[Multimodal evoked potentials and heart rate variability in comatose patients. 2. Visual evoked potentials and computed tomographic findings].

Visual evoked potentials (VEPs) were examined as an integrated parameter in a multimodal EP study of brain bioelectrical activity in 33 comatose patients (Glasgow Coma Score 3-6) after craniocerebral trauma. Recordings were performed not only from the occipital (01-02) but also from the central (Cz-C3, Cz-A1, Cz-C4) and frontal (F3-F4) areas. The results show an attenuation of the vertex VEP (SNR 3.2 +/- 1.7) and an increase in the occipital VEP (SNR 4.8 +/- 2.3) as compared to the control group (SNR vertex 7.0 +/- 3.0/SNR occipital 3.9 +/- 2.0). In addition, 16 of the comatose patients showed a very considerable loss of the normally polyphasic character of the occipital VEP in favour of a potential that looked almost monophasic; this was either left or right dominant, and was recorded as a negative or positive potential, respectively, in bipolar recording (01-02). Comparison of the electrophysiological and computer tomography findings showed a laterally correlated change in the occipital VEP: contralateral to the injured hemisphere, the VEP was dominant, ipsilateral it was reduced or extinguished.

Special radiologic imaging of paranasal sinuses. A prerequisite for functional endoscopic sinus surgery.

Endoscopic diagnosis and surgery of inflammatory diseases of the lateral nasal wall and the paranasal sinuses require a very detailed preoperative knowledge of the individual anatomical conditions and pathological changes. Pluridirectional tomography and computed tomography are used best to visualize this region. The complicated radiologic anatomy of the lateral nasal wall together with the most important anatomical variations are demonstrated and the underlying pathophysiologic principles of chronic recurrent sinusitis are discussed.

[The computed tomographic spectrum of pulmonary aspergillosis].

The authors describe the CT findings in 10 cases of pulmonary aspergillosis which include allergic bronchopulmonary aspergillosis, aspergilloma and invasive pulmonary aspergillosis. CT reveals a more distinct radiologic-pathologic correlation than plain films and shows suggestive features of pulmonary aspergillosis earlier and in a more detailed fashion. Thus the use of CT in the appropriate clinical setting should allow earlier diagnosis than conventional radiography.

A study of clinical suicide.

A retrospective analysis of the clinical records of 149 patients suicide subjects and the same number of control subjects representative of the entire admitted clinical population was carried out using a larger set of demographic, psychosocial, and clinical variables. The results of the univariate analysis of the data indicated a serious and incapacitating course of the mental illness in the suicide group. The multivariate stepwise logistic regression analysis yielded a set of five variables including sex, Research Diagnostic Criteria diagnosis, previous and recent suicidal behavior, and social exits that discriminated best between both groups. These findings have to be taken into consideration when estimating the suicidal risk. The assessment of the suicidal danger, however, remains an individualized procedure.

[Functional assessment, using micturition cystourethrography, of the unstable bladder in childhood].

The accuracy and reliability of a modified method of micturating urethrocystography for the diagnosis of uncontrolled detrusor contraction in unstable bladders (sphincter-detrusor dyssynergy during bladder filling) was studied in 100 patients aged between four and 15 years. A comparison with a urodynamic method showed an accuracy of 92%, sensitivity of 93% and specificity of 90.7%. The technique and indications are described and the clinical relevance of the results is discussed.

Netilmicin: new approach to treating gonorrhoea.

Resistance and side effects to antibiotics or other chemotherapeutic agents are the main limiting factors in the treatment of gonorrhoea. As no such side effects have been reported with a new aminoglycoside, netilmicin, the therapeutic efficiency of this substance was evaluated in 1200 patients (690 women, 510 men) with uncomplicated gonorrhoea, who each received a single dose of 300 mg netilmicin by intramuscular injection. Efficacy of treatment was assessed by smear microscopy and culture, which gave results in all 1200 patients. No serious side effects were reported. These results indicate that netilmicin is an extraordinarily safe and efficient agent for treating uncomplicated gonorrhoea.

[Surgical treatment of chronic recurrent sinusitis--the Caldwell-Luc versus a functional endoscopic technic].

More than 50 patients who underwent intranasal or radical (Caldwell-Luc) antrostomy for chronic recurring sinusitis and who had still the same symptoms as before the operation, were examined by endoscopy and polytomography/CT. More than 50% still had polypoid changes of the maxillary sinus mucosa. No correlation was found between the degree of the patient's symptoms and the degree of the maxillary sinus pathology or the patency of the naso-antral window. CT demonstrated anterior ethmoid disease in all of the patients, correlating well with the endoscopic findings and the patient's symptoms. This again proves that the anterior ethmoid holds the key position for re-infection or cure of the larger dependent sinuses. Functional endoscopic sinus surgery therefore aims at the primary infective foci in the anterior ethmoid and usually cures disease in the larger sinuses without an attack upon the latter sinuses.

[HTLV-III monitoring in prostitutes in Vienna].

Since the end of May 1985 registered Viennese prostitutes are regularly screened for HTLV-III/LAV antibodies. Up to the end of January 1986 839 prostitutes had been investigated and 7 of them (0.8%) revealed positive test results. 5 persons are intravenous drug addicts, whilst 2 only admitted connection to drug addicts. Furthermore, HTLV-III antibodies were detected in 1 non-registered prostitute and in 5 men (3 homosexuals, 2 drug addicts). 11 out of these 13 seropositive patients have confirmed lymphadenopathy syndrome, whilst latent infection has to be assumed in the remaining 2 cases.

Retrocaval ureter--a rare anomaly.

Retrocaval location of the ureter is a rare ontogenic vessel anomaly. In the present paper the authors report on four cases, discussing their diagnostic, clinical and therapeutic aspects.

Blastogenetic responsiveness of peripheral blood lymphocytes from patients with adult periodontitis, evaluated by a reverse hemolytic plaque assay.

Peripheral blood lymphocytes were obtained from 10 patients with advanced periodontitis and from 5 normal control subjects. The lymphocytes were cultured with dental plaque, supernatant of homologous fibroblast cytoplasm and Pokeweed-mitogen (PWM). The number of immunoglobulin-secreting cells (ISC) was assessed after 7 days using a reverse hemolytic plaque assay (RHPA). The stimulation levels of lymphocytes from periodontitis patients and control subjects stimulated with dental plaque and PWM were not statistically different (p greater than 0.05). In contrast, the number of ISC produced after stimulation with fibroblast cytoplasm was increased significantly in the patients group (p less than 0.05). These data support the hypothesis that autologous substances play an important rôle in the cellular immune responses of patients with severe chronic inflammatory periodontal disease (CIPD).

Unstable bladder in children: functional evaluation by modified voiding cystourethrography.

A slightly modified technique for voiding cystourethrography was applied in 100 patients between 4 and 15 years of age and used to study uninhibited detrusor contractions in the so-called unstable bladder syndrome. Uninhibited detrusor contractions result in a slowing or cessation of the flow of contrast material into the bladder and a simultaneous minimal or marked opening of the bladder neck, with variable filling of the posterior urethra. Comparison of the radiologic findings with the results of urodynamic studies demonstrated an overall accuracy of 92%, sensitivity of 93%, and specificity of 90.7% for the radiologic technique.

Sonographic demonstration of lumbar disc herniation.

In a limited series of examinations the capability of ultrasonography in the assessment of intervertebral disc herniation by a transabdominal approach is evaluated. Normal anatomical findings on transabdominal spinal sonography are demonstrated. Two cases of disc herniation proved either by operation or lumbar myelography and spinal computed tomography are reported. The potential capabilities and known limitations of ultrasonography in this field are discussed.
