The coracoclavicular joint.

The coracoclavicular joints are genetically determined anatomic variants. There are problems in determining their presence by dissection examination of osteologic material as well as by radiography. Based on a survey of published material and a prospective radiological survey carried out in Malaysia, it is clear that the anomaly is more frequent in Asia than in Europe and Africa. There is a high incidence in people from Southern China. The joints are subject to osteophytic marginal lipping without development of symptoms or disability.

Diagnostic radiology: geography of a high technology.

A survey was made of the manpower employed, facilities available and utilization of radiology in countries in tropical Africa and Southeast Asia. The data obtained are compared with those from industralized societies in temperate climates. The level of use of radiographic services varied by a factor as high as 40. The differences between the level of radiology manpower available could be over a hundredfold and for x-ray technologists, many hundred times. The concentration of services in capital cities is characteristic of many tropical societies. Measures to improve radiologic health care in tropical countries, with particular emphasis on rural areas, are discussed. The main need is for availability of radiodiagnosis close to the primary care level in rural areas to deal with the important clinical problems of trauma, chest infections, and obstetrics. Simplification of equipment and employment of technicians with short training can help relieve maldistribution of services.

Lung density: clinical method for quantitation of pulmonary congestion and edema.

The density of a defined volume of the human lung can be measured in vivo by a new noninvasive technique. A beam of gamma-rays is directed at the lung and, by measuring the scattered gamma-rays, lung density is calculated. The density in the lower lobe of the right lung in normal man during quiet breathing in the sitting position ranged from 0.25 to 0.37 g.cm-3. Subnormal values were found in patients with emphsema. In patients with pulmonary congestion and edema, lung density values ranged from 0.33 to 0.93 g.cm-3. The lung density measurement correlated well with the findings in chest radiographs but the lung density values were more sensitive indices. This was particularly evident in serial observations of individual patients.

Xanthoma disseminatum. Clinical, metabolic, pathologic, and radiologic aspects.

A 55-year-old man with the classical mucocutaneous lesions of xanthoma disseminatum has been followed up for a period of 13 years. The special features of this case, which make it unique, are as follows: (1) the availability of histologic data on multiple lesions for more than a ten-year period; (2) the progressive nature of the multiple osseous lesions; (3) the metabolic studies that show no evidence for accumulation of abnormal sterols in a xanthoma, the blood, or intestinal aspirate; (4) the development of hypothyroidism and symptoms or signs, or both, of an intracerebral and an intraspinal lesion; (5) the partial regression of the cutanous symptoms and lesions while receiving clofibrate, in spite of progression of the mucous membrane and osseous lesions, and (6) the failure to develop diabetes insipidus to date.

Post mortem coronary arteriography in Nigerians: a radiological review.

The findings in 147 unselected consecutive post mortem coronary arteriograms carried out at the University College Hospital, Ibadan, are analysed. The so-called third primary division of the left coronary artery was noted in 35% of cases which is much less than the 74% quoted for the South African Bantu. In atrial blood supply, there was a definite predominance of one coronary artery. The predominant artery to both atria originated in one-third of cases from the right coronary, in another third from the left coronary and in the remaining third from both coronary artery systems respectively. Only in eight instances was atherosclerotic disease noted and even then, this was of the mildest degree with only minimal and slight intimal irregularity. This further explains the known rarity of myocardial infarction as a cause of death in the Nigerian. The study forms a useful basis for the baseline anatomy of the coronary arteries in Nigerians as a prelude to in vivo studies in the cardiomyopathies.
