Laparoscopic-assisted serial biopsy of the bovine kidney.

A serial renal biopsy with endoscopy was done, using 10 calves and a cow. In 8 of the calves, biopsy materials were collected 3 to 5 times over a period of 15 to 41 days. A laparoscope was inserted into the peritoneal cavity from the center of the right paralumbar fossa through the outer cannula of the trocar. Under laparoscopic observation, the renal parenchyma was collected with the cutting prong of a biopsy needle. Of 34 biopsy attempts, 32 were successful; in 2, there was incomplete penetration of the trocar assembly into the peritoneal cavity. In one instance, the trocar was misguided to the small intestine which was closely adherent to the peritoneum. The error may have been the result of the presence of a persistent blood clot resulting from the preceding biopsy. An immediate saline rinse of the blood clot attached to kidney and the surrounding tissues was effective in preventing such adhesions. The size of the average sample obtained was 1.5 mm in diameter and 16 mm (2 to 40 mm) long. The tissues were collected in 61 times from the 72 insertions of the biopsy needle to the renal tissue. The 11 unsuccessful attempts were caused by bluntness of the cutting parts of the needle. Adverse clinical signs were no observed after the serial biopsy of kidney, except for the presence of blood in he urine persisting for 1 to 5 days. Although a dehydrated blood clot was found in the renal pelvis of one animal at necropsy, any complication such as obstruction of urinary flow was not observed.

[Continuous venous infusion of 5-fluorouracil (5-FU) under total parenteral nutrition (TPN) in advanced gastric cancer].

Continuous venous infusion of 5-FU was investigated in patients with advanced gastric cancer under TPN. In preliminary research Yoshida Sarcoma-bearing rats were continuously infused with 5-FU at a dose of 20 mg/kg/day under TPN. The 5-FU level in the tumor was 2-5 times higher than that in the kidney, stomach, intestine and liver, and 40-50 times higher than that in serum. Advanced gastric cancer patients were continuously infused with 5-FU at a dose of 500 mg/day under TPN. Serum 5-FU level was constantly maintained at 0.05 mcg/ml during the infusion period. The effects of 5-FU continuously infused with other anticancer agents and TPN were satisfactory with regard to anticancer response and general condition. In conclusion, in advanced cancer, 5-FU is recommended for use in continuous infusion at a dose of 500 mg/day under TPN.

Isolation and characterization of I5B2, a new phosphorus containing inhibitor of angiotensin I converting enzyme produced by Actinomadura sp.

A new inhibitor of angiotensin I converting enzyme, I5B2, was isolated from the culture broth of Actinomadura sp. No. 937ZE-1. This compound contains N-methylvaline, tyrosine and 1-amino-2-(4-hydroxyphenyl)ethylphosphonic acid. The microorganism also produced another inhibitor, I5B1, which is identical with K-4 isolated from Actinomadura sp. as an antihypertensive agent.

[Effect of preoperative adjuvant therapy of esophageal carcinoma].

Between January 1970 and December 1983, 148 patients of esophageal carcinoma were treated surgically in the 2nd Department of Surgery, Osaka University Medical School. Among these patients, 70 (48.6%) with suspected invasion to neighboring structures were treated with preoperative chemotherapy, radiotherapy or a combination of both. The chemotherapeutic agents used were tegafur (FT-207)-suppository, bleomycin (BLM) or peplomycin (PEP). Radiotherapy (3000-4000 cGy) for selected cases was begun at the same time as the chemotherapy. Three to four weeks after the chemotherapy and radiotherapy were completed, esophagectomy was performed. The effects of the preoperative adjuvant therapies were investigated in these patients, and the following results were obtained: A marked histological effect, according to the Guide Lines in Clinical and Pathologic Studies for Carcinoma of the Esophagus (Japanese Society for Esophageal Diseases, 1976) was found in 47.4% of the radiotherapy plus FT-207 group, 39.1% of the group receiving radiotherapy alone and 28.6% of the group receiving radiotherapy plus PEP or BLM. Radiotherapy plus FT-207 showed excellent effects (77.8% of this group showed marked histological effects) on well differentiated squamous cell carcinoma, as shown histologically by biopsy specimens. Tumors exhibiting sharp edged margins radiographically and endoscopically, showed a very good histological effect after preoperative radiotherapy. Metastatic lymph nodes present in the irradiation field, whose primary lesion showed a marked histological effect, also gave excellent results. Postoperative radiotherapy is also expected to be equally effective on these cases.

[The metabolic changes and nutritional management following radical surgery of esophageal cancer and total gastrectomy].

The metabolic changes after major surgery such as esophagectomy and total gastrectomy are remarkable. Postoperative levels of plasma catecholamines and glucagon were significantly higher in major surgery than those in the other operations (distal gastrectomy, cholecystectomy, mastectomy, etc). As the consequence of these hormonal changes, resting metabolic expenditure (RME) increased up to 30-40% above the predicted basal metabolic expenditure and blood glucose elevated to a high level. The relationships between RME, N-balance and energy intake after major surgery indicated that the intake of non-protein energy expenditure equal to RME was necessary in order to maintain N-balance. For the administration of enough energy after major surgery, TPN solution composed of glucose 180-240g, fructose 90-100g, xylitol 30-40g and amino acids 68-91g (the approximate weight ratio of G, F and X is 4: 2: 1) was developed since 1975 and in some cases, lipid solution was added as non-protein energy source. This solution was infused immediately after operation and no metabolic complications (hyper glycemia, acidosis, etc) were observed. This nutritional therapy has maintained the nutritional state of postoperative patients and improved our operative results. There are, however, many problems in the nutritional managements of the patients with severe complications and after the discharge of hospital. In the future, special nutritional therapy for these cases should be progressed.

[The reciprocal pattern of release observed in PRL and TSH following various types of administration of TRH].

Alterations of plasma TSH and PRL levels following various types of administration with TRH were studied in the luteal phase of women with normal menstrual cycles. The results were as follows: Plasma PRL response to a single i.v. injection of tRH with DHEA-S drop injection was not significantly different from that observed without it. The data suggest that DHEA-S does not affect the feedback mechanism in the short-term course of the experiment. Alterations in the plasma levels of PRL during intermittent as well as continuous infusion with TRH were quite different from those with TSH. Plasma TSH increased following the beginning of a continuous infusion with TRH and reached a peak at approximately 180 min, maintaining a plateau until the infusion was withheld. On the other hand, plasma PRL levels increased rapidly to reach peak levels in 30 min after the start of the infusion gradually following a decline in spite of continued TRH stimulation. Furthermore, a stepwise increase of TSH levels was observed following 4 intermittent injections of TRH; however, plasma PRL rapidly reached peak levels in 30 min following the 1st injection of TRH and a further 3 injections did not raise the levels of PRL. The results suggest that in humans, the synthesis and release of PRL occur independently in the pituitary.

Prevalence of severely impaired visual acuity among diabetic patients in Fukuoka Prefecture with special emphasis on diabetic retinopathy: a survey by the Fukuoka Diabetes Clinic Group.

From December 1981 to January 1982, 3841 diabetic patients were seen in 16 diabetic clinics in Fukuoka Prefecture. Among them, 3289 diabetics had been examined for the presence of ophthalmological complications during the past year, and 979 cases (29.8%) had diabetic retinopathy, in which the proliferative type manifested itself in 152 cases (4.6%). Of 3841 diabetics in our clinics, there were 275 cases (7.2%) with severely weakened visual acuity (assessed by corrected acuity below 0.1), of which 153 cases (4.0%) resulted from diabetic retinopathy. Of these, 92 (2.4% of total diabetics) had visual defects in both eyes. Analyses on these 153 cases revealed several features, i.e., there were 10 cases with maculopathy and 4 cases with secondary rubeotic glaucoma, the age of the patients was 55.9 +/- 11.5 (mean +/- S.D.) years, and the presumed duration of diabetes, time lapse until appearance of visual defect(s) from onset of diabetes and the untreated period were 13.8 +/- 6.6 (n = 150), 10.5 +/- 6.0 (n = 124) and 4.2 +/- 4.9 (n = 101) years, respectively.

Effect of morphinone on opiate receptor binding and morphine-elicited analgesia.

Specific binding of 3H-naloxone to opiate receptors was found to be irreversibly inactivated by morphine. This inactivation exhibited pseudo-first-order kinetics. The presence of sulfhydryl compounds or morphine during incubation with morphinone proved good protection. Morphinone-pretreated mice blocked the analgesic effect of morphine. The possible mechanism for these observations is proposed as follows: morphinone binds covalently to sulfhydryl group of opiate receptors, and inactivates irreversibly opiate binding sites, thus blocking the analgesic effect of morphine.

Protective effect of sulfhydryl compounds on acute toxicity of morphinone.

The ability of sulfhydryl compounds to provide protection against the acute toxicity of morphinone was investigated in mice. Subcutaneous administration of morphinone produced a reduction of hepatic non-protein sulfhydryl concentration. Pretreatments of mice with glutathione or cysteine significantly increased the survival rate of mice given a lethal dose of morphinone, whereas morphinone lethality was markedly potentiated by diethyl maleate. On the other hand, the administration of morphine produce a dose dependent reduction of hepatic non-protein sulfhydryl contents. However, neither glutathione nor cysteine protected mice from the acute toxicity of morphine. A possible explanation for these observations was proposed as follows: morphine is oxidized by morphine 6-dehydrogenase to morphinone, and the morphinone thus produced decreases the sulfhydryl contents in the liver. This mechanism is supported by the fact that morphinone reacts easily with glutathione and cysteine in vitro.

A new antibiotic K-82 A and minor components, produced by Streptomyces lavendulae, strain No. K-82.

From the results of taxonomic studies, Streptomyces sp. strain No. K-82 isolated from a soil sample collected in Kumamoto city, was identified as a strain belonging to Streptomyces lavendulae WAKSMAN & HENRICI 1948. The strain produced an active new antibiotic called K-82 A and minor components named the B complex. Antibiotic K-82 A was isolated as dark reddish needles by silica gel column chromatography and found to have both antibacterial activity and high phage induction activity. The K-82 B complex was found to consist of at least five components, among which K-82 B2 and B3 were isolated as crystals. Substances K-82 B2 was identified as benzoic acid from its physicochemical properties. Substance B3 like B2 had only marginal antibiotic activity.

Demonstration of immunoreactive somatostatin-like substance in villi and decidua in early pregnancy.

With the use of the radioimmunoassay for growth hormone--releasing inhibiting factor (GIF), it was found that measurable amounts of GIF-like substance existed in the chorionic villi and decidua of pregnant women. The indirect immunofluorescent method revealed that the higher intensity of GIF-like immunofluorescence was presented in cytotrophoblasts rather than in syncytiotrophoblasts of the villi and in stromal cells of the decidua.

[Demonstration of immunoreactive GIF-like substance in villi and decidua by radioimmunoassay and immunofluorescence (author's transl)].

Since Arimura et al (1975) reported the radioimmunoassay for somatostatin (GIF), the concentration of GIF in various organ and brain regions were determined by radio-immunoassay. Dubois et al (1975) reported that immunohistochemically somatostatin was located in the discrete cells of the pancreas as well as the hypothalamus, and from this result, they presented the concept of local hormone instead of systemic hormone which was up to that time accepted in endocrinology. In this study, we developed the high specific anti-GIF serum using rabbits, and with the micro immunodiffusion method, we demonstrated that the precipitin band formed a circular fusion between the GIF and anti-GIF serum. This pattern of reaction was also seen in decidual immunodiffusion. In addition, we developed the radioimmunoassay for GIF using this anti-serum and measured immunoreactive GIF-like substances in villi and decidua of early pregnancy. The concentration of GIF-like substances with 2 N acetic acid extracted of villi and decidua were 0 to 30 pg/0.1 g dry weight. At the same time, we demonstrated the presence of GIF-like substance-containing cells in the villi and decidua by indirect immunofluorescent method. The intensity of immunofluorescence was in cytotrophoblast rather than syncytiotrophoblast, and decidual stromal cell also reacted to the immunofluorescence.
