[Combined treatment in patients with carcinoma in situ of the urinary bladder using intravesical pirarubicin, irradiation and hyperthermia].

The treatment with hyperthermia in combination with radiation and intravesical pirarubicin (THP-ADM) was preliminary investigated in 5 patients with urinary bladder carcinoma in situ. Following intravesical administration of 30 mg THP-ADM, external irradiation of 3.0 Gy was delivered to the urinary bladder. Immediately then, hyperthermia using Thermotron RF-8 was performed for 50 min (intravesical temperature: 42-43 degrees C for 35 min). After five courses of the treatment, complete response has been maintained for 6, 8, and 9 months in 3 patients. However, in a patient with complete response, urinary cytology became positive in the 6th month after the treatment. In the remaining patient treatment was interrupted after only 3 courses due to urinary irritation, urinary cytology didn't become negative. The side effects of the combined treatment were limited to the transient symptoms of bladder irritation in all patients and thermal burn in 2 patients. These preliminary results suggest that this combined treatment may represent an effective conservative therapy for patients with urinary bladder carcinoma in situ.

Analysis of Epstein-Barr viral genomes in lymphoid malignancy using Southern blotting, polymerase chain reaction and in situ hybridization.

109 malignant lymphomas were surveyed by Southern blot analysis and polymerase chain reaction (PCR) for Epstein-Barr virus (EBV) DNA and compared with 16 examples of non-neoplastic lymphadenopathy and 4 normal thymuses. In specimens positive by the method of Southern and PCR, in situ hybridization studies were performed on formalin-fixed, paraffin-embedded sections. By Southern blot analysis, two of seven Hodgkin's disease samples (29%) (one of mixed cellularity and the other of lymphocyte predominance type), three of 56 B-cell lymphomas (5.6%) and five of 46 T-cell lymphomas (11%) demonstrated EBV DNA. However, the 16 examples of lymphadenitis and the 4 normal thymuses showed no EBV DNA. With PCR, EBV DNA was identified in one B-cell lymphoma, nine T-cell lymphomas, ten lymphadenitis specimens and two of the normal thymus, in addition to the positive specimens determined by the Southern blotting method. These results indicate that the presence of EBV DNA is not related to lymphoid malignancy, but enhancement of the DNA is demonstrated in some neoplastic conditions. By in situ hybridization, EBV genomes were not detected in all PCR-positive cases, but only in those positive by Southern blot analysis.

[A case of right renal pelvic and ureter cancer with hepatic metastasis showing complete response by M-VAC].

A 69-year-old man was admitted for further examination of right hydronephrosis in November 1988. Radiologically, excretory urography showed right non-functional kidney and antegrade pyelography revealed filling defect of right renal pelvis and ureter. Cytology findings of right renal urine indicated transitional cell carcinoma. Computed tomography revealed multiple mass of liver. The patient was administered M-VAC (methotrexate, vinblastine, doxorubicin and cisplatin) combination chemotherapy. After the 4 courses of treatment, the mass of the primary lesion and hepatic metastasis disappeared and he achieved complete remission. The side effects noted were mild myelosuppression, alopecia and gastrointestinal symptoms such as nausea and vomiting.

[Intra-arterial infusion chemotherapy in bladder cancer].

Between 1976 and 1985, 35 patients with bladder cancer were preoperatively treated with intra-arterial doxorubicin (ADM) chemotherapy. We separated these patients into 2 groups according to regimen: A) 1-3 shots of ADM infusion with or without radiotherapy, and B) 3-day courses of ADM infusion with low-dose radiotherapy. The 5-year actual survival rate according to stage (T category) was 100% in T1-2, 37.5% in T3 and 23.3% in T4. In T1-2, bladder preservative operations were done on 50% of the patients in group A and 100% in group B. In T3-4, the 5-year actual survival rate in group A was 22.5%, while in group B it was 52.8% (p less than 0.01). The bladder preservative operations were done on 33.3% of the patients in group A and 73.3% in group B. The patients who received ADM intra-arterial chemotherapy combined with low-dose radiotherapy for bladder cancers, who achieved complete or partial remission, might be able to preserve their bladder.

[Endourological management of ureteral strictures].

Endourological management was employed in five cases with ureteral strictures. The cause of stricture included ureteroileal anastomoses in two cases, open pyeloplasty in one, radical hysterectomy in one and retroperitoneal fibrosis in one. In four cases, strictures were dilated using a balloon dilation catheter (diameter 4 to 8 mm), followed by placement of ureteral stent (6 to 8.3 Fr), for from 8 days to 43 days. In one case, graduated flexible dilator was passed until a 10 Fr opening was obtained. A 6 Fr ureteral stent was placed for 42 days. Of the 5 strictures 3 were dilated successfully as judged by excretory urogram. Followup ranged from 7 months to 19 months. Unsuccessful dilation occurred in 2 cases. In one case, subsequent exploratory operation revealed retroperitoneal fibrosis. In another case, subsequent CT scan disclosed the metastasis of rectal cancer obstructing the site of ureteroileal anastomosis. Endourological procedure is valuable in the treatment of benign post-operative ureteral stricture.

[Metastatic tumor of the spermatic cord from gastric cancer].

A 70-year-old male who had undergone gastrectomy due to gastric cancer 6 years previously visited our department complaining of perineal and bilateral scrotal mass. Biopsy specimen of the perineal mass showed well-differentiated adenocarcinoma. The spermatic cords were resected en bloc with the entire scrotal content, penis and perineal skin. Histological examination of the spermatic cord, epididymis and perineal subcutaneous tissue revealed well differentiated adenocarcinoma similar to that of the lesion resected from the stomach 6 years earlier. The patient was followed for 2 years with no evidence of recurrence. Metastatic tumors of the spermatic cord and epididymis resulting from gastric cancer are rare and 31 cases previously reported in the Japanese literature are reviewed.

[Renal artery thrombosis following surgical treatment of chyluria].

A case of segmental renal arterial thrombosis occurring after bilateral surgical stripping and interruption of renal pedicle lymphatics for chyluria is presented. This case showed elevation of serum creatinine (6.6 mg/dl) and lactate dehydrogenase (1131 IU/L) on the 4th day after operation, and on the 14th day, renal scintigraphy with 99 m-Tc diethylenetriamine penta acid acetic showed bilateral marked decrease of glomerular filtration rate. Aortography and selective left renal angiography demonstrated left segmental renal thrombosis, but right renal arteries were intact on the 20th day after operation. Systemic anticoagulation therapy was performed, but left renal function was destroyed completely. Excessive and prolonged retraction of the renal artery with vascular tape may have resulted in this unfortunate complication.

[Renal cell carcinoma in the solitary kidney].

We have treated surgically 5 patients with renal cell carcinoma in the solitary kidney. The cause of renal absence was nephrectomy for renal stones, in 2 patients and renal tuberculosis, renal cyst and renal hypoplasia in 1 patient each. Four of the 5 patients died. One of the 4 patients died 5 days after surgery due to gastrointestinal bleeding, 1 due to metastasis, 1 due to gastric cancer and one due to hemodialysis complications. Surgical management of renal cell carcinoma of solitary kidney is discussed.

[Inverted papilloma of the ureter].

Two cases of ureteral inverted papilloma are reported. Case 1: A 48-year-old male had asymptomatic gross hematuria and filling defect of left middle ureter on intravenous pyelography. Segmental resection of ureter was performed. The specimen was a 1 cm polypoid lesion with histologic features resembling "multifocal bud-like proliferation", which was reported as the initial stage of the inverted urothelial tumor by Kunze et al. Case 2: A 64-year-old female with asymptomatic gross hematuria and complete obstruction of left middle ureter on ante- and retrograde pyelograms. Total nephroureterectomy was performed. A 4 cm lobulated and pedunculated lesion with histologic features of typical inverted urothelial papilloma was resected. Twenty-one cases of ureteral inverted papilloma in the literature, including our cases, are analyzed.

[One-stage operation of ureteropelvic junction and ureterovesical junction stricture].

One-stage surgical management for ureteropelvic (UP) and ureterovesical (UV) strictures is reported. A 3-year-old boy with UP and UV strictures on left side, underwent nephrostomy at 4 months old. The plastic operations which were Anderson-Hynes method for UP stricture and submucosal tunnel method with tailoring of dilated ureter for UV stricture were performed at the same time. The post-operative intravesical pyelogram and renal scintigraphy showed the improvement of hydronephrosis. Many authors have described two-stage operations for such cases. Our findings revealed that one-stage operation, carefully preserving the blood supply to the ureter, would be an alternative surgical management for UP and UV strictures.

[Prophylactic effects of a combined use of a cephem antibiotic, latamoxef, and tobramycin on postoperative infection in obstetrics and gynecology].

To prevent postoperative infection in obstetric gynecology, latamoxef (LMOX) and tobramycin (TOB) were intravenously administered to 81 patients at daily doses of 2 g and 120 mg, respectively, for 5 postoperative days, and the preventive effect of the combined drugs on postoperative infection were evaluated in terms of the clinical effects and safety. 1. The fever index was low as a whole; 3.38 +/- 2.30 degree hours in patients receiving simple panhysterectomy (n = 61), 3.21 +/- 3.84 degree hours in those given cesarean section (n = 12), and 3.53 +/- 2.78 degree hours in those given other surgical procedures (n = 8). 2. There were no abnormalities in hematological findings or liver and kidney functions 14 days after the surgery. PIVKA-II was observed in 3 of 32 patients 14 days after the operation, at a rate of 9.4%. 3. Except mild diarrhea which was observed in 7 of the 81 patients (8.6%) in 4 to 6 days after the operation, no subjective or objective side effects or postoperative complications were observed. The combined use of LMOX and TOB seems useful for preventing postoperative infection in obstetrics and gynecology in terms of its efficacy and safety.

The role of peripheral blood T and B lymphocytes in mitogen responses during human pregnancy.

Studies on the role of T and B lymphocytes of pregnant and post-partum women were performed in a pokeweed mitogen (PWM) stimulated culture system. The responses were assessed by immunofluorescent staining for intracytoplasmic immunoglobulins and solid-phase radioimmunoassay for Ig secretion into culture supernatants. As compared with non-pregnant control subjects, a slight decrease of helper T cell function and accelerated suppressor activity of T cells in pregnant and post-partum women in B cell differentiation promoted by PWM were demonstrated. Conversely, B lymphocytes in pregnant and post-partum women were hyperreactive in the presence of T lymphocytes from non-pregnant control subjects in the mitogen stimulated culture system.

Prophylactic long-term treatment of bladder tumors with oral chemotherapy (tegafur).

A total of 107 patients with bladder tumor (from Ta to T2) was studied. Patients were treated with or without prophylactic intravesical chemotherapy using mitomycin C or doxorubicin hydrochloride (Adriamycin) after the excision of tumor. After the preliminary treatment, 55 patients were given tegafur 600 mg p.o. daily (study group), and the other 52 patients (control group) were treated without tegafur. Both groups were followed up with periodic cystoscopy every three months. Actuarial nonrecurrence rate of the two groups were examined statistically. The results revealed that tegafur was a good prophylaxis against recurrence of the bladder tumor, significant differences at P from less than 0.001 to 0.025 were noted at every observation period except at six months. A significant difference at P less than 0.005 was also recognized from an overall comparison between two nonrecurrence curves.

Inhibition of retrovirus RNA-dependent DNA polymerase by novobiocin and nalidixic acid.

Inhibitors of bacterial DNA gyrase and eukaryotic DNA topoisomerase (novobiocin and nalidixic acid) were investigated with respect to their effect on the activity of RNA-dependent DNA polymerases from murine and avian retroviruses. Purified RNA-dependent DNA polymerase from AKR virus was inhibited more than 90% by 0.3 mg/ml and almost completely by 1 mg/ml of the drugs when poly(A) X oligo(dT)12-18 was used as a template-primer. In contrast to the enzyme from AKR virus, purified enzyme from avian myeloblastosis virus was less sensitive, i.e. nearly 50% activity remained even in the presence of 1 mg/ml of the drugs with the same template-primer. RNA-dependent DNA polymerase activity in AKR virus particles was inhibited, but was resistant to low concentrations of the drugs. The inhibition was not due to specific interaction between drugs and the template-primer or labelled precursor, since RNA-dependent DNA polymerase was inhibited by the drugs with activated calf thymus DNA or poly(C) X oligo(dG)12-18 as the template. Endogenous DNA synthesis by AKR virus particles was inhibited by novobiocin to the same extent.
