[Radiological detection of the Medina worm (Dracunculus medinensis)].

Atypical linear or convoluted calcification in the subcutaneous tissues of patients from tropical areas in Africa and Asia may be caused by D. medinensis. The radiological appearances and differential diagnosis are described. Dead, calcified worms may occur in the soft tissues of the thorax, abdomen, pelvis or extremities and may cause complications due to a foreign body reaction, particularly when related to major joints.

[Computed tomographic control of the conventional x-ray findings in space-occupying lesions of the bony pelvis].

The CT and conventional radiological findings in 30 patients with space-occupying lesions of the pelvic bones were compared. 73% of the tumours diagnosed by CT were shown by conventional means and in 71% their extent was correctly estimated. Lesions at the ilio-sacral junction are not demonstrated as well by conventional methods as those near the hip joint. The lesions most frequently missed by conventional radiography lie in the dorsal part of the ilium and in the lateral mass of the sacrum.

[Malignant mesenchymal liver tumors in the computed tomogram].

The computer tomographic appearances of very rare sarcomas of the liver have been demonstrated in five cases (four haemangiosarcomas, one spindle-cell sarcoma). On the plain scan, all the tumours appeared less dense than the liver parenchyma, but in haemangiosarcomas there was a marked increase in density after the infection of contrast up to an isodense level. During angio-CT, densities equal to intrahepatic vessels were observed. There was only a slight increase in density of the spindle cell sarcoma after contrast injection. The CT appearances have been compared with the pathological and anatomical structure of the tumour.

[Respiratory distress syndrome of the adult in the computer tomograph].

Pulmonary changes due to the adult respiratory distress syndrome, which are visualized in the roentgenogram as spreading from the hilus to the periphery, often show a distribution in dorsocentral direction in the computer tomogram, a phenomenon which occurs especially during the oedema phase under the influence of gravity. In addition, there are ARDS developments involving the expected spread from the root of the lung, particularly so with patients of advanced age and with septic-toxic diseases. The causes and significance of these differences are discussed.

[CT findings in ileo-caval thrombosis (author's transl)].

Thrombosis of a large vein can be demonstrated by CT. The thrombosed vein has a hypodense lumen, with a somewhat increased diameter which does not opacify after intravenous contrast injection, but which shows a narrow hyperdense margin. Collateral vessels in the pelvic or paravertebral plexus and dilated veins in the abdominal skin are indirect evidence of ileo-caval thrombosis. CT can also demonstrate the cranial extent of a caval thrombosis. Venous thrombosis demonstrated by CT is an important additional finding in the pre-operative examination of the abdomen.

[Multicystic renal dysplasia in the adult (author's transl)].

In the literature 59 cases of multicystic dysplasia of the kidney in adults have been reported. Many authors use different descriptions to characterize the same aspect of identical disease causing a confusing nomenclature. The disease is caused by a malformation in development. On the basis of three clinical case reports diagnosis, differential diagnosis and prognosis is discussed. One patient had an additional malformation (M. Klippel-Feil).

[Rating and limitations of ultrasonic examination of the biliary system compared with contrast radiography (author's transl)].

The rating of sonography compared with contrast roentgenography is examined for the biliary system. A high quota of accuracy equivalent or superior to contrast radiography is achieved by sonography, as demonstrated by a comparison or oral cholecystography with sonography (accuracy 69% to 95.5%), IV cholangiocholecystography (accuracy 86.5% to 88.9%) and ERCP (accuracy 69.9% to 67.3%).

Urological complications in malignant and inflammatory intestinal diseases.

Secondary involvement of the ureter or urinary bladder in inflammatory and malignant diseases of the intestine is described with case reports and documented by figures. With involvement of the bladder wall, intractable dysuria or later pneumaturia and fecaluria arises. The symptoms from the intestine are masked by urological symptoms. If the intestinal process leads to a disease of the ureter, urological pathology is mostly limited to urographic findings; symptoms are circumscribed. However, the ureteric wall is usually already irreparably damaged.

Significance of supraclavicular lymph node biopsy for staging, therapy and prognosis of testicular tumors.

Supraclavicular lymph node biopsies were performed in 40 patients with germinal testicular tumors. The radiologic visualization of both the thoracic ducts and the supraclavicular lymph nodes was reviewed and no relation to retroperitoneal lymph node involvement was found. In 5 patients with retroperitoneal but without distant metastases, supraclavicular lymph nodes revealed tumor metastases, two of which had been clinically suspected. Depending on the staging system applied, the tumor stage had to be changed in 1 or 3 patients, respectively. For comparison, in 4 patients with distant metastases no metastatic involvement of the supraclavicular nodes was demonstrable. It is concluded that supraclavicular tumor spread is not indicative of distant metastasis and incurability. Therefore, in these cases primary radical surgery should not be considered contraindicated.

[Bone metastases of prostatic cancer in relation to tumor size and grade of malignancy (author's transl)].

In 153 patients with prostatic cancer total body bone scintigraphy was performed. Forty six patients showed bone metastases while there was no evidence of skeletal involvement in 101 cases. Our results show that the tendency of bone metastases increases when low differentiated type of malignancy is present: Patients with low grade malignancy (G1) showed bone metastases in 7.3%, with middle grade malignancy (G2) in 40%, and with high grade malignancy (G3) in 38.5%. We were also able to establish a correlation between tumor size and frequency of bone metastases: Patients with tumor size T1 showed bone metastase in 7.1%, with tumor size T2 in 19.3%, with tumor size T3 in 39.2%, and with tumor size T4 in 65%. Furthermore, we found that the incidence of low differentiated tumors appeared to be more common in increased tumor size. These results show the value of early detection of prostatic cancer.

[The clinical and radiological picture of degenerative disease of the extra- and intra-cranial vessels (author's transl)].

The clinical and angiograms of 500 patients with degenerative disease of their cerebral vessels were evaluated. The clinical severity of the ischaemic changes was graded and compared with localisation and extent of the angiographic abnormality. In these patients there is an increased risk during angiography and a careful technique has to be employed. Two examples are used to show that it is possible to clarify even complex neurological and angiographic problems by means of an appropriate technique, 90% of our patients with a definite cerebral infarct gave a history of previous transitory ischaemic attacks. In our opinion patients with these attacks are of particular interest to the angiographer and the surgeon.

[Recurrent collapse of one lung (author's transl)].

A case of recurrent collapse of one lung is reported. It developed within 90 minutes after reexpansion of a completely collapsed lung induced by pneumothorax. There was no demonstrable cause for the recurrence of the atelectasis. Treatment was by means of a bird respirator (ventilation via a mouthpiece) with respiratory resistance. Factors possibly responsible for the secondary atelectasis are discussed.

[Gastroscopy and rinsing of the gastric wall under sight in drug intoxication (author's transl)].

Gastric rinsing during gastroscopy is presented as a new, effective, uncomplicated method of primary elimination of poison, as a part of a new diagnostic and therapeutic concept for patients intoxicated with hypnotics. The advantage of this active therapy is a fast elimination of the toxic substance and prevention of secondary complications. The diagnostic value of gastroscopy and radiological examination in patients with unexplained comatous states is emphasized.
