Studies on lymphocytotoxicity in acute and chronic liver disease.

The cytotoxicity of lymphocytes from patients with chronic active hepatitis, chronic persistent hepatitis, acute hepatitis B and rheumatoid arthritis as well as from normal controls was studied in a microcytotoxicity assay according to COHEN et al. using 125I-iododeoxyuridine labeled embryonal liver cells and Chang cells as target cells. Unfractionated lymphocytes of the peripheral blood from patients with chronic active hepatitis and rheumatoid arthritis showed a high frequency of cytotoxic activity. The lymphocytotoxicity in chronic active hepatitis was significantly increased in comparison to normal controls at the EC/TC of 10:1 and 100:1. Specificity of the cytotoxic reaction to target cells could not be demonstrated. Addition of autologous serum to the cytotoxic assay blocked the lymphocytotoxicity in patients with chronic active hepatitis. A weak potentiating effect on lymphocytotoxicity was observed in patients with hepatitis B after addition of autologous serum. It is discussed that this reaction is due to the presence of HB-antigen in the serum since addition of HB-antigen from other sources increased also the lymphocytotoxicity in hepatitis B patients. This effect was observed neither in HB-antigen positive nor in HB-antigen negative patients with chronic active hepatitis or chronic persistent hepatisis.

Vasoactive intestinal peptide: a secretin-like partial agonist for pancreatic secretion in man.

The responses of pancreatic volume flow and bicarbonate output to intravenous vasoactive intestinal peptide (VIP, 0.8 to 3.2 microgram per kg per hr) and synthetic secretin (32.2 to 129 ng per kg per hr) were compared intraindividually in 5 healthy volunteers. Pure pancreatic juice was obtained by endoscopic cannulation of the main pancreatic duct. The mean +/- SEM observed maximal response of secretin-stimulated juice flow was 248 +/- 7 microliter per kg per 5 min, whereas the observed maximal response for VIP-evoked juice flow was 48 +/- 6 microliter per kg per 5 min. The observed maximal secretin-induced bicarbonate output was 30 +/- 2 muEq per kg per 5 min, and the maximal VIP-related response was 4.3 +/- 0.9 muEq per kg per 5 min. In addition to low efficacy, high dose requirements, and side effects (significant rise in pulse rate and cutaneous flushing at 3.2 micrograms per kg per hr) argue against a major physiological role of VIP as a hormonal stimulant of human pancreatic bicarbonate secretion.

Endoscopic papillotomy.

Obstruction of the common bile duct can now be relieved by endoscopic electrosurgery. This report describes our experience with 267 patients. In 192 of 222 patients with choledocholithiasis all calculi were evacuated by endoscopic papillotomy (EP). The remaining patients had EP because of papillary stenosis. Complications of EP included nine instances of pancreatitis, seven of bleeding, and two perforations. In 2 of 32 patients having EP for papillary stenosis, restenosis has appeared on follow-up. The two fatalities were attributable to purulent cholangitis and acute bleeding. This required to manage these situations. The endoscopic method requires less hospitalization and recuperation. EP and stone extraction are the methods of choice for managing common duct obstruction in high risk patients before cholecystectomy, for retained or reformed stones after cholecystectomy, and for papillary stenosis.

[Results of surgical treatment of early carcinoma of the stomach (author's transl)].

From 1961 to 1975, 53 patients were operated on for early carcinoma of the stomach, 24 of the total of 57 cancer sites being limited to the mucosa, 33 having already infiltrated into the submucosa. Macroscopically, most of them were ulcerative carcinoma, while histologically the intestinal type predominated. The cumulative five-year survival rate (excluding hospital deaths) was 86.5%, while after "typical" carcinoma resection by the Billroth II procedure with removal of the greater and lesser omentum it was 100% for mucosal and 93% for submucosal carcinoma.

[Persistent diarrhea in rheumatic disease].

The case history of a 52-year-old male with weight loss, steatorrhea and arthritis is presented. During clinical pathological conference, Whipple's disease was strongly suggested. The diagnosis could be proved morphologically. Antibiotic therapy with tetracycline 1 g daily for three months caused a prompt improvement. Small bowel biopsy showed disappearance of PAS-containing macrophages.

[Malignant systemic diseases of the digestive tract].

In about 5-6% of all patients with malignant myeloproliferative disorders the gastrointestinal tract is involved with macroscopic lesions. The most common sites of digestive manifestations are stomach, small bowel, pancreas and colon. Multiple irregular ulcers, doughy infiltration of the bowel wall, multiple nodules and diffuse polyposis of the colon are classic patterns of secondary lymphoma of the gut. Prognosis is considerably impaired by gastroenterological symptoms.

Biliary Tract Diseases↗

[Manifestations of systemic diseases in the gastrointestinal tract].

Under the aspect of systemic diseases and their manifestation in the gut the following conclusions can be drawn: 1. The skin is the mirror of the intestinal tract; not only in primary gastroenterological disorders one should look for dermatological complications, but should also think in chronic skin lesions of concomitant intestinal alterations. 2. In all patients with collagen diseases a gastrointestinal involvement is very common. 3. In all endocrine disorders except in hypothyroidism diarrhea is a very common finding. 4. Infiltrations of gastrointestinal tract can be demonstrated in many cases by gastric, small bowel or rectal biopsy. 5. In all forms of dysgammaglobulinemia giardiasis is very common. 6. In right heart failure protein-losing enteropathy should be considered, in left ventricular insufficiency bowel ischemia.

Transgastroscopic ultrasonography.

A newly designed ultrasonography probe is presented which can be advanced through the instrumentation channel of an operative endoscope (TGF-Olympus). The A-scan allows the differentiation between solid and cystic masses in the upper abdomen leading to gastric impression. Guided puncture of pancreatic pseudocysts is based on a solid diagnosis and can be performed from the gastric lumen thus avoiding the development of pancreato-cutaneous fistulas.

[The function of the hydrokinetic and ekbolic pancreas after acute pancreatitis].

Thirteen patients were investigated 3-4 weeks (I. examination) and 3-5 months (II. examination) after the ceasing of the signs symptoms of an acute pancreatitis concerning their hydrokinetic and ekbolic pancreas-functions. In addition, the pancreatic ducts were demonstrated by ERCP. Reduction of the secretion were shown in 31% at the I. examination and in 23% at the II. examination. There were significant differences in the outputs of bicarbonate, trypsine and chymotrypsine compared to healthy test persons at the I. examination. At the II. examination, however, no significant differences could be observed compared to healthy test persons. The exocrine function of the pancreas was getting worse during the interval between the I. and II. examination in 2 cases but it became normal in 5 cases. Pathological alterations at the pancreativ-duct-system were not demonstrable shown at all.
