[Long-term drug therapy of bronchial asthma in childhood. Follow-up 1981-1983].

The effect of a long-term drug therapy in childhood asthma was investigated in all children seen in our outpatient clinic for the first time in 1981 and followed over the next two years. The drug schedule consisted of a step-wise increasing medication with regard to severity and form of bronchial asthma. Most children of grades II to IV improved during the study period. Children with rarely occurring episodes of asthma (grade I) received therapy only during asthmatic episodes and did not improve. The results of this study are discussed with respect to the natural course of the disease and immunotherapy (hyposensitization).

[Femoropatellar joint. Aspects of anatomy, physiology and pathophysiology].

The variety of anatomic conditions is critically confronted with the so-called dysplasias of the patella. Is chondromalacia or anterior knee pain a clinical diagnosis? The physiological aspects of the tension forces acting on the patella with its position in the midst of tendons and ligaments in valgus and in varus knees are discussed. What may the automatic rotation in extension effect on the patellofemoral joint-compartment?

[Carpal tunnel syndrome--a leading symptom in generalized tendomyopathy].

Among 75 of 100 patients with generalized tendomyopathy, carpal-tunnel syndrome developed in the course of the disease and was demonstrated both clinically and electromyographically, requiring operative intervention in 10%. Furthermore, 17 of 26 patients previously operated on for carpal-tunnel syndrome, had chronic tendomyopathy requiring treatment, and in six there was severe generalized tendomyopathy. Thus generalized tendomyopathy can be the precipitating underlying disease in a large number of cases with so-called idiopathic carpal-tunnel syndrome.

[Lyme arthritis (erythema migrans arthritis)].

In the light of an observation of a patient with Lyme arthritis living near the Swiss-German border, the authors describe "Lyme or erythema migrans disease". The clinical findings include erythema chronicum migrans, meningoradiculitis and arthritis, and, occasionally, carditis. Also, it has recently become possible to describe this entity serologically, by detection of antibodies against the etiologically responsible spirochetes, which are usually transmitted by tick bites. This diagnostic procedure can also be expected to provide a better overview of the incidence of Lyme arthritis in our region.

[Heberden arthroses in judo athletes].

Increased occurrence of Heberden nodes has been observed in judo practitioners. Nine out of 30 members of the Swiss national team showed such nodes clinically on several fingers, whereas sportsmen in other disciplines did not. X-ray showed that all judokas examined have more or less severe osteoarthrosis of the distal interphalangeal joints (DIP), whether Heberden nodes were present or not. At the same time, in most cases osteoarthrosis of the proximal interphalangeal joints (PIP) was found by X-ray and clinical examination. Osteoarthrosis of DIP and PIP in this young age group is due to overstress and injury to the joints involved. Acute injuries to the finger joints evidently cause Heberden nodes with, in some instances, only minor osteoarthrotic lesions visible by X-ray, while overstress of DIP and PIP is the cause of osteoarthrosis in these joints, which is only detectable by X-ray.

CT-analyses of cerebral tumors under steroid therapy.

In order to determine, by CT density and volume measurements, the influence of steroid therapy on cerebral tumors, on their perifocal edema and on the uninvolved cerebral tissue, CT follow-up studies of 37 patients were analysed. In general a decrease of tumor density is to be seen within the first 10 days of therapy. Under continuous steroid application absorption coefficients then increase again, so that no cortisone effect on the tumor remains. The tumor size does not alter: in particular at no time is growth retardation detectable under cortisone therapy. Intensity and extension of the perifocal edema decrease in two stages. From this delayed course we conclude the cytotoxic component of the tumor edema to be more extensive than supposed. Deviations appear dependent on tumor histology, which should lead to individualized steroid application.

In vivo allergen tachyphylaxis in guinea pig lung is mediated by endogenous prostaglandin E biosynthesis.

The term "allergen tachyphylaxis" (AT) describes a progressively decreased bronchial reactivity to allergen exposition after repeated allergen challenge [in our test system measured by sequential inhalative antigen challenge of sensitized guinea pigs (GP)]. The hypothesis that AT is mediated by endogenous prostaglandin E (PGE) biosynthesis was tested in vivo on GP sensitized to ovalbumin (OA). Different groups of animals were challenged with simultaneous inhalation of OA (repeatedly at time 0, 10, 20, 60 and 70 min) together with inhibitors of PGE-biosynthesis ( parachloromercurobenzoic acid = PCMB, copper sulfate = Cu, copper dithiotreitol complex = CuDTT , dithiotreitol = DTT and dimercaptopropanol = DMP) or agents increasing PGE production (aurothioglucose = Au, zinc dithiotreitol complex = ZnDTT and reduced glutathion = GSH). Bronchial obstruction was measured by whole body plethysmography . PCMB, Cu, CuDTT , DTT and DMP inhibited AT, whereas Au and ZnDTT enhanced AT. Acetylsalicylic acid (ASA) treatment prevented AT. Aerosols of PGE2, but not of prostacyclin or prostaglandin D2 restored AT in ASA treated animals. In addition to these in vivo experiments in vitro investigations showed that PCMB, Cu and DTT decreased while ZnDTT increased PGE biosynthesis of allergen challenged GP lungs. It is concluded that AT, an important self-protecting mechanism of GP bronchial asthma, is mediated at least in part via endogenous PGE.
