OAK knee evaluation. A new way to assess knee ligament injuries.

The Orthopädische Arbeitsgruppe Knie (OAK) documentation is based on clinical evaluation during functional knee stability testing near extension and near flexion. Increased compartmental translations and rotations, which result from structural defects because of anatomic lesions in a given knee injury, are clinically evaluated. The synopsis of the clinically detectable abnormal knee motion is graphically documented. A clinically applicable grading system of the true and reversed pivot shift phenomena completes the assessment of compartmental knee instability. Important contributing factors such as the constitutional laxity, the morphotype, and the range of motion are registered on the documentation form. The evaluation form presents four categories that represent subjective, objective, and functional criteria reflecting the overall results of repaired ligaments. This evaluation format establishes selected criteria to compare results from different centers.

Application of liquid-liquid partition chromatography in the simultaneous purification of sex-hormone-binding globulin and corticosteroid-binding globulin.

Two human serum proteins, corticosteroid-binding globulin (CBG) and sex-hormone-binding globulin (SHBG), were purified to homogeneity by the application of a combination of three different modes of chromatography. Human pregnancy serum was fractionated with ammonium sulphate. SHBG (50% pellet) and CBG (80% pellet) were then purified by affinity chromatography on tresyl-activated Sepharose with 15-aminopentadecanoic acid (for SHBG) and 1,12-diaminododecane (for CBG) as spacers and 17 zeta-aminoethyl-5 alpha-androstan-3 beta,17-diol (for SHBG) and 17 alpha-hydroxy-4-androsten-3-one-17 beta-carboxylic acid (for CBG) as specific ligands for these two proteins. The eluate was injected into a Mono Q anion-exchange column. Fractions containing SHBG or CBG were finally purified by liquid-liquid chromatography on Lipar-Gel 750. This chromatographic sequence clearly separated SHBG and CBG from other proteins, mainly serum albumin, without a loss of protein or binding activity.

[The quality of primary care of rheumatic diseases in childhood].

Primary treatment of 123 children with joint conditions up to referral to the rheumatological outpatient department of an university hospital was investigated. Only 81 (66%) of the children were referred with a diagnosis. This could in turn be confirmed only in 24 children (20%). The most frequent false diagnoses were "infection arthritis" (18%) and "rheumatic fever" (7%). Inadequate therapies were likewise frequent during primary care: 30 children (24%) received plaster casts which in five children (17%) led to a contracture which was difficult to influence. Only 21 children (17%) received physiotherapy. Twenty-one children (17%) were treated with corticoids, 16 children (13%) with pyrazolidine derivatives and 33 (27%) were treated with antibiotics. Further treatment in the clinic then revealed that corticoids were indicated in only five cases and pyrazolidine derivatives in no case. Acetylsalicylic acid was generally sufficient to eliminate symptoms. Our results indicate that in terms of the early correct diagnosis and commencement of adequate therapy which is important for the prognosis, primary treatment of children with rheumatic diseases is inadequate. Appropriate training and postgraduate education of the doctors concerned is necessary in order to improve the situation.

The inhibitory effect of dithiothreitol on the assembly of the filamentous phage fd.

Assembly of the filamentous phage fd is preceded by the formation of a complex between the viral single-stranded (ss) DNA and the virally coded gene 5 protein (gene 5 protein-ssDNA complex). The presence of 5 mM dithiothreitol in the growth medium prevents phage production; however, phage infection, phage DNA replication and phage genome expression are still observed. In contrast, the gene 5 protein-ssDNA complex is not formed in the presence of dithiothreitol in vivo, although the complex is not affected by the disulfide reducing agent in vitro. Furthermore, host lipid composition is altered by growth in the presence of dithiothreitol. The zwitterionic lipid, phosphatidylethanolamine, increases while the cationic phospholipid content, cardiolipin and phosphatidylglycerol, decreases. This suggests a role for lipids or membranous structures in the process of gene 5 protein-ssDNA complex formation.

A new V gene expressed in lambda-2 light chains of the mouse.

We have partially sequenced the light chain variable regions expressed in three IgM-producing hybridomas generated from newborn mice or from manipulated animals suppressed for IgM production. In these lines a new V gene (V-lambda-X), exhibiting less than 60% homology to any known lambda or kappa V gene, is rearranged to J-lambda-2. The light chains produced by these cells contain the lambda-2 constant domain, but are not recognized by goat antisera raised against conventional mouse lambda light chains.

The early receptor potential (ERP).

The ERP contains information on the function of the outer segments of the retinal photoreceptors. The authors have established normative values of the ERP and the factors which might be of influence hereupon. ERP findings in patients suffering from various eye diseases are given. Value and limitation of ERP recording are discussed in relation to the other clinically available electrodiagnostic tests.

Topochemical demonstration of zinc in meningiomas.

In agreement with the atomic absorption tests, histochemical demonstration of zinc in meningiomas using the dithizone method revealed a higher level of zinc in fibromatous meningiomas than in its meningiotheliomatous variant. Calcified psammoma bodies and vessels exhibited an intensely positive reaction on their surface. The metabolic function of zinc in the synthesis of fibre proteins, as well as in metallo-enzymes associated with mineralization, explains these findings.

Balance of T1-weighted images before and after application of a paramagnetic substance (Gd-DTPA).

Ten patients with encephalitis disseminata and 10 patients with cerebral ischemia underwent computed tomography and magnetic resonance imaging examinations. The results before and after the application of contrast media were compared. It is shown that T1-weighted MRI images, even after application of gadolinium-DTPA, are of little value for differential diagnosis of these diseases. In contrast, T2-weighted images did support the diagnosis of an acute stage of encephalitis disseminata versus acute ischemia. This was also true for lesions that had not been adequately depicted by computed tomography.

Contribution to the problem of giant cell astrocytomas.

Histological, immunocytochemical, and biological features of 38 giant cell gliomas were investigated. The invasion of these tumors and its giant cells by histiocytes, lymphocytes, plasma cells, and especially by eosinophilic granulocytes is viewed as an immune response, which may explain a favorable clinical course. Fifty-three percent of the patients were younger than 45 years at the time of surgery. The average postoperative survival of 27.4 months was clearly longer than in glioblastoma. These biological features suggest a differentiation from glioblastoma multiforme. The classification of this entity as "monstrocellular astrocytoma" is proposed.

Idiopathic and symptomatic plastic bronchitis in childhood. A report of three cases and review of the literature.

During a one-year period we could observe 3 children with the rare disease of plastic bronchitis. Two of them showed a symptomatic form, one with constrictive pericarditis, the other with bronchial asthma as primary diseases. No underlying disease could be found in the third case. Bronchial casts had to be extracted during endoscopy in all children. No relapse occurred after 2-3 years. Reviewing the literature, 37 cases of plastic bronchitis in childhood could be found, most of them had been reported before 1954.

Persistently circulating C3 nephritic factor (C3 NeF)-stabilized alternative pathway C3 convertase (C3 CoF) in serum of an 11-year-old girl with meningococcal septicemia--simultaneous occurrence with free C3 NeF.

Hemolytic complement was found to be absent in the serum of an 11-yr-old girl (R.N.) with meningococcal septicemia. C1, C4, and C2 were slightly decreased, C3 was absent, C5-C9 within the normal range. B levels immunochemically and electrophoretic mobility of B were normal. C3d was greater than 1000% of a pooled EDTA-plasma standard indicating hypercatabolism of C3. On incubation of the patient's serum with normal human serum activation of C3 occurred even in the presence of 0.04 M EDTA. The amount of C3b generated was, however, greater without any chelating agent or in Mg-EGTA. On gel filtration of the serum two protein containing peaks were found to be responsible for activation of C3: the IgG containing peak was able to activate C3 in normal human serum without chelating agents and in Mg-EGTA but not in the presence of EDTA. The IgM-containing peak activated the third component of complement even in the presence of EDTA. The factor responsible for this phenomenon was termed C3 converting factor (C3 CoF). The IgG fraction of the patients serum caused activation of C3 in Mg-EGTA. However, in the presence of EDTA no activation of C3 could be induced even if physiological concentrations of the patients IgG were added to normal human EDTA-plasma. Thus the activity of the patient's IgG did not differ from typical C3 nephritic factor. The decay of C2 in EAC42 intermediates in the presence of the patient's IgG was uninfluenced indicating that it did not carry autoantibody activity against the classical pathway convertase C4b,2a, an activity recently termed NFc.(ABSTRACT TRUNCATED AT 250 WORDS)

The fibrositis syndrome: diagnosis, differential diagnosis and pathogenesis.

The fibrositis syndrome represents a clinically definable entity, which is characterised by spontaneous pain, especially in the lumbar and cervical region, and more rarely in other movable parts, multiple tendomyosis, tendinitis and insertion tendinitis, as well as a wide variety of functional syndromes, vegetative symptoms and psychological disturbances. The course of the illness is rather variable; its commencement can be slow to develop or acute, initially presenting a localised clinical picture similar to a lumbar or cervical condition, and gradually, or in jumps, showing a deterioration accompanied by a generalisation of complaints. Various factors are probably instrumental in triggering off the fibrositis syndrome. The most important ones are emotional stress whereby symptoms of fear, depression, etc. lead to muscle tension and insertion tendinitis. Somatic factors such as malposition of the spinal column, may also contribute towards the manifestation of the clinical picture. The so-called secondary fibrositis syndromes should be defined from the point of view of differential diagnosis; they can develop within the framework of inflammatory rheumatic conditions, through infections and endocrinopathy. Differential diagnosis is very difficult considering depression alongside pain in the movable parts and "psychogenic rheumatism". Smooth transitions are in existence. Polymyalgia rheumatica and polymyositis, which produce similar clinical pictures, must be differentiated from the fibrositis syndrome.
