Prostaglandin synthesis in dental cysts.

The synthesis of prostaglandins by dental squamous cell cysts was studied. Radiothinlayer-chromatography (RTLC) showed C14-labelled arachidonic acid to be converted into the prostaglandins PGE2, 6-oxo-PGF1 alpha, PGF2 alpha and PDG2. In addition, hydroxyfatty acid synthesis in excess of total prostaglandin production was observed. The substances mentioned are thought to be causally involved in the osteolytic activity of dental cysts.

Evidence that PGI2-generation in human dental cysts is stimulated by leucotrienes C4 and D4.

Various prostaglandins, particularly PGE2 and PGI2, appear to play a major role in osteolytic processes. The bone-destructive action of dental squamous cell cysts is thought to be caused or at least mediated by prostaglandins. Both leucotrienes and prostaglandins are synthesized from a common precursor substance, i.e. arachidonic acid, in a series of reactions. There is a special relation between leucotrienes and PGI2 in that PGI2 synthesis in endothelial cells can be enhanced by LTC4 and LTD4. R-TLC showed conversion of the arachidonic acid added into PGI2 in 8 out of 12 cases, as reflected by the stable degradation product 6-oxo-PGF1 alpha. In 8 cases bioassay showed PGI2 synthesis without addition of LTC4 and LTD4. Incremental LTC4 and LTD4 additions were found to cause a highly significant increase of PGI2 synthesis rates. In summary, LTC4 and LTD4 stimulate PGI2 synthesis in chronic inflammatory processes in vivo and may thus elicit or accelerate osteolysis.

Lectin histochemistry of kidney tumours and its pathomorphological relevance.

Thirty kidney tumours of various histological type were histochemically investigated under the light microscope by means of the ABC-method. We used four biotinylated lectins which are known to bind also to normal renal tubular epithelial cells of different nephron segments. The nuclear grade, the histological growth pattern, the cell type and the histogenesis of the tumours were studied. The lectin of Lotus tetragonolobus bound to nine out of ten renal cell carcinomas with low nuclear grade, but in contrast no binding was seen in eight poorly differentiated ones. Four carcinomas mimicking collecting duct epithelium were also negative after treatment with this lectin, but showed strong staining after treatment with peanut agglutinin, Dolichos biflorus agglutinin and soybean agglutinin. Five oncocytomas showed a high affinity only for Dolichos biflorus agglutinin and only one case was positive with Lotus tetragonolobus agglutinin; three tubulo-papillary adenomas of the renal cortex without any oncocytes were negative with all of the lectins used. The value of lectin histochemistry in tumour pathology and its significance in routine pathological examination of kidney tumours are discussed.

The value of immunohistochemistry in medullary thyroid carcinoma: a systematic study of 30 cases.

Thirty cases of medullary thyroid carcinoma were investigated by immunoperoxidase staining techniques to evaluate the diagnostic significance of neuron-specific enolase (NSE), carcinoembryonic antigen (CEA), somatostatin (SOM), a-subunit of human chorionic gonadotrophin (a-hCG), serotonin (5-HT) and adrenocorticotropic hormone (ACTH) immunoreactivity as diagnostic markers in comparison to different calcitonin (CT) staining patterns. Twenty three cases exhibited a strong (group I) or moderate (group II) staining intensity for CT and did not need further immunocytochemical proof for classifying them as medullary carcinoma. From seven cases which showed only a weak or borderline CT-immunoreactivity (group III), six stained positively for NSE and four positively for CEA. SOM-positive cells were identified in six cases and a-hCG or 5-HT-positive cells respectively in three cases of group III. Twenty follicular and 20 papillary carcinomas also included in this study did not react with any of the above mentioned antibodies. Therefore, NSE and CEA represent useful additional diagnostic markers particularly for the identification of medullary carcinoma with weak or borderline CT-immunoreactivity. The identification of other peptides may also be helpful in demarcating it from thyroid tumours of follicular cell origin.

Possibilities and pitfalls of indium-111 platelet scintigraphy in the monitoring of renal transplant recipients.

During the last four years, we have performed two studies on the clinical application of 111In-platelet scintigraphy after renal transplantation. In the first study, we collected 75 patients who were treated with prednisolone and azathioprine. The platelet deposition in the graft was expressed as a platelet-uptake index (PUI). The results suggested that the platelet scan was a valuable tool for early detection of acute graft rejection, which was indicated by an increase in PUI. Quite different results were obtained in 50 patients immunosuppressed with cyclosporin A (CSA) and prednisolone. Acute interstitial rejection escaped detection by this diagnostic procedure. Significant elevations of PUI were observed in acute vascular rejection and microvascular CSA nephrotoxicity, resembling the haemolytic uraemic syndrome (HUS) only. Therefore, platelet deposition in the graft can be regarded as a non-specific phenomenon, occurring in two entities which require completely different therapeutic approaches. In spite of the monitoring by means of the platelet scan, percutaneous biopsy is still necessary for differential diagnosis of graft dysfunction.

Cardial decompensation caused by hypertension and polyglobulia associated with multiple renal oncocytomas.

A 43 year old man was referred to the Internal Department for the examination of cardial decompensation, hypertension, polyglobulia and atrophy blanche of the lower legs. Clinical and supersonic examination revealed two tumors in the left kidney (diameter 8 and 3.5 cm) and some small tumors in the right kidney. Laboratory studies disclosed erythropoietin levels up to 170 mU/ml (standard 14-61 mU/ml) and a red blood cell count of up to 8 X 10(6)/ml. After recompensation, a radical left nephrectomy and the removal of three small tumors of the right kidney four weeks later was initiated. Postoperative erythropoietin levels and red blood cell count sank to the normal range leading to the reduction of antihypertensive therapy. The clinical course indicates a strong etiological connection between hypertension as well as polyglobulia with the multiple bilateral oncocytomas. Polycythemia also led to the atrophy blanche which had resulted from disturbances of microcirculation.

The cyclo-oxygenase and lipoxygenase pathways in human oral cancer tissue.

This paper reports on the synthesis of prostaglandins in oral squamous epithelial carcinomas. Using radio-thin-layer chromatography, we observed the conversion of 14C-labelled arachidonic acid into the prostaglandins PGE2, PGF2 alpha, PGI2 and PGD2. We also observed an approximately equal production of hydroxy fatty acids. The role of the above-mentioned substances in the growth of malignant tumours and the process of metastasis is discussed.

[Role of computed tomography in malignant tumors of the oral cavity and pharynx].

The CT findings of 22 patients with histologically verified malignant tumors of the oral cavity and pharynx were evaluated to demonstrate the value of CT. Comparison with conventional radiological methods was made. In 16 patients an attempt was made to define tumor expansion and 6 patients were examined after surgery and radiation therapy with regard to tumor recurrence. In the first group we found tumors of different stages: in advanced cancer it was evident that with CT it is possible to describe tumor expansion very exactly and its relationship to surrounding structures, as well as the invasion of the intracranial space or spinal cord. In malignomas of the tongue and mouth floor measurements of size by CT were very precise, as proved by surgery. Our results in the first group of patients show, that CT has a great influence on therapeutic management and is also very important for clinical staging of these malignomas. In the second patient group it is shown that by CT differentiation between postoperative changes and recurrent tumor is as difficult as it is in other organs. Based on the postoperative examination, appropriate follow-up is necessary.

[Tumors of the endocrine pancreas].

37 patients suffering from apudomas of the pancreas are reported (18 insulinomas, 6 isletcell-hyperplasias, 9 Zollinger-Ellison syndroms, 1 glucagonoma, 3 without hormone production). In preoperative localization computerized tomography and angiography were the best with 65% positive findings. Insulinomas were enucleated, all free of recidives. 50% of operated isletcell hyperplasias had a postoperative resisting hyperinsulinism. Either gastrectomy or tumour enucleation was performed in the Zollinger-Ellison syndrome. The five-years survival rate was 43%.

[Electron microscopic and immunohistologic studies of patients with Horton's temporal arteritis].

Temporal arteritis is a systmic disease with a predilecation for the cranio-temporal vascular area. Histologically it is a panarteritis. Diagnosis is based on the presence of lymphocytes, histiocytes and foci of epitheloid cells in the media of the temporal artery. The presence of giant cells is, however not obligatory. The present study emphasizes the value of biopsy of the temporal artery in diagnosing this disease. It, furthermore, also points out the 10-percent possibility of false negative biopsy results based on patchy vascular lesions. Tenderness to touch of the temporal artery, characteristic of temporal arteriitis, can be explained by perineural inflammatory infiltration of nerves in the adventitia of this artery. Examination under the electron microscope reveals almost complete destruction of the smooth muscles of the media by epitheloid cell granulomas. Massive neogenesis of collagen ensues. Furthermore, numerous myofibroblasts, macrophages and histiocytes are observed. Several macrophages close to each other create the impression of giant cells in the light microscope. The electron microscope image allows for clear differentiation between temporal arteritis on one hand and of arteriosclerosis on the other. Using the immunoperoxidase method in temporal arteritis, immune globulines are found intracellularly in plasma cells. Extracellularly, however, neither immune golbulins nor complement deposits are found in the vascular wall. Thus, the assumption that temporal arteritis represents a immune complex disorder cannot be maintained. The most frequent ophthalmologic complication in temporal arteritis is ischemic optic neuropathy. Histologic examination of a bulbus presenting anterior ischemic optic neuropathy in a case of temporal arteritis revealed predominantly lymphocytic infiltrations of the short and long ciliary arteries. No inflammatory infiltration was found in the central retinal artery. The development of anterior ischemic optic neuropathy can be explained by impaired perfusion or by occlusion of the short posterior ciliary arteries. In 60% of patients suffering from temporal arteritis, we found anticollagen antibodies in the serum. Collagenization of the vascular wall as observed in our electron-microscopic examinations must, therefore, be considered the paradoxical consequence of an immune reaction caused by collagen auto-antibodies. Collagen auto-antibodies play a decisive role in the maintenance and chronicity of the inflammatory process in temporal arteritis. In therapy, corticosteroids should not be administered according to rule but rather in doses adjusted to individual requirements.(ABSTRACT TRUNCATED AT 400 WORDS)

[Immunohistochemical detection of prostaglandins using immunoperoxidase (PAP) exemplified by a human tumor tissue].

The possibility of detecting prostaglandins by means of indirect immunoperoxidase-method is reported allowing a differentiation between actual synthesis and inactive metabolites. This technique allows the detection of all important prostaglandins in tissue samples and opens a new route for discovery of prostaglandin synthesis profile of a particular cell type or the electron microscopical localization. Its application in normal and pathological tissue structures will assess the patho-diagnostic value of this new method in the near future.

[Apud-cell tumors (carcinoids)--immunohistochemical and pathomorphologic results].

47 carcinoid tumours originating from different sites were classified on a histological basis and immunohistochemically investigated by means of a modified PAP method using 10 different antisera. The biological behaviour and the extent of anaplasia were correlated. The results were compared with those described in the literature. 19 carcinoids showed a positive immune reaction with the antisera used. 5 hormone-producing tumours were poorly differentiated and in 9 cases metastases were recognized. More than one peptide hormone was detected in 7 carcinoids, 3 of which were malignant. Metastases or invasion of lymph and blood vessels were observed in 9 cases although the tumour cells showed no greater degree of cellular atypia. A good correlation between biological behaviour and cytological features was obtained in only 5 atypical carcinoids. Carcinoid tumours represent a group with complex biological, histological and immunohistochemical properties. The secreted peptide hormones might be of importance as tumour markers in some cases.

[Prostacyclin therapy in EPH gestosis (preliminary report)].

EPH-gestosis is still today one of the most dangerous complications of pregnancy, threatening both, maternal and fetal life. Until now only symptomatic treatment was possible. A new mode of treatment of EPH-gestosis may evolve from the use of prostacyclin, a potent vasodilator and inhibitor of platelet aggregation. We tried prostacyclin therapy in three patients with severe gestosis. The results of prostacyclin infusion on this condition, the pregnancy, the fetus, the placenta and the morphology of the umbilical artery are discussed.

[Adenocarcinoma of the larynx as a recurrence of an irradiated squamous cell carcinoma].

In a 55-year old male patient who underwent radiotherapy because of a low-differentiated carcinoma of the squamous epithelium, a highly differentiated adenocarcinoma developed at the same site two years after a successful treatment of the original tumour. This extraordinary case forms the basis for a brief discussion of the histogenesis of adenocarcinoma of the larynx, followed by a discussion of the significance of radiotherapy in cases of malignant tumours of the larynx.

[Prostacyclin synthesis in human umbilical vessels].

Umbilical arteries exhibit the highest capacity to synthesize PGI2 of all human blood vessels. This capacity is not only severely suppressed in adults, but also in umbilical arteries in mothers suffering from diseases accompanied by vessel wall alterations such as diabetes mellitus, hypertension and with smoking, for example. In parallel, severe morphological changes of endothelium and smooth muscle cells of umbilical arteries can be observed. It is assumed that a decrease in PGI2 formation might cause a disturbance in local haemodynamics.
