[Indications for the shunt treatment of children with cerebellar astrocytoma].

In the years 1980-1987 the number of children aged from 11 months to 15 years treated for cerebellar astrocytoma was 113, the mean age was 5.4 years. Shunt insertion preceding the main surgical treatment was done in 10 cases (9%). After the operation shunt was necessary in 15 cases, that is 15% of cases not undergoing shunt insertion before the operation. In all, shunting was necessary in 22% of patients with cerebellar astrocytoma. In the conclusions the authors suggest indications to shunting in various age groups in cases of cerebellar astrocytoma.

[Results of combined treatment of neoplasms of the posterior cranial fossa in children].

The effectiveness of combined treatment of posterior fossa tumours was analysed using the clinical material of a neurosurgery department. The material comprised 242 patients treated in the years 1980-1987 with 211 suboccipital craniotomies performed. In cases of cerebellar astrocytoma one-year survival rate was 9% and five year survival in 89% of cases. In the group of medulloblastoma treated in the period 1980-1982 the one-year survival rate was 75%, three-year 39% and five-year 25%. In the group treated in the years 1985-1987 the one-year survival rate was 95% and three-year was 60%. In ependymoma cases the one-year rate was 90%, three-year was 40% and five-year--33%. The use of combined treatment in cases of ependymoma and medulloblastoma improved significantly the therapeutic results.

[Treatment of cerebral vein aneurysms in children].

Aneurysms of the vein of Galen are very rare vascular anomalies found mainly in newborns and infants. In the years 1982-1989 five children with these aneurysms were treated. Their age was from 6 weeks to 16 months, all were males. The diagnosis was based on computed tomography (initial examination) and cerebral angiography. Two cases with hydrocephalus were treated with valves. The main procedure was clipping of supplying vessels. One child died during diagnostic procedures. The results in four operated on patients are good.

[Astrocytomas of the cerebellum in children: treatment and results].

Between 1980 and 1987, 113 children with astrocytoma of the cerebellum were treated. The range of the age was 11 months to 15 years. 103 children were treated by surgery only and 10 received surgery and postoperative irradiation. Shunting was necessary in 22% patient with cerebellar astrocytoma. One-year survival for patients with cerebellar astrocytoma was 97%, five-year 89%.

[Problems with diagnosis and treatment of brain tumors in children with tuberous sclerosis].

Problems are discussed connected with the occurrence of brain tumours in children with tuberous sclerosis with a survey of the most modern Polish and foreign literature. For illustration two cases of such tumours are reported in a group of over 50 children with tuberous sclerosis. The authors discuss also the problems related to surgical treatment in cases of brain tumours developing in tuberous sclerosis.

Tissue catecholamine concentrations in spontaneously hypertensive rats.

Tissue concentrations of noradrenaline (NA), dopamine (DA) and adrenaline (A) were compared in spontaneously hypertensive (SHR) and normotensive (NCR) rats, aged 1, 3, 8, 14 and 24 weeks The organs analyzed included the brain, subdivided into prosencephalon and rhombencephalon, heart, adrenal glands and kidney. Brain catecholamines were significantly lower in SHR than in NCR, and the difference appeared already at the age of 3 weeks. Concomitant increase was found in the adrenal NA and A concentrations of the SHR. Concentration of NA in the heart decreased in the SHR following onset of hypertension. It is concluded that the diminished NA, DA and A concentrations in the brain as well as the augmented adrenal NA and A levels in the SHR may be causally related to the development of hypertension, while the heart NA level reflects the secondary, hypertension -- related changes.

[Infratentorial ependymomas in childhood].

Between 1980 and 1987, 30 children with infratentorial ependymomas were treated at the Department of Neurosurgery, Children Medical Centre. The age range was 1.5 years to 9 years. All children with infratentorial ependymomas were treated surgically. Shunting was necessary in 36% patients in this population. Conventional irradiation was given to five children, cobalt Co60 therapy to 24 patients. Chemotherapy was used in nine cases. One-year survival for patients with infratentorial ependymoma was 90%, three year -40% and five year -33%.

[Indications for shunting in children with tumours of posterior fossa].

Between 1980-1987 in the Neurosurgery Department we treated 113 patients with cerebellar astrocytoma, 52 children with medulloblastoma and 30 patients with ependymoma. Before operation shunting was used in 10 patients and after operation in 15 patients, generally shunting was necessary in 22% patients with cerebellar astrocytoma. In cases of medulloblastoma before operation shunting was used in 6 children and after in 15 patients, generally shunting was necessary in 21 (40%) patients. In cases of ependymomas shunting was used in 6 patients before operation and 5 cases after operation, generally shunting was necessary in 11 (36%) patients.

[Lateral ventricle choroid plexus papillomas in infants].

Choroid plexus papillomas are rare intracranial tumours occurring most frequently in children. In the period 1983-1988 in the neurosurgery department 10 infants were treated for choroid plexus papillomas. Their age was from 4 weeks to 12 months. The diagnosis was based on the results of ultrasonography and CT. Eight patients had shunts implanted for hydrocephalus. In most cases the histological examination demonstrated benign tumours. Radical removal of the lesion was done in 90% of cases, but, despite this, remote results were satisfactory in only 40% of cases.

[Treatment of medulloblastomas in children].

Fifty-two children were treated surgically in the years 1980-1987. Their mean age was 5.3 years, range 10 months-13 years. In 58% partial removal of the tumour was done, in 4% only biopsy was performed. Shunt treatment was necessary in 40% cases. The analysis showed that in the cases treated in the years 1980-1984 1-year survival was 75%, 3-year survival was 39%, 5-year survival was 25%. Statistical analysis demonstrated that in the cases treated in the years 1985-1987 the 1-year survival was 95%, 3-year survival was 60%, and 5-year survival was 51%.

[Diagnosis and treatment of brain abscesses in children].

Fourty children were treated for brain abscess in the period 1983-1988 at the Neurosurgery Department, Children's Health Centre, Warsaw. Their age was from 4 weeks to 16 years. In 38 cases the treatment was surgical, followed by antibiotic therapy, in 2 cases only antibiotics were given. In 7 cases shunts were implanted for treatment of hydrocephalus developed in infants. Two patients died during the treatment. Problems encountered in various methods of brain abscess treatment are discussed.
