In late pregnancy about 10% of the patients suffer from dizziness and a shock symptomatic if placed in a supine position. These symptoms are reversible if the patient turns to the lateral position. In the present paper experiments in human and sheep are reviewed. The cause for this disease is the occlusion of the inferior vena cava by the gravid uterus. The venous backflow to the maternal heart is reduced and the cardiac output decreased by about 1 1/min. The total peripheral resistance (TPR) is increased (Lees et al. 1967b). The increase in TPR is due to the compression of the vena cava by the uterus and not related to an increased arterial tone during the first time course. The fall in mean arterial blood pressure is of varying degree and paralleled by an increase of the blood pressure in the vena cava below the compression and the uterine vein. The fall of the mean arterial blood pressure war related to the decrease of the perfusion pressure. The fall in arterial blood pressure and most important the rise of the uterine vein blood pressure led to a decrease of the perfusion pressure across the uterus. The uterine blood flow fell proportional to the drop of perfusion pressure. The uterine vasculare resistance remained unchanged. The fall of uterine blood flow was associated with a fall of fetal heart rate.

The relationship between dip area (DA) of fetal heart rate (FHR) and the acid base status of fetus and mother during the first and second stage of labor were studied on 39 women at term. The observations reveal a significant correlation between the fall in fetal base excess (BE) and the measured DA: deltaBE = 0.01 - 0.143 DA (2alpha less than 0.01). There is no difference in the correlation of these parameters between the first and the second stage of labor despite the fact of a greater fall in BE and a greater DA per time compared to the first:BE meq/1/10min. 0.14 (SD 0.26) and 0.76 (SD 0.62); DA cm2/10 min. 0.99 (SD 1.19) and 4.28 (SD 3.06), respectively. With increasing DA the variance of the fall of fetal BE (S2BE) rose also: DA of 0-5 cm2: S2BE 0.66; DA greater than 15.1cm2:S2BE 3.76. With the fall in fetal BE there was also a fall in maternal BE, more pronounced during the second stage of labor:-BEF=0.38.DELTA BEM-0.70(2alphaless than 0.01). The fraction of the fall in fetal BE per DA increased with the fall in maternal BE (2alpha less than 0.05). This correlation reveals that there might be a slight influence of maternal BE on fetal BE. The present observation shows that measuring DA serves only as an approach to predict fetal condition during labor. It therefore should be combined with fetal scalp blood sampling. The influence of maternal BE on fetal BE seems to be only of less importance.
Under the conditions of an optimized (with regard to caries prevention) fluoride content of the drinking uater, the authors studied (in the framework of an oral hygiene measure covering 32 months) in 149 children 6.5-8 years of age the effects of supervised daily dental and oral care on dental health. The improvement in oral hygiene (OHI) by 33% is in harmony with an additional caries reduction by 33.3% (DMF/S index) and a decrease of the PM index by 47%. A wider use of oral hygiene actions as secondary preventive measures is, therefore, recommended also for towns with fluoridated drinking water.
12 years after water fluoridation (1971), the repercussions of the quantitative and qualitative changes in caries prevalence on the paedodontic care were studied on the basis of statistical findings from more than 36 000 subjects aged from 6 to 18 years. It was shown that the result of oral rehabilitation is substantially influenced by the increase in the number of subjects with primarily healthy dentitions and by the reduction in the number of carious teeth. Furthermore, the reduced number of cavities and fillings involving two or more surfaces and the less frequent occurrence of secondary caries have repercussion on the time spent by the paedodontist. Not least the caries reduction also influences the paedodontist/children ratio which has changed from 1:1659 in 1959 to 1:3208 in 1971.
In a 3-year, 4-year and 5-year studies, the authors tested clinically and radiographically the caries-inhibiting efficiency of topically applied amine fluoride in defined groups (fluoride group with Elmex fluid and Elmex tooth-paste for unsupervised dental and oral hygiene at home, placebo group and control group). After the fifth study year, the caries increment in the children who were 11 years of age at this time had decreased by 30.6% (DMF/T index) and 32.1% (DMF/S index).

Cariostatistical comparisons made since the start of drinking-water fluoridation in Karl-Marx-Stadt suggested territorial differences in the prevalence of dental decay. In the present paper an attempt is made to analyse the problems involved, with due consideration being given to both demographic and ecological factors.

The aim of the present paper was to investigate if the glucose concentration of the fetal blood is reduced already during parturition. It was further of interest if there is a relationship between the glucose concentration in the maternal blood and the acid-base-balance of the maternal and the fetal blood, respectively. The observations comprised 40 patients during labor. Blood was sampled from the hyperemized fetal scalp and the umbilical artery. The maternal blood was collected from the hyperemized earlobe and fingertip, respectively. The blood was analyzed for pH, PCO2, base excess and blood glucose. The dip area (DA) was taken from the cardiogram and measured by planimetry. During labor the blood glucose increased in the fetal blood from 67 mg% (SD 12) to 87 mg% (SD 23) (2 alpha less than 0,001) and in the maternal blood from 88 mg% (SD 14) to 113 mg% (SD 29) (2 alpha less than 0,02). There was a significant correlation between the fetal and maternal blood glucose concentrations. The increase of the fetal glucose concentration is, however, less with increasing maternal blood glucose. (b = 0,66). The base excess in the maternal and fetal blood fell significantly. The rise of the maternal and fetal base excess (= base deficit) was related to the increase of the glucose concentration (2 alpha less than 0,001). If the base excess was zero, the fetal and the maternal blood glucose was 46 mg% and 78 mg%, respectively. The difference between the maternal and fetal blood glucose was 28 mg%. With increasing DA the fetal blood glucose increased (2 alpha less than 0,001.). From the observations it is concluded that there developes no hypoglycemia during parturition. This is due to the correlation found between fetal and maternal blood glucose and due to the rise in fetal blood glucose during hypoxia. Obviously, the decrease in fetal glucose following delivery is caused by a lack of glycogen which is enduced during labor and strengthened by a deficit of enteral glucose supply.
