[The caries-protective efficacy of 2 fluoride varnishes in a 2-year controlled clinical trial].

The aim of this randomized, double-blind study was to measure the cariostatic effect of Bifluorid 12 (VOCO GmbH), containing 6% sodium fluoride and 6% calcium fluoride and Laweflour-Schüttellack (LAW), containing 5% sodium fluoride in comparing of placebo varnish. The caries study included 400 schoolchildren, aged 12-14 years. The tests according to the DMFS were carried out by two independent examiners. The 400 children were divided into 3 test and 1 placebo group, each group consisting of 100 subjects. After two years there was a significant inhibition of caries increment in all test groups compared to placebo group. Percentage caries reduction ranged from 25 to 30%. The highest effect was stated at proximal surfaces.

[Comparison of oral health in metropolitan and nonmetropolitan district of Leipzig in 1979 and 1989 (ICS I replication study)].

In 1989 the Leipzig-ICS-I study from 1979 was replated according to the same methodology on randomized probands (N = 2693). A significant reduction of the dmft-value (27%) and DMF-value (50 resp. 27%) with clear increased f(F)-value was found at 8/9 and 13/14, years olds. At the same time there was a caries increment of 1.9 DMFT from 1979 to 16 DMFT in 1989 in the age group of 35-44 years. Again in this group was found a very low edentulousness (0.3%) and a very high number of natural teeth (25.9). During 10 years the prevalence of dentofacial anomalies decreased about 25.6% (13 to 14 years old) and in the adult group the prostetic treatment needs decreased by half. The results reveal an essential improved oral health status in consequence of intensive dental care. Unsatisfied is the periodontal status. 90% of subjects need oral hygiene measures.

[Clinical controlled study on the treatment of deep carious lesions in deciduous molars].

Compared with a step-by-step procedure, the one-step excavation of deep carious lesions in primary molars proved to be the treatment of choice in a clinically, microbiologically and histologically controlled trial. The stepwise method failed to produce any significant reduction in pulp exposure. Sensitivity assessment and percussion findings did not result in a reliable and unambiguous evaluation of treatment success. Histology showed inflammation-free pulps in 67% of the deciduous molars irrespective of the treatment method used. Microbiological examination revealed slightly softened dentin to be significantly more infected than clinically acceptable, hard dentin; although only 59.3% of the cavity floors were free of microorganisms. Chronic pulp inflammations were correlated with the presence of streptococci and lactobacilli, whereas actinomycetes were associated with inflammation-free pulps.

[Tooth health and periodontal condition in adults with and without root caries].

In a population sample of 2261 men and women aged between 35 and 74 years the prevalence of root caries increased for both sexes consistently with age and ranged between 49 and 73%. In the mean 1.8 to 2.9 teeth were affected. Subjects with root caries had fewer teeth, more teeth with untreated coronal caries, and more teeth which needed conservative treatments. The number of extractions required was the same in subjects affected by root caries. In the age older than 65 years subjects affected by root caries had more teeth in the average of 2 teeth and significantly more fillings in their teeth. If a DMF/T-index of 15 to 19 was recorded nearly half of all subjects were affected by root caries. In individuals with root caries lesions aged between 35 and 54 years significant severe periodontal disease were seen.

[Priority health targets in the new Germany from the view point of oral health].

The global pattern of caries prevalence has undergone some changes since the seventies. This has occurred with some remarkable geographic differences. Comparative statistical analysis shows an acceptable situation at the current moment for the population of the new "Bundesländer". The globally given goals of oral health for the various age groups can be reached or have already been accomplished in part. For the youth population the state of good oral health corresponds always to the caries protective availability of fluorides, for the adult population to the remarkable state of a tooth-preservative oriented curative care. For the senior citizen population the high loss of teeth is an indication of past neglect in dental care. Future development of oral health of the population depends largely on all-encompassing preventive and tooth-preservative concepts of dental care.

[Caries protective effectiveness of fluoride varnish in animal experiments and clinical research].

Fluoride containing varnishes represent clinically proved means for individual caries prevention by topical application due to their prolonged contact time between fluoride and enamel. The fluoride accumulation in vitro and in vivo, the inhibition of demineralization, the experimentally and clinically proved reduction of caries and the toxicological safety during varnish application are reviewed. A reduction of caries between 20-50 per cent may be registered after semiannual application of such varnishes due to their high caries protective properties.

[Caries protective effect of a NaF containing toothpaste with and without calcium carbonate polishing agent].

A caries preventive effect depending on the free fluoride concentration was registrated on rats using dentifrices with NaF or MFP (alone or in combination) despite the presence of CaCO3 as abrasive material because the composition of these dentifrices was changed. The free fluoride concentration of dentifrices had induced the accumulation of fluoride into enamel. The ionized fluoride content of NaF-dentifrices was unchanged for 3 years. MFP was hydrolized and had continuously released fluoride ions. The caries preventive effect which was increased by HCO3- was reproducible in correspondence with the fluoride concentration after the storage for two years.

[Plaque and gingivitis reduction by an amine fluoride/stannous fluoride combination].

A clinical study on children has demonstrated that the accumulation of plaque and the development of experimental gingivitis can be reduced by a daily mouthrinse with AmF/SnF2. Low, reversible yellowish tooth staining can almost be prevented by mechanical oral hygiene. The efficacy of AmF/SnF2 was nearly the same as chlorhexidine and the F combination can be recommended for preventive plaque control, especially on high-risk patients.

["Goldblatt phenomenon of the uterus" and latent kidney disease as the cause of pregnancy-induced hypertension--epidemiology and therapeutic consequences].

The aetiology of pregnancy-induced hypertension (PIH) is at present unknown. Epidemiological data lead us to assume, that two main mechanisms could be responsible for the development of PIH. They are not well understood in their complexity, but result in the same pattern of signs and symptoms: Oedema, proteinuria and hypertension. 1. "Goldblatt-Phenomenon" in the uterus, as a result of a disturbed maternal immune response to the cytotrophoblast, followed by the reduction of uterine blood flow and the liberation of vasoactive substances from the placenta. 2. Renal factors, especially in elderly women, result in a manifestation of nephrotic diseases during pregnancy. No screening methods are at the present time available to diagnose PIH in advance. It is therefore necessary, to look at typical clinical manifestations, i.e. the development of hypertension. It is also important to estimate the weight gain and the occurrence of oedema before rising blood pressure demonstrates a general vasoconstriction in the maternal vascular system. The basic therapeutical concept is to reduce the peripheral vascular resistance, to prevent maternal complications and to reduce the uterine vascular resistance to improve foetal oxygenation. In many cases an improvement of the oxygen supply to the foetus is not possible, since irreversible alterations of the uterine arterial vascular bed have already taken place. For the treatment of PIH, different drugs are available which act on different targets. In cases of special medical history, the early application of magnesium and acetyl salicylic acid (ASA) should be included in the therapeutical concept of prophylaxis.

[Passive immunotherapy with polyvalent immunoglobulins in women with habitual abortions].

Intravenous immunoglobulins (IVIG) were used in an alternative approach to immunotherapy with allogeneic leukocytes to prevent recurrent spontaneous abortions (RSA). Twenty five women with a history of greater than or equal to 3 unexplained RSA were treated with IVIG during their next pregnancy. An initial dose of 30 g (0.5-0.6 g/kg body weight) was administered at gestational week 5-6. Treatment was continued every 3 weeks with 20 g (0.3-0.4 g/kg) and terminated by week 25. In primary RSA the success rate was 82-85%, which is in the same range as for leukocyte therapy. In contrast, IVIG was successful in only 40% of secondary aborters. We conclude that IVIG can be an effective treatment for prevention of primary RSA. Compared to leukocyte therapy the risks of transmission of infections and of HLA immunization can be avoided.

[Intraoral pH measurement by determination of acidogenic potentials of dietary materials].

The intra-oral production of acid by plaque bacteria from sucrose, glucose and sorbitol and test products with sugar substitutes as well was estimated by pH-telemetry which permits the digital registering and collection of data. A pH-drop was not observed when sorbitol or xylitol were offered as a substrate to the dental plaque. The small differences of intensity and duration of the telemetric estimated acid production by dental plaque due to the test products (with sucrose or sorbitol/glucose) exhibited different cariogenic challenge in the animal experiment and in the intra-oral cariogenicity test, but with high probability they have not any practical significance for the preventive dietetic recommendations.
