Stretch-induced delivery is independent of the functional fetal role and dysfunction of the amnion and decidua: a morphologic and enzyme cytochemical study.

The role of the fetus, amnion, and decidua in stretch-induced delivery was studied in six normal pregnant women (group 1) and six in whom the fetuses had been dead in the uteri for more than a week (group 2). The cervices were distended by laminaria for 48 hours; this was followed by balloon inflation with traction. During the balloon application, regular uterine contractions were initiated and increased in all patients, with four patients in group 1 and five in group 2 aborting or being delivered within 24 hours. Light and electron microscopic studies of the amnion obtained during apparent uterine activity or after delivery revealed normal cell morphologic features in both groups although an increased fat deposition in cytoplasm was noted in group 2. Decidua parietalis exhibited normal cell morphologic features in both groups, and cytochemical demonstration of acid-phosphatase activity in decidual cells confirmed no leakage of the lysosomal enzyme into the cell cytoplasm. These findings indicate that stretch-induced delivery is initiated independent of the functional fetal role and without dysfunction of the amnion and decidua.

Gallium-67 imaging in the evaluation of thyroid malignancy.

One hundred thirty-six patients with suspected thyroid malignancy were studied prospectively with Ga-67 imaging of the thyroid and whole body. All patients subsequently had diagnoses proven by biopsy or surgical resection. Patients with malignant tumors (anaplastic carcinoma, malignant lymphoma, adenocarcinoma, medullary carcinoma, adenocarcinoma with squamous cell transformation, and metastases to the thyroid from other tumors) were examined serially after appropriate therapy. The authors draw on their own experience and a literature review to propose the following indications for Ga-67 imaging in patients with thyroid disease: 1) suspected anaplastic carcinoma or malignant lymphoma of the thyroid gland, 2) detection of distant metastases of anaplastic thyroid carcinoma and malignant lymphoma, 3) evaluation of efficacy of therapy, 4) suspected metastases to the thyroid gland from other malignancies, and 5) differentiation of malignant lymphoma from chronic thyroiditis. The strengths, limitations, and potential pit-falls of the technique are discussed.

Conditions of the cervix for the increase of plasma levels of 13,14-dihydro-15-keto-prostaglandin F2 alpha after amniotomy at term.

Amniotomy was performed in 12 multiparas at term but not in labor. In 6 of these patients (group I), the fetal head and cervix condition were favorable for amniotomy, and in the other 6 (group II), they were not favorable. In all group I patients, a sudden and progressive descent of the fetal head, and onset and progress of labor were noted within 5 hours. Plasma 13,14-dihydro-15-keto-prostaglandin F2 alpha (PGFM) levels increased significantly (P less than 0.05) in 4 of these cases with time. In group II patients, descent of the head was less than that in group I patients (P less than 0.05), and neither strong labor nor rise of PGFM levels was noted within 5 hours. These data support our view that amniotomy at an appropriate time results in the onset and progress of labor, and the rise of plasma PGFM in virtue of the sudden and exponential increase of the head to cervix force, but amniotomy at an inappropriate time does not, because this force is unchanged.

Lack of desensitization of 3',5'-cyclic AMP response to thyrotropin stimulation in differentiated thyroid carcinoma.

The course of the cAMP response to thyrotropin (TSH) in 23 specimens of differentiated thyroid carcinoma and the adjacent normal tissue was studied. In the carcinomatous tissue, the cAMP concentration in slices incubated with TSH was significantly greater after 2 h than after 15 min of incubation; the level was almost the same at both times in normal tissue. The effects of an initial exposure of thyroid slices to TSH (50 U/l) on the subsequent cAMP responsiveness to the hormone were investigated in seven more differentiated thyroid carcinomas. In normal tissue, the cAMP level in slices exposed to TSH, washed, and exposed again was lower after the third incubation than in slices exposed to TSH only in the third. In contrast, the cAMP level tended to be greater after the second of 2 TSH incubations in 5 out of the 7 specimens of carcinomatous tissue. The data suggest that differentiated thyroid carcinoma lacks desensitization of the cAMP response to TSH.

High-level conjugation of chelating agents onto immunoglobulins: use of an intermediary poly(L-lysine)-diethylenetriaminepentaacetic acid carrier.

Diethylenetriaminepentaacetic acid (DTPA), a strong chelating agent, was covalently linked to murine monoclonal anti-HLA IgG1 antibody (H-1) with the use of poly(L-lysine) (Mr 14,000) as a multivalent, intermediary carrier, via thiol-disulfide exchange reaction. The conjugates contained up to 42.5 mol DTPA per mol antibody, and retained over 90% of their antibody activity in vitro. The conjugates incorporated gadolinium (Gd) through an exchange reaction with Gd-EDTA, used to prevent colloid formation and nonspecific binding of the free metal. The IgG-poly(L-lysine)-DTPA-Gd had a greater effect per mol on proton relaxation rates than DTPA-Gd itself. Use of poly(L-lysine) as an intermediary carrier for attachment of chelating agents to IgG thus offers great potential for achieving high-specific-activity conjugates, particularly for use as biologically specific contrast agents in nuclear magnetic resonance imaging.

Isolation of hydrogen-oxidation gene from Alcaligenes hydrogenophilus and its expression in Pseudomonas oxalaticus.

A gene bank of a megaplasmid encoding the hydrogen-oxidizing enzyme system (Hox) in Alcaligenes hydrogenophilus was constructed using a broad host range cosmid vector pVK102, and established in Escherichia coli. Hybrid cosmids containing hox genes were identified by transferring the bank into Pseudomonas oxalaticus OX1 and screening colonies for the ability of H2-dependent autotrophic growth. About 800 colonies were formed under autotrophic conditions. One of the Hox+ transconjugants was isolated and its hydrogenases activities were measured. Although soluble hydrogenase was not detected, the Hox+ transconjugant had four times the membrane-bound hydrogenase activity of A. hydrogenophilus.

Correlation of the main peripheral branches of the facial nerve with the cytoarchitectonic subdivisions of the facial nucleus in the guinea pig.

Correlation of the main peripheral branches of the facial nerve with morphological subdivisions of the facial nucleus was examined in the guinea pig by the retrograde horseradish peroxidase method. The facial nucleus of the guinea pig was divided cytoarchitectonically into the dorsolateral, lateral, intermediate, medio-intermediate, medial, and ventromedial divisions; the ventromedial division was further divided into the major, dorsal and lateral parts. Six main branches of the facial nerve were identified; the zygomatico-orbital, cervical, posterior auricular, anterior auricular, superior labial, and inferior labial branches. After applying HRP to the main branches of the facial nerve, the pattern of distribution of HRP-labelled neuronal cell bodies within the facial nucleus was examined: the dorsolateral division, dorsal part of the ventromedial division, major part of the ventromedial division, lateral part of the ventromedial division, or medial division contained the cell bodies of respectively the zygomatico-orbital, cervical, posterior auricular, anterior auricular, or superior labial branches, while each of the lateral, intermediate, and medio-intermediate divisions contained the cell bodies of both the superior labial and inferior labial branches.

Collagenolysis in human vaginal tissue during pregnancy and delivery: a light and electron microscopic study.

Biopsy specimens of the human vagina were studied by light and electron microscopy. In the nonpregnant state, the collagenous fibers were tight and regular. During pregnancy and after delivery the collagenous fibers were irregular and more spread out. Pronounced edema, increased vascularity, and dilated blood vessels were also characteristic findings; however, no interstitial hemorrhage or perivascular granulocyte infiltration was noted. At electron microscopy, collagenous fibers were thick and compact in the nonpregnant state, but thin fibrillar components with amorphous material were observed during pregnancy and post partum. Fibrillar elements were sparse around the fibroblasts and fat droplets in the fibroblasts were noted at term and after delivery. The possible involvement of prostaglandins released from fibroblasts and/or blood vessels is suggested in these processes.

Induction of squamous cell carcinoma in the rat lung by 1,6-dinitropyrene.

The carcinogenicity of 1-nitropyrene [(1-NP) CAS: 5522-43-0] and 1,6-dinitropyrene [(1,6-DNP) CAS: 42397-64-8] was examined by their direct injection in a beeswax-tricaprylin vehicle into the lung of male F344/DuCrj rats. Of 28 rats given 0.15 mg of 1,6-DNP, 21 (75%) developed squamous cell carcinomas, 2 (7%) developed undifferentiated carcinomas, and 2 (7%) had squamous metaplasias in the lung by 72 weeks. In 32 rats that received 1.5 mg of 1-NP, neither carcinoma nor squamous metaplasia was induced. In all 19 rats (100%) given 0.5 mg of 3-methylcholanthrene [(MCA) CAS: 56-49-5], squamous cell carcinomas were induced earlier than in rats treated with 1,6-DNP. In 1 of 31 rats (3%) given the beeswax-tricaprylin vehicle only, squamous metaplasia was induced. Distant metastases of induced tumors were observed in 4 rats treated with 1,6-DNP and in 1 rat receiving MCA. Two lung tumors induced by 1,6-DNP were successively transplanted into the same strain of rats for 3 generations.

[Clinical studies on pulmonary metastases from differentiated thyroid carcinoma--characteristics of patients with pulmonary metastases which appear before thyroid operation or afterward].

At Ito Hospital we have experienced 70 primary cases of differentiated thyroid carcinoma with pulmonary metastasis during the last 32 years. These patients were divided into two groups; patients who presented with the metastasis before thyroid operation (M-1 cases) and afterward (M-O cases). Contrary to our expectation, the over-all survival rate was more favorable in M-1 cases. We therefore studied on the clinical characteristics of these patients to see if there may be any differences between the two groups. The over-all survival rate of M-1 and M-0 cases was 85.7% and 42.9%, respectively. This difference was statistically significant (p less than 0.001). In M-1 cases percentage of male patients was higher, mean age was lower, papillo-follicular carcinoma was more predominant, and the outcome of RI therapy was more favorable. There were no significant differences on the roentgenographical type of metastatic foci and on the intraoperative findings between the two groups. From these results it is suggested that differentiated thyroid carcinoma with pulmonary metastasis in younger patients may be pathogenetically different from those in the older, and that biological characteristics of the disease that shows pulmonary metastasis in early stage differ from that which shows later.

Experimental evidence for the progress of labor with the increase in the force of cervical dilatation after rupture of the membranes.

To examine the mechanical forces involved in cervical distention, actual tensions of the fetal membranes up to the bursting level were measured experimentally with the use of five circular membrane holders that simulate various cervical dilatations (radii 1 to 5 cm). The results show that the force of cervical dilatation with the head alone increases remarkably as the cervix dilates, and this force is always larger than that when the membranes are intact. Thus the membranes appear to interfere with effective cervical dilatation and progress of labor after fixation of the fetal head to the cervix.
