[The therapeutic effects and postoperative complications of cataract surgery in Laiwu City].

OBJECTIVE: To decrease the prevalence of blindness and low vision in Laiwu City. METHODS: The therapeutic effects and postoperative complications of each method of cataract surgery were investigated and statistically analyzed. The cases with postoperative complications were treated after surgery. RESULTS: In the investigation, 1,274 eyes of 1,089 cases had undergone cataract surgeries, including couching for cataract, intracapsular cataract extraction (ICCE), extracapsular cataract extraction (ECCE) and ECCE with intraocular lens implantation. The prevalence of blindness and low vision was 4.87% and 23.70%, respectively. There were postoperative complications in 650 eyes (51.02%). They were corneal decompensation, secondary glaucoma, opacity of posterior capsuler, cystoid macular edema and retinal detachment. After treatment for the postoperative complications, the prevalence of blindness and low vision was decreased to 1.96% and 6.01%, respectively. The cases having escaped the blindness and handicap after treatment were lowest in the groups of couching and ICCE. CONCLUSIONS: The survey on the population having undergone cataract surgery in rural district is important. The postoperative complications can be treated in time and the prevalence of blindness and low vision can be decreased.

[Preliminary clinical study on the management of angle-closure glaucoma by phacoemulsification with foldable posterior chamber intraocular lens implantation].

OBJECTIVE: To investigate the clinical results of phacoemulsification with foldable posterior chamber intraocular lens (PC-IOL) implantation in the management of angle-closure glaucoma with cataract. METHODS: Phacoemulsification with PC-IOL implantation performed on 47 eyes (42 cases) with angle-closure glaucoma and cataract was retrospectively studied, including 26 eyes with primary acute angle-closure glaucoma, 12 eyes with primary chronic angle-closure glaucoma, 5 eyes with secondary acute angle-closure glaucoma and 4 eyes with secondary chronic angle-closure glaucoma. RESULTS: After a mean postoperative follow-up of (18.40 +/- 9.51) months, the intraocular pressure was reduced from a preoperative mean of (25.47 +/- 18.43) mm Hg to a postoperative mean of (11.99 +/- 4.48) mm Hg (t = 4.918, P < 0.001). The mean anterior chamber depth was (1.69 +/- 0.46) mm preoperatively and (2.28 +/- 0.36) mm postoperatively (t = 7.738, P < 0.001). The best-corrected visual acuity was improved in 41 of 47 eyes (87.2%). CONCLUSION: Phacoemulsification with posterior chamber foldable intraocular lens implantation can be a good alternative in treating angle-closure glaucoma with cataract.

Nuclear chromatin characteristics of breast solid pattern ductal carcinoma in situ.

OBJECTIVE: To characterize nuclei from breast solid pattern ductal carcinoma in situ (DCIS) by their karyometric features and to search for the presence of statistically significantly different subsets of nuclei. STUDY DESIGN: One hundred nuclei from each of 6 normal, 13 solid DCIS, (9 low and intermediate grade and 4 high grade DCIS) histopathologic samples of breast tissue were digitally recorded. Karyometric features were computed and subjected to a nonsupervised learning algorithm (P-index) to identify significantly different subgroups. RESULTS: Nuclei in low grade lesions displayed a diploid/near diploid pattern, while the majority of intermediate grade lesions fell into a range beyond 5N. The high grade lesions showed substantial genomic instability and represented three statistically different subsets or phenotypes. CONCLUSION: There is a progression of nuclear abnormality from low grade to high grade DCIS. The nuclei from high grade DCIS form a heterogeneous set that represents three phenotypes. One of these phenotypes shows a nuclear chromatin pattern that more closely resembles poorly differentiated, infiltrating disease. The observation of such a phenotype may have prognostic implications.

Identification of metastasis associated gene G3BP by differential display in human cancer cell sublines with different metastatic potentials G3BP as highly expressed in non-metastatic.

OBJECTIVE: To investigate genes involved in cancer metastasis, mRNA differential display was used to compare the levels of gene expression in two cell sublines derived from human giant cell carcinoma of lung (PG) which had different metastatic potentials. METHODS: Using in vivo tumorigenicity and a spontaneous metastasis assay in nude mice, two sublines (BE1, LH7) from human giant cell carcinoma of lung (PG) with different metastatic potentials were isolated and characterized. mRNA differential display was used to compare the levels of gene expression between them and the obtained results were confirmed by Northern hybridization. RESULTS: One differentially expressed band was nearly identical (99% homology) to Ras-GTPase-Activating protein SH3 domain binding protein (G3BP). G3BP displayed a strong expression in LH7 (non-metastatic in recipient nude mice) and a very weak expression in BE1 (100% metastatic frequency). The same different expression level of G3BP was detected in Northern hybridization with another panel of cell sublines with different metastatic potentials (established in our lab) derived from human prostate carcinoma cell line PC-3M. CONCLUSION: Our results indicate that G3BP was implicated in cancer metastasis because of its differential expressions in the two panels of cell sublines with different metastatic potentials.

Immune responses to the expressed products of the CSP antigen gene of plasmodium falciparum southern China isolate FCC1/HN in Hela cell.

OBJECTIVE: To construct a eukaryotic expression system with pcDNA3-PfCSP/Hela for the Circumsporozoite protein (CSP) gene of Plasmodium falciparum (P. falciparum), to observe the immune responses in BALB/c mice induced by the expressed proteins. METHODS: The recombinant plasmid pcDNA3-PfCSP was transformed into the Hela cell line. The expressed protein was isolated and analyzed by using SDS-PAGE and used for immunization of BALB/c mice by subcutaneous, intravenous, and intraperitoneal administration. Enzyme-linked immunosorbent assay (ELISA), Dot-ELISA, Western blot, T lymphocyte proliferation test, natural killer cell (NKC) activity assay, and CD4+ and CD8+ T cell detection were used for observation of humoral and cellular immune responses. RESULTS: Immune sera strongly reacted with the expressed protein, antibody titer was up to 1:6400 as detected by ELISA. Western blot analysis revealed a specific band at 38.3 Kda. When the spleen cells of normal and immunized BALB/c mice were specifically stimulated with expressed protein, the optical densities were 0.12 +/- 0.03 and 0.34 +/- 0.04, respectively. The latter were significantly higher than the former (P < 0.01). We used the MTT colorimetric assay to measure NKC activity of mice spleen. The results showed that the NKC activity of immunized BALB/c mice was remarkably higher than that of the controls (P < 0.05). CD4+ and CD8+ T cells were detected by using monoclonal antibody immunofluorescence methods. The results showed that the percentage of CD4+ and CD8+ T cells of immunized group were significantly higher than that of control group (P < 0.05). CONCLUSIONS: The humoral and cell-mediated immune responses and elevated NKC activity to products made with a eukaryotic expression system could be specifically detected in BALB/c mice. These findings indicate that the expressed protein could enhance the immune function in mice.

Differentiation of NIH3T3 fibroblasts into adipocytes induced by peroxisome proliferator activated receptor gamma 2 expression.

OBJECTIVE: To express mouse peroxisome proliferator activated receptor gamma 2 (mPPAR gamma 2) in NIH3T3 fibroblasts mediated by the recombinant retrovirus and study its function. METHODS: The mPPAR gamma 2 gene was subcloned into retrovirus vector pGCEN to generate the recombinant pGCEN/mPPAR gamma 2. Then it was packaged into PA317 cells and selected with G418. Viral supernatants were harvested and then used to infect NIH3T3 fibroblasts. PPAR gamma activator 5, 8, 11, 14-eicosatetraynoic acid (ETYA) was used to induce the mPPAR gamma 2-expressing NIH3T3 cells into adipocyte differentiation. RESULTS: The recombinant retrovirus pGCEN/mPPAR gamma 2 was constructed, and the higher titers of the viral supernatants were obtained. mPPAR gamma 2 was expressed in NIH3T3 cells mediated by the recombinant retrovirus. Lipid accumulation obviously existed in these induced adipocytes which morphologically resembled mature adipocytes in vivo and expressed tissue specific adipocyte P2 (AP2) and Leptin genes. CONCLUSIONS: An adipocyte differentiation model in vitro was successfully established. The work is the basis for further research on the molecular mechanism of adipocyte differentiation induced by PPAR gamma 2.

Increased expression of Hsp70 and co-localization with nuclear protein in cells infected with the Hantaan virus.

OBJECTIVE: To investigate the effect of Hantaan virus infection on the expression of stress genes. METHODS: Techniques of virus infection, Western blot, immunohistochemistry, dual-immunofluorecsence staining, laser scanning confocal microscopy, RNA dot blot and in situ hybridization were used. RESULTS: Expression of heat shock protein 70 (Hsp70) was observed in cells infected with HTV as well as the translocation of Hsp70 from cytoplasm to nucleoli following virus infection. The variable distribution of Hsp70 was related to the various time after infection. Double-label indirect immunofluorescence of nuclear protein (NP) and Hsp70 in infected cells demonstrated co-localization of these proteins in the cytoplasm. CONCLUSION: Overexpression of Hsp70 can be induced directly by Hantaan virus, which may be associated with virus protein assembly. The Hantaan virus proteins co-localize with, and possibly form a physical complex with cellular Hsp70 in infected cells.

[Preliminary study on skeletal muscle derived tumor suppressor].

OBJECTIVE: To investigate the effect of new born rat skeletal muscle conditioned medium (MCM) on proliferation of tumor cells, and its physicochemical characteristics and mechanisms of action. METHODS: The effects of MCM were tested on a variety of cells by MTT assay. MCM was also tested following ultrafiltration, boiling at 100 degrees C and treatment with trypsin. Morphology and apoptosis of the MCM-treated cells were examined. RESULTS: Proliferation of mouse myeloma SP2/0, rat carcino-sarcoma Walker 256, and tumor cell lines of human origin, including leukemia K562 and HL-60, colon adenocarcinoma LS-174-T, prostatic carcinoma PC3-M, and lung adenocarcinoma with low(PLA801-C) and high(PLA801-D) metastatic potential were significantly decreased when cultured with MCM(P < 0.01-0.05) in a dose-dependent manner. The proliferation of normal cells(RGP-2 rabbit joint epiphysial disk cells) was not affected by MCM. Proliferation of the lung adenocarcinoma with high metastatic potential was more susceptible to the growth inhibitory effect of MCM as compared to that with low metastatic potential. On ultrafiltration, the inhibitory activity of MCM existed in the fraction with a molecular weight < or = 10,000. It was trypsin resistant but thermolabile. Apoptosis was not seen but the cytoplasmic membrane of tumor cells was destroyed after cultured in MCM. CONCLUSION: Rat skeletal muscle cells produce low molecular weight tumor suppressor(s) capable of selectively inhibiting tumor cell proliferation in vitro. Its activity is neither species- nor tumor-specific. The skeletal muscle derived tumor suppressor(s) may play a key role in the rarity of tumor metastases in skeletal muscles.

[The placental immunopathology of intrauterine growth retardation].

OBJECTIVE: To observe the immune complex distribution in placenta of intrauterine growth retardation (IUGR). METHOD: Using immunohistochemical technique (SP method), authors studied the deposition of IgG and IgM in the placenta of idiopathic IUGR patients (including 8 samples of (ACA)-positive and 14 samples of ACA-negative) and patients with Pregnancy induced hypertension (PIH) complicated IUGR. RESULTS: There is no significant difference of IgG deposition in villous vessels among the three groups. The positive rate of IgM in villous vessels, the positive rate of IgG, IgM in decidual vessels and the deposition of IgM in syncytiotrophoblast of both idiopathic IUGR and PIH complicated IUGR were much higher than those of control group (P > 0.05). The positive rates of IgG, IgM in decidual vessels and villous vessels between the ACA-positive group and ACA-negative group in the idiopathic IUGR were not significantly different. CONCLUSIONS: The immunopathological lesion in placenta might play an important role in the development of IUGR. Idiopathic IUGR and PIH complicated IUGR have the similar placental immunopathological changes. The etiology of IUGR induced by ACA needs more studies.

[The development of a respiration and temperature monitor].

This paper introduces the design of a monitoring system to measure the respiration and temperature of a body with an 8Xc196 single-chip microcomputer. This system can measure and display the respiration wave, respiration frequency and the body temperature in real-time with a liquid crystal display (LCD) and give an alarm when the parameters are beyond the normal scope. In addition, this device can provide a 24 hours trend graph of the respiration frequency and the body temperature parameters measured. Data can also be exchanged through serial communication interfaces (RS232) between the PC and the monitor.

Comparison of the effects of two types of intraocular lens.

OBJECTIVE: To evaluate the effectiveness and safety of implantation of flexible open-loop anterior chamber intraocular lens (FOAC-IOLs) and scleral fixated posterior chamber intraocular lens (PC-IOLs). METHODS: Sixty-eight eyes of 68 patients with implanted intraocular lenses in the absence of posterior capsular support were reviewed retrospectively. According to the type of intraocular lens, patients were classified into two groups. In group I (30 eyes), FOAC-IOLs was implanted primarily or secondarily. In group II (38 eyes), scleral fixated PC-IOLs was implanted primarily or secondarily. By gonioscopy and ultrasound biomicroscopy (UBM), accurate positions of IOLs' haptics and the relationship between the haptics and surrounding tissues were observed postoperatively and used to evaluate the influence of the two types of IOLs on ocular anterior segments. Follow-up was 6 to 20 months. RESULTS: Best corrected visual acuity of 20/40 or better was achieved in 27 eyes (90.0%) in group I, and 35 eyes (92.1%) in group II and the difference was not statistically significant (P > 0.05). In group I, 23 eyes (76.7%) had a total of 40 complications, while 13 eyes (34.2%) had 19 complications in group II (P < 0.05). Gonioscopy and ultrasound biomicroscopy showed that in group I, all haptics of IOLs contacted with the iris completely and compressed the iris to different degrees, sometimes causing the anterior chamber angles to widen. Anterior synechia of the iris was caused by the haptics of FOAC-IOLs in 12 eyes. Among the 60 IOLs haptics, 39 foot plates of the haptics were properly fixed at the ciliary band; 21 haptics (12 eyes) penetrated through the iris into the stroma of the ciliary body with accompanying recurrent uveitis. In group II, among the 76 IOLs haptics, 52 were adequately fixed in the ciliary sulcus regions, and 8 (8 eyes) were placed below the iris, causing goniosynechia. This type of angle closure was localized, with an open angle on each side of the haptics. The remaining 16 haptics were fixed onto the ciliary crown. CONCLUSIONS: The current sutured mode of scleral fixated PC-IOLs can not ensure that IOLs haptics are placed in the ciliary sulcus. The haptics of FOAC-IOLs compress the iris and may penetrate through the iris into the stroma of the ciliary body. This can cause peripheral iris anterior synechia and chronic recurrent uveitis. The implantation of scleral fixated PC-IOLs is safer and shows better effects than that of FOAC-IOLs.

[Appraisal of renal failure reversing therapies in lupus nephritis].

OBJECTIVE: This study aimed to explore the therapies of reversing renal failure in lupus nephritis. METHODS: The research strategy was a retrospective cohort study. 173 patients were included in the study. All of them were consecutive in-patients of newly diagnosed renal failure (serum creatinine > 180 micromol/L) with lupus nephritis from 1991 to 1999 in the university hospital. The research factors included steroids, cyclophosphamide and cyclosporin. Steroids were divided to oral prednisone, intravenous dexamethasone and intravenous methylprednisolone pulse; cyclophosphamide dose intensity was divided into three grades: none, low dose (average dose 0.25 g/m(2) body surface area per week). 51 demographic, clinical and laboratory data were included in the study as possible confounding factors. The study end-point was recovery of renal function that was defined as a level of serum creatinine <or= 116 micromol/L in the azotemia patients or a 50% reduction of serum creatinine and discontinuation of dialysis in the uremic patients. The improvement had to be maintained for at least three months. Collection of the study factors and outcome variable was conducted by two separate, blinded groups of clinical specialists with two structured abstracting forms. RESULTS: The number of complete following up patients was 142 (82.1%). After over 6 months of following up, 93 cases (65.5%) recovered their renal function and 17 cases (12.0%) died. In COX proportional hazard regression model, cyclophosphamide therapy was significantly associated with the recovery of renal function. The adjusted hazard ratio (HR) to recovery of renal function was 4.41, 95% CI = 1.03 - 18.94 in low dose; and the HR was 13.29, 95% CI = 3.11 - 56.82 in high dose cyclophosphamide therapy. The gender, duration of renal dysfunction, small renal size, severe anemia and serum creatinine were also significantly associated with renal function recovery. Cyclophosphamide therapy was not significantly associated with death. The variables associated with death were low level of serum albumin and edema. CONCLUSION: Cyclophosphamide pulse was an efficient therapy in reversing renal failure of lupus nephritis, especially in the patients with short duration of renal failure, non-small in renal size, and non-severe anemia.

[Malignant hypertension presenting with renal damage: a clinical and pathological analyses].

OBJECTIVE: To evaluate the clinical features and pathological characteristics of malignant hypertension (MHPT) with renal damage. METHODS: The blood pressure, eye ground, urinalysis, renal function and extrarenal presentations of malignant hypertension patients, who had been hospitalized in recent eleven years, were analyzed retrospectively. There were 27 patients, 21 males and 6 females, aged from 19 to 51 years. At the same time, the clinical and pathological features of primary MHPT were compared with those of renal parenchymal MHPT. RESULTS: Among these patients, primary hypertension was diagnosed in 10 cases (37%), glomerulonephritis in 10 cases (37%), renovascular hypertension in 1 case (3.7%), and undetermined etiology in 6 cases (22.3%). The common renal presentations were those of rapidly progressive glomerulonephritis (RPGN) and progressive renal failure. Compared with those who were diagnosed as renal parenchymal MHPT, primary MHPT patients had lower urinary protein excretion (P = 0.001) and most of them had family history of hypertension. Renal biopsy was done in 13 cases, revealing primary MHPT in 4 cases and glomerulonephritis in 9. Typical pathological findings for primary MHPT were fibroid necrosis, marked intimal hyperplasia ("onion skin" like change) and ischemic changes of glomeruli. Severe glomerulonephritis, hyalinosis and thickening of renal arterioles were the main changes in MHPT with renal parenchymal diseases. After intensive therapy, the renal functions of some primary MHPT patients improved markedly. CONCLUSION: Malignant hypertension presenting with renal damage was not very rare. It was usually misdiagnosed. The clinical presentations, pathological findings and prognosis of primary MHPT were different from those of renal parenchymal MHPT.

[The effects of dialysate and ultrafiltration flow rate on solute clearance during continuous renal replacement therapy].

OBJECTIVE: To study the solute clearance during various forms of continuous renal replacement therapy(CRRT) and test the formulas that allow the prediction of the influence of dialysate and ultrafiltration flow rate on small solute removal during CRRT. METHOD: Five patients with acute renal failure were included in the study and were treated by venovenous CRRT using the PRISMA predilution system. Solute clearance of urea nitrogen(UN), creatinine(Cr), uric acid(Ua), phosphate(P) and beta(2)-microglobulin(beta(2)-M) were evaluated during CRRT with different dialysates and ultrafiltration flow rates. RESULTS: The determined clearance of small molecular solutes during continuous venovenous hemofiltration (CVVH) and continuous venovenous hemodialysis(CVVHD) was similar with the following formulas: K(UF) = (Q(UF)/60) x Q(B)/(Q(B) + Q(UF)/60) (in CVVH), Kd = Q(D)/60 (in CVVHD), where K is the clearance, Q(B), Q(D) and Q(UF) are blood, dialysate and ultrafiltration flow rates, respectively. There was very significant correlation between calculated values of K(UF) and observed clearances of small solutes such as UN, Cr, Ua and P during CVVH, between calculated values of Kd and observed clearances of UN, Cr, Ua but not P during CVVHD (P < 0.001). Clearances of UN, Cr, Ua and P during CVVHD were greater than those during CVVH, but clearance of beta(2)-M during CVVHD was less than that during CVVH. Interaction between convection and diffusion was found during continuous venovenous hemodiafiltration (CVVHDF). CONCLUSIONS: The previous formulas can provide with the prediction of the clearance of small molecular solutes during CVVH and CVVHD. The present results demonstrate that diffusion is more efficient in removing small solutes than convection but less efficient in removing large solutes than convection. There is interaction between convection and diffusion during CVVHDF.

[The effects of high dose atropine on function of isolated diaphragmatic preparation of rats with omethoate poisoning].

OBJECTIVE: The study was designed to examine the influence of high dose atropine on respiratory muscle of rat isolated phrenic diaphragm. METHODS: Rats were divided into 2 groups: (1) a toxic group administered with 2LD(50) omethoate, SC. (2) a contrast group with 0.9% NS, SC. The function of isolated rat phrenic diaphragm was observed by MS-302 analysis instrument physiologically and pharmacologically. RESULTS: In both groups the function of isolated rat phrenic diaphragm was significantly reduced with atropine dose increasing gradually to 0.6 mg (P < 0.01). When the atropine dose amounted to 0.8 mg. The function of isolated rat phrenic diaphragm almost disappeared in the picture. When the atropine in the diaphragm was weed out, the function of isolated rat phrenic diaphragm recovered completely in 50 minutes. CONCLUSION: It is suggested that the cause of respiratory muscle paralysis may be related to high dose of atropine.

[Adipocytes differentiation of NIH3T3 cells induced by peroxisome proliferator activation receptor gamma 2 expression].

OBJECTIVE: To express the mouse peroxisome proliferator activated receptor gamma 2(mPPAR gamma 2) in NIH3T3 cells mediated by the recombinant retrovirus and study its function. METHODS: mPPAR gamma 2 gene digested from the recombinant plasmid pcDNA3/mPPAR gamma 2 and confirmed to contain the target gene segment with fluorescence-sequencing was subcloned into retrovirus vector pGCEN to generate the recombinant retrovirus pGCEN/mPPAR gamma 2. The recombinant retrovirus pGCEN/mPPAR gamma 2 and pGCEN were packaged with PA317 cells and anti-G418 clones of PA317 cells were selected. Viral supernatants were collected and used to infect NIH3T3 cells. Peroxisome proliferator activated receptor gamma 2 (PPAR gamma 2)-expressing NIH3T3 cells cultured in the differentiation media containing PPAR gamma activator ETYA were induced into adipocytes. RESULTS: The recombinant retrovirus pGCEN/mPPAR gamma 2 was constructed, 5 x 10(4) CFU/ml of the viral supernatants containing pGCEN/mPPAR gamma 2 and 6 x 10(5) CFU/ml of the viral supernatants containing pGCEN were obtained. mPPAR gamma 2 was expressed in NIH3T3 cells mediated by the recombinant retrovirus. Lipid accumulation obviously existed in PPAR gamma 2-expressing NIH3T3 cells at 10 days postdifferentiation and the lipid-containing cells morphologically resembled the mature adipocytes in vivo. CONCLUSION: An adipocyte differentiation model in vitro was established. The work is the basis for further researches on the molecular mechanism of adipocyte differentiation induced by PPAR gamma 2.

[A clinical and pathological analysis of 41 patients with anti-glomerular basement membrane antibody related diseases].

OBJECTIVE: To investigate the clinical and pathological characteristics of anti-glomerular basement membrane (GBM) antibody associated diseases with different clinical patterns. METHODS: The clinical and pathological data of 41 patients with anti-GBM antibody associated diseases, screened in our institute in recent six years, were retrospectively analyzed. RESULTS: 22 of the 41 patients presented as Goodpasture syndrome, 2 of them having normal renal function. 32 of the 41 patients had anti-GBM antibody positive only (single positive). 31 of the 32 single positive patients were male with an average age of (26.8 +/- 9.7) years. The remaining 9 patients were both anti-GBM antibody and antineutrophil cytoplasmic antibody (ANCA) positive (double positive); 7 of the 9 were female with an average age of (44.5 +/- 19.6) years. There was significant difference in gender and age between the single and double positive groups (P < 0.05). 30 of the 32 patients with renal biopsy had crescentic glomerulonephritis and the remaining two with normal renal function had mild mesangial proliferative glomerulonephritis only. 24 of the 30 (80%) crescentic glomerulonephritis patients had all glomeruli affected by crescents with severe glomerular capillary loop and Bowman's capsule damage. With immunofluorescence examination, 16 of the 23 cases had typical IgG and C(3) linear deposition along glomerular capillary walls, but the remaining 7 had fine granular IgG and/or C(3) deposition. In a few cases, granular deposition was found even in mesangium. There was no correlation between the immunofluorescence patterns and the severity of glomerular damage (P > 0.05). All patients had anemia, hematuria and proteinuria, 7 of the 41 cases presented as nephrotic syndrome. After intensive immunosuppressive therapy, only four patients, including the two patients with normal renal functions, achieved remission, the remaining patients either were in renal replacement therapy or died. CONCLUSIONS: Anti-GBM antibody associated diseases are not rare in China. Single positive patients are mainly young male, while double positive patients are mainly middle-aged female. Most of the patients with anti-GBM antibody associated diseases have crescentic glomerulonephritis (rapidly progressive glomerulonephritis, RPGN) with a bad prognosis. However, not all the patients have typical linear deposition of IgG and C(3) along glomerular capillary walls. Only a few non-oliguric patients with normal renal function or mild renal damage may achieve remission.

[Clinico-pathological features and possible pathogenesis of rifampicin-induced acute renal failure].

OBJECTIVE: To explore the clinico-pathological features and possible pathogenesis of rifampicin-associated acute renal failure (ARF). METHODS: The clinico-pathological features of 4 patients with Rifampicin-associated ARF were analyzed. Rifampicin-dependent antibody in the patient's serum was detected with Coomb's test. RESULTS: 4 cases with rifampicin-associated ARF was distinguished from 202 in-patients with ARF in 5 years(1.98%). Most of them had infection before the occurrence of ARF. Clinical features were characterized by fever, fatigue and gastro-intestinal symptoms after taking rifampicin (especially when it was reused). Oliguria or anuria developed suddenly. Laboratory data showed hemolytic anemia, thrombocytopenia and decline of renal function. Some also had impaired liver function. Renal biopsy revealed acute tubular necrosis in 3 cases and acute interstitial nephritis in 1 case. Serum rifampicin-dependent antibody was positive in all the cases. CONCLUSION: Rifampicin-associated ARF is not rare clinically, especially in those rifampicin re-exposure patients. Renal function should be closely monitored in susceptible patients. Detection of serum rifampicin-dependent antibody may be helpful to distinguish this group of ARF patients.
