[Pathogenesis of chronic subdural hematoma].

In a retrospective examination, the developmental process of chronic subdural haematomas (CSDH) was investigated by means of sequential CT studies. All CSDHs were found to arise from subdural hygromas by means of encapsulation and bleeding into the hygroma content: hypodense, isodense and "mixed" hyper/hypodense CSDHs were shown to develop due to this mechanism. It is suggested that low intracranial pressure represents the main factor for membrane formation and the development of CSDH of traumatic as well as non-traumatic origin. It is hypothesised that CSDHs are space-filling rather than space-occupying and represent some sort of adaption process to intracranial hypotension. The developmental process described is well suited to explain specific features of CSDH, including membrane formation and the latent interval.

Chronic Lyme disease with an expansive granulomatous lesion in the cerebellopontine angle.

Expansive granulomatous lesions in the posterior cranial fossa are rare and have not been reported in conjunction with Lyme disease. We report a patient with verified Borrelia burgdorferi infection who developed a tumor in the cerebellopontine angle. Rapid growth of the tumor led to signs of cerebral compression and to hydrocephalus. Surgical intervention was required despite florid meningitis. The histological examination showed inflammatory, nonspecific granulation tissue. The origin of this tissue is almost certainly causally related to the B. burgdorferi infection. Signs of inflammation resolved rapidly after subtotal resection. The clinical, radiological, and biochemical course is documented. This is the first report of an expansive cerebral lesion in the chronic phase of Lyme disease.

[Morphometric distribution of HLA-DR-coded immunocytes in gingiva biopsy specimens with periodontal disease].

Gingiva biopsy specimens from 14 patients with chronic periodontal disease were analysed immunohistologically. Using monoclonal antibodies and the immunocytochemical APAAP-technique, HLA-DR-associated macrophages, helper/inductor-lymphocytes and dendritic reticulum cells were evaluated. The results show that the specific immunocompetent cells were predominantly found in the upper and middle part of the periodontal pocket. All biopsy specimens were reinforced by macrophages representing the predominant cell fraction. The density of all examined cells was diminished with decreasing API, GI and PDI. It becomes evident by the significant reduction in immunoreactive cells during periodontal therapy that chronic inflammation represents an altering pathogenic environment. The demonstration of HLA-DR positive cells suggests a genetic control of the infection-induced immune response.

[Decentralized psychiatry. Evaluation of the first 5 years of psychiatric service based on a general hospital].

In 1977 the state parliament of Berne took the decision to reform the existing psychiatric health care. decentralized, community oriented institutions were to be established with the aim of preventing secondary disabilities due to psychiatric illness, and of enhancing or instigating rehabilitative measures. 4 different psychiatric services based in general hospitals were installed. The present report presents an analysis of statistical data collected during the first 5 years on one of these services. We found a linear increase of admission which appears to be due to several factors: demand, image, manpower, cooperation with other institutions, breadth of the offered service. Furthermore, the distance between the institution and the place of living proved to be an important factor influencing the incidence of psychiatric treatment and the degree of cooperation with other care services--this aspect being consistent with the arguments for a decentralization of care. The degree of consistency in the distribution of diagnoses and the changes of treatment was surprising: an average of 50% of our patients were referred on an out-patient basis, approx. 40% by general hospital wards, 16% remain under the care of the general hospital. Our statistical analysis shows that in an average of 61% of the cases in which a referral to a psychiatric hospital was discussed, this measure could be avoided (although there is a certain degree of subjectivity in this judgement). The psychiatric service does not appear to compete with private practice care: 44% of the patients are referred to general practitioners and practing psychiatrists for after care, while only half as many are referred from private practices.

[Relevance of routine diagnosis of Chlamydia and HIV infections in Vienna prostitutes].

At the Control Center for Veneral Diseases of the Vienna Health Office, 5000 examinations for Chlamydia infection were performed on registered and non-registered prostitutes between 1980 and 1988. During this period, we observed a continuous decrease of the rate of infections from 20.4% to 4% among registered prostitutes and from 31.4% to 12.4% among non-registered prostitutes. Between 1985 and 1988, the cumulative number of HIV infections only rose from 7 to 8 cases among registered prostitutes and from 2 to 3 cases among non-registered prostitutes. All these HIV carriers were associated with i.v. drug abuse. The correlations between these two infections as well as the implications regarding health politics are discussed.

On characterizing new psychiatric long-stay patients.

In order to characterize new long-stay patients at the Psychiatric Clinic of Berne University, a comparison of all new long-stay patients of a 12-months period (n = 44) with a random sample of hospital admissions (n = 58) was carried out. Demographic data, circumstances of admission and illness data were recorded. Psychiatric symptoms were documented by means of the Brief Psychiatric Rating Scale (BPRS), intelligence from the Progressive Matrices Test (PMT) and inadequate behaviour based upon a modified "Check List of Nonfunctional Behaviors". As expected, findings show that new long-stay patients differ from the control group not only in diagnoses, symptoms and inadequate behaviour but also in physical illnesses, education, intelligence and social status.

Differentiation of T lymphocyte subpopulations, macrophages, and HLA-DR-restricted cells of apical granulation tissue.

Sixteen specimens excised from apical granulation tissue were examined for immunocompetent T lymphocytes, mononuclear phagocytes, and HLA-DR-determined cells. Light microscopic visualization of the cells was performed using monoclonal antibodies and the immunocytochemical alkaline phosphatase anti-alkaline phosphatase technique. Quantitative assessment of the biopsy specimens showed that in five diagnosed cysts, eight granulomas, and three specimens taken from scar tissue, macrophages represented the dominating inflammatory cell fraction, followed by T lymphocytes. In the latter group, helper/inducer T cells were most frequent. Cysts showed a statistically significant increase in cell populations of helper/inducer, suppressor/cytotoxic T cells, macrophages, and HLA-DR-coded cells. There were only a few activated T lymphocytes.

Hypotensive properties of antibodies directed against an endogenous pressor peptide isolated from rat blood.

Hypertensive factor (HF), a compound isolated from the erythrocytes of rats and tentatively identified as a peptide, has been shown to influence tissue calcium metabolism and induce prolonged blood pressure elevation. In the present study, we investigated the biological properties of antibodies directed against this peptide. Partially purified antibody preparations significantly decreased HF stimulation of lanthanum-resistant calcium uptake in rat aortic tissue in vitro. Infusion of the antibody preparation into spontaneously hypertensive (SH) or normotensive Sprague-Dawley rats resulted in a rapid decline in mean blood pressure of 54 and 34 Torr (1 Torr = 133.332 Pa), respectively. In contrast, infusion of the serum immunoglobulin preparations from controls (unimmunized and ovalbumin-immunized rabbits) had no significant effect on the blood pressure of SH or normotensive rats. The systolic blood pressure of SH rats was reduced for at least 72 h following a single injection of the antibody preparations, whereas the blood pressure of normotensive rats had returned to normal levels within 24 h following antibody injection. The results indicate that the anti-HF antibody preparation antagonizes the stimulation of calcium uptake by the peptide and acutely lowers blood pressure in SH and normotensive rats.

[The current examination protocol for prostitutes at a venereal disease counseling clinic, Vienna Public Health Service].

Registered Prostitutes are seen weekly for medical examination in the STD-Clinic of the Public Health Office Vienna. 1987 no syphilis was seen in this special riskgroup. The incidence of both gonorrhoe and chlamydial infection was. Chlamydias were found 1.6 times more, as gonococcal infections. There was no evidence of HIV infection. Screening for cervical neoplasia was started in reduced. 1988 and yielded a tenfold incidence of abnormal findings, requiring conisation often than compared to preventive checkups amoung the general female population.

[Computerized tomography image of entero-enteric invagination].

A case demonstrating the computertomographic appearance of entero-enteric intussusception in an adult patient is presented. Unlike conventional barium studies CT is able to demonstrate completely the intussusception complex. Its characteristic CT-appearance allows a definite diagnosis.

[Computerized tomography in the diagnosis of allergic bronchopulmonary aspergillosis].

In four patients with immunologic and laboratory findings consistent with allergic bronchopulmonary aspergillosis a CT of the chest was performed. CT revealed central bronchiectasis in all patients. Other features of the condition revealed by CT were mucoid impaction, peripheral alveolar infiltrates, and segmental or subsegmental atelectasis. Owing to their central location the bronchiectatic bronchi always appear parallel to the image plane. This means they can generally be visualized by CT.

[Computer-assisted diagnosis of bone tumors and tumor-like skeletal diseases: critical evaluation of its clinical use].

Thirty-four patients with bone tumours and tumour-like abnormalities of the skeleton, of varying ages, were examined by a computer-aided diagnostic program; the accuracy, clinical usefulness and specific advantages and disadvantages of the program have been evaluated. This was done by two statistical methods, both with showed high accuracy and reliability of the system (97% and 88.2%). In addition to the diagnostic results, the growth rate of the lesion could be estimated. This indicates the biological behaviour of the tumour independently of the histological diagnosis.

[Dynamics and pathogenesis of traumatic subdural hygroma].

Of 2005 patients, who had suffered blunt skull trauma, 108 developed post-traumatic subdural hygromas (TSH). CT was used to observe the course of subdural hygromas not treated surgically, with particular reference to the time of their appearance and regression. The relative incidence of TSH, and of the severity of head trauma quantified on a 5-grade scale was determined in different age groups. From the traumatic changes demonstrated on CT, some conclusions could be drawn regarding the pathogenesis. A hypothetical model has been developed which explains TSH as a result of shearing forces.

Density and localization of lymphocytes with natural-killer (NK) cell activity in periodontal biopsy specimens from patients with severe periodontitis.

Natural killer lymphocytes (NKL) in gingival biopsy specimens of 13 patients with advanced periodontitis and 6 normal control subjects have been evaluated by the monoclonal antibody NC 1, anti-B4 and a double immunoenzymatic labeling technique, combining the peroxidase-anti-peroxidase (PAP) method with the alkaline-phosphatase-anti-alkaline-phosphatase (APAAP) technique. Mononuclear cells with NK-activity (NKA) were mainly accumulated in the upper to middle third of the periodontal pocket. The concentration of NC-1-positive cells (CNC) of the baseline biopsy specimens was significantly increased at p = 0.05 compared to the CNC after hygiene training. The statistical difference rose to p = 0.01 following scaling, root planing and curettage therapy. The healthy gingiva of control subjects showed an average of 1-2 NKL, and no statistical difference could be found when compared to the CNC of the biopsy specimens of patients after periodontal therapy. These data support the hypothesis that antibody-dependent cytotoxic immunoreactive lymphocytes in chronic inflammatory periodontal disease can be reduced by scaling and root planing therapy.

[Pulmonary involvement in tuberous sclerosis].

The authors present a case of tuberous sclerosis with marked pulmonary involvement, confirmed by both radiological and pathological studies. The radiological manifestations and basic pathology of this rare condition are reviewed with emphasis on differential diagnosis.
