A comparison of prednisolone with azathioprine and prednisolone with intramuscular gold in rheumatoid arthritis.

Pulse prednisolone hemisuccinate therapy (500 mg given intravenously on three occasions over two weeks) has been combined with either intramuscular sodium aurothiomalate or azathioprine in an assessment of 30 patients with rheumatoid arthritis. Significant improvement in a variety of clinical and biochemical assessments was seen in both groups. Both treatments were well tolerated by the patients and prednisolone appeared to accelerate the response to sodium aurothiomalate and azathioprine but there was no great evidence that it enhanced it.

[Thermologic studies in inflammatory rheumatic diseases--general presentation and personal results in rheumatoid arthritis].

The registration of the temperature of the body surface gains an increasing significance among others for the diagnosis of inflammatory rheumatic diseases. A criterion of the inflammation of the joint, the hyperthermy as well as the disturbances of the microcirculation may be made evident by means of the thermological methods. Own results in healthy persons and patients with rheumatoid arthritis as well as evidence of the literature confirm the possibilities of the measurements of temperature for the rheumatology. On account of the great inter- and intra-individual variability of the skin temperature an orientation at the temperatures of normal values of certain measuring regions is not reasonable. When the results of the temperature measurement are estimated, therefore, a comparison with temperature patterns of healthy persons must be done. Using dynamic measuring methods the evidence of thermological investigations may essentially be enlarged. The evaluation of the results of the measurement is increasingly performed with the help of computers.

[Diagnostic and prognostic significance of so-called rheumatoid vasculitis--2].

51 patients with rheumatoid arthritis and high rheumatoid factors (mean titres 928) underwent examination for the demonstration of an extraarticular organ manifestation within the scope of the cooperation between the Department of Medicine of the Karl-Marx-University Leipzig and the Institute for Rheumatology of the Academy of Medical Sciences of the USSR in Moscow. The frequency of nodous rheumatism (about 60%) is comparable with the frequency of polyneuropathy. In 20% of the patients a systemic muscle atrophy, a hepatomegaly as well as a Raynaud-syndrome were stated. By means of skin biopsy in 28% perivascular infiltrates were found. Altogether in 6 patients (12%) a participation of the lungs and the pleura, respectively, could be proved. Only rarely a clinically manifest heart disease caused by the rheu-we we found an pericardial effusions in 3 cases. In systemic manifestation, such as myositis, participation of the eyes or vasculitis of the digital arteries with necrosis, were only sporadically to be established. Among 22 patients we found an pericardial effusions in 3 cases. In systemic manifestation in most cases increased parameters of activity were found. From the practical point of view apart from increased titers of the rheumatoid titres and circulating immune complexes (C1q-BT) increased concentrations of the C-reactive protein are prognostically significant. The presence of high rheumatoid factor titres alone as well as the isolated presence of rheumatic nodes must not always be connected with an unfavourable prognosis. When severe extraarticular manifestations are present a possibly early, intensive occasionally extracorporeal treatment is indicated.

[Effectiveness of plasma exchange therapy in patients with rheumatoid arthritis and systemic manifestations].

For the advantage of the therapeutic plasma exchange in the active rheumatoid arthritis there exist at present contradictory opinions. Three patients with rheumatoid arthritis and systemic manifestation as well as high activity of the disease, in whom the conventional therapy with antirheumatic drugs showed only little effectiveness and which was stopped due to severe side effects, respectively, underwent 6 plasma exchange treatments within 14 days. The subjective condition as well as standardized clinical and laboratory-chemical activity parameters quickly ameliorated. A positive therapeutic effect was to be proved still after 6 to 12 months. In 2 patients a proposed synovectomy could be renounced. The reasonable indication for the plasma exchange will be given when there is an acute immunological reaction which can be shortened by a rapid reduction of pathogenic protein fractions before irreversible morphological injuries occur.

[Sex ratio in the spectrum of chronobiopathology].

With regard to the deficiency of representative data on the different course of the ischaemic heart disease depending upon sex a total statistics (death data) of the GDR for 5 years was elaborated and compared with the total mortality since 1903 to 1958 of the area of the former German Reich, the GDR and the FRG with still-born and deceased babies. Here conspicuous synchronous courses depending upon the season were the result. The female sex quite generally predominates the well-known course of the season of all individual mortality data, whereas the males are somewhat more endangered during the summer months. The differences established between the sexes proved in most cases as significant, in larger groups as highly significant. Moreover, the statistical reference guarantees an up to now not proven representativeness . The result of the distribution characteristics within the 10 categories examined renders possible cautious conclusions concerning the causal connection of the process.

[Diagnostic and prognostic significance of so-called rheumatoid vasculitis].

23 patients with rheumatoid arthritis and high rheumatic factors (mean titres of rheumatic factors 1 : 1 434) within a prospective study on the diagnostic and prognostic significance of the rheumatoid vasculitis which was performed together with the Research Institute for Rheumatism Moscow underwent examinations for the proof of an extraarticular organ manifestation. In correspondence with the data in literature we found rheumatic nodes (70%) as most frequent extraarticular organ manifestation. In 1 patient each a participation of the lungs and a rheumatic exudate of the pleura, respectively, could be diagnosed. In another patient there were references to a vasculitis at the fundus of the eye. In only 2 patients there was a vasculitis of the digital arteries with necroses, in one of them a generalized vasculitis. 5 of the 13 neurologically examined patients showed polyneuropathic symptoms. Of the biochemical findings the C-reactive protein and the blood sedimentation rate showed clear increases and correlations.

Neuraminidase deficiency presenting as non-immune hydrops fetalis.

A newborn infant with oedema, ascites and hepatosplenomegaly is described. In ascites fluid foamy macrophages were found, in a liver biopsy cytoplasmic inclusions and membrane-bound vacuoles were seen. Furthermore the child excreted excessive amounts of sialic acid-rich oligosaccharides in the urine, and therefore a neurovisceral degenerative disorder was assumed. The diagnosis of sialidosis was confirmed by enzymatic assay in cultured fibroblasts, in which a complete deficiency of the lysosomal enzyme neuraminidase could be demonstrated. After recurrent septicaemias the child became dystrophic and died at the age of 6 months. Our case is compared with sialidosis observed by other authors, the wide phenotypic diversity within this biochemical defect is emphasised. The occurrence of hydrops fetalis in lysosomal storage diseases is discussed.

[Significance of intermittent slow waves with right posterior accentuation in the EEG's of psychiatric patients].

The study was based on the frequent occurrence of intermittent slow waves right-posterior accentuation (IRP) in the EEGs of psychiatric patients. With regard to the EEG-phenomenon we present a detailed morphological and functional description as well as an evaluation from a developmental point of view. According to case histories a clinico-psychopathological characterization of the patients with IRP is given. The IRP-phenomenon can be interpreted electrogenetically against the background of and in connection with the so-called slow alpha variant rhythms as well as the posterior slow waves characteristic of children and adolescents. These patterns have in common a certain tendency to right-sided accentuation. In accord with a hypothesis (which has been derived from other observations and considerations) of a "maturation gradient" which favours the left hemisphere, we try to explain the IRP-phenomenon as an expression of a maturation deficit. Whereas the slow alpha variant rhythms and the posterior slow waves characteristic of children and adolescents appear bilaterally for the most part, IRP by definition, limited to the right hemisphere, may be considered as a less pronounced form in comparison. Deriving from clinico-psychopathological assessment the relationships are as follows: Patients with IRP account for about 5% of the in-patients in our psychiatric hospital. The IRP phenomenon seems to be closely linked to the male sex. Although a clear relationship with nosological categories (ICD) could not be proved, it seems that patients suffering from schizophrenic psychoses (ICD No. 295) are more frequently represented among the patients with the IRP-phenomenon than others. For the group of schizophrenic patients with IRP we found in contrast to a control group of schizophrenics without IRP a tendency to earlier onset of their disease. Compared with the control group it is found that the IRP groups consists of younger patients at the time of conducting this study. The two comparative groups of schizophrenics with and without IRP can be characterized by differences in the more or less pronounced manifestation of a fairly large number of AMDP items. Whereas for the IRP group a symptom pattern emerges with a hypermotoric -expansive accentuation, the control group seems to tend more towards a syndrome with depressive shading. The slight tendencies towards increased hereditary disposition as well as towards increased perinatal complications among the schizophrenic IRP patients necessitate further studies.(ABSTRACT TRUNCATED AT 400 WORDS)

[Surgical stabilization of fractures of the thoracic and lumbar portion of the spine using paraspinal metal plates].

Surgical treatment of lesions of the vertebral column is indicated for concomitant disturbed neurological functions showing an increasing tendency. In cases of a primary complete transverse paralysis, an improvement cannot be expected. The influence of the applied paraspinous plates on the rehabilitation is judged differently. The operation is carried out from a dorsal access. After laminectomy and decompression of the spinal marrow or cauda, stabilisation is effected by means of two paraspinous osteosynthesis plates similar to the method after Roy- Camille . Our own procedure is described, the results are demonstrated by the example of seven patients treated in this way. The authors are of the opinion that it is justified to continue employing this method and extend the range of indications to include also severe instable luxation fractures of the thoracic and the lumbar part of the vertebral column without disturbed neurological functions being present . The care of such patients is facilitated by the fact that pain is improved and degenerative changes and late damage prevented.

[Various periodical and aperiodical variations of heart infarct mortality in the DRG].

On the basis of 66.900 cases of death from ischaemic heart disease from 1970 to 1974, patient aged 70 years and younger, total statistics, an average quotient for the house/clinic-relation was calculated. The variations of the house/clinic-relation observed are subdivided into 3 stable periods: 24-hour periodicity, season periodicity and weekday periodicity (socially conditioned). These periods are superimposed by aperiodical influences. The relations between increased house/clinic-relation of the cases of death from ischaemic heart disease and the increased influences of the geomagnetic field as well as geomagnetic tempests (SSC). Here a significant positive connection for the indicators of activity as well as for the SSC was the result. One can issue from the fact that, the higher the ratio is the more unfavourable conditions for a specialised therapy of the complications of the ischaemic heart disease are existing. The cases in question in this phenomenon are new chronobiological knowledges which have a direct relation to the medical practice. Instances are given for the practical use in the prevention of complications of the ischaemic heart disease.
