Gastrointestinal fiberoptic endoscopy in pediatric patients and juveniles.

Over a period of 8 years 351 upper gastrointestinal endoscopies and 72 coloscopies were performed in infants, children and juvenile patients. Development of special pediatric instruments provides increasing application even to the youngest patients. Fiberendoscopy is a safe and sensitive diagnostic tool also in the pediatric age group; furthermore the therapeutic possibilities of operative endoscopy can be used without disadvantage.

Peroral cholangioscopy in choledocho-duodenostomy--patients using the pediatric fiberscope.

In patients with wide-open bilio-digestive anastomoses peroral cholangioscopy using the pediatric endoscope GIF-P2 is easily performed. The common bile duct is visualised down to the papilla and up to the distributaries of the hepatic ducts. The instrumentation channel allows mobilisation of incarcerated concretions, extraction of food particles (cholangiophytiasis) and guided biopsy of pathological findings. The experience in 12 patients with this method is promising.

[Septic temperatures in questionable ascites].

The case history of a 40 year old male with septical temperatures and ascites is presented. Because of skin-pigmentation, hypotension, low rate of electrolytes, augmented renin-activity, and septical temperatures there was a suspicion of Addison's disease. A protein-rich ascites without tumor cells and erythrocytes, but with lymphocytes gives the diagnosis of peritonitis tuberculosa. Addison's disease and peritonitis tuberculosa are late manifestations of an organ tuberculosis, about 20 years after pulmonal tuberculosis.

[Peptone-stimulated gastric secretion and antacids. Effect of antacids with various content of calcium, magnesium and aluminiumions on the peptone-induced secretion of gastric juice, double-blind study].

A double-blind-trial was performed to prove the effect of antacids containing Mg (OH)2, Al (OH)3 and CaCO3 on peptone-stimulated gastric secretion (intragastric titration). A high efficacy of antacids given after meals was demonstrated. After application of antacids no significant rise in serum gastrin levels during intragastric titration was found. Serum calcium and magnesium levels remained unchanged. Peptone-stimulated gastric secretion was not influenced by application of different antacids within a period of 2 hours.

[Current ulcer surgery from the internist's view point: vagotomy versus resection].

The internist is usually not concerned with the successfully operated ulcer patient now free of symptoms, but primarily with the sequels which follow the surgical intervention more or less immediately or many years later. In the resecting procedures these are stomal ulcer and dumping, weight loss, osteoporosis, iron deficiency anemia and stump carcinoma, in vagotomy ulcer recurrency, diarrhea, dumping symptomatology and an increased incidence of cholelithiasis. Of the currently practiced procedures parietal cell vagotomy seems to offer the best results in duodenal ulcer patients although long-term follow-up examinations are not yet available.

[The glandular cyst, a polypoid lesion of the gastric mucosa (author's transl)].

Confusion in the nomenclature of gastric polyps and the resulting uncertainties regarding prognosis and treatment have made a new classification necessary, consisting of focal hyperplasia, polyp of manifold aetiology, adenoma, and benign hyperplasiogenic polyp, the latter the most common one, found only in the stomach. But 110 cases of polypoid mucosal changes could not be classified. These "polyps" grow to be at most 8 mm in diameter and are characterized histologically by non-inflammatory cysts of varying size located within the intact fundal glands. Possible causes are hamartoma or functional secretory disorders. These glandular cysts have not previously been described. They do not fit the pattern of cystic gastritis. The clinical significance lies in the differentiation from gastric polyposis.

[Congenital gastrointestinal diseases with manifestations in adulthood].

A genetic background is discussed in many disorders of the gastrointestinal tract with a disposition in addition to environmental factors. The pathophysiology of most hereditary diseases is unknown although the mode of inheritance is established. Biochemical analysis may show molecular defects or inborn lack of enzymes, cytogenetic studies may reveal chromosomal abnormalities. The knowledge of genetic factors in gastrointestinal disorders may contribute to the early detection of persons afflicted but not yet symptomatic, in some rare syndromes genetic counseling may become mandatory. Finally, there are many congenital malformations which may not cause symptoms for many years so that doubts may arise whether they are developmental anomalies or acquired conditions.

A new catheter for endoscopic manometry of Oddi's sphincter.

With a newly designed manometry catheter and a modified pull-through maneuver a high presure zone can reproducibly be recorded between the duodenum and the pancreato-biliary tree corresponding to Oddi's sphincter. Catheter marking allows in addition calculation of the length of this zone. Since the sealed end of the catheter remains during pressure recording within the ductal system repeated push-and-pull maneuvres can be performed through the sphincter area.

[The aging alimentary tract (author's transl)].

The physiological process of aging causes changes in volumen and composition of saliva, gastric and pancreatic juice and bile flow, a diminished absorption and an altered bacterial flora. However, these changes are compensated and do not cause symptoms. Besides of an increased disposition to develop carcinoma of the gut the alimentary tract in contrast to the cardiovascular system has no specific symptomatology as signs of wear. In old age, too, functional disorders rank first in clinical symptomatology.

Inhibition by somatostatin of secretin-stimulated pancreatic secretion in man: a study with pure pancreatic juice.

The action of somatostatin on compostition and flow rate of pure pancreatic juice obtained by endoscopic cannulation of the main pancreatic duct was evaluated in 5 healthy volunteers. Synthetic secretin (0.06 CU/kg-h) was intravenously infused throughout the 80-min study. Bicarbonate concentrations in pancreatic juice achieved constant levels (117 +/- 3 muEq/ml) after 10 min, whereas a steady state of juice flow (7.3 +/- 1.4 ml/5 min) was attained after 15 min of secretin infusion. In the third 20-min period, cyclic somatostatic (5 mug/kg-h i.v.) was given, leading to a decrease in pancreatic flow rate by 47% after 10 min, and by 67% after 15 min of somatostatin administration. Alrady 5 min after the infusion of somatostatin had been discontinued, pancreatic flow rate gradually recovered; presomatostatin levels, however, were not reached within 20 min. Cyclic AMP varied roughly in accordance with bicarbonate concentrations, whereas the chloride concentrations were reciprocally related. Bicarbonate, sodium, potassium, protein, and cyclic GMP concentrations did not change substantially due to somatostatin.
