An enzyme immunoassay (EIA), Chlamydiazyme (Abbott Laboratories), was evaluated for its determination of MICs of 15 antimicrobial agents against Chlamydia trachomatis (MRC-1, LB, TRIC/GB/MRC-1 Gf [ATCC VR-1]). The inoculum size, incubation time, and enhancers were defined for the assessment of chlamydial antigen synthesis in HeLa 229 cells seeded as monolayers in 96-well plates. MICs were determined and defined as the lowest antibiotic concentrations required to inhibit, after 24 or 48 h of incubation, antigen production as determined by the EIA. The MICs (after 48 h) were similar to those determined by the peroxidase-antiperoxidase staining of inclusions. MIC determinations after 24 h were suitable for screening the activities of quinolones, but less so for measuring the susceptibility of C. trachomatis to macrolides and tetracyclines. MIC determination by EIA was rapid, appropriate for standardization, and less cumbersome than determination by quantification of inclusions.
Sixty strains of Haemophilus influenzae were isolated from the genitourinary tracts of adults: 19 from cervicovaginal secretions, one from a woman with bartholinitis, 37 from urethral exudates, and three from urine. Non-capsulated strains were recovered predominantly, and biotype III accounted for 28 isolates and biotype IV for 25. Many of the H influenzae strains were found to be resistant to one or more of the antibiotics commonly used against sexually transmitted diseases. Resistance to tetracycline was prevalent and was found in 17 of the strains. Ten strains were ampicillin resistant and beta lactamase producing. Kanamycin resistance was less common (two strains). Trospectomycin (U-63366), a new spectinomycin analogue, was eight to 16 times more active than spectinomycin. All the quinolones tested were very active against all strains and may provide an effective alternative for the treatment of resistant H influenzae in genitourinary infections.
In a consecutive series of 65 patients treated by allogeneic bone marrow transplantation, we have compared the efficacy on prevention of bacterial infection of total gut decontamination with oral non absorbable antibiotics or with oral absorbable broad spectrum antibiotics. On day-8, all patients were randomly allocated to one group: Group I received orally pefloxacin 400 mg/day and penicillin 3 M UI/day. Group II received capsules containing cephalothin 250 mg, gentamicin 20 mg and bacitracin 150 UI. Each patient received 9 to 12 capsules per day according to body weight. Patients less than 5 years old or with severe hepatic abnormalities were excluded from the study. All patients were treated in LAF room with usual precautions of asepsis. They received sterile food. In addition, they were on ketoconazole for prophylaxis of fungal infections and acyclovir for prevention of herpes infections. Antibiotics were started on day-8 before bone marrow transplantation and stopped 15 days after discharge from the hospital. 32 patients were allocated to Group I and 33 in Group II. There was no difference between both groups according to age, sex, diagnosis. After transplant, the one year survival and the complications were similar in both groups. The median time of decontamination was 65 days, the mean number of days of agranulocytosis was 20 days, the mean number of days of fever was 7 days in both groups. The compliance was better in Group I but the treatment had to be modified in 9 patients of Group I because of liver abnormalities.(ABSTRACT TRUNCATED AT 250 WORDS)

Antibodies to Chlamydia were assayed by complement fixation (CF) and inclusion indirect immunofluorescence (IFI) in sera collected from 379 patients with salpingitis: 30.3% of the patients had total and IgM antibodies at IFI and CF antibodies (profile I); 26.6% of the patients had total and IgM antibodies at IFI without CF antibodies (profile II); 31.6% of the patients had only total antibodies at IFI without specific IgM and without CF antibodies (profile III); 11.3% of the patients were Chlamydia antibody negative (profile IV). In the control group of 50 pregnant women apparently non infected, the profile distribution was 2% profile I, 8% profile II, 38% profile III, and 54% profile IV. Detection of IgM antibodies to Chlamydia trachomatis in 57% of patients with salpingitis, taking only one specimen, suggested recent or active chlamydial infection. CF antibodies indicated diffuse infection. Total antibodies correlated well with IgG antibodies detected by ELISA. Their finding was by no means diagnosis for Chlamydia being the cause of tubal infection, although titers observed in salpingitis patients were higher than in controls.

The in vitro activities of new beta-lactam antibiotics and new quinolones were studied against 96 Capnocytophaga strains, including a beta-lactamase-producing strain which was resistant to ampicillin, amoxicillin, carbenicillin, cephalothin, and cefamandole. All strains were susceptible to the combination of amoxicillin and clavulanic acid, ureidopenicillins, cefoxitin, broad-spectrum cephalosporins, and imipenem. Cephalothin and cefamandole did not show good activity against most strains. All Capnocytophaga spp. were uniformly susceptible to the five new quinolones tested.
Cytomegaloviruses are characterized by the long duration of their reproducible cycle, their narrow host range, their latency of infection properties of the subfamily beta-herpesviridae. The cell infection leads either to the production of infectious viruses, as in the human fibroblast which is the only permissive cell in vitro, or to a latent infection possibly reactivated in cases of cellular immunodepression. Although these viruses shave some of the properties of the oncogenic viruses, there is no definitive evidence that they cause human tumors. They should be considered as cocarcinogens.
CMV can be isolated within 24 or 48 h post-infection of human diploid fibroblasts cells cultures by detection of immediate early antigen with immunological techniques using E13 monoclonal antibody. Optimal enhancement of CMV detection is obtained by centrifugation at 4,000 g for 45 minutes of specimen inoculum onto cell culture monolayers. Rapid immunodiagnosis can be applied to cells from clinical specimens faster than with culture but it is less sensitive. This rapid diagnosis of CMV infection enables early prescription of appropriate antiviral chemotherapy.
We compared the in vitro activities of 9 aminoglycosides including trospectomycin sulfate against 250 selected strains of Neisseria gonorrhoeae according to their penicillin resistance (penicillinase production), streptomycin resistance, spectinomycin resistance. Trospectomycin sulfate inhibited all spectinomycin-sensible strain at a concentration of 4 mg/l but the spectinomycin-resistant strain needed 32 mg/l for its inhibition. Among the other aminoglycosides tested netilmicin had the most in vitro effectiveness. Its minimal inhibitory concentrations were comparable to that of trospectomycin sulfate: furthermore this compound demonstrated in vitro activity against the spectino-resistant strain.
Ofloxacin is a new fluoroquinolone with excellent in vitro activity against N. gonorrhoeae. 32 adult males with acute uncomplicated gonococcal urethritis were treated with a single-dose of orally administered ofloxacin (400 mg). 1 out of 32 isolates was penicillinase-producing N. gonorrhoeae. Urethral cultures were obtained before treatment and on day 3-5. Microbiological cure was achieved in all patients (100%). No side effects were observed. According to these results, single-dose ofloxacin therapy (400 mg) is effective and safe against uncomplicated gonococcal urethritis in adult males.
Four patients developed legionnaires' disease after bone marrow transplantation. Two cases occurred early after transplant and were considered as part of a hospital epidemic due to contamination of water supply. The other two cases were considered to be sporadic because they occurred 3-4 weeks after hospital discharge. The outcome was good in two patients. In the third patient, recurrent disease was probably due to acquired resistance to macrolides, and complete cure was achieved after treatment with pefloxacin and rifampicin. The fourth patient died of overwhelming infection despite early treatment with erythromycin and pefloxacin. During the same period we treated 14 patients with pefloxacin for prevention of bacterial infection, of whom none developed Legionella pneumophila infection, while three of the patients reported here were in a group of 11 patients who received only oral non-absorbable antibiotics for gut decontamination. The fourth patient in this report was receiving no antibiotics. Thus pefloxacin seems to be effective as prophylaxis against L. pneumophila infection. When the hospital water supply was heated to 60 degrees C and chlorinated, the nosocomial cases in the hospital completely disappeared.
The specific antibody response to Epstein-Barr virus (EBV) antigens of 41 bone marrow transplant recipients with leukemia or aplastic anemia was examined retrospectively by immunofluorescence test (IF) over 1 year. We observed high titers (greater than 640) of IgG-viral capsid antigen (VCA) with emergence of IgG-early antigen (EA) and frequent absence or low levels of Epstein-Barr nuclear antigen (EBNA) antibodies. After absorption to remove rheumatoid factor (RF), five of the 41 recipients had IgM-VCA antibody to EBV, which appeared between weeks 26 and 48 after BMT and persisted for 1-4 months. No heterophil antibodies were detected in these sera, and none of the five recipients had a history of infectious mononucleosis.
