[Omental metastasis in primary ovarian cancer].

Omentectomy was performed in 74 patients with primary cancer of the ovary. Omental metastasis was found in 60.8% (45 cases). The highest rates of omental metastasis were observed in serous carcinoma (56%) and endodermal sinus tumor (67%). Omental metastasis was correlated to types of tumor, amount of ascitic fluid, survival rate and extent of resection of omentum. The authors believe that omentectomy should routinely carried out in patients with primary ovarian cancer.

Cyclosporin A inhibits activation-induced cell death in T-cell hybridomas and thymocytes.

One mechanism by which the immune system develops the ability to discriminate self from nonself is the deletion of autoreactive T-cell clones during thymic maturation. The drug cyclosporin A (CsA) has been shown to interfere with this process, allowing the escape of normally 'forbidden' T-cell clones and the appearance of autoimmune disease. Recently, it has been demonstrated that immature thymocytes undergo programmed cell death (apoptosis) upon activation via the T-cell receptor. A similar phenomenon of activation-induced cell death (AICD) has been observed in T-cell hybridomas. Here we show that AICD in T-cell hybridomas in vitro and in thymocytes in vivo is blocked by CsA. Thus, clonal deletion may involve AICD when self-reactive, immature T cells are induced by self antigen, and CsA may cause autoimmunity by interfering with this process.

Kinetics of murine delayed-type hypersensitivity response to Eimeria falciformis (Apicomplexa: Eimeriidae).

Mice recovering from a primary infection with an intestinal protozoan parasite, Eimeria falciformis (Apicomplexa: Eimeriidae), showed a classic delayed-type hypersensitivity (DTH) reaction to oocyst antigen challenge. This reaction was characterized by a biphasic pattern of footpad swelling. The first swelling peaked at 2 h after antigen challenge, whereas the second swelling peaked at 24 to 48 h after challenge. The DTH reaction was transferable with a T-cell-enriched spleen cell population from mice that had recovered from E. falciformis infection. Cytotoxic depletion of immune T cells with anti-L3T4 antibody and complement abrogated DTH transfer, indicating that L3T4-positive T cells were required. A T-cell-enriched spleen cell population from acutely infected mice suppressed the transfer of DTH with immune cells from recovered animals, implicating the existence of infection-induced immunoregulatory cells controlling the parasite-specific immune response during infection. Immune spleen cells also transferred resistance to infection as measured by oocyst production and death rate of recipients. Together, these results indicate that the DTH reaction, induced by infection with E. falciformis, is mediated by L3T4-positive T cells and is associated with resistance to infection.

[Effect of testosterone preparations on the function of the hypothalamic pituitary axis in Klinefelter's syndrome].

This article prospectively observed the suppressive effects of the testosterone preparations on the elevated gonadotropin levels in Klinefelter's syndrome (KS). After sublingual administration of methyl testosterone 5 mg Bid, the serum T levels increased 97 and 126 ng/ml respectively in 6 normal men and 9 patients of KS and the serum FSH and LH levels did not changed significantly. After intramuscular injection of testosterone propionate 25 mg 3/week testovirone 100 mg 1/10 days and testosterone enanthate 200 mg/20 days, the serum T levels increased significantly both in 6 normal mon and 17 patients with KS. It took about 1 month for the LH levels to decrease to normal range and 3-6 months for the FSH levels to become normal. Our result indicates that it is necessary to treat the KS patients for long period to normalize the increased hypothalamic pituitary function.

[The clinical manifestations and endocrine changes of anorexia nervosa].

We analyzed the clinical manifestations and endocrine changes of 15 cases of anorexia nervosa. One male, aged 42, and 14 females, aged 14-26 were involved in the study. The precipitating causes were chronic psychiatric trauma (60%), strain (27%) and abnormal fear of being fat (13%). All showed clinical manifestations of hypometabolism and malnutrition. Body weight decreased by 36.3 +/- 11.6% on average and 93% of the patients had lost more than 20% of their original weight. All women patients over 16 years old had amenorrhea. The daily intake of grain was less than 150 gm and had decreased by more than 67% as compared with earlier intakes. Although the patients were very thin, 3/4 could maintain ordinary indoor activities. The patients with anorexia nervosa had obvious endocrine disturbances, such as low serum FSH, LH and Ts levels and high basal serum GH and midnight serum cortisol concentrations. The response of TSH to TRH and of FSH and LH to LHRH were delayed, but serum GH and PRL levels rose normally after stimulation. All of these changes are secondary to severe body weight loss or hypofunction of the hypothalamus.

[Ectopic ACTH syndrome caused by bronchial carcinoid].

A 35-year male patient with clinical and biochemical manifestations of severe hypercorticism was thought to have Cushing's syndrome of pituitary origin. However, the surgically removed pituitary adenoma was not confirmed by pathological examination. There was no improvement after transsphenoidal microsurgery Chest CT scan showed a small mass located at the upper-lobe of the right lung. This tumor (1.8 x 1.0 x 1.0 cm) was removed and one course of radiotherapy with linear accelerator was given. Remission was achieved clinically and biochemically after these therapies. The diagnosis of bronchial carcinoid was confirmed by pathological findings. Metastasis of a lymph node was also proved. The tumor cells was found to contain ACTH and related peptides with radioimmunoassay, immunocytochemistry and electron microscopy. Gel filtration of the tumor extracts showed molecular heterogeniety of ACTH related peptides and that the big-form were in large percentage.

Reduction of the effects of growth hormone release inhibiting factor enhances plasma growth hormone response to GHRH.

Responses of serum growth hormone (hGH) to glucagon (G), growth hormone releasing hormone (GHRH) and G/GHRH were measured in 8 normal adults and 6 patients with growth hormone deficiency (GHD). In normal adults, serum hGH reached its peak value (12.7 +/- 1.6 ng/ml) at 150 +/- 10 min, as blood glucose declined to its minimum after a transitory hyperglycemia in G test. The normal adults were responsive to GHRH test (GH peak 14.7 +/- 2.3 ng/ml at 30 +/- 0 min). In GHD, the responders to both G and GHRH tests showed a strongly positive response in G/GHRH test, with a serum hGH peak value of 34.6 +/- 4.1 ng/ml at 131 +/- 8 min being much higher than that of either single G or GHRH test (P less than 0.01), but without significant difference to the sum of the two single tests (P greater than 0.10). Among GHD patients, only 2 responded to GHRH and G/GHRH tests with hGH peak values 6.8 +/- 0.7 and 6.9 +/- 0.7 ng/ml at 45 +/- 15 and 90 +/- 0 min, respectively, both peak values being essentially similar (P greater than 0.10). We suggest that the mechanism of stimulation of pituitary hGH secretion in G test might involve inhibition of release of hypothalamic GH release inhibiting factor (GHRIF) caused by hypoglycemia after a transitory hyperglycemia following G injection. These results may further confirm our previous postulation (1986) that insulin hypoglycemia may increase hGH release by inhibiting hypothalamic cell secretion of GH release inhibiting factor.

Evaluation of hypothalamic dopaminergic function in patients with pituitary prolactinoma.

This study was carried out using a dopaminergic agonist (carbidopa plus levodopa, CD + LD) and antagonist (metoclopramide, MCP) respectively for dynamic tests to observe the variations of serum prolactin (PRL), thyrotropin (TSH) and luteinizing hormone (LH) levels in 7 normal women and 11 women with pituitary prolactinoma. It was shown that CD + LD resulted in minimal suppression of serum PRL (18.4 +/- 3.4%) in tumor patients, with this being significantly less than that in normal women (80.7 +/- 4.6%). However, similar degrees of TSH and LH suppression were observed after CD + LD in patients (23.8 +/- 4.2% and 28.2 +/- 2.1%, respectively) and in normal women (27.9 +/- 2.4% and 34.7 +/- 9.0%, respectively). MCP greatly increased PRL levels in the normal women as compared with the patients (892.1 +/- 195.3%, 16.4 +/- 6.5%), but increased TSH and LH levels were much higher in the patients than in the normal women (291.4 +/- 36.1% vs 19.9 +/- 3.3% and 96.9 +/- 7.4% vs 24.9 +/- 5.5%, respectively). It was also found that the levels of TSH or LH after MCP strongly correlated with basal PRL levels in the patients (r = 0.858, P less than 0.001 and r = 0.737, P less than 0.01, respectively). These results indicate that synthesis, turnover and release of hypothalamic dopamine are normal and the hypothalamic tone is relatively high in patients with PRL-secreting pituitary tumors.

Murine major histocompatibility complex and immune response to Eimeria falciformis.

The genetics of the immune response to Eimeria falciformis were investigated in three inbred and six congenic strains of mice. There were significant differences among strains in oocyst production and age-related mortality from parasitic infection. Genes within the H-2 complex and also non-H-2 genes share in the immune response to eimerian infection.

Chlamydial infection in a Chinese gynecologic outpatient clinic.

In an attempt to detect the presence, if any, of cervical chlamydial infection, we obtained endocervical smears from 1000 female patients attending the outpatient gynecologic and family planning clinics at Women's Hospital, Zheijiang Medical University, Hangzhou, People's Republic of China. Using direct immunofluorescent monoclonal antibody staining technique, we identified elementary bodies typical of the Chlamydia trachomatis organism in ten of the 1000 slides (1%). The only characteristic of statistical significance in this small group of infected women was the complaint of infertility for more than two years' duration, noted in six of the ten cases.
