The neurosurgical management of brain metastasis from colorectal cancer: a report of three cases.

We report herein three cases of brain metastasis from primary colorectal cancer in which the metastatic lesion was resected to effectively relieve neurological symptoms. Case 1 was a 61-year-old woman with a solitary brain metastasis from colorectal cancer and no other metastases; case 2 was a 59-year-old woman who died from liver metastasis 11 months after resection of the metastatic brain tumor; and case 3 was a 68-year-old woman with multiple metastases to the lungs and bones detected before the brain metastasis. According to 16 cases previously reported in the Japanese literature and our 3 cases, the interval between diagnosis of the primary cancer and discovery of brain metastasis was 23 months on average, while the median survival after the discovery of brain metastasis was 7 months. Brain metastases with liver and lung metastases were seen more frequently than brain metastases alone. In these three cases, chemotherapy appears to have been of no use in preventing recurrence. Thus, we believe neurosurgical management to be appropriate for a solitary lesion and that it should be actively pursued to prolong survival and improve quality of life.

Surfactant apoprotein A secretion by human tracheobronchial epithelial cells.

To determine whether surfactant apoproteins are produced locally by the airway walls, we used a two-site simultaneous immunoassay with monoclonal antibodies and measured the surfactant apoprotein-A (SP-A) content in media cultured with human airway explants or cultured epithelial cells. Tracheobronchial explants were cultured for 2 successive periods (periods I and II). Significant SP-A concentrations were detected in both periods. Methacholine (MCh) or isoproterenol (ISP), added to the medium at the beginning of period II, reduced the ratio of SP-A concentration in period II to that in period I, compared to samples without treatment. The SP-A concentration in the medium at the confluent period of cultured epithelial cells was significantly higher than at the mid-period, indicating that SP-A secretion is dependent on the cell number. The supernatant from explants stimulated by MCh was capable of reducing SP-A secretion from cultured epithelial cells. Immunohistochemical study of SP-A using monoclonal antibody demonstrated positive immunoperoxidase staining in the cytoplasm of epithelial cells. Further, Western blots of electrophoresed proteins from epithelial cells showed the characteristic properties of SP-A. These findings indicate that tracheobronchial epithelium can secrete SP-A.

Morphometric analysis of airways in Sjögren's syndrome.

Sjögren's syndrome is characterized by a decrease in exocrine gland secretion and the destruction of secretory tissue and cells, that is, salivary and lacrimal glands. Although patients with Sjögren's syndrome have chronic cough and sometimes sputum, we have little knowledge concerning the morphology of their airways. We performed morphometric analysis of goblet cells and bronchial glands and compared them with intraluminal mucus volume by measuring the mucus-occupying ratio (MOR) in the airways of patients with Sjögren's syndrome. A total of 6 nonsmoking patients with Sjögren's syndrome who had no significant prolonged airway infections (Group SG) (6 women, 55 +/- 2 yr) were compared with four control patients with no pulmonary diseases (Group NL) (1 woman and 3 men, 64 +/- 2 yr) and five patients with chronic bronchitis (Group CB) (1 woman and 4 men, 52 +/- 5 yr). Autopsied lungs from 15 patients were used for morphometry by a digitalizing computer. Area proportion of bronchial glands (gland %) to bronchial wall, goblet cell-occupying ratio to total epithelial layer (goblet %), and MOR were measured in central (3-8 mm in diameter) and peripheral airways (2 mm or less in diameter). Gland % and goblet % in central and MOR in peripheral airways from Group SG were significantly larger than those from Group NL: 15 +/- 2 versus 6 +/- 1% (p < 0.01), 6 +/- 1 versus 2 +/- 1% (p < 0.05), and 9 +/- 2 versus 0.6 +/- 0.3% (p < 0.05), respectively. Further, these values from Group SG did not significantly differ from those from Group CB.(ABSTRACT TRUNCATED AT 250 WORDS)

Morphometric analysis of airways in idiopathic pulmonary fibrosis patients with mucous hypersecretion.

Five idiopathic pulmonary fibrosis (IPF) patients with sputum since the initial period of the disease (IPF SP+, more than 15 ml/day) were compared with five IPF patients without sputum throughout the course of the disease (IPF SP-) and four control subjects without pulmonary disease matched for age and sex. No significant differences in the duration of symptoms, pulmonary functions, or glucocorticoid therapy were observed between the two IPF groups. Autopsied lungs fixed by immersion into formaldehyde were used for morphometry by digitizing computer. The volume proportion of glands to bronchial wall thickness (gland%), volume proportion of goblet cells to total epithelial layer (goblet%), and luminal mucous volume were measured in central and peripheral airways. The gland percentage in the central airways of the IPF SP+ group was 18 +/- 1% (mean +/- SE), which was significantly greater than 7 +/- 0.6% of the IPF SP- group (p less than 0.001), similar to the 6 +/- 1% of control subjects. Luminal mucous volume in the peripheral airways of the IPF SP+ group was 11 +/- 2%, which was significantly greater than 3 +/- 1% of the IPF SP- group (p less than 0.05) or 0.6 +/- 0.3% of the control subjects (p less than 0.01). Furthermore, luminal mucous volume in both the central and peripheral airways significantly correlated with gland% (p less than 0.01, each). No significant difference in other parameters such as goblet% and cell infiltration between the IPF SP+ group and IPF SP- group was observed. These findings suggest that IPF with hypersecretion is associated with mucous glandular hypertrophy and the accumulation of mucus in the airways.

Perivascular fibrosis of muscular pulmonary arteries in chronic obstructive pulmonary disease.

We performed a morphometric analysis of peribronchiolar and perivascular fibrosis in lungs obtained at autopsy from six patients with chronic bronchitis, six with pulmonary emphysema, and four normal control subjects. The areas of fibrosis outside the smooth muscle layer of bronchioles and outside the external elastic lamina of muscular pulmonary arteries were measured and their thickness was then calculated by assuming a round airway or artery. Patients with chronic bronchitis had significantly thicker peribronchiolar fibrosis in bronchioles of 1 mm or less in diameter and also thicker perivascular fibrosis of the adjacent muscular pulmonary arteries than the other two groups. The extent of perivascular fibrosis was significantly correlated with peribronchiolar fibrosis only in the muscular pulmonary arteries adjacent to the bronchioles but not in those away from the bronchioles. These findings suggest direct extension of chronic inflammation from bronchioles to the adjacent muscular pulmonary arteries in chronic bronchitis but not in pulmonary emphysema. Such perivascular fibrosis might lead to sustained pulmonary hypertension.

[Brachytherapy of prostatic carcinoma, 60Co high dose rate, remote after-loading methods].

Six patients with prostatic carcinoma, including 2 with Stage B, 2 with Stage C, 2 with Stage D1, received transurethral irradiation (TUI) using 60Co high dose rate, remote afterloading methods, in combination with external irradiation by 4 MV X-ray. Total tumor doses were 97.0 to 129.3 in TDF and needle biopsy performed 3 mo after the treatment proved to be free of cancer cell in 5 patients and almost no viable cancer cell in one patient. No adverse reactions were encountered in any patient. We believe that TUI is a useful treatment for prostatic carcinoma.

[Improvement of plaque score after oral hygiene instruction in patients with periodontitis].

The purpose of this study was to investigate the variations in oral hygiene conditions after oral hygiene instruction. We divided the patients with periodontitis into three groups according to the number of times oral hygiene instruction had to be given to achieve the O'Leary plaque control record (PCR) of 20%. The first group was those who achieved PCR 20% quickly (early achievement group). The second group was those who achieved PCR 20% gradually but slowly (slow achievement group). And the third group was those who showed no progress at all (non achievement group). Results showed that there were statistical differences among the average changes in PCR and residual plaque score (PS) of the teeth surfaces in each group. Especially the early achievement group were significantly superior to the other groups in improvement of PS of mandibular lingual surface with one oral hygiene instruction. We also have investigated differences in the probing depth, age and sex at initial treatment among these three groups. The average probing depth at initial treatment was significantly deeper in the non achievement group than in the early achievement groups, and there were more males than females in the early achievement and non achievement groups.

Immediate elimination of labile HbA1c with allosteric effectors of hemoglobin.

Our aim was to find a simple method of removing labile glycosylated hemoglobin (HbA1c) from blood samples before it is measured by cation-exchange chromatography. Labile HbA1c is formed by the binding of glucose to the NH2-terminal valine of the beta-chain of HbA. We sought a more competitive binder for the same site to dissociate labile HbA1c to glucose and HbA. Inorganic phosphates were found to have a strong allosteric effect and a great ability to eliminate labile HbA1c. We developed our method with 4 mM tetrapolyphosphate in the hemolyzing solution to eliminate labile HbA1c during the automatic processing (at pH 6 and heated for 2 min at 45 degrees C) of blood samples for HbA1c estimation. This may be useful when estimating HbA1c by the manual method.

Choroid plexus arteriovenous malformation in a full-term neonate. Case report.

A full-term newborn infant with intracranial hemorrhage secondary to an arteriovenous malformation (AVM) of the choroid plexus is presented. Although angiography was not undertaken before surgery, a vascular nodule containing an AVM nidus was found in the hematoma wall. The authors stress that an AVM should be considered as a cause of intracranial hemorrhage and that the hematoma wall be searched thoroughly in neonates without predisposing factors.

Clinical evaluation of the sedative effect of HEMA solution on the hypersensitivity of dentin.

The sedative effect of the dentin primer, 35% hydroxyethyl methacrylate (HEMA) solution, on dentin hypersensitivity was clinically evaluated. Exposed root surfaces of teeth were cleaned with neutralized 0.5 mol/L EDTA and treated with 35% HEMA solution followed by the application of a commercial dentin bonding agent. The extent of sedation was estimated by comparison of the responses before and after treatment to irritation by a cold water spray, compressed air blast and scratching with an explorer. The degree of the response to these irritations was decreased significantly upon the combined application of HEMA and commercially available dentin bonding agents though it was still ineffective in nearly 30% of the 48 cases tested.

[Laboratory and clinical studies on cefdinir in pediatric field].

Clinical trials of cefdinir (CFDN) in pediatric infections were carried out. Results are summarized as follows. 1. Mean half-lives of CFDN in serum in children when administered on an empty stomach were 1.24 hours (3 mg/kg per os) and 1.85 hours (6 mg/kg per os). 2. Mean 8 hour urinary excretion rates of CFDN were 19.0% (3 mg/kg/per os) and 10.5% (6 mg/kg per os). 3. CFDN was administered to 28 children with various infections: 12 patients with tonsillitis, 8 with bronchitis, 2 with pneumonia, 4 with urinary tract infections, 1 staphylococcal scalded skin syndrome and 1 with impetigo. The overall efficacy rate was 89.3%. 4. Diarrhea was noted in 1 patient. Abnormal laboratory test values encountered were eosinophilia in 2 patients, thrombocytosis in 1.

A new ultrasound-guided brain biopsy technique through a burr hole. Technical note.

There is a disadvantage in brain biopsy by ultrasonic guidance comparing it with the CT guided approach. It is the fact that the ultrasonic method requires a small craniotomy or multiple trephinations because the size of the transducer exceeds the diameter of the burr hole and the echographic information from the trocar which houses the biopsy forceps, becomes poor when the sound beams run nearly parallel with the site of its insertion. The authors devised a brain biopsy technique through only one burr hole under real time monitoring, using a small foot-print transducer, 12 mm in diameter, and a special trocar with engraved scales on its surface. We believe that this new biopsy technique surpasses the traditional CT directed and ultrasound-guided methods in many respects. A representation case biopsied by this technique is presented.

Huge pineal teratoma mimicking a dermoid cyst. Case report.

A case of huge pineal teratoma mimicking a dermoid cyst is reported. Although both computed tomography and surgical findings were entirely similar to those of a dermoid cyst, close histological examinations using the serial sectioning technique of a small solid portion attached to the pineal region revealed tissue components of both mesodermal and entodermal origins among mostly epidermal and dermal components. The importance of serial sectioning of the surgical specimens of seemingly a dermoid cyst is stressed.

[Lobar intracerebral hemorrhage secondary to cerebral amyloid angiopathy: a clinicopathologic study of three operated cases].

Three operated cases of lobar intracerebral hemorrhage (LICH) related to cerebral amyloid angiopathy (CAA) were studied clinicopathologically. They constituted about 8% of all LICH cases (n = 37) operated upon in our institute (DUSM) during the past 3 years. Case 1, 2 and 3 aged 71, 67 and 73 years, respectively. There were 2 males (Cases 1 & 2) and 1 female (Case 3). Only one case (Case 3) had both hypertension and dementia before hemorrhage. In all 3 cases, neurologic symptoms deteriorated after admission. The hematoma involved the right temporo-parietal in 1 (Case 1), the right parieto-occipital in 1 (Case 2) and the left fronto-parietal region in 1 (Case 3). Case 1 developed a new hematoma in the right occipital lobe on the day following surgery. On CT, the hematoma was multilobular in shape and located very superficially extending to the subarachnoid space in all cases. There was no abnormal enhancement in and around the hematoma upon contrast infusion. Angiography showed only an avascular mass sign in case. At surgery, the hematoma was extruded onto the cortical surface in all cases. The surgical outcome was good in 2 (Cases 1 & 2) and fair in 1 (Case 3). Removed hematomas, solid nodular tissues and adjacent brain tissues were examined histologically using hematoxylin and eosin, Azan-Mallory, elastica van Gieson, silver and Congo red stains. Arteries in the hematoma wall, the subarachnoid space and the adjacent brain parenchyma were intensely stained with Congo red and showed birefringence on polarized light.(ABSTRACT TRUNCATED AT 250 WORDS)

Glycated hemoglobin in fractionated erythrocytes.

Erythrocytes were fractionated, by centrifugation, on the basis of cell density, which is proportional to cell age. Glycated hemoglobin (Hb A1c) in the fractionated cells was measured and compared with the Hb A1c in whole blood and with the fructosamine concentrations in plasma of normal and diabetic subjects. In normal subjects, Hb A1c increased according to the age of the erythrocytes, but hyperbolically rather than linearly, indicating the slow conversion of Hb A to Hb A1c with saturability. The greatest correlation of Hb A1c with plasma fructosamine was in younger middle-aged cells. A subgroup of subjects with impaired glucose tolerance had high Hb A1c in older cells. Hb A1c of the young cells reflects recent glycemic status, for monitoring the initial stage of the treatment of diabetics, whereas Hb A1c of the older middle-aged cells more specifically reflects the longer term glycemic status, for screening and for the long-term monitoring index for treatment of diabetes.
