Effects of a bezafibrate sustained release formulation on plasma lipoproteins in patients with hypercholesterolemia. Importance of timing of tablet intake for efficacy.

The therapeutic effect of a single sustained release tablet of bezafibrate (Cedur retard, 400 mg) in primary hypercholesterolemia type IIa and type IIb was investigated in a placebo-controlled randomised study comparing the efficacy of morning vs. evening intake. The decrease in total cholesterol with the morning intake was 18.5% vs. 16.5% for the evening intake (n.s.). HDL-cholesterol increased more in patients taking bezafibrate retard at morning (29.6% vs. 22.4%, p less than 0.05). Bezafibrate was well tolerated. Animal experiments and precursor studies of cholesterol synthesis in man indicate peak activity of HMG-CoA reductase between 1 a.m. and 3 a.m. The data suggest that with normal eating habits during day time other modes of action of bezafibrate besides HMG-CoA reductase inhibition such as reduction of VLDL-synthesis in the liver and an increased fractional catabolic rate, could contribute to the therapeutic effect.

The effect of ovariectomy on lipoprotein metabolism during a period of one year.

In a follow-up study over one year the influence of sex hormones on lipoprotein metabolism in women after ovariectomy was investigated. During this relatively long period we could observe several changes of lipid and apolipoprotein levels in serum which may be caused by adaptive processes after withdrawal of sex hormones. The first phase of variation of lipids and apolipoproteins (2 weeks after ovariectomy) is strongly influenced by the operation trauma and characterized by a decrease of apo A-I, apo A-II, TC levels and an increase of the triglyceride level. The second phase (6-18 weeks after operation) shows an increase of the levels of apolipoprotein of HDL (apo A-I and apo A-II) and also apo C-II and apo C-III. The pattern in the third phase (one year after operation) differs from that of the two earlier phases and is similar to that in the natural postmenopausal state (elevation of apo B, TC, nonsignificant elevation of apo A-I and significant elevation of apo A-II). It seems that ovariectomy causes permanent changes of the LDL-level whereas the fluctuation of HDL is temporary. The late changes in the lipoprotein spectrum following ovariectomy in the reported direction may be important for the increase of ischemic heart disease observed by many investigators.

Effects of high doses of oestrogens and androgens on lipoproteins: observations in the treatment of excessive growth with sexual hormones.

In a prospective study we investigated the changes of lipoprotein metabolism under therapy with high doses of oestrogens or androgens, applied to stop the excessive growth of very tall girls or boys. Therapy with 2 mg ethinyl oestradiol sulfonate per week for one year in 11 girls resulted in an increase in serum triglycerides, which was reversible after cessation, and a minimal rise of total cholesterol and HDL-cholesterol in the phase of adaptation to this treatment. Therapy with 1000 mg testosterone oenanthate per month for one year lead to a fall of triglycerides and HDL-cholesterol. These changes are considered as a regulative phenomenon, without consequences for the application of the high dosage therapy with these steroid hormones in the treatment of excessive growth.

Intestinal biosynthesis of apolipoproteins during active fat absorption in rabbits.

To gain some insight in the biosynthesis of apolipoproteins in small intestinal walls of rabbits we investigated the in vivo incorporation of [14C]leucine into mesenteric lymph lipoproteins during active fat absorption. The labeled amino acid was predominantly incorporated into the apo B-containing lipoproteins, but also in HDL. More than 40% of total lymph radioactivity were associated with the lipoprotein bound apolipoproteins of chylomicrons, VLDL and HDL. About half of this radioactivity was incorporated into apo B, predominantly into that of the triglyceride-rich lipoproteins. Apo A-I and apo A-IV contained about 17 and 3% of total lymph radioactivity, respectively. In contrast, no radioactivity was detectable in the gel zones corresponding to plasma apo E or lower molecular weight peptides (apo C-II and C-III). During active fat absorption the small intestine of rabbits secretes apo B and apo A into mesenteric lymph, which are obviously necessary for the lymphatic transport of lipids.

Pituitary and adrenal hormones in patients after myocardial infarction under ergometer load.

The levels of human growth hormone (HGH), ACTH and cortisol in the plasma of 100 middle-aged men were measured by means of radioimmunoassay (12 patients in the phase of hospitalization after myocardial infarction, 47 patients in convalescence, 31 patients in post-convalescence, 10 healthy men). Twenty patients in the phase of convalescence and all patients in post-convalescence did exercises on bicycle ergometer with submaximal loading. Patients after myocardial infarction showed significantly lower basic levels of HGH than healthy persons, and the increase in the HGH level induced by exercise was significantly lower. The hormones ACTH and cortisol showed only slight differences. The secretion of the pituitary hormones, mainly HGH, seems to be altered in patients after myocardial infarction.

[Uric acid determination in dilute serum with an enzyme electrochemical and enzyme-free sensor].

The paper describes the development of electrochemical methods for analytical determination of uric acid basing on amperometric sensors: with an uricase enzyme electrode, termed "electrochemical-enzymatic" method; with an enzyme-free electrode by indication of the anodic oxidation of uric acid, termed "electrochemical" method. The comparison of analytical quality of both methods shows a sufficient specificity, precision, and accuracy. They show a good correlation with regard to the specific uricase-catalase reference method (UCM): yelectr./enz. = (0.943 xUCM + 19.8) mu mol/l; r = 0.9948 yelectr. = (0.964 xUCM + 6.31) mu mol/l; r = 0.9917 The analysis by the "electrochemical" method can be done with a higher frequency of samples per hour completely without need of enzyme. Both methods are principally suitable for an application in analytic biochemical laboratories.

[Alcohol and lipid metabolism].

Alcohol has a direct and/or indirect influence on the fat metabolism, evoking a HLP and this again may lead to secondary defects. Liver defects, pancreatitis, haemolytic conditions may be the sequel of the alcohol as well as of the HLP. When a HLP, particularly of type IV. is present, these statements demand a careful alcohol anamnesis. A control after absolute alcohol abstinence is necessary. If within a few days the HLP shows tendencies to involution, the alcohol can be proved as pathogenetic factor. The exclusion of the noxa is of decisive therapeutic importance and often spares a little effective and loaded with side-effects pharmacotherapy.

Effects of sexual maturation on serum lipoproteins.

In 503 healthy Dresden children aged 1 to 17 years we evaluated triglyceride (TG) and total cholesterol (TC) concentrations. The mean TG level decreases up to the 7th year of life followed by a slow increase. During adolescence we found a difference with higher TG levels in females than in males. TC concentrations remain constant from the first to the 11th year of life in both boys and girls. In the 12th and 13th year TC sharply decreases in girls followed by a rise one year later. The same phenomenon occurs in males 1-2 years later. In a second study 372 pupils were investigated consisting to two age groups: 250 boys and girls aged 8 years and 167 young men and women aged 17 years. We estimated TG, TC, high-density lipoprotein cholesterol (HDL-C), and in quite a number of cases HDL apoproteins, estradiol (E2), and testosterone (T). HDL-C concentrations do not differ between sexes in childhood, whereas they do after puberty. We found reduced HDL-C levels in young men. The apoprotein pattern of HDL in male adolescents differs from those of boys, girls, and young women showing an increased relative amount of apo A II. The comparison of E2 and T levels with HDL-C concentrations allows to draw conclusions on the importance of testosterone for the HDL metabolism
