[Pathogenesis and pathology of acute respiratory infections].

The paper deals with modern results on the pathogenesis of acute respiratory infections (ARI) caused by viruses. The course of the disease depends on exogenic (viral) factors and intrinsic individual conditions. The earliest steps of infection are only understood by immunologic, biochemical and virologic considerations until the first, only by electronmicroscopy detectable structural lesions of cells, the mucosa and the lung tissue can be seen and finally more or less severe damages of the respiratory tract with bronchitis and pneumonia occur. At first this is considered from a general point of view for all ARI with regard to local pathologic changes of bronchial tree and lung, to doubtlessly existing viremias and general toxic and circulatory damages of the whole organism. Thereafter some typical but different findings caused by the most important viruses are shortly described.

Detectability of the acoustically evoked composite response (40 HZ potential) near threshold.

In 56 persons with normal hearing and 10 patients with sensorineural hearing loss, acoustically evoked potentials were recorded at stimulus repetition rates of 10/s, 20/s, 30/s and 40/s. With a sufficiently low lower amplifier cut-off frequency, one obtains at the repetition rate of 40/s a composite response described by Galambos et al. (1981) as 40 Hz potential, which consists of the ABR evoked by the given stimulus and superposed MLR-waves evoked by the preceding stimuli. In both groups the amplitude mean value of the potentials recorded at the stimulus rate 40/s was significantly higher than at lower rates. The detectability of the near-threshold response for the subjects with normal hearing was distinctly higher than the responses recorded at lower rates.

[Coincidence of silicosis and sarcoidosis. 1. Observations in a selected patient sample].

In a group of 1,975 patients with sarcoidosis we observed 11 patients with an additional histologically proved silicosis and 4 further patients with a silicosis found out by X-rays. In 57 other patients of this group we could only state a professional exposure to quartz dust. In this 1st communication we point out casuistries of literature, own experiences, possibilities and necessaries for a secure diagnosis of a coincidence of both diseases without regards of causal and pathogenetic connections. The physician at the hospital will take the last responsibility for diagnosis; it is based as a rule on X-ray-series. Most reliable and uncontradictedly recognized diagnosis of coincidence of silicosis and sarcoidosis is got by histological examination of lymph nodes and lung tissue obtained by microthoracotomy.

Wound healing of spleen ruptures by ultrasonic welding in animal experiments.

Violent ruptures of the spleen caused by accidents are regarded as an indication for necessary splenectomy. Experiences of modern ultrasonic surgery, e.g. in ultrasonic welding of bone fractures prompted us to experimentally investigate ultrasonic welding of spleen ruptures in dogs. We immediately obtained a safe closing of the ruptures with a durable scar and thus the method may give an alternative to splenectomy with saving of the organ.

[Present findings on the determination of mycobacterial resistance].

The change of the signification of the appearance of tuberculosis is explained (1) in a cognition-theoretical, (2) under a more medical practical aspect and (3) in the real prevalence of tuberculosis in autopsies nowadays. The pathology of the tuberculosis and of its different manifestations were known in the main before the detection of tubercle bacteria by Robert Koch. However, its nature and its cause and the connexions of the different forms of tuberculosis were wrongly interpreted in relations to the timely knowledge. Not till the detection of the tubercle bacteria and confirmation of tuberculosis as a bacterial infectious disease the pathology gained new possibilities to take part in further investigations in the disease with great success. The disease of tuberculosis goes back to earliest time of mankind. Since this time it existed until the middle of 20th century in nearly the same manner, not curable with a high morbidity and mortality and heavy courses. A change of its course to lower mortality and a decrease of the heavy exudative forms could be observed after getting a higher social standard of life in many countries, by the modern prophylaxis and therapy of the tuberculosis and related to a disease drifting into the older ages of men with a predominance of milder forms of tuberculosis in this age. We use our observations of 53,167 autopsies of adults with a total number of 2277 (4.3%) tuberculosis, (1249 = 2.4% inactive and 1028 = 1.9% active tuberculoses) to explain some facts about the present situation of tuberculosis. In this time the relative amount of tuberculosis among the whole number of autopsies decreased from 6.4% to 3.2%, the active tuberculosis from 3.8% to 0.8%. In 1961 we still found 39% exudative-cavernous tuberculosis of lungs, 1981 only 21%. But the amount of tuberculosis miliaryis pulmonum increased in this period from 3% to 26%, the generalised tuberculosis miliaris from 17% to 53%. 1981 only 1 tuberculosis deceased was younger than 50 years. But alarming is the fact that more than 50% of active tuberculosis and 63% of the miliar tuberculosis in the last 10 years were not diagnosed in lifetime but only by autopsy.

[Tumor-like necrotizing granulomatosis of breast].

The observation of a tumor-like granulomatosis of the breast with vasculitis, necrosis and fibrosis is the reason for the discussion of the pathogenesis and the type of this special inflammatory lesion. The histological picture is that of Wegener's granulomatosis. The delimitation of a true tumor by clinical exploration is difficult or impossible and requires the histological examination in each case.

[The pathology of the tuberculosis 100 years ago and its present role in mortality].

On occasion of the centenary of the detection of the tubercle bacteria by Robert Koch we try to give a survey on the knowledge of pathology of tuberculosis 100 years ago. Besides mystic considerations since the antique medicine, the cause of the tuberculosis was seen in a phthisic habitus, as endogenous, not perceptible, the disease as not curable, its nature as a hereditary illness, as a metabolic disorder or, in the time of Robert Koch, mostly as a malignant neoplasia. Only few physicians considered the tuberculosis as an infectious disease with inflammatory properties. Since his discovery of the tubercle bacteria, Robert Koch was sure that the tuberculosis could be cured and eradicated like other infectious diseases. He believed that this aim could be attained in a short period. At the end of this paper, a brief review is given about the frequency and importance of the tuberculosis as a cause of death in the past and especially nowadays after the consequent fight against this disease.

[Problems of the pathogenesis and of the pathology of tuberculosis once and at present].

The paper is giving review of some important points in the development of pathology and especially of pathogenesis of tuberculosis. Highly different conceptions of the cause and the kind of tuberculosis existed in the past times up to the detection of the tubercl bacteria by Robert Koch and even later on. About 100 years ago in medical research the mean discussion of tuberculosis problem was determined by two men, Robert Koch and Rudolf Virchow. The last part of the paper deals with some aspects and actual problems of tuberculosis in the last decades.

[On problems of the term "atypical adenoma" of the thyroid gland (author's transl)].

Besides a separation of reactive-hyperplastic nodules from true adenomas in the sense of autonomic tumours of the thyroid gland "follicular neoplasias" often make difficulties in the daily diagnostic work. This collective term integrates the follicular adenoma and the follicular carcinoma of the WHO-Classification of thyroid tumours (1974). The distinction between a benign "simple adenoma" and an even benign "atypical adenoma", between encapsulated well-differentiated follicular carcinoma and differentiated follicular carcinoma with wide infiltration of the capsule besides other less differentiated follicular carcinomas is very useful. In spite of the good prognosis of the atypical adenoma, clinicans should know the special problems of these tumours and provide a very careful post operationem control.

[The function of pathology at service of clinical special disciplines in past and presence (author's transl)].

The intent of this presentation shall be a contemplation about the character and the position of the pathology in the past and its determination in the presence after changing of many tasks. The allsurrounding pathology as a general system and doctrin of diseases continues in the pathology in spite of the separation of pathophysiology and pathobiochemistry in teaching and researching methods. The necessary development of an organ-pathology supported by the clinical specialisation again demands a conform functional and structural style of thinking in pathic processes. Way and realization possibilities of this demand in the daily practice especially for lung pathology are discussed and proposed. The pathology is a service of other clinical specialities and has to promote advances of knowledge.

[Considerations on a pathology of the diaphragm. 1st Report: the pathology of the diaphragm as a part of a pathology of respiration (author's transl)].

The first part of this paper deals with the idea of a "pathology of the diaphragm" and its special relations to the respiration on phylogenetical and ontogenetical base. A pathology of diaphragm does not exist until now. The diaphragm represents an important respiration organ in orthological and pathological conditions. The function of this organ is described and derivated from its specific anatomical structure and topographical site. In this conception the pathology of the diaphragm is a part of the complex pathology of the respiration. A systematic presentation of diaphragm diseases will follow in a second part of this paper.

[Alveolar soft-part sarcomas in the skeleton (author's transl)].

The rare alveolar soft-part sarcomas, tumours with well defined histological pattern but uncertain histogenesis, are up to now described only in soft tissues, especially in those of the extremities of adolescents and young adults. We can report two cases of alveolar soft-part sarcomas in the sceleton, both in the right femur of 22 years old women. The delimitation of these tumours against others with similar histological pattern may be very difficult. The occurrence of these tumours in the sceleton does not require to substitute the term alveolar soft-part sarcoma by an other one.
