[Differential diagnosis of retinoblastoma].

An 8-year-old boy presented with a white pupillary reflex. This clinical picture suggested a diagnosis of retinoblastoma or Coats' disease. Ultrasonography did not reveal the characteristic signs often seen in retinoblastoma. However, diagnostic vitrectomy and subsequent enucleation confirmed the diagnosis 'retinoblastoma'.

Persistent atrioventricular block in Lyme borreliosis.

Cardiac manifestations are reported in 0.3%-4.0% of European patients with Borrelia burgdorferi (B.b.) infection. Usually symptoms disappear within 6 weeks. We report a case with persistent impairment of atrioventricular (AV) conduction. Diagnosis was confirmed by demonstration of IgM antibodies and increase of IgG antibody titers against B.b. in serum, by isolation of the spirochete from skin biopsy material and by the typical clinical combination of erythema migrans, Bannwarth syndrome (meningoradiculitis), and complete heart block. Despite immediate antibiotic therapy with ceftriaxone, first degree AV block and second degree block Wenckebach with atrial pacing at 100 beats/minute persisted for 2 years. We conclude, that Lyme carditis can cause long-standing or irreversible AV conduction defects despite adequate and early antimicrobial therapy.

Toxicity and clearance of intravitreal cefotetan.

Direct intravitreal injection of antibiotics plays an important role in the management of bacterial endophthalmitis. In the present study we investigated the toxicity and clearance of intravitreally injected cefotetan in a rabbit model. No toxic ocular side effects could be detected by electroretinography (ERG) or light and electron microscopy up to and including a single intravitreal dose of 1000 micrograms. Intravitreal injection of 2000 micrograms cefotetan resulted in mild degeneration of photoreceptor outer segments and, sporadically, in cataract formation. After intravitreal injection of 4000 micrograms, moderate toxic degeneration of photoreceptors occurred, with displacement and mitochondrial swelling of inner segments. In addition, lysosomal lamellar inclusion bodies could be detected in pigment epithelial cells. After a single intravitreal injection of 1000 micrograms cefotetan, concentrations greater than the minimum necessary for the inhibition of most commonly occurring intraocular pathogens (except Pseudomonas aeruginosa and Staphylococcus epidermidis) were maintained in the vitreous humor for greater than 48 h. Cefotetan may be a potentially important drug for intravitreal injection, especially in cases of gram-negative and suspected anaerobic endophthalmitis.

[Pharmaceutical characterization and spectroscopy of AWD 26-06].

To fix parameters for the pharmaceutical characterization of AWD 26-06 test methods converting identification, purity, content and stability as well as their results are submitted. Spectroscopic investigations are intended to give information on the constitution of the molecule.

Anti-Ulcer Agents

Investigation of visual field defects in strabismic and anisometropic amblyopes with the Octopus program G1.

Using the Octopus G1 program, we investigated the visual fields of amblyopic and healthy contralateral eyes in 21 patients with strabismic amblyopia and 14 patients with anisometropic amblyopia. All subjects had a visual acuity of 0.7-0.06. A central scotoma was detected in 85.7% of the patients with strabismic amblyopia and in 79% of those with anisometropic amblyopia. The mean maximal depth of central scotomas in subjects with strabismic amblyopia was 6.81 +/- 4.74 dB (mean +/- SD); in those with anisometropic amblyopia, it was 6.64 +/- 4.34 dB. This difference was statistically not significant. However, in patients with anisometropic amblyopia, the visual field indices MD (mean defect) and CLV (corrected loss variation) were significantly higher than in those with strabismic amblyopia (P less than 0.05 vs P less than 0.05); this was predominantly due to additional flat defects that occurred paracentrally and peripherally in subjects with anisometropic amblyopia. In both groups of patients, we found a significant negative correlation between visual acuity, on the one hand, and the maximal depth of the scotomas and the visual field indices (MD and CLV), on the other.

[Occupational fields of AIDS specialists in the public health office].

Preexisting supplies of a local public health office and other institutions are taken into account in developing main activities of an AIDS-counsellor within the AIDS-programme at public health offices in Western Germany. School education and individual AIDS counselling are predominant. Sexology at schools in Bavaria is restricted by the education privilege of parents and ethical principles supported by the catholic church. Further activities for domestic nursing personnel and medical doctors were introduced successfully. The controversy of epidemic hygiene predominating over psychological counselling is discussed. Efficacy of working at schools and future developing topics are discussed.

Acquired Immunodeficiency Syndrome

[HIV tests as preventive responsibility of public health offices].

4,382 blood samples were examined for HIV antibodies between August 1985 and the end of December 1988 at the Public Health office of the city of Augsburg. The prevalence of positive tests was 0.57% (n = 25). The rate at which various consultation offers were made use of was calculated for 1988. From 1985 to 1988 approximately 200 registered female prostitutes were regularly tested for HIV. It was only in 1985 that the HIV antibody test was positive in two women. Compared with 1984-1986 as basis there was a marked drop in the incidence of gonorrhoea to 71.7% and in 1988 to 6.5%.

AIDS Serodiagnosis

A first report of the Department of Defense external civilian peer review of medical care.

In mid-1985, the Department of Defense announced a plan for an external civilian peer review of selected medical and surgical cases in military hospitals. A medical advisory panel consisting of practicing clinical representatives from nine medical specialty societies helped develop written criteria and standards for specific surgical and medical cases. Abstracted data were compared with computer algorithms created from the medical standards and criteria. Those cases failing the review criteria were reviewed by a physician peer review panel. The sampled medical records represented approximately 7% to 8% of the total hospital discharges. Less than 1% of the overall sampled cases were in noncompliance with stated standards. The highest rate of noncompliance was for cases of diabetic ketoacidosis. The most frequent noncompliance event was hysterectomy. Although comparisons are not available, the quality of care among selected cases, as measured by compliance with civilian physician specialty group-endorsed standards, appeared high.

Algorithms

[The purification and characterization of Cordemcura].

3-Amino-5-(4-pyridinyl)-1,2-dihydro-pyrid-2-one (1) is an amphoteric compound and forms one crystalline sodium salt and two hydrochlorides. Physicochemical properties UV, NMR and MS are described. TLC has been used mainly and is the most sensitive method for estimation of 1-byproducts. Coloured byproducts, generated by hypochlorite or air oxidation during synthesis and handling in solution, are monitored by vis-spectra, diminished by sulfite addition and removed by alkaline precipitation. The purification procedure is able to produce 1 with only 0.1% of precursors or byproducts.

Aminopyridines

[Porencephaly as a cause of ophthalmologic symptoms].

Two cases of hypoplasia of the optic nerve associated with porencephaly and one case of porencephaly without ophthalmologic signs are described. The clinicopathological correlation of the double-ring sign is presented and the possible mechanism of optic nerve hypoplasia being a secondary degeneration is discussed.

Epithelialization of the anterior chamber as a complication of penetrating keratoplasty.

Only a few cases of anterior chamber epithelialization after penetrating keratoplasty have been reported, and in most of these the keratoplasty had been preceded by another event that could have been the cause of the process. In the patient whose case we are reporting here, epithelialization of the anterior chamber developed after penetrating keratoplasty.
