into poly (dA).oligo (dT) and oligo (dG), respectively. The specific chain-terminating activity of dFTTP has proved for DNA sequencing to be at least equivalent to ddTTP.

[Histopathologic study following optic keratoprosthesis. A case report].

The authors present the case history of a patient in whom an optical keratoprosthesis was implanted after recurrent corneal ulcers in aphakia at the age of 80. The globe was examined histologically post mortem. A solid retroprosthetic membrane, a hemorrhage in the vitreous and retinal detachment after trauma were found to be the causes of the visual disorder. Wound healing following implantation of a keratoprosthesis is discussed in the light of the histopathological findings.

[Relations of the ultrasonic image of the inferior vena cava and central venous pressure].

In a prospective study of 50 patients a good correlation was found between the central venous pressure and the sonographic appearance of the inferior caval vein (ICV) behind the liver in right paramedian scanning. Dependent on central venous pressure (CVP) there were typical variations in the configuration, width and respiratory movements. These changes allowed a reliable estimation of the CVP. Decrease of the width in anteroposterior diameter in connection with a reduction of respiration-dependent caval movement indicated a deficiency of circulating blood volume. These alterations were always positive in cases of dehydration. Elongation of the vessel together with a widening over 2 cm and the lack of the normal end-inspiratory collapse were signs of an increased CVP. This non-invasive method is without risks or inconvenience for the patient. It is easily and quickly applied and very reliable for analyzing ICV haemodynamics. ICV ultrasonography can be recommended as a diagnostic aid for dehydration and hyperhydration as well as right heart failure, especially unrecognized, that means cardiac insufficiency without peripheral edema, constrictive pericarditis, tricuspid valve disease and for the assessment of the course of right cardiac failure.

[Therapeutic aspects of endoscopic polypectomy].

466 endoscopic polypectomies of the upper and lower gastrointestinal tract were analysed according to their curative value. In 254 removed polyps of the upper gastrointestinal tract the result of the histological examination was twice a proof of a carcinoma. One it had to be after-resected, once a carcinoid, five times bleeding polyps were removed. Four times prolapsing gastric polyps were removed in the bulbus duodeni. In these cases intermittent disturbances of the passage could be repaired. Thus the endoscopic polypectomy in the upper gastrointestinal tract was of therapeutic value in 4.3%. In the lower intestinal tract in 204 polypectomies 16 focal carcinomas or invasive carcinomas removed in the healthy tissue, 1 carcinoid and 98 bleeding adenomas were cut away. Thus the coloscopic polypectomy was connected with a therapeutic use in 56.3%. 81.8% of the polyps was tubular, papillary or villous adenomas. Taking into consideration the "adenoma-cancer-sequence", the coloscopic polypectomy must thus be regarded as a prophylactic and curative method, while in the polypectomy in the upper gastrointestinal tract the diagnostic value is in the first place.

[The value of fiber gastroscopy and targeted gastrobiopsy in the diagnosis of stomach ulcers and tumors].

In 2,000 patients the fiber-gastroscopic and bioptic findings were tested with the help of control examinations, results of operations and course observations. Here the fibergastroscopic diagnosis tumour was confirmed in 89.4%. 13 times a wrong diagnosis of tumour was made. Here different substrates were in question (ventricular ulcers, polypous hyperblastic mucous membrane and others). 11 carcinomas were gastroscopically not recognized as tumour. In 25 cases (20%) of the histological investigation of bioptically taken tissue did not result in a confirmation of the fiber-gastroscopic diagnosis of tumour. But 19 times of them the histology was falsely negative. The supplementation of the gastroscopy by biopsy resulted in a right diagnosis in 93.6%. By control examinations a further increase of the accuracy to 96.8% could be reached.
