Diagnosis of epidermoid cysts by metrizamide CT cisternography.

The authors describe three cases of intracranial epidermoid cysts specifically diagnosed by metrizamide CT cisternography. In CT cisternography of epidermoid cysts, metrizamide enters deep in the tumor clefts and depicts the lobulated margin of the tumor. We consider these findings are specific for this tumor and similar to the findings reported previously in pneumoencephalography or positive contrast cisternography.

Detection of bovine coronavirus in feces by reversed passive hemagglutination.

A reversed passive hemagglutination (RPHA) method was developed for the detection of bovine coronavirus in fecal specimens. Sheep erythrocytes fixed with glutaraldehyde, and then treated with tannic acid were coated with anti-bovine coronavirus rabbit antibodies purified by affinity chromatography using bovine coronavirus linked to Sepharose 4B. The RPHA test was carried out by a microtiter method. Erythrocytes coated with purified specific antibodies were agglutinated by bovine coronavirus, but not by bovine rotavirus or enterovirus. The reaction was inhibited by antiserum to bovine coronavirus, confirming the specificity of the reaction. The RPHA test detected bovine coronavirus in 13 of 22 fecal specimens (59 per cent), from natural cases of diarrhea, while the positive rates were only 14 per cent (3/22) and 22 per cent (5/22) for immunofluorescent staining of primary cultures of calf kidney cells infected with the specimens, and immune electron microscopy respectively. The advantages of the RPHA method are its simplicity, high sensitivity and rapidity.

CT findings in patients with gliomas, surviving more than 10 years.

After reviewing the latest CT scans of 23 mostly low grade glioma patients who survived more than 10 years after treatment, it was suggested that loss of the original tumour bulk and frequently the presence of porencephalic cavity replacing the original tumour, with a smooth lining which is not contrast-enhanced is essential to assume a clinical "cure" in the case of intrinsic cerebral gliomas. In the case of gliomas growing within the ventricle or subarachnoid space, tissue with calcium deposit but without contrast-enhancement was noted, although the number of cases was limited.

A case-control study on liver cancer with special emphasis on the possible aetiological role of schistosomiasis.

In order to elucidate the relationship between liver cancer and suspected aetiological factors, a case-control study was carried out at seven hospitals in Yamanashi prefecture, where the mortality rate of the disease was significantly higher than the corresponding national figure. This prefecture was also recognised as one of the endemic areas for Schistosoma japonicum in Japan. Sixty-two cases of liver cancer, and the same number of controls were selected during the two-year study period. A skin test for Schistosoma japonicum, serum tests for HB virus and a questionnaire study were carried out. The results of matched pair analysis revealed that liver cancer was significantly highly correlated with three factors, that is, HBs antigen (HBsAg), history of schistosomiasis, and daily intake of alcohol, and that their odds ratios were 10.0, 9.5 and 3.2 respectively. The values of percentage population attributable risk for these three factors were estimated to be 21.3%, 71.8% and 39.8% respectively. The above findings suggest that HBsAg and history of schistosomiasis are both important risk factors for liver cancer and that schistosomiasis may be much more important than HB virus in the area. The analysis of combinations of these three factors suggest that each factor might be related to liver cancer multiplicatively rather than additively.

Moyamoya disease in children and its surgical treatment. Introduction of a new surgical procedure and its follow-up angiograms.

Moyamoya disease is an unusual form of chronic cerebrovascular occlusive disease characterized usually by bilateral stenosis of distal internal carotid arteries and their vicinity, by a hazy network of collateral circulation at the base of the brain called moyamoya vessels and clinically by recurring hemispheric ischemic attacks in children. This disease was first reported by a Japanese neurosurgeon and many reports and studies on this disease have been published in Japan. We report here the recent progress in the diagnosis of the disease and introduce a newly developed operative procedure which we think is an ideal surgical method for treating this disease in children.

A cohort study on the causes of death in an endemic area of schistosomiasis japonica in Japan.

In order to elucidate the chronic effects of schistosomiasis japonica, a retrospective cohort study was carried out. The mortality experience of 2,067 inhabitants over 30 years old in the endemic area of schistosomiasis in Yamanashi Prefecture were followed up from 1958 to 1982 concerning the underlying cause of death. Total person-years at risk were 18,694.1 for males and 21,524.9 for females. The observed-expected ratio (O/E) was significantly high for cirrhosis of the liver (4.05 for males, 5.53 for females), cancer of the liver (males only, 2.30) and cancer of the colon (females only, 2.25). Increasing O/E values were observed for cirrhosis of the liver and cancer of the colon (females only) in relation to duration of residence in the area before 1957. These results suggest that schistosomiasis japonica is one of the important risk factors for cirrhosis of the liver, cancer of the liver and cancer of the colon.

[A case of metrizamide encephalopathy with triphasic waves on EEG].

Metrizamide (Amipaque), a water soluble nonionic contrast medium has less toxic effect in comparison with other contrast media, and it is now widely used for myelography, cisternography, ventriculography and cerebrospinal fluid dynamic imaging. However, as the number of cases in which this medium has been utilized has gradually increased, incidents of toxic manifestations have been reported. Among these, there are a considerable number of case reports referring to metrizamide encephalopathy, but only a few authors reported the appearance of triphasic waves on EEG when they occurred. The authors experienced one case of metrizamide encephalopathy accompanied by frequent appearance of triphasic waves on EEG. A 31-year old male was admitted to our hospital with the complaint of right homonymous hemianopsia. At that time he was fully conscious and mentally alert. On CT, 39 mm X 45 mm partially enhanced isodense mass was revealed on the enlarged sella turcica. Laboratory findings showed high titer of prolactin (10200 ng/ml). Premedication of 100 mg phenobarbital i.m. was followed by the tomography of the sella turcica, using 8 ml of 250 mgI/ml metrizamide injected into L 3-L 4 subarachnoid space. Several hours after the examination, he complained of slight nausea and was kept in bed with his head placed in an elevated position. The next morning, he was found to be in a drowsy state. He was disoriented and could not respond adequately to questions asked. His naming of daily necessities was also poor, although he knew how to use them.(ABSTRACT TRUNCATED AT 250 WORDS)

Inducement of cytopathic changes and plaque formation by porcine haemagglutinating encephalomyelitis virus.

ESK cells were shown to be a good medium for propagating the 67N strain of porcine haemagglutinating encephalomyelitis virus, although no cytopathic effect was observed. The virus induced a readily recognizable cytopathic effect in ESK cells, when a noncytotoxic amount of diethylaminoethyl-dextran (DEAE-dextran) was incorporated in the culture medium. Based on this finding, a sensitive, practical assay method for the virus was developed. When DEAE-dextran was incorporated in the agar overlay medium, 67N virus formed plaques in ESK cell monolayers. The cytopathic effect as well as the plaque formation were specifically inhibited by antisera against the virus. Neutralization tests were developed on the basis of these findings. Neutralization and haemagglutination-inhibition tests on swine serum samples indicated a wide dissemination of haemagglutinating encephalomyelitis virus or antigenically-related viruses in Japanese pigs.

[Clinical study on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

A clinical investigation on the efficacy of T-1982 (cefbuperazone), a new cephamycin antibiotic, was performed in the field of obstetrics and gynecology. T-1982 was administered to 17 cases: 6 cases of parametritis, 3 cases of endometritis after artificial abortion, 3 cases of puerperal fever, 2 cases of febrile abortion, 1 case of Bartholin's abscess, 1 case of adnexitis and 1 case of infection of vaginal hematoma. Seventeen cases of these infections receiving in total 6 to 20 g of T-1982 demonstrated excellent results in 8 cases and good in the remaining 9 cases. Neither side effect nor clinical test abnormality was observed. From the present study, T-1982 is considered to have excellent efficacy and safety.

[Autopsy case of atypical medulloblastoma in an adult].

A 24-year-old female complained of headache and vomiting. The brain-CT scan demonstrated a tumor shadow in the right cerebellar hemisphere. The tumor was partially resected, and irradiation therapy was started. She died of intraventricular hemorrhage about 6 months after the onset of symptoms. Autopsy revealed a recurrent tumor mass in the cerebellum extending to the brain stem. It showed systemic metastases to the leptomeninx, liver, bones and ovaries. Histological examination showed a tumor which was a primarily composed of typical medulloblastoma cells with occasional Homer-Wright type rosettes. It partly showed glioblastoma-like configuration. Some tumor cells were positive for GFAP by the PAP method, suggesting glial differentiation.

Antigenic relationships between rotaviruses from different species as studied by neutralization and immunofluorescence.

Antigenic relationships between the Wa, Hochi, Ito, Hosokawa and Nemoto strains of human rotavirus, the SA-11 strain of simian rotavirus, the Lincoln strain of bovine rotavirus, the OSU strain of porcine rotavirus and the R-2 strain of lapine rotavirus, which were all established in cell culture, were studied by neutralization (NT) and immunofluorescence (IF) using guinea pig antisera against these strains. The strains could not be distinguished by indirect IF staining of infected cells because of marked cross reactions. On the other hand, the NT test clearly distinguished these strains, although there was a one-way antigenic relationship between some of them. The five strains of human rotavirus were clearly distinguished from each other by NT test. These human strains exhibited much higher homologous titers than heterologous titers, the former being at least eight-fold higher than the latter. The presence of at least five serotypes of human rotavirus was indicated.
