[Will root caries become a treatment problem of the future?].

Based upon the present prevalence of root caries an attempt is taken to evaluate the future importance of this carious type. A population sample of 2,261 men and women aged 35 to 74 years were examined. The participation rate was 81%. Root caries was recorded when the lesion was obviously soft and located on cementum. The prevalence of root caries increased for both sexes almost consistently with age and ranged between 49 and 73%. In the mean 1.8 to 2.9 teeth were affected. Root caries was found more often among men than women. Over the sixth decades the reduction of the relative frequency of root caries was stated. Up to this age the rates of tooth extraction were higher than the rates of incidence. If it succeeded that in the future aged people will retain more teeth than nowadays a increase of root caries can be expected.

[The plaque- and caries-inhibiting efficacy of an amine fluoride/tin fluoride combination in an animal experiment on rats].

A similar plaque inhibition by was found after daily topical application of CHX or AmF/SnF2 in comparison with a placebo on rats. The fluoride combination exhibited a significant caries protective influence which was equal to AmF when the last was used in the same concentration. More concentrated AmF has shown a higher caries reducing effect and higher fluoride accumulation in the enamel surface layer of teeth. AmF/SnF2 could been recommended to individual plaque control with parallel caries protective effect.

[The microbial colonization of the dentin close to the pulp in the permanent teeth following deep caries therapy].

In a clinical microbiological study the microflora of cavity floors in 41 permanent teeth was investigated after several month of caries excavation and with or without Calxyl application on hard dentine prior sealing with zincphosphatecementum and amalgam. The cavity floors were only lowly infected. Actinomycetes or acidogenic and aciduric streptococci and lactobacilli dominated in the CFU. After application of Calxyl actinomycetes (A. israelii) were found 3-fold more frequently in the remaining population than streptococci (S. mutans) and lactobacilli. Without application of Calxyl their ratio was 1:1. S. mutans seems to be important for the carious progression in dentin.

[Root caries--scanning electron microscopic observations].

Sound and carious root surfaces of 24 extracted human teeth with extensive periodontal attachment loss were examined by SEM. The microflora covering the radicular surfaces was a complex flora consisting of filamentous and fusiform bacteria, short and long rods. Cocci and coccoid bacteria were observed on root surfaces. Bacterial invasion in the exposed peripheral root dentin was delayed by sclerotic dentin.

Etilefrine and amezinium reduce uterine blood flow of pregnant guinea pigs.

Etilefrine and Amezinium are used during pregnancy to prevent hypotension and fetal growth retardation. The effect of these drugs on uterine blood flow (UBF), uterine vascular resistance (UVR) and fetal growth are, however, unknown. 31 guinea pigs were injected daily with Etilefrine (3 X 0.14 mg/kg) and Amezinium (0.14 mg/kg) from day 30 to day 60. Uterine blood flow was measured with radioactive-labeled microspheres. Uterine vascular resistance was calculated from arterial blood pressure and uterine blood flow. 10 guinea pigs treated with 0.9% sodium chloride served as controls. As a response to Etilefrine and Amezinium, UBF fell by 68 and 48%, respectively, accompanied by an increase in UVR. The fetal weight decreased only slightly. It is concluded that long-term application of anti-hypotensive drugs may be hazardous to uterine blood flow and fetal growth.

Native fluorescence of the cervix uteri as a marker for dysplasia and invasive carcinoma.

A non-invasive and non-destructive fluorescence technique developed recently for an in situ detection of melanomas has been applied for determining in vitro dysplasia and invasive carcinomas in the cervix uteri. The cervices uteri exhibit a fluorescence band with a peak at about 475 nm if excited with 365 nm. The fluorescence intensity increases concomitantly with the degree of dysplasia, ranging from 30 counts/100 ms (healthy) to approximately 200 counts per 100 ms (CIN 3). At the rim of a malignancy, the intensity is 250 counts/100 ms and higher and decreases towards the healthy region. In the tumor region, the intensity is about zero or very small, at the most. The naturally occurring chromophore being responsible for the fluorescence observed seems to be NADH.

Diabetes mellitus in pregnancy. Management and outcome of diabetic pregnancies in the state of Hesse, F.R.G.; a five-year-survey.

From 1982 to 1986, data of 446 pregnancies in diabetic women were compared to equivalent information on 111,390 unselected non-diabetic pregnancies with the help of the Hessische Perinatalstudie (Hessian Perinatal Study, HEPS), a computerized system of collecting information on obstetrical care in the state of Hesse, F.R.G. Patient histories, pregnancy risks, birth risks, fetal outcome and maternal well-being were evaluated to survey the current situation of diabetic pregnancies in the specific constellation of widely decentralized obstetrical management and to point out possible benefits of stronger centralization of these high-risk pregnancies. Perinatal mortality in children of diabetic mothers (4.89%) remains substantially higher than in children of non-diabetic mothers (0.63%), with two thirds of the fetal loss occurring before birth. Infant morbidity, including macrosomia, shows the same impact of maternal diabetes. Maternal post-partum morbidity is increased in diabetic women. 37.9% of children of diabetic mothers were delivered in obstetrical units equipped for maximal care, 17.5% in primary care level hospitals. Perinatal mortality and morbidity as well as maternal complications indicate that diabetic women should receive obstetrical care in those centers that can provide all the necessary facilities.

Prevention of recurrent spontaneous abortion by intravenous immunoglobulin.

Intravenous immunoglobulin (IVIG) treatment was attempted as a novel therapeutic approach for unexplained recurrent spontaneous abortions (RSA) occurring in the first trimester of pregnancy. Twenty women with a history of RSA were treated with IVIG during pregnancy. Therapy was commenced at week 5 of gestation with 1 dose of 0.5-0.6 g IVIG/kg body weight. Infusions were repeated every 3 weeks (0.3-0.4 g/kg) and terminated by week 22 to 24. Of 20 women, 11 delivered healthy infants at term. 5 women are still pregnant, 3 in the third trimester. Only 3 patients suffered abortions and 1 presented with ectopic pregnancy. The overall success rate was 82-86%. Thus, the therapeutic effect of IVIG is comparable to that of the conventional transfusion/vaccination regimen with allogeneic leukocytes, but avoids the risk of transmission of infections and/or HLA immunization, has no major adverse effects and is applicable to 'nonresponders'.

[Periodontal disease prevalence (CPITN) in the adults of an F-optimized drinking water area].

The prevalence of periodontal diseases has been determined by means of CPITN in 35- to 44-year-olds (N = 330) residing for more than 26 years in an F-optimized water area and compared with samples taken of people of the same age in 14 county towns of the German Democratic Republic (N = 1261) and also the Spremberg district (N = 1,116). A relation with the systemic availability of fluoride could neither be deduced from the frequency of calculus nor the prevalence of periodontitis.
