The assessment of development of bone mass in children by quantitative ultrasound through the proximal phalanxes of the hand.

The purpose of this study was to determine the relationship between quantitative ultrasound and age and body size, and to establish reference ranges for speed of sound (Ad-SoS) in the phalanxes of children. Healthy children (433), aged from 9 to 15 y were studied (226 girls and 207 boys). Ultrasound device (DBM Sonic 1200, IGEA, Italy) measuring the Ad-SoS through the phalanxes was used. Up to 11 y, the mean values for boys and girls were not significantly different. The increase of values was observed in the girls at 11 y, in the boys 2 y later. Stepwise multiple linear-regression analysis has shown that, in girls, the main factor influencing Ad-SoS was age and, in boys, height and weight. In the study, the ability of measurements of proximal phalanxes to reveal skeletal changes in healthy Polish children was shown. The study enabled us to establish the reference curves for healthy Polish children.

Quantitative ultrasound of the heel and serum and urinary cortisol values in assessment of long-term corticotherapy side effects in female bronchial asthma patients.

Female asthma patients (26) with and without corticotherapy were studied. The control group included 19 healthy women. Skeletal status was assessed by ultrasound measurement of the heel (Achilles, Lunar, Madison, WI, USA) and serum and urinary corisol expressed adrenal function. Ultrasound and hormonal values were significantly lower in patients treated with glucocorticosteroids (GC) than in controls. In patients without GC, cortisol parameters were normal and ultrasound measurements were moderately diminished. 57% of women with and 33% of women without GC-therapy had an ultrasound T-score less than -2.5. Decrease of ultrasound values estimated by linear regression in relationship to time of asthma duration was highest in women with GC therapy. Several significant coefficients of correlation between ultrasound and adrenal function parameters were noted only in patients treated with GC. These data suggest that bone and endocrinological side effects due to steroid therapy in asthma patients show similar trends. Results obtained in present study require further longitudinal investigations.

[Secondary osteoporosis in patients with steroid-dependent asthma].

The aim of the trial was estimation of frequency and degree of osteoporosis in steroid-dependent asthma patients (48 persons: 18 men and 30 women). Control group created 36 healthy persons. Following measurements were done: quantitative computed tomography, radiological estimation of thoracic and lumbal spinal column and hands, in serum total alkaline phosphatase, calcium, phosphate, in urine excretion of calcium, phosphate and creatinine. Quantitative computer tomography revealed secondary osteoporosis in 40.5% asthma patients. These results were correlated with estimation of roentgenograms of thoracic and lumbal spinal column and metacarpal measurements and less well with total alkaline phosphatase. Time of duration of steroid therapy and time of asthma had influence on degree of osteoporosis. Moderate development of osteoporosis is probably the result of low dose of steroids.

[Alendronate--a new drug for treatment of osteoporosis].

Bisphosphonates are new drugs in the therapy of osteoporosis. Till now, several drugs were described, among them alendronate. General features of bisphosphonates and some recent results of clinical trials are presented. The authors conclude, that alendronate is a well tolerated new treatment option reducing the risk and severity of new vertebral fractures, reducing the risk of nonvertebral fractures, and reducing the height loss.

[Levels of endothelin-I in bronchoalveolar fluid of patients with selected respiratory tract diseases].

UNLABELLED: Endothelin-I (ET-I) levels in BALF of symptomatic (n = 7) and asymptomatic (n = 10) asthmatic patients, sarcoidosis (n = 10), allergic alveolitis (n = 6) and healthy volunteers (n = 6) was evaluated. In all patients BALF level of endothelin-I was assessed by radioimmunoassay. We observed that patients with symptomatic asthma had more increased amounts of ET-I in BALF in comparison with asymptomatic asthmatics, patients with sarcoidosis, allergic alveolitis and control group. CONCLUSIONS: 1. Presence of ET-I in BALF indicates that this peptide is involved in the pathogenesis of bronchial asthma, sarcoidosis and allergic alveolitis. 2. Endothelin-I is involved in bronchial smooth muscle contraction.

[The role of physical exertion in prevention and treatment of osteoporosis].

The prevention of osteoporosis consists of two components: building up high peak bone mass and reducing the rate of bone loss during menopause and aging. The animal trials prove positive influence of physical activity on bone mass but retrospective studies in humans are not quite confirmative. Prospective trials on large populations indicate positive role of physical activity in humans. The weight exercises show the best results. Proper long-term physical activity contributes both to gaining high peak bone mass in the young and to slowing down the rate of bone loss (in conjunction with pharmacotherapy) in the elderly.

[Osteogenesis imperfecta tarda--case report].

The case of type IA of osteogenesis imperfecta tarda was described. Following results were presented: anamnesy, physical, ophthalmic and ORL examinations, laboratory findings, radiological and densitometric data. Clinical characteristic and mentioned above examinations allowed to take the definitive diagnosis. Family data showed features of this disease in the next generations and should be a base for diagnostic evaluation in whole family. Confirmation of diagnosis ought to be the important information concerning future life style, work etc. in the next generations.

[Reduced loading of one lower limb as a cause of local osteopenia].

An impact of diminished mechanical loading of lower extremity on bone densitometry has been quantitatively assessed with ultrasound. Seventy-three females with lower limb fracture history, coxarthrosis or scoliosis were evaluated. Both heels were submitted to measurements with Achilles densitometer (Lunar, USA). The following parameters were analyzed: Speed of Sound, Broadband Ultrasound Attenuation and Stiffness Index. Decreased values of all parameters were found in unloaded extremities. Speed of Sound was reduced most and in the group with fracture history. To avoid local bone loss short immobilization, early rehabilitation and pharmacotherapy is suggested.

[The role of locally synthesized growth factors and cytokines in pathogenesis of osteoporosis].

Bone remodelling is a complex process involving a number of cellular functions directed toward the coordinated resorption and formation of new bone. Bone remodelling is regulated by systemic hormones and by local factors. The local factors are synthetized by skeletal cells and include growth factors and cytokines. The end product of remodeling is the maintenance of a mineralized bone matrix, and the major organic component is the collagen. Local factors play an important role in pathophysiology of osteoporosis as systemic hormones.

[Evaluation of bone loss in postmenopausal women during the first 5 years after menopause].

Aim of the trial was estimation of the degree of bone loss in first 5 years of menopause. 184 postmenopausal women were evaluated. Bone status was assessed by using quantitative ultrasound. Both parameters decreased in term of 1-5 years of menopause, what was especially expressed in first 2-3 years of menopause. Densitometric values were lower in comparison to British, French and Japan women population. Also dynamic of bone loss was greater in Polish women population. Results confirm necessity of early prevention of osteoporosis in postmenopausal women.

[Levels of endothelin-1 in bronchoalveolar lavage fluid of patients with bronchial asthma].

UNLABELLED: To evaluate the role of endothelin in the pathogenesis of bronchial asthma endothelin-1 levels were studied in the bronchoalveolar lavage fluid of symptomatic (n = 7) and asymptomatic (n = 10) asthmatic patients and healthy volunteers (n = 6). In all patients BAL fluid levels of endothelin-1 was assessed by RIA, total protein concentration by Lowry method. Patients with symptomatic asthma had increased amount of ET-1 in BAL fluid than asymptomatic asthmatics and control group. CONCLUSIONS: 1. Presence of endothelin in BAL fluid indicate that this peptide is involved in the pathogenesis of bronchial asthma. 2. Endothelin-1 BAL fluid levels may correlate with the severity of bronchial asthma.

[The role of prevention in osteoporosis].

Prevention of osteoporosis plays the major role in care of osteoporotic patients because of limited efficacy of therapy in late stages of the disease. Prevention ought to begin the whole life and its main aims are: achieve maximal peak bone mass, stabilize bone mass in adults and slow bone loss in elderly. The most important are: long-term high diet calcium consumption, physical activity avoiding secondary reasons of osteoporosis and wide use of hormone replacement therapy.

[Usefulness of biochemical markers of bone turnover in diagnosis of osteoporosis].

In diagnosis of osteoporosis biochemical markers of bone turnover play an important role as the tool of estimation of bone metabolism rate. Bone formation markers are alkaline phosphatase, osteocalcin, procollagen; bone resorption markers are pyridinoline, deoxy-pyridinoline, hydroxyproline. Biochemical markers allow together with other diagnostic methods better evaluation of bone status.

[The influence of patient age on the effectiveness of bronchodilators].

The beta adrenergic receptors are exposed to variety of factors modulating their activity. We analyzed the sensitivity to Salbutamol in asthmatic patients divided into three group (I-30 yr; II-30-50 yr; III-50 yr). The effectiveness of beta adrenergic receptors was assessed by means of estimation of plasma cAMP level before and after Salbutamol infusion. We didn't found any significant differences in plasma cAMP changes between investigated group of patients.
