[Validity of a web-based color vision test for screening examinations of color vision].

BACKGROUND: With standardized examination conditions, a web-based color vision test using pseudoisochromatic color plates can deliver test results comparable to those yielded by conventional color vision tests. The aim of the study is to analyze to what extent a web-based color vision test can be used as a screening test on the internet after visual monitor calibration. METHODS: A German-language web-based color vision test with 25 pseudoisochromatic color plates based on the color plates of Velhagen and Broschmann and of Ishihara was developed, which can be seen at http://www.farbsehtest.de. Volunteers were recruited by means of specific information distributed on the internet, in various print media, and on the radio. RESULTS: Over a period of 12 months, 2,126 of the initial volunteers [541 women and 1,588 men; mean age 34.27 (+/-14.1) years] with a valid test result for 25 color plates performed the web-based color vision test, 1,700 (506 women and 1,194 men) of whom passed it. Of the 426 volunteers who did not pass the test, 32 were women and 394, men. Counter-testing was performed on 101 volunteers (34 women and 67 men) with a mean age of 36.74 (+/-11.6) years. CONCLUSIONS: The results recorded in the patients who underwent counter-testing suggest that, if handled correctly, the web-based color vision test can be used as a color vision screening test on the internet.

[Chlamydial diseases of the eye. A short overview].

In this short review, new data on the taxonomy of Chlamydia and the association of these pathogens with various ocular diseases are presented. Clinical diagnosis and laboratory tests for ocular C. trachomatis infection are discussed. The actual therapy consists in oral azithromycin.

[Therapy and prognosis of bacterial keratitis].

This review discusses the current therapy for bacterial keratitis, especially that using quinolones of the fourth generation. In addition, the importance of additive steroids is discussed. Steroids should be administered at a low dose and tapered off slowly, whereas antibiotics should be applied at a high dosage and discontinued abruptly.

[Bacterial keratitis. Current diagnostic aspects].

Keratitis remains the third most common etiology for blindness worldwide. Whereas bacteria still predominate as causative organisms in temperate climates, fungal and mixed infections are more common in tropical and semitropical areas. In recent years, a shift in the causative organisms, as well as predisposing factors, has been reported. Risk factors that may have gained in importance, such as wearing contact lenses and corneal surgery, have been identified. Microorganisms, especially Pseudomonas spp. and mycobacteria have been frequently isolated in these patients. A changing pattern in microorganism infection has been observed that might be caused by inappropriate use of potent antimicrobial agents. Because of the sight threatening nature of bacterial keratitis, proper diagnosis and antibiotic selection are required. Management should be guided by the appropriate diagnosis, severity of clinical symptoms and underlying risk factors. Molecular techniques, such as polymerase chain reaction, have increased our diagnostic options, even when they cannot replace established procedures. This article reviews the current data and procedures available for the diagnosis of bacterial keratitis.

[Bibrocathol eye ointment is efficacious in blepharitis. Results from a randomized, double-blind, controlled clinical trial].

Bibrocathol (Noviform) 5% ointment is a frequently used therapy for blepharitis. The study included 197 patients who were randomized to be treated over 2 weeks with bibrocathol or an ointment vehicle, respectively. Slit lamp examinations were performed on day 1 (baseline) and day 14 (end of study) to assess lid edema, lid erythema, debris, and pouting of Meibomian glands; patients rated their subjective complaints. Bibrocathol was superior to the control treatment in improving the sum score of the 5 scales (by 7.0 vs 4.7 points, p<0.001), especially in patients with primarily severe symptoms (by 8.3 vs 4.8, p<0.0001). Antiseptic therapy with bibrocathol eye ointment 3 times daily over 2 weeks was found to be efficacious in the treatment of acute forms of blepharitis which do not require antibiotic therapy. Bibrocathol treatment is well tolerated and safe.

[Anti-infective drug therapy in ophthalmology--part 4: acanthamoeba keratitis].

An early diagnosis of an acanthamoeba keratitis can usually be made on the basis of the patient's history and slit-lamp examination. Further helpful diagnostic steps are confocal microscopy, microbiological, and histopathological examination. In the early stages topical application of antimicrobials (combined application of aminoglycosides, polyhexamethylene biguanide 0.02 %, propamidine isoethionate 0.1 %) is effective. In the late stages a penetrating keratoplasty is often necessary.

[Alternative medicine in ophthalmology?].

In an overview new data from the literature regarding alternative/complementary medicine are presented. Naturopathy, acupuncture and homeopathy as well as new findings in placebo-research and their correlation to alternative medicine are discussed. In addition, as a synthesis a patient-orientated so-called anthropologic medicine is recommended to make alternative/complementary medicine unnecessary.

[Corneal wound healing after experimental penetrating keratoplasty after excimer laser trephination in free form].

BACKGROUND: The postoperative clinical course after penetrating keratoplasty and trephination in free form using a guided excimer laser beam has been published before. Here the findings of light-microscopy comparing corneal wound healing after experimental penetrating keratoplasty after laser trephination and after conventional mechanical trephination are presented. MATERIALS AND METHODS: Homologous penetrating keratoplasty was performed on 12 NZW rabbits (6 animals with mechanical trephination, 6 animals with excimer laser trephination). The cutting edges achieved by both trephination techniques were examined by light microscopy in the remaining donor rings. During the postoperative follow-up animals were sacrificed at 3 and 6 weeks and at 3 months. Corneal specimens were retrieved and corneal healing processes were evaluated by light microscopy. RESULTS: The cutting edges of corneal excisions with the excimer laser demonstrated a high precision with only minimal collateral damage to adjacent tissue structures. At the different intervals both trephination groups demonstrated comparable stages of corneal wound healing regarding epithelial regeneration, stromal fibroblast migration with collagen synthesis and Descemet repair by endothelial synthesis of basement membrane. After 6 months corneal specimens of both groups demonstrated complete healing with nearly parallel orientation of newly synthesised collagen lamellae. Corneal thickness in the wound areas did not differ significantly from normal corneal tissue. CONCLUSIONS: Experimental follow-up studies to evaluate the feasibility of the developed technology of laser trephination in the living eye have shown no differences between conventional mechanical and excimer laser trephination with a guided beam. The present histology study also does not demonstrate any significant differences in corneal wound healing between the two trephination groups. Although excimer laser trephination along metal masks has now been established for several years, the here presented technique for trephination with individualised transplant geometries appears to be a valuable addition to the surgical repertoire in the presence of selected corneal pathologies.

[Diagnostics of fungal infections of the eye and of its adnexa].

The spectrum of fungal infections of the eye and of its adnexa is presented with respect to their clinical pictures as well as to their laboratory diagnostic monitoring. The various types of localisation are different in their symptomatologies, in their risk factors and in their aetiology.

Systemic iodine absorption after preoperative antisepsis using povidone-iodine in cataract surgery-- an open controlled study.

After preoperative conjunctival and periorbital antisepsis with povidone-iodine (PVP-I), the systemic absorption of iodine after cataract surgery was measured to evaluate the risk of thyroid side effects. Five different combinations of PVP-I alone or in combination with PVP-I-free antiseptics were applied to the conjunctiva and periorbital skin. An iodine-free product served as control. Iodide and creatinine in urine were analyzed before intervention and 24 and 48 h postoperatively. Depending on the concentration and application site, 0.3-4.5% of the total applied iodine or 3.6-45.4% of the free iodine were absorbed. The range of urine iodine excretion was between 11.7 and 71.0 mug iodine/g creatinine, depending on the PVP-I concentration and the site of application. The increase in iodine excretion was significant at 24 h postoperatively in trials receiving PVP-I both periorbitally and conjunctivally, depending of the concentration used. Because the iodine absorption is only slight and of doubtful clinical relevance, presurgical conjunctival antisepsis can be achieved with 1.25% PVP-I; so far clinically manifest anamnestic thyroid disorders are excluded. Presently, periorbital skin antisepsis with PVP-I cannot be recommended until data on thyroid metabolism in the population have been collected and evaluated, especially in a region currently or previously deficient in iodine.

[Anti-infective therapy of the eye -- part 3: mycotic infections].

This review presents the main antifungal drugs, which can be used systemically as well as topically in form of drops or intravitreal injection. In addition, the symptoms of keratomycosis and fungal endophthalmitis are briefly presented. Finally, the pros and cons of an additive application of steroids are discussed.

[Diagnostic and therapeutic recommendations for mucous membrane pemphigoid of the eye].

Mucous membrane pemphigoid (MMP) is often accompanied with a chronic cicatrizing conjunctivitis that may eventually lead to loss of vision. The study is intended to investigate the current scientific knowledge on the diagnosis of and therapy for MMP involving the eye. Previous studies published before December 2004 have been systematically reviewed for their level of evidence. Consequently, recommendations for patient management are provided.

[Investigation of color vision using a web-based color vision test].

BACKGROUND: Screening tests of visual functions using the Internet are theoretically possible. To use these tests as a screening test, they must deliver comparable results with conventional test procedures. A web-based color vision test was developed based on pseudoisochromatic color plates. METHOD: The web-based color vision test was developed according to the pseudoisochromatic color plates by Velhagen and Broschmann using the programming-languages HTML, Java, and Perl. Sixty-five voluntary subjects, including nine color-deficient subjects, were examined by luminescence color plates (via web-based color vision test) and pigment color plates (via book). The statistical analysis was performed by determining the correspondence and the 95%-confidence interval. RESULTS: The correspondence of the test results for all subjects was 0.98 and the 95%-confidence interval was within 0.91 and 0.99. The correspondence of the test results in the group of color-deficient subjects was 1.0 and because of the limited number the 95%-confidence interval was within 0.71 and 1.0. CONCLUSIONS: The web-based color vision test with luminescence color plates for color-efficient and color-deficient subjects delivers test results comparable to pigment color plates under standardized examination conditions. Further studies are needed to examine if the web-based color vision test can also be used as an Internet screening test.

[Use of an electronic patient record system at the Department of Ophthalmology, Otto-von-Guericke University of Magdeburg].

BACKGROUND: The electronic or paper-based patient record is the most important documentation file. The electronic patient record has many advantages compared to a paper-based record. METHODS: An example of an electronic patient record (EPR) system accessed by several departments of the Faculty of Medicine of Magdeburg is presented. RESULTS: To guarantee faculty-wide communication with sharing of clinical pictures and findings, e.g., with the Departments of Radiology and Pathology, the system of MEDOS Inc. is used in our department. Several examination devices, for example fundus camera, slit lamp, and ultrasound, are equipped with an digital picture storage and communication system via a DICOM interface to the MEDOS system. The software AnalySIS is used as a gateway for picture transferation. The MEDOS system also supports the clinical workflow. CONCLUSIONS: The use of an electronic patient record system in ophthalmology is already possible, but still needs further tuning and improvement.

[Anti-infectives in eye injuries].

BACKGROUND: We present the current concepts for the use of anti-infectives parallel to surgery of traumatic eye injuries. MATERIAL AND METHODS: Data from the national and international literature were collected using the PubMed, Medline, Drugs and Pharmacology, ISI, and Cochrane databanks. In addition, the experience from our tertiary center was taken into consideration. RESULTS: For palpepral and globe injuries prophylactic and therapeutic anti-infectives are recommended as supporting measures to surgical repair. These include antiseptics (PVP-iodine), antibiotics (cefuroxime, vancomycin, ceftazidime), antimycotics (voriconazole) and anti-inflammatory substances (prednisolone). CONCLUSIONS: Several drugs are useful for the prevention and/or treatment of infection following eye injury.
