[Anti-infective drug therapy in ophthalmology -- Part 1: bacterial infections].

This review presents a rational antibacterial therapy. First, systemic versus topical administration of the drugs is discussed including substances specific for the respective routes. Secondly, ocular diseases requiring systemic therapy are presented followed by a discussion of infections suitable for topical administration. Finally, the pros and cons of a combined antibiotic-steroid therapy are discussed and examples described.

[Charles-Bonnet-syndrome].

BACKGROUND: The Charles Bonnet Syndrome (CBS) is characterized by complex visual hallucinations in psychologically normal people. It occurs frequently in elderly, visually handicapped patients. CASE REPORTS: Three patients are presented whose age ranged between 75 and 85 years old and they had impaired vision because of age-related or myopic macular degeneration. They complained about complex visual hallucinations and all patients were aware of the unreal nature of their hallucinations. A neurological examination of two patients revealed no abnormalities and none of the patients required treatment. DISCUSSION: About 12-13% of people with impaired vision have visual hallucinations. Neurologic and psychiatric diseases should be excluded before making a diagnosis of CBS which has little impact on the general feelings of well-being in the majority of patients. Many patients do not consult a doctor about CBS for fear of being considered insane. It is recommended that all patients with reduced vision are asked about visual hallucinations. Many patients will benefit from an explanation of these phenomena and the assurance of their harmless nature. Treatment of CBS is rarely needed.

[Immune suppression by combination therapy with basiliximab and cyclosporin in high risk keratoplasty. A pilot study].

BACKGROUND: The risk of postoperative immune reaction/transplant rejection is increased in the presence of distinct risk factors. In these situations, the use of established immunosuppressive agents (steroids, cyclosporin) is indicated. However, immune reactions can occur despite this therapy. Basiliximab is a monoclonal antibody with a high specific binding affinity to the IL-2 receptor of activated T-cells. After renal transplantation, the combination therapy with basiliximab and cyclosporin reduces the risk of acute rejection by 30% against placebo. The aim of this study was to investigate the effect of complementary immune suppression with the IL-2 synthesis inhibitor cyclosporin and the IL-2 receptor antagonist basiliximab on the incidence of transplant rejection after high-risk keratoplasty. PATIENTS AND METHODS: In an open pilot study, seven patients with high-risk keratoplasty were treated with basiliximab perioperatively and cyclosporin postoperatively. All patients had penetrating keratoplasty with a non-HLA-matched corneal graft. RESULTS: Seven patients were operated on from 12/98 through 12/99 with a corneal transplant diameter between 7.1 and 9.5 mm. Risk factors included extensive corneal neovascularization in all patients (indications for surgery: 4 cases with corneal scars after herpetic disease, 2 cases with conditions after alkali burns, 1 case with corneal ulceration after thermal burn). During the follow-up of 14-25 months, no immune reaction has occurred, and all transplants are clear. The mean visual acuity increased from 0.04 to 0.41 postoperatively. DISCUSSION: The preliminary data on a combination therapy with basiliximab and cyclosporin are promising. For further evaluation a prospective multicenter study is planned.

[Quantitative and objective topometrical analysis of drusen of the optic nerve head with the Heidelberg retina tomograph (HRT)].

BACKGROUND AND PURPOSE: Optic nerve head drusen (ONHD) are one of the most frequent causes for congenital swelling of the optic nerve head. Visual field and retinal nerve fiber layer defects are reported in cases of ONHD. The Heidelberg Retina Tomograph (HRT) allows a 3-dimensional topometric analysis of the optic nerve disc and measurement of the peripapillary mean retinal nerve fiber layer. PATIENTS AND METHODS: A total of 18 eyes from 9 patients with sonographically confirmed drusen were analyzed with the HRT. Data were compared to a control group of 18 eyes from 9 matched healthy individuals. Statistical analyses were performed by using ANOVA (univariate). All patients with ONHD underwent a computerised visual field test (30 degrees, Octopus 101). Due to a bad reliability factor of over 10 in the visual fields by 4 out of 18 eyes, only measurements from 14 eyes were included in the study. We correlated visual field and HRT parameters and calculated the Pearson's correlation coefficient (r). RESULTS: We found a significant difference in the measured parameter mean retinal nerve fiber layer (RNFL) thickness ( p<0.05) between the two groups. In the ONHD group a negative correlation coefficient was found between the peripapillary mean RNFL thickness and the loss variance (r=-0.50, p=0.03) as well as between the peripapillary RNFL cross-sectional area and the loss variance (r=-0.47, p=0.04). CONCLUSIONS: The HRT is able to detect a peripapillary RNFL thinning in cases with ONHD. The mean RNFL thickness correlated with the loss variance. The HRT should be used to perform a quantitative and objective topometric analysis in cases with ONHD.

[Options and limits of visual function tests available on the internet].

BACKGROUND: The quantitative capability of the visual system can be tested using graphic presentations with defined size, form and colour. For presentations a chart projector or personal computer (PC) can be used. In the meantime the number of visual function tests on the internet is continually increasing. These tests are potentially available for over 400 million internet users with different hard- and software requirements. METHODS: From 38 on-line visual acuity tests and 19 on-line colour vision tests found by means of search engines on german language web pages, the offerer of the test, the type of chart used and the data for the test conditions, as well as hard- and software adjustment were registered. RESULTS. Various visual function tests, such as visual acuity tests, the Amsler grid, stereo and colour vision tests, can already be planned via internet. The variability of the tests ranges from a simple presentation of graphic elements to a laboriously programmed interactive input by the user to specify the test result. Most of the tests are presented by opticians and the optical industry. DISCUSSION: The analysed tests allow for a considerable variability of the test conditions, for example by missing data concerning the room lighting and lacking monitor calibration. Furthermore, problems of the web-based colour presentation were not considered or not considered sufficiently in most of the on-line colour tests. CONCLUSIONS: Because of the test deficits determined, the results of visual function tests on the internet are to be classified as doubtful. At present, most data of test offerers are not sufficient with respect to the test conditions. Standards should be defined for internet-based visual function tests and further studies should be followed to check the effectiveness of these tests for screening examinations.

New aspects of the tolerance of the antiseptic povidone-iodine in different ex vivo models.

Investigating new possibilities for the application of 1% (v/v) iodophors, povidone-iodine (PVP-I) was better tolerated in the HET-CAM or explant test than 1% (w/v) silver nitrate or tetracycline. After application to the eye, at least 2.6% of used iodine were adsorbed. Therefore PVP-I is more effective than silver nitrate or erythromycin, meaning a possible alternative for the prevention of ophthalmia neonatorum. PVP-I is more active against methicillin-resistant Staphylococcus aureus (MRSA) in a human ex vivo skin model, which results in a complete eradication of S. aureus in the nasal cavity of volunteers after 2 daily applications and will be better tolerated by human nasal cilial epithelium than chlorhexidine. Having the same clinical tolerance as mupirocin, PVP-I is a useful alternative for the antiseptic therapy of germ carriers of MRSA. The synthesis of proteoglycans in articular cartilage of bovine sesamoid bones was increased after application of 5% (v/v) PVP-I without any increase in catabolism revealing possibilities for the use as irrigation solution in the joint.

[TFF peptides. New mucus-associated secretory products of the conjunctiva].

The molecular architecture of the human ocular mucus is not yet completely understood. Recently, TFF peptides (formerly known as trefoil factors or P-domain peptides) could be identified as new constituents of this delicate mucus. Members of the TFF-peptide family are typical secretory products of mucous epithelia and three are known in humans and designated as TFF1, TFF2 and TFF3. They enhance cell migratory processes (motogenic effect), they show anti-apoptotic effects and are inflammatory modulators Both TFF1 and TFF3 expression could be monitored by the reverse transcription-polymerase chain reaction (RT-PCR) in the human conjunctiva; in contrast, TFF2 transcripts were not detectable. Using immunohistochemistry, TFF1 and TFF3 peptides were found in varying concentrations solely in secretory vesicles of conjunctival goblet cells. This localisation matches precisely that of the secretory mucin MUC5AC. Thus, conjunctival TFF1 and TFF3 have to be considered as typical mucin-associated peptides probably modulating the rheological properties of the ocular mucus and the tear fluid. Future investigations are in progress to elucidate the role of TFF-peptides during pathological conditions of the eye as well as their diagnostic and therapeutic potential.

[Electrophysiological studies in breast carcinoma patients with tamoxifen retinopathy].

PURPOSE: Crystalline plaques in the cornea or the retinal nerve fiber layer are well-known side effects of tamoxifen therapy. We investigated whether electrophysiological methods for determining the function of the retinal nerve fiber layer and retinal pigment epithelium demonstrate changes in tamoxifen retinopathy. PATIENTS AND METHODS: We compared the right eyes of four women with breast cancer and mono- or bilateral tamoxifen retinopathy to ten right eyes of age-matched, eyehealthy patients who had not received tamoxifen by means of electrophysiological investigations (e.g., pattern-reversal electroretinography, flash electroretinography for the maximal combined response, electrooculography; ISCEV standard conditions). RESULTS: No significant differences were observed between patients with tamoxifen retinopathy and controls regarding mean visual acuity, basal level of the electro-oculographic standing potential, basal level on the Arden index, or any of the other electrophysiological potentials. CONCLUSIONS: Pattern-reversal electroretinographic measurements revealed no damage of retinal ganglion cells in the presence of crystalline plaques in the retinal nerve fiber layer. Electro-oculography did reveal differences, but these were not statistically significant, possibly due to the small number of cases.
