[Sublingual buprenorphine in the therapy of postoperative pain].

40 Patients undergoing elective orthopaedic surgery of the lower limb randomly received either Buprenorphin sublingual (0,4 mg.) (Bsl) or Pentazocin i.m. (30 mg.) (Pim) for postoperative pain therapy. They were neither premedicated nor sedated. Analgesia was measured with a Visual Analogue Scale (VAS) and assessments by the anaesthetist and patient. Physiological measures were: Blood pressure, heart rate, arterial blood gases and serum cortisol. Side effects were registered. After Pim well known data were obtained: onset (10-30 min.) and duration (2 hours) of action and a significant pain reduction 20, 30 and 60 minutes after injection, whereas with Bsl onset was 60-120 min. and duration 6-7 hours of action and significant pain reduction was seen 120 and 180 min. after administration. 8 dropouts after Bsl up to 120 min. post applications are interpreted as a result of late onset or lack of analgesic action whereas the 13 drop outs 3 hours after Pim can be explained with the short lasting analgesic action of Pentazocin. Serum cortisol levels were better after Bsl, but did not correlate with the VAS. In Blood gas analyses, there were no group differences, especially no hypoxic or hypercapnic periods. There were no severe cardiocirculatory side effects, but significantly more hypertonic reactions and tachycardia after Pim. Other side effects were rare in both groups. The methods are discussed. Bsl has late onset but long duration of action with a higher efficacy than Pim. Bsl is strong and long acting. After an initial intravenous injection Bsl (0,4 mg.) should be administered at 6-hourly intervals.

[Brain tumor removal with an ultrasound aspirator].

A report is given on the application of an ultrasound aspirator for the removal of cerebral tumours. After the description of physical and surgical fundamentals, a report is given on the respective patients. Then hazards an advantages are described as far as they can be summarized already today.

Brain Neoplasms↗

[Fundamental studies of ultrasonic surgery. 5. Ultrasonically forced polymerization of Ligament-Fimomed in joining biological tissue].

About basis investigations to the mode of action, the energy transformation, and to the polymerization heat of Ligament-Fimomed adhesive is informed over the ultrasonic osteosynthesis. A prompt activation of the monomeric tissue adhesive is attained at the contact region with the osseous tissue by means of low frequency power ultrasound. The mechanism of the connecting formation and characteristic temperature-time course are shown.

[Cross sectional echoencephalography in infants].

Importance of echoencephalography increased during development of neonatal intensive care. In addition to investigations in neonates 2-D-echoencephalography even during pregnancy and infancy can be used for detection of intracranial pathology. A standardized method of investigation and the knowledge of the pathology of the expected lesions are of fundamental importance. During neonatal period and infancy intraventricular and subdural hemorrhage, brain tumors, postinfection sequelae, hypoxic brain damage, alteration of intracranial perfusion and cerebral malformations can be detected.

Age Factors↗

[Tenacity of bacteria in the airborne state. III. Model studies on the epidemiology of Pasteurella multocida influenced by a tropical climate].

The viability of airborne P. multocida in a static aerosol chamber was determined at temperatures of 21, 24, 28, 34, 40 and 42 degrees C and relative humidities of 44, 70 and 87%. The greatest viability with 35.49 min half-life time was at 24 degrees C and 70% relative humidity. There was no separate influence of one of the climatic components temperature and relative humidity to the airborne viability of the germs. The lowest resistance of P. multocida in the airborne state was as they were solitary germs, but as agglomerates (1.5-3.5 micron) they survived also worse environmental conditions.

Air Microbiology↗

[Value of the indirect immunofluorescence test in syphilis and toxoplasma diagnosis].

The examination tests annually necessary for a serological examination for syphilis and toxoplasmosis are in the first place as to their quantity in the diagnostics of the infection. For the performance of this large number of tests the use of a method is necessary which corresponds to the necessities of rationalisation of the laboratory work in the same way as to the demands to quality, which is characterized by specificity, sensitivity and reproducibility. For these criteria in the framework of the syphilis serology the TPHA-test is in a higher degree sufficient (with the exception of the early syphilis) than the FTA-ABS-test. The small quantity of the imported test set which is at present at our disposal does not allow a general application of this test, so that the FTA-ABS-test is the most usual method. Moreover, by means of FTA-ABS-IgM and 19 S-IgM-FTA-ABS-test valuable diagnostic additional informations (early diagnosis of the infection, L. connata diagnostics, estimation of the success of therapy) can be got which neither the TPI nor the TPHA-test are able to give. For the diagnosis of toxoplasmosis the IFT shows at present a higher value of precision than ELISA. It is still to be clarified in how far in the higher sensitivity of toxoplasmosis ELISA unspecificities play a role.
