Cytologic and histologic investigations of the uterine cervix and studies of the lymphocyte functions were performed in human immunodeficiency virus-infected and human immunodeficiency virus antibody-negative women to study possible linkages between human papillomavirus-induced dysplasia and degree of human immunodeficiency virus-induced immunosuppression. Cytologic smears of the uterine cervix of 111 human immunodeficiency virus-infected women were compared with findings in 76 female intravenous drug users negative for human immunodeficiency virus antibodies and in a group of 526 women of the outpatient population of the hospital. Cervical dysplasia-neoplasia (including five cases of invasive carcinoma) was seen in 41% of the human immunodeficiency virus-infected patients. In human immunodeficiency virus-negative intravenous drug users dysplasia-neoplasia was seen in 9%, and in the sample from outpatients in 4%, including two cases of invasive carcinoma (p less than 0.01). Cytologic features that were attributable to infection with human papillomavirus were observed in human immunodeficiency virus-infected women four times more often than in the sample from the outpatient population (p less than 0.01). Frequency and severity of dysplasia appear to increase with diminishing numbers of CD4+ helper/inducer T lymphocytes and correlated significantly (p less than 0.01) with a loss of blastogenic response to phytohemagglutinin, pokeweed mitogen, and tetanus toxoid. These results suggest an increased risk for the development of dysplasia of the uterine cervix in women with human immunodeficiency virus infection, which is related to the degree of immunosuppression.
The amniotic band sequence includes a number of typical fetal developmental anomalies that appear as disruption, amputation or deformation, dependent on the time of origin of the amniotic bands. In rare cases, amniotic bands can cause intrauterine fetal death by constriction of the umbilical cord. Clinical and pathologic-anatomical findings of two cases of intrauterine fetal death in the 21st and 34th week of gestation after torsion and strangulation of the umbilical cord by amniotic bands are presented.
A woman with severe immunosuppression provoked by the acquired immunodeficiency syndrome developed a disseminated cytomegalovirus infection of the genital tract. Using in situ hybridization and the APAAP technique, cytomegalovirus was detected in epithelial and endothelial cells as well as in macrophages in the vulva, the vagina and the cervix uteri.
Omentectomy was conducted on a 49 year old patient at the Dept. of Surgery of the UFK Berlin-Charlottenburg because of a mucinous adenocarcinoma, without prior detection of the primary tumour site. Relaparotomy revealed proliferative mucinous adenoma of the Fallopian tube and a consecutive pseudomyxoma peritonei. Up to now, this seems to be the first case report of a mucinous adenoma of the Fallopian tube.
Clinical and histopathological findings of a woman with AIDS and an ulcerated vulvovaginitis are presented. The preliminary diagnosis of a herpes vulvitis was corrected after in-situ-hybridisation of the formalin-fixed and paraffin-embedded tissue. CMV was found in macrophages and endothelial cells in the vulva and vagina. In view of the increase in the number of immuno-suppressed patients these findings should direct clinical attention to this so far unusual manifestation of a CMV-infection.

The patient was a 32-year-old para 2 who developed within a few hours after the first symptoms a lethal HELLP-syndrome. Despite initially normal blood pressure an hepatic failure, hemolysis and cerebral bleeding appeared. The autopsy revealed an eclamptic liver and symptoms of a disseminated intravasal coagulation (DIC). As a consequence a quick delivery instead of a conservative-expectant management because of fetal prematurity is recommended.
The data available for the epidemiology of AIDS and HIV infection among women in the Federal Republic of Germany describe the distribution of risk groups to urban areas. The highest rates of seroprevalence are found in cities like Berlin, Munich and Frankfurt. These data do not support the initial fear of an explosive heterosexual spread in the female population. Since 1986 the incidence of pregnancies of HIV-infected women in Berlin has remained constant. The number of pregnancies carried to full term decreased because of an increased number of terminations. Another growing problem are gynecological complaints, depending on the duration of the HIV infection, ranging from repeated infectious diseases of the genitals to cervical cancer.
Between 1985 and 1989 108 HIV-positive patients underwent cytologic examinations at the Dept. of Obstetrics and Gynecology, Univ. of Berlin. These patients showed 15 times more pathologic smears (group IIId to V) than all other patients of the hospital investigated. By histological controls 10 cervical intraepithelial neoplasias (CIN I to III) and 5 invasive carcinomas were found. A significantly high percentage (40.3%) of the smears showed signs of a papilloma-virus-infection, even the pathologic smears in a percentage of 80.7%.

In an experimental study with laboratory animals we studied the relation between the extent of a disorder of regional wall motion as determined by echocardiography and size of a perfusion defect following occlusion of a coronary vessel for 5 hours. It was demonstrated that wall motion is not uniform in a normal left ventricle and that there is a wide range of variability in wall motion within a given myocardial segment. For this reason we determined the extent of a disorder of regional wall motion in two echocardiographic planes with reference to defined normal values. Analysis of interobserver and intraobserver variability showed that reproducible determinations of the circumferential extent of a disorder of regional wall motion and the ejection fraction are possible with an acceptable degree of certainty. There was a significant correlation between morphological determinations of the size of a perfusion defect in the left ventricle and the circumferential extent of a disorder of regional wall motions as demonstrated in the echocardiogram (r = 0.83). The regression curve (y = 4.26 + 0.95x) for determinations of the size of the perfusion defect approached the identity line with a standard error of estimation for the echocardiographic examination of 7.4%. Size of the zone of infarction was overestimated by an average of 8% with echocardiography (r = 0.81), with a standard error of estimation of 6.6% (y = -2.41 + 0.85x). There was no significant correlation between ejection fraction and size of the perfusion defect or the size of infarction.(ABSTRACT TRUNCATED AT 250 WORDS)
Keratin was identified with the aid of polyclonal antisera in the cytoplasm in over 90% of the transitional cell carcinomas investigated. The intensity of staining increased with the degree of dedifferentiation. Detection of cytokeratin with monoclonal antibodies was successful in over 80% of samples. All squamous cell carcinomas of the bladder were strongly positive for keratin and cytokeratin. CEA was found in 20% of the G1 and 40% of the G2 and G3 carcinomas of the bladder. Both the prostatic epithelium markers PSA and PAP and the monoclonal antibody Ca1 were negative in all cases.
Keratin was found in more than 90% of transitional cell carcinomas of the bladder in the cytoplasma with polyclonal antibodies. Intensity increased with dedifferentiation. Cytokeratin was detected with monoclonal antibodies in more than 80%. Squamous cell carcinoma of the urinary bladder was always strongly positive for keratin and cytokeratin. CEA was found in 20% of G1 and 40% of G2 and G3 carcinomas of the urinary bladder. The prostatic epithelium markers PSA and PAP were always negative also Ca1.
38 pure and 11 mixed seminomas were studied with the peroxidase-anti-peroxidase method for the presence of chorionic gonadotropin (beta-HCG) and pregnancy-specific beta 1-glycoprotein (SP1). HCG was found in 8 of 49 cases, SP1 in 5 of 49 cases in syncytial and mononuclear giant cells. The 5 pure seminomas with positive tumor markers appear to have no worse prognosis than pure seminomas without HCG or SP1 production. None of the seminomas was found to contain carcinoembryonic antigen or alpha-fetoprotein.
Prostate acid phosphatase (PAP), prostate-specific antigen (PSA), carcinoembryonic antigen (CEA) and keratin were determined immunohistochemically in paraffin sections from 64 prostatic carcinomas fixed in formalin according to the conventional method. The results obtained with PSA led to the correct diagnosis of prostatic carcinoma in 90.7% of the cases. 80.3% of the diagnoses obtained with PAP were correct. The intensity of the staining of the marker decreased with increasing differentiation. 3 utricular carcinomas were positive for PAP and PSA. CEA and keratin may be considered unspecific tumor markers only. However, metaplastic squamous epithelium from poorly differentiated carcinomas was always positive for keratin. PAP and PSA are also suitable for differentiating between tumors of prostatic and nonprostatic origin and could thus be successfully used to determine immunohistochemically the histogenesis of 15 invasive, poorly differentiated carcinomas of the prostate and bladder. PSA again proved to be a more specific epithelial marker than PAP.
Metastases of 47 known prostatic carcinomas were subjected to the unlabeled immunoperoxidase procedure to localize prostate acid phosphatase (PAP) and prostate-specific antigen (PSA). PAP was found in 64% and PSA in 78% of bone marrow, lymph node, lung and liver metastases investigated. There was no significant difference between the intensity of staining in primary and metastatic neoplasms. Staining of PAP and PSA was found to be less intense in poorly differentiated metastases of prostatic adenocarcinomas. The data suggest that the demonstration of PAP and PSA is a practical and sensitive test for determining the prostatic origin of a clinically and histologically unclassifiable metastasis.
