[Results of different long term treatments of patients in the early stages of rheumatoid arthritis].

In a clinical 5-year-study on 200 patients with rheumatoid arthritis with the help of the dynamics of radiological changes (radiological progressing), the joint deformation, the reduction of function and the new affection of joints (clinical progressing) as well as the medium position of activity the progressing of the disease and a good, satisfying or missing success of the long-term therapy were estimated. From the characteristics of the individual groups of therapy conclusions for the therapy in the early phase of the rheumatoid arthritis were derived.

[Early diagnosis of rheumatoid arthritis].

On the basis of own observations of courses the author adopts a definite attitude to the early symptomatology of the rheumatoid arthritis. During the first weeks of the rheumatoid arthritis the following symptoms are found: articular syndromes, more frequently in form of obstinate polyarthralgias, mono-oligoarthritis, accompanied by morning rigidity and accelerated BSR as well as impairment of the general condition. In the majority of the patients only the tentative diagnosis rheumatoid arthritis may be made. After a one to three months' course of the disease the diagnosis becomes more probable. It is above all based on constancy and symmetry, characteristic localisation of the articular process, morning rigidity, radiologically paraarticular loosening of the structure and morphological symptoms of an acute and subacute synovialitis. 6 to 12 months after the beginning of the disease a clinical picture forms which allows to make the diagnosis of a certain or classical rheumatoid arthritis in accordance with the criteria of the ARA. The occurrence of a high activity of multiple affection of the joints (permanent symmetrical polyarthritis including the small joints of the hands and feet), distinctive morning rigidity, high fever and much accelerated BSR, beginning with the first weeks of the disease, speaks for the possibility of the development of an arthrovisceral form of the course of rheumatoid arthritis.

[Early diagnosis of spondylarthritis ankylopoietica].

For the early recognition of the ankylopoietic spondylarthritis subjective and objective criteria are explained and the importance of X-ray-morphological, cytological, biochemical, immunogenetic and coagulation-physiological investigations are explained. By accentuation of two types of constellation of the course the possibility of an early diagnosis is given still before the appearance of severe irreparable X-ray-morphological changes in about half the cases of disease. Further observations and controls of courses are necessary for the confirmation of evidence.

[Protein-analytical studies on the serum and urine of patients with visceral lupus erythematosus].

Serum and urine of patients with ascertained lupus erythematodes visceralis were separated by means of the polyacrylamide gel pore gradient electrophoresis on the same experimental conditions. Deviations from the normal electropherogram were found in the serum within the gamma-globulins. For the demonstration of antinuclear factors the loose body test, the LE-cell test as well as the immunofluorescence test were carried out. In the uropherogram a highly molecular protein band could be proved which did not appear in the serum. In this fraction an immune complex which developed in the area of the kidney might be in question. It is referred to the possibility of the use of the PAA-polyacrylamide gel electrophoresis for the early recognition of renal changes.

[Differential-diagnostic and prognostic significance of antinuclear factors].

On the patients of the consulting point for rheumatic diseases of the policlinical institute of the Karl-Marx-University Leipzig analytic examinations of the course for the existence of the LE-cell factor were carried out. We used the loose-body-test after van Soeren as screening test, controlled positive test results for several times under the same experimental conditions and supplemented it by the LE-cell test after Zinkham and Conley or later on by the immune fluorescence test. All patients with positive proof of LE-cells were examined for reference signs concerning a visceral lupus erythematodes, in which cases at the beginning of the examination nobody fulfilled the criteria of the diagnosis of a visceral lupus erythematodes. We tested the constancy of the proof of the LE-cells as well as the diagnosis in the course of longer periods. Typical changes of a visceral lupus erythematodes were seen only rarely. In 2 patients the joint processes were concomitant symptoms of a chronic aggressive hepatitis. In the p.c.p. at stage II to IV with positive LE-cell factors in the first place must be thought of a proof of LE-cell factors induced by drugs. In these cases gold is of practical importance. We could confirm that in contrast to the typical active visceral lupus erythematodes in p.c.p. the antinuclear factors have only a weakly positive result and are above all inconstant.

Glycerol release in vitro from adipose tissue of obese (ob/ob) mice treated with thyroid hormones.

Glycerol release from adipose tissue of adult genetically obese (ob/ob) mice was greater than that from adipose tissue of lean litter-mates. However, a higher concentration of adrenaline was required to stimulate lipolysis in ob/ob adipose tissue. Since oxygen consumption and the uptake of radioactive iodine by the thyroid were reduced and the biological half-life of thyroidal radio-iodine was lengthened, it is suggested that ob/ob mice are hypothyroid. The poor sensitivity of ob/ob adipose tissue to low doses of adrenaline was improved by treatment with either tri-iodothyronine or thyroxine. The lipolytic responses to both dibutyryl 3':5'-cyclic adenosine monophosphate and aminophylline were enhanced in thyroid hormone-treated lean and ob/ob mice. A higher dose of tri-iodothyronine was required to enhance lipolysis in ob/ob mice than in lean controls. The possibility that hypothyroidism is responsible for the change in response of ob/ob adipose tissue to lipolytic hormones is discussed.

[Practical experiences in the intra-articular treatment of chronic rheumatic diseases].

Local instillations of various medicaments with the aim of a so-called chemical synovectomy are compared with the experiment of a possible causal therapy by operative synovectomy. Under this aspect corticosteroids are used still relatively frequently also for the intraarticular treatment. They diminish transiently the inflammation of the pannus, but they do not remove it. On the basis of its good local compatibility in relapsing effusions of the large articulations, mostly of the knee-joints, which appeared in the course of a chronic progressive rheumatoid arthritis of Bechterew's disease, cyclophosphamide was applied intraarticularly and was compared with the results after injections of prednisolone. Apart from the clinical findings cytological and biochemical examinations of the synovial membrane, which more exactly than clinical parameters allow an objective estimation of the local inflammatory activity, were used for the judgement of the success. With the help of intraarticular treatment of cyclophosphamide within a complex chemotherapy an improvement of the findings and a stabilisation of the process can be achieved. Local applications of prednisolone often lead to a rapid, however, in most cases not remaining reduction of the activity at the joint. In uninfluencable high local activity of the process, objectified by examinations of the synovial membrane, an early synovectomy is indicated for the prevention of the formation of irreversible chondropathies.

[Early recognition of adverse effects, especially of kidney lesions, in patients with progressive chronic polyarthritis under D-Penicillamine therapy].

A report is given on the control of the course of 36 patients with progressive chronic polyarthritis under treatment of D-penicillamine. Despite the favourable therapeutic effect side effects of various kinds are observed. As the severest complication the development of renal disturbances with proteinuria is to be regarded. With the help of the disc electrophoresis of the urine it is possible to establish the beginning disturbances of the permeability of the basal membrane and to achieve a retrogression by reduction of the dose or finishing of the treatment with the medicament, respectively.

[Holiday effect in myocardial infarct].

Aimed measures of the organisation of the combat against infarction demand also the observation of temporary frequencies. On the basis of the evaluation of certificates of death of the month December of the years 1969 to 1973 of the GDR with differentiation according to so-called prehospital dead (persons who died at home and on the way to the hospital) and patients who died in the hospital with high significance an unwarrantedly high prehospital mortality during the period from Christmas to the end of the year (25th to 31st December) was established compared with the preceding week (18th to 24th December). Since in contrast to this the hospital cases and the cases "on the way" do not show any significant differences main tasks for the beginning of improvements concerning health policy may be deduced, all the more since the so-called holiday effect, expressed by a high home/clinic-relation of patients who died of myocardial infarction, could be restricted to 6 counties of the GDR on account of the analysis of further localities. From the results the tendency of a retrogression of the holiday effect is to be read off in the course of years. In the discussion an explantation of this peculiarity is attempted, and practicable conclusions for the removal and thus for the improvement the infarct situation are formulated.
