Pancreatic juice protein associated with chronic pancreatitis and adenocarcinoma of the pancreas.

125I-protein-radioiodinated pure pancreatic juice samples from patients with adenocarcinoma of the pancreas, chronic pancreatitis, or intact pancreas were analysed by high-resolution SDS-polyacrylamide gel electrophoresis and subsequent autoradiography. Experiments resulted in the detection of a 180K protein, probably a glycoprotein, in the pure pancreatic juice from pancreatic carcinoma (93%) and chronic pancreatitis (73%) patients, which was completely absent from pancreatic juice from intact pancreas. Sephadex G-200-isolated 180K protein was found to be different from carcinoembryonic antigen (CEA) when traced by a commercial CEA radioimmunoassay, but it seemed identical in pancreatic juice samples from patients with pancreatic carcinoma and chronic pancreatitis, at least with regard to isoelectric point. In brief, the present results suggest that 180K protein identification in pancreatic juice permits adenocarcinoma of the pancreas and chronic pancreatitis to be differentiated from normal conditions but that distinction between pancreatic carcinoma and chronic pancreatitis is unlikely.

[Therapy of acute hemorrhage - the value of antacids].

In contrast to prospective studies demonstrating the efficacy of antacids in the prevention of stress ulcers there are only a few reports on the successful treatment of bleeding lesions with antacids. Although by a constant pH exceeding 5.4 acid-induced dissolution of thrombocyte aggregation can be avoided, the high amounts of antacids cause volume problems and side effects. No experience exists on the combination of formula diets with antacids in the treatment of acute hemorrhage.

Acute Disease↗

[Segmental pancreatitis].

Segmental pancreatitis is an intermediate type of pancreatitis showing certain traits typical in acute and other traits typical in chronic pancreatitis. Alteration of pancreatic ducts, which can be demonstrated by endoscopic retrograde pancreatography is typical for chronic pancreatitis, even calcifying pancreatitis, whereas on the other hand pancreatic function is normal. Segmental pancreatitis is localized mostly in the tail of the pancreas, however circumscript pathological changes may be found as well in the body, the head or the uncinate process of the pancreas. Divisum pancreatitis is a special form of segmental pancreatitis, where both parts of the pancreas may be affected.

[Ulcerative processes of the stomach].

Ulceration of the gastric mucosa is a consequence of a pathological process and it can be subdivided into erosions and ulcers according to depth of penetration. In most cases the lesions are of peptic origin; they may however, be caused by disturbed local circulation, disintegrating tumor tissue and specific diseases like sarcoidosis, tuberculosis, Crohn's disease and syphilis. Such lesions may also be caused by acid burn. Since macroscopic findings during endoscopy may be misleading, all gastric ulcers should be investigated by biopsies, with the possible exemption of diffuse acute or chronic erosions.
