INTRODUCTION: Acute ileum intussusception is a frequent and mostly benign condition in childhood. Conversely, it is a rare condition during adulthood and generally associated with an underlying malignancy. We report a familial form of benign inflammatory fibroid polyps, revealed by an acute ileum intussusception. EXEGESIS: A 41-year-old man, whose mother had undergone three surgical procedures for acute ileum intussusception associated with inflammatory fibroid polyp, was admitted for a abdominal pain that started three month ago. The patient displayed alteration of the intestinal transit, weight loss and sub-occlusive syndrome. Upper and lower endoscopies, performed before admission, were normal. In the emergency room, the abdominal computed tomography-scan revealed an acute intussusception of the last loop of the small intestine, probably caused by a tumor and leading to an occlusive syndrome. Surgical resection and histological analysis concluded to an inflammatory fibroid polyp. Clinical outcome was excellent. CONCLUSION: Inflammatory fibroid polyp is always a benign tumor. It is usually isolated, expressing itself mainly in the form of an acute intussusception when located in the small bowel. The familial form presented here is exceptional and illustrates the possibility of an inherited transmission of this disease. However the pathogenesis of this type of polyp is still unclear.
Idiopathic retroperitoneal fibrosis is a rare cause of venous occlusion that is mainly treated conservatively or surgically. However, endovascular techniques can be proposed for its treatment. We report here a case of a 64-year-old man who was admitted for bilateral lower limb swelling due to iliocaval occlusion caused by idiopathic retroperitoneal fibrosis. Due to high anesthetic risk, related mainly to congestive heart failure, the patient was treated percutaneously by balloon angioplasty and stenting instead of receiving surgical treatment. Clinical and radiological results were excellent, both early and in midterm, confirming that endovascular techniques constitute an interesting alternative to surgery, especially in fragile patients.
UNLABELLED: Ultrasonography Doppler in the assessment of arterial peripheral circulation. PURPOSE: The ultrasonography-Doppler study of peripheral arterial circulation requires measurements of arterial area and arterial blood flow. The aim of this study was to assess their variabilities, intra- and interobserver. METHODS: We studied brachial arterial circulation of 13 volunteers. Two experimented investigators carried out the examinations. Arterial blood pressure measurements were obtained at the level of the brachial artery of the left arm by means of a semi automated oscillometric device. The brachial artery was approached by ultrasound scan in semi-decubitus position, the arm in exterior rotation and abduction. Brachial artery cross-sectional areas were obtained by a transversal view with a depth of 4-15-cm upstream the antecubital fossa of the forearm. Cross sectional areas were measured at end diastole (SD) and end systole (SS). We measured maximal systolic and diastolic velocities (Vmax and Vmin), mean velocity (Vmoy) and velocity integral (ITV) of the blood flow. We deduced brachial output (D), resistance and pulsatility index (IR, IP), cross-sectional compliance coefficient (C). RESULTS: Intra- and interoperator variability as assessed by the Bland-Altman approach was acceptable SS, SD, ITV, D, Vmax, Vmoy, Vmin C, IR, and IP, in regard with values of variation coefficient, biais, and concordance limits. CONCLUSION: Conventional B-mode ultrasonography may offer a reliable method for arterial peripheral circulation evaluation.

To determine the influence of a saturation dive on cardiac function, Doppler-echocardiographic measurements were compared at sea level and during a 36 atm (3,650 kPa) He-O2 dive (gas density: 7 g/liter) in four healthy men. Left ventricular systolic function was studied from time motion measurements. Transmitral flow (E:A ratio) and isovolumetric relaxation time were used to assess left ventricular diastolic function. Cardiac output was derived from systolic pulmonary and aortic valvular flows. Cardiac output decreased 4.4 +/- 0.8 vs. 5.9 +/- 1.2 liter/min at sea level) whereas stroke volume, left ventricular ejection fraction, atria and ventricular diameters remained unchanged. Thus, the decrease in cardiac output was attributed to bradycardia (56 +/- 8 vs. 73 +/- 9 beats/min at sea level) which probably resulted from the slight hyperoxia (PI(O2), 0.4 atm). We found no evidence of left ventricular diastolic dysfunction. nor did we find valvular regurgitation or pulmonary hypertension. We conclude that Doppler-echocardiography can be used safely to investigate cardiac function during human saturation dives. Our results suggest that a 36 atm He-O2 dive does not modify cardiac or systolic and diastolic function except for a slight decrease in cardiac output correlated to bradycardia.
Vertebrobasilar-distribution stroke is a rare but sometimes severe complication of chiropractic neck manipulation. We report two patients with dissections of the vertebral arteries authenticated two and six days after the cervical manipulation. In the first case, a Wallenberg's syndrome occurred due to a dissection of the right intracranial vertebral artery; the patient was treated with anticoagulant therapy but little improvement of the disorder was noted. The second patient had transitory neurologic manifestations which led to the discovery of an intimal tear of the ostium of the right vertebral artery with a floating clot. Further embolic complications were avoided by performing a venous bypass between the right common carotid and the vertebral artery at the base of the skull. Therapists should be aware of vertebrobasilar complications after spinal manipulations and should ask for early explorations (brain CT, cerebral angiography) to institute rapidly the most appropriate treatment.

1. This placebo-controlled study assessed once daily sustained-release (SR) diltiazem, 200 and 300 mg, in 182 stable angina patients with positive exercise test. 2. Exercise testing was performed at baseline after a 7 day placebo run-in period, and repeated after 7 days of treatment, 25.0 +/- 0.1 h postdose. 3. Diltiazem (200 and 300 mg) produced respectively a 68% and a 64% decrease in weekly angina episodes, and placebo a 15% decrease (P < 0.05). Similarly, both dose levels produced a 70% decrease in nitroglycerin consumption, whereas no difference was obtained with placebo (P < 0.01). The increase in time to ischaemic threshold was significantly superior for 200 mg and 300 mg diltiazem when compared with placebo (75.2 and 91.5 s respectively vs 47.0 s) (P < 0.05); increase in time to anginal threshold was also significantly greater for diltiazem when compared with placebo (84.6 and 85.9 s respectively vs 43.9 s) (P < 0.05). 4. Only one patient experienced worsening of angina and had to be withdrawn from the study. 5. This study demonstrates 200 and 300 mg SR diltiazem is effective when given once-a-day in the prophylaxis of stable exertional angina. This once daily formulation should improve patients' compliance and comfort.
