[Primary liver carcinoma. Results of 268 autopsies].

268 patients with primary liver cancer have been autopsied: 81% had hepatomas, 14% cholangiocellular carcinomas, 5% mixed carcinomas (hepatocholangiocellular) and 1% had mixed tumors (hepatoma and angiosarcoma). In contrast to cholangiomas, hepatomas were found more often in males than in females. The age peak of primary liver cancer was between 61 and 70 years. 79% of hepatomas developed in a cirrhotic liver, 71% of cholangiomas were without cirrhosis of the liver. Metastases were found in 66-67% of hepatomas and cholangiocellular carcinomas, especially in blood vessels, lymph nodes, lungs and within the liver.

[Ischaemic colitis (author's transl)].

Attention must be drawn to the fact that especially in elderly patients with acute onset of abdominal pain concomitant with haemorrhagic, mucous diarrhoea, ischaemic colitis must be considered in the differential diagnosis. Ischaemic colitis arises as a consequence of insufficient blood flow through the peripheral branches of the middle colic artery and inferior mesenteric artery and mostly strikes the left colonic flexure, the colon descendens or sigmoid. Because of the segmental involvement and the macroscopic appearance on X-ray and endoscopy the disease is sometimes indistinguishable from Crohn's disease or ulcerative colitis. In every day clinical life ischaemic colitis is a relatively rare phenomenon, in spite of the high incidence of arterial occlusive disease in the patients treated at this unit. The increasing frequency of ulcerative colitis in elderly patients, as reported in the literature, may be partly associated with unidentified cases of ischaemic colitis and this possibility is discussed.

[Hypercalcemia in liver tumors (author's transl)].

In 32 patients with primary liver tumors incidence and etiology of hypercalcemia were evaluated. Two patients revealed hypercalcemia from osteolytic metastases, one had coexistent primary hyperparathyroidism and in one patient hypercalcemia was paraneoplastic. The last two patients are presented in detail by case reports. Differential diagnostic problems appear often in patients in bad general condition, since invasive diagnostic methods are limited. The most potent substance to treat acute hypercalcemic syndrome is mithramycin. In those cases where surgical treatment is possible, complete resection of the primary liver tumor or the adenoma of the parathyroid gland is recommended.

Morphological changes in the umbilical arteries of babies born to pre-eclamptic mothers: an ultrastructural study.

The umbilical arteries of babies from ten normal pregnancies and from ten babies born to pre-eclamptic mothers were studied. Ultrastructurally, there were marked abnormalities of umbilical arterial morphology in the pre-eclamptic group. A loss of intercellular junctions, distension of the endoplasmic reticulum and an increase in cell organelles in the endothelial cells leading to a partial de-endothelialization was noted. Smooth muscle cells showed the morphological features of metabolic activation. These ultrastructural findings can be attributed to fetal hypoxia and to disturbances in cellular lipid metabolism; they suggest that there may be an abnormal fetal arterial blood flow in maternal pre-eclampsia.

Kawan bronchial brush/cell block technique. Facilitation of the routine diagnosis of bronchial neoplasms.

OBJECTIVE: To evaluate cell block preparations as a tool for the diagnosis of pulmonary malignancy and to correlate the findings with those from brush cytology and histology from forceps biopsy. STUDY DESIGN: One hundred twenty consecutive samples from patients with primary or metastatic lung cancer were routinely processed and stained for cytologic and histologic examination. In addition to smears, a cell block was prepared from each brushing sample. Brush smears, cell blocks and biopsy specimens were compared and evaluated for their diagnostic accuracy. RESULTS: Brush cytology samples were interpreted as positive for malignancy in all 120 cases. In 42 cases immunohistochemistry performed on cell blocks led to the final precise type diagnosis. Owing to technical problems (stenosis, bleeding, peripheral location), forceps biopsy specimens were obtained from only 51 patients. CONCLUSION: In addition to lower patient risk, cell block preparation yields high diagnostic accuracy and may thus be considered an improvement in quality assurance.

Aneuploid and polyploid cellular DNA heterogeneity in insect cell material of diptera species analyzed by flow cytometry.

Flow cytometric 1-parameter DNA analysis and 2-parameter DNA/protein analysis have been performed with cell material of the diptera species Chironomus thummi, Drosophila melanogaster, Calliphora vicina and Musca domestica using an impulse cytophotometer with a new quartz objective, that was especially manufactured for cytofluorometric investigations. The occurrence of heterogenous cell populations with aneuploid and polyploid DNA content within the cell material of different developmental stages of diptera species have been determined, whereby in larvae polyploid cell populations and in imagos aneuploid cell populations predominate. Partially separation of 2 C cells from other cell populations with higher DNA content can be done by Ficoll-Hypaque centrifugation as demonstrated with cell material from Chironomus larvae. For flow cytometric DNA analysis of insect cell material a simple and rapid cell preparation and staining technique is presented by using the DNA-specific fluorochrome DAPI in combination with the protein fluorochrome sulforhodamine 101. Employment of flow cytometry in diptera genetics might be a new tool for cytological and cytogenetic investigations as shown with the classical genetic objects Chironomus and Drosophila.
