Endoscopic treatment of precanceroses and early gastric carcinoma.

A plea is made for individual age-adjusted endoscopic therapy of precancerous lesions and, under certain conditions, of early gastric carcinoma. True adenoma and borderline lesion have to be resected, if technically feasible, by snare ectomy. Tumour remnants or local recurrences can be destroyed by argon laser application. In poor-risk patients with a well-differentiated mucosal carcinoma situated close to the cardia, the risk of dying from metastases left behind after endoscopic resection seems less than that of fundectoma or gastrectomy. Endoscopic polypectomy was successfully practised in 6 patients with early gastric carcinoma with no tumor recurrence.

Treatment of choledocholithiasis using the transpapillary perfusion technique.

In 5 patients with common bile duct stones a dissolution therapy with the octanoate mixture Capmul 8210 was performed using the transpapillary perfusion technique. In 3 out of the 5 cases a passage of the stones without further endoscopic operative procedures could be achieved. Complaints were endurable except in one case. Further studies to dissolve common bile duct stones with a modified Capmul solution via a transpapillary tube seem justified.

[Chronic gastritis].

Chronic gastritis is a histological diagnosis, relying on separate biopsies from antral and fundic mucosa. According to Strickland type A gastritis corresponding to pernicious anemia should be differentiated from type B gastritis, maybe induced by duodeno-gastric reflux. Intensity of inflammatory infiltration and atrophy of the specific glands correlate by and large with acid secretion, however, not with alcohol or nicotine abuse or iron deficiency. Chronic atrophic gastritis may lead to gastric carcinoma of the intestinal type, in gastric ulcer patients localization of the ulcer crater is determined by the spread of gastritis. The histological diagnosis of chronic gastritis has no therapeutic consequences; however, in type A gastritis regular endoscopic follow-up studies in 2-5 year intervals seem advisable.

Controlled trial of carbenoxolone sodium vs. cimetidine in duodenal ulcer.

The healing-rate of duodenal ulcer after short-term treatment with carbenoxolone was not significantly different from that after treatment with cimetidine. There seems to be no evidence of a significant difference in the tendency of duodenal ulceration to relapse following successful short-term therapy with either treatment. In the case of cimetidine treatment the rate of 58% relapse within 12 months after withdrawal of the drug corresponds well to the data of other authors (3, 19) and does not exceed the relapse rate during placebo medication. The post-treatment relapse study was markedly hampered by a low compliance of patients to cooperate in the long-term follow-up. The drop-out rate was greater in patients previously treated with carbenoxolone than in those of the cimetidine group.

[Expert judgement of patients with gastro-intestinal disease (author's transl)].

At present there are two special situations which offer considerable difficulties in expert judging of gastro-intestinal disease: (1) patients with gastro-intestinal disease acquired during the war or in the immediate post-war period, and (2) quantification of functional disorders. Gastro-intestinal disorders do have considerable importance in social medicine. Physical unfitness for special professional work was caused in 3.5-4% of all male cases by gastro-intestinal disease during the last decade, physical unfitness for work in general in 2.2-2.6% of all male cases, the corresponding figures for female persons being 1.6-2.0% and 2.2-1.5%. General rules for establishing an expert opinion cannot be given; some points however are made in regard to how a diagnosis can be established and how unfitness caused by gastro-intestinal disease may be ascertained or excluded.

[Selective proximal vagotomy without drainage in uncomplicated duodenal ulcer. Technic and frequency of recurrence].

899 cases of slective promimal vagotomy were operated between 1974-1978 in the Surgical University Hospital Erlangen. The underlying disease was in all cases a chronical duodenal ulcer. The primary operative mortality was 0,6%. Intraopeatively measurements to control the sufficiency of the vagotomy Burge and Grassi proved to be necessary. We could verify by endoscopy a recurrence rate of 5,8-16,5% in the 1.-4. postoperative year. Only 58% of the recurrences could be found by x-ray investigations. 75-82% of the patients had a clinically perfect and good result. In patients with a proven recurrent ulcer there was nevertheless a good clinical result in 64%. Clinical results and complaints of the patients are therefore not sufficient as follow-up after selective proximal vagotomy and early detection of a recurrent ulcer. We believe, that one endoscopy per year is necessary.

[Long-term effect of calcium carbonate containing antacids on basally and peptone-stimulated hydrochloric acid secretion].

Basal and meal-stimulated gastric acid secretion was measured before and after a 3-week-period of regular consumption of a calcium carbonate-containing antacid with 6 therapeutic doses per day in normal healthy volunteers. Furthermore the effect of a single large dose of 2000 mg calcium ions was studied on meal-stimulated gastric acid secretion before and after this period. The results suggest that (1) even a large single dose of calcium ions seems to have no major effect on meal-stimulated gastric acid secretion and (2) chronic consumption of calcium carbonate containing antacids over a 3-week-period does not change basal- and meal-stimulated gastric acid secretion in healthy volunteers nor does it increase the response to 2 g calcium ions.

[A prospective multi-centre study of the response of metastatic gastrointestinal tumours (author's transl)].

In a prospective, multi-centre, randomized study of 109 patients with metastatic gastro-intestinal adenocarcinomas the response rate, survival time and side-effects of two drug combinations, carmustin +5-fluorouracil and carmustin + ftorafur, were compared (same carmustin dosage in both groups). Response to the treatment was 32.7% in those receiving carmustin +5-fluorouracil, 26.3% in those on carmustin + ftorafur. This difference occurred among the 42 patients with gastric adenocarcinoma (33.3% compared with 25%), as well as in 11 with pancreatic adenocarcinoma, and in 56 with colorectal adenocarcinoma (32.1% and 28.6%). Median survival time for 5-fluorouracil + carmustin was 330 days, double that for ftorafur + carmustin (163 days). Bone-marrow toxicity (leukopenia, thrombopenia) was below 10% for both drug combinations. Alopecia occurred in only a few patients. Gastro-intestinal toxicity was common (20% and 18.5%, respectively), but there was no difference between the two groups. The somewhat lower effectiveness of ftorafur compared with 5-fluorouracil was probably due to the deliberately smaller dosage of the former.

[Irritable colon. Possibilities for an objective diagnosis and its value for the therapy].

Diagnosis of functional gastrointestinal disorders consists mainly in exclusion of organic diseases. A more positive diagnosis may be established by the identification of characteristic symptoms. Myography and manometry within the colon sigmoideum reveal significant patterns in irritable bowel patients, which also may allow control of therapy.

[Endoscopic papillotomy (author's transl)].

Endoscopic papillotomy is a method of incising the papillary spincters without opening the abdominal wall or the duodenum. It is successful in over 95% of cases. In over 80% of cases passage of choledochus concrement occurs, about half of these stones being excreted spontaneously and the rest having to be extracted actively with a small Domia basket. For large concrements which cannot be removed endoscopically even after wide papillotomy incision, there is the electrohydraulic lithotrite which crushes them so that they can be passed spontaneously or be actively extracted. Dissolution by a substance instilled through a catheter passed through the papilla into the common bile duct is still in the experimental stage. Altogether for this reason endoscopic papillotomy and removal of stones is an advance for the patient at risk, as it involves a lower lethality than the corresponding surgical method.
