[Bacteremia in therapeutic endoscopy].

In a prospective study of 70 patients undergoing therapeutic endoscopy, the incidence of bacteremia was established. It was observed in 20 after bougienage, 30% after the placement of an endoprosthesis, 15% after endoscopic sphincterotomy, and 25% after percutaneous biliary drainage. Although clinical symptoms were observed in only a few of these patients, perioperative antibiotic therapy is recommended in elderly patients with malignant diseases, diabetes mellitus, renal insufficiency or liver disorders.

[The "upside-down stomach"--a diagnostic pitfall].

Upside down stomach is an extreme variant of a paraesophageal hernia. The cardia and pylorus are located next to each other, while the rest of the stomach lies within the thorax. Radiology is mandatory for accurate anatomical documentation, while endoscopy is necessary to identify pathology, primarily bleeding sources. This is a rare condition found during endoscopy, with an incidence of 1.5%. Since there is a risk of volvulus, surgical correction is usually indicated.

Efficacy of cisapride in the treatment of epigastric pain and concomitant symptoms in non-ulcer dyspepsia.

Non-ulcer dyspepsia is gaining increasing interest among gastroenterologists even though the pathogenetic mechanisms in individual patients are still unknown. On the basis of a number of studies, it can be concluded that in about 60% of patients impairment of gastric evacuation may contribute to the symptomatology (epigastric pain, postprandial fullness, early satiety, bloating, nausea and vomiting). This review summarizes the results of 10 placebo-controlled trials which evaluated the effects of cisapride (3 x 5 or 3 x 10 mg/day) in strict non-ulcer dyspepsia or functional postprandial dyspepsia. In seven of the trials, cisapride proved significantly superior to placebo in relieving epigastric pain and concomitant symptoms in patients with non-ulcer dyspepsia. In the three studies examining chronic functional dyspepsia, belching, postprandial bloating, early satiety and heartburn were significantly improved. In all 10 trials, cisapride was significantly superior to placebo.

[Risk diseases for colorectal cancer].

Among the risk conditions with an increased prevalence or incidence of colorectal carcinomas hereditary and acquired disorders have to be differentiated. While in hereditary forms new in vitro tests may play a role in the identification of patients at risk we have to rely on screening and follow-up programmes in the other group of patients. The intervals of these surveillance programmes should be acceptable to the patient since many risk groups seem to be questionable on a cost-benefit evaluation.

A comparison of roxatidine acetate 150 mg once daily and 75 mg twice daily in gastric ulcer healing.

In 363 outpatients with endoscopically confirmed gastric ulcers the efficacy and safety of roxatidine acetate 150 mg at night was compared to 75 mg twice daily. After 8 weeks' treatment substantial reductions in gastric ulcer diameter were obtained in addition to healing rates of 83.7 and 86% for the twice daily and night-time dosing, respectively. Daily reductions in day and night-time epigastric pain were obtained with no significant differences between treatment groups for pain scores or antacid tablet consumption. Furthermore, cigarette smoking did not influence the healing rates produced by either treatment schedule. 26 patients reported 32 adverse reactions and 5 patients discontinued treatment because of side effects, although only 1 of these was a severe reaction. The present data suggest that a single night-time dose of roxatidine acetate 150 mg is as safe and effective as the twice daily dose regimen for the management of acute gastric ulceration.
