Practical experiences with the routine application of the intravascular PO2 probe.

The practical experience with the routine use of 53 Roche PO2 probes, introduced through the radial artery in artificially ventilated patients of all age groups, during up to 8 1/2 days, is described. It may be concluded that many observations are only possible by continuous measurement, which, for example, makes possible a quicker and more accurate control of breathing and respiration. In some cases the cost-benefit calculation even favors continuous measurement.

[Fiber-optic endoscopy demonstration, incidence and clinical significance of intestinal lymphangiectasis].

The intestinal lymphangiectasia can be proved enteroscopically, laparoscopically as well as histologically after preceding aimed or unaimed biopsy. In 2,250 fibre-endoscopic examinations of the upper gastrointestinal tract we found 54 times endoscopically and 48 times also histologically a lymphangiectasia. Of these 54 patients 9 had an advanced carcinoma of the stomach. The evoking causes of the lymphangiectasia are discussed. When an intestinal lymphagniectasia is diagnosed it should always be sought for an evoking cause and last not least should be thought of a primary or secondary neoformation in the region of the epigastrium.

[Endoscopic demonstration of heterotopic gastric mucosa in the duodenum].

On the basis of 8 observations in 3,140 duodenoscopies is referred to the endoscopic picture of heterotopies of the gastric mucous membrane. The heterotopic mucous membrane of the stomach was found in polypoid rice-grain-sized to pea-sized lesions of the duodenum. The histological examination four times resulted in a mucous membrane of the stomach of corpus type, three times of antrum type and once only in foveolar epithelium. As to other findings endoscopically were twice observed erosions of the gastric mucous membrane or of the mucous membrane of the bulb, respectively, once an antrum polypus, in no case ulcers. Once an achlorhydria was present. A hyperchlorhydria was to be proved in no case. The clinical significance of the heterotopia of the gastric mucous membrane in the duodenum as well as the cause of its existence are discussed.

Neutralization of heparin after extracorporeal circulation.

In a two group-study of patients with and without postoperative haemorrhagic diathesis after ECC tests were undertaken to ascertain whether a postoperative haemorrhagic diathesis after neutralization of Heparin is due to the use of protamine-chloride or stet to a preoperative existing disturbance of the coagulation system. At the end of the ECC, a strongly marked fibrinolysis has been found to take place, together with an increased turnover of coagulation factors i.e. DIC. These results indicate that we are not concerned with a primary fibrinolysis, which would be characterized by plasminaemia, but rather with a reactive fibrinolysis. This coagulation disturbance was more marked in the group with postoperative haemorrhagic diathesis and it became even more pronounced after neutralization with protamine-chloride. From the results we obtained in our tests, the bleeding which occurs after the neutralization of heparin with protamine-chloride can be explained by increased DIC. We think a so-called "heparin-rebound-effect" is improbable as a cause of this postoperative bleeding complication. When there is DIC at the end of the by-pass time, one should avoid neutralizing the heparin and continue the heparin treatment for the same reason.

Isolation of DNA Strand-specific early messenger RNA species in cells infected by human adenovirus 2.

Hybridization to the separated light (L) and heavy (H) strands of adenovirus 2 DNA in 50% formamide at 37 degrees was used to isolate undegraded virus-specific RNA molecules from the polyribosomes of cycloheximide-treated human KB cells early after infection with adenovirus 2. About 20% of polyribosomal RNA labeled with [(3)H]uridine from 4 to 7 hr after infection was virus-specific. Twice as much labeled RNA was homologous to the L strand as to the H strand. Polyacrylamide gel electrophoresis of RNA selected with unfractionated adenovirus DNA resolved a major component of virus-specific RNA in the 19-20 S region of the gel and smaller amounts of viral RNA in two heterogeneous fractions migrating at 15-18 S and 21-26 S. Selection with individual DNA strands showed that the 19-20 S main size class of early mRNA consists of two homogeneous RNA species with slightly different mobilities, the transcripts from the L and H strand having molecular weights of 7.4 x 10(5) and 7.7 x 10(5), respectively. The 15-18 S RNA hybridized with the L strand and the 21-26 S RNA with the H strand.
