[Effect of 0.1% dexamethasone and 1.0% prednisolone acetate eyedrops on the blood-aqueous humor barrier].

In a randomized, double-blind clinical trial we studied the protective effect of prednisolone-acetate 1.0% and dexamethasone 0.1% on the blood-aqueous barrier after cataract-extraction and posterior chamber lens implantation. Pre- and postoperative anterior chamber fluorophotometry was performed after i.v. administration of 10% fluorescein-sodium in 20 eyes of 20 patients of whom 16 finished the study (mean age 68.7 +/- 11.0; 13 female, 3 male). The topical application of either drug 5 times daily for 5 days did show a significant difference in surgery mediated disturbance of BAB in each group before and after phacoemulsification. However, when comparing the effect of the two drugs with each other, a difference could not be detected (p = 0.35). To attain a statistical 95% probability that there is no significant difference between the two treatment groups, a number of 690 patients would have had to be investigated. The results can be interpreted as to be due to a better cornea penetration of prednisolone-acetate which compensate the higher glucocorticoid-potency and receptor-affinity of dexamethasone. The untreated fellow-eyes did not reveal a significant difference between the pre- and postoperative anterior chamber fluorescein-concentrations in either treatment group. Anterior chamber fluorophotometry is able to demonstrate the potency and influence of both prednisolone-acetate 1.0% and dexamethasone 0.1% eye-drops on the BAB following phacoemulsification and IOL-implantation. The difference of both drugs does not seem to be of clinical importance in this context.

The effect of flurbiprofen 0.03% eye drops on the blood aqueous barrier in extracapsular cataract extraction with IOL implantation.

Preoperative and postoperative anterior chamber fluorophotometry were performed after intravenous administration of fluorescein sodium in patients undergoing extracapsular extraction and posterior chamber lens implantation. The topical application of aqueous flurbiprofen sodium 0.03% solution before and after surgery significantly decreased the surgery mediated disturbance of the blood aqueous barrier as compared to vehicle application (placebo controlled). In contrast to similar studies, cortical steroids were not given topically or systemically to either group of patients during the study. The present fluorophotometric results correlate well with slitlamp biomicroscopy of postoperative inflammation. The data of this study indicate that flurbiprofen ophthalmic solution is effective in protecting the blood aqueous barrier (BAB) in human eyes during cataract surgery and IOL implantation.

Erythrocyte apheresis in combination with elimination of fibrinogen and plasma proteins of higher molecular weight in macular disease and in uveal effusion syndrome.

Reduction of hematocrit as well as elimination of fibrinogen and plasma proteins of higher molecular weight are effective approaches in hemorheology. A combination of these therapeutical concepts was applied in one patient with uveal effusion syndrome and in 16 patients with maculopathy. The hematocrit was reduced by erythrocyte apheresis. Fibrinogen and plasma proteins were eliminated by plasma exchange. In the case of uveal effusion syndrome the filling of the scleral veins as well as the uveal effusion were reduced. Furthermore visual function was improved significantly. In 9 of the 16 patients with maculopathy a significant increase in visual acuity occurred. The therapy described lowers significantly the following parameters: hematocrit, fibrinogen, serum proteins, plasma viscosity, erythrocyte aggregation and apparent whole blood viscosity (native and standardized). In a multiple linear regression analysis we found correlations as follows: plasma viscosity correlates with fibrinogen and proteins of higher molecular weight (r = 0.82), erythrocyte aggregation with fibrinogen and proteins of higher molecular weight (r = 0.88), standardized apparent whole blood viscosity with fibrinogen and proteins of higher molecular weight (r = 0.88) and native apparent whole blood viscosity with hematocrit, fibrinogen and proteins of higher molecular weight (r = 0.91).

[Artificial iris diaphragm in silicone oil surgery].

An artificial PMMA iris diaphragm is described, which is similar to an inferior iridectomy after Ando. The diaphragm can be implanted in aniridic eyes requiring silicone oil surgery. It successfully inhibits contact between the silicone oil bubble and the corneal endothelium.

[Clinical specular microscopy. Technic, organization and simple calculator morphometry].

The use of specular microscopy (SM) as a routine clinical method for qualitative morphological and quantitative morphometric analysis of the corneal endothelium requires a standardized examination technique. Organizational and apparative means are necessary for the analysis, documentation and storage of the data. The results of the immediate analysis are documented on a result form. The morphological changes are classified in 4 different groups (endothelial, pre-endothelial, retro-endothelial, and non-endothelial changes). The morphometric analysis is performed immediately: the mean cell density (mcd) is estimated by comparing the endothelial mosaic in the camera viewfinder with 3 standardized specular micrographs with known cellular density. The good correlation between the results of estimation and definite counting allows this rapid method to be used as a simple clinical screening techniques. The definitive morphometric analysis is carried out directly from the photographic negative using the fixed frame principle. The mcd and mean cell area (mca), m +/- sd, are determined by a simple method which saves time and money, using a scientific pocket calculator programmed in Basic. A specially designed slide rule serves to define the frame. The calculator may also be used for statistical analyses. This is demonstrated in a group of 127 patients (aged 51-87) before IOL implantation. The mcd and mca of the examined population is 2249 +/- 290 cells/mm2 and 444 +/- 57 microns 2. There is no statistically significant difference in mcd between the right and left eye (t-test: p = 0.93). Although statistically mcd decreases significantly with advancing age (p = 0.05), there is a large range of results in this population.(ABSTRACT TRUNCATED AT 250 WORDS)
