[Experimental and clinical studies of cefmenoxime in the field of obstetrics and gynecology].

The study group was organized to evaluate the usefulness of cefmenoxime (CMX) injection, a new synthetic cephalosporin, for the treatment of infections in the field of obstetrics and gynecology. Fundamental and clinical studies were made by the society and the following results were obtained. 1. The peak distribution of CMX's MIC for E. coli, Klebsiella sp., Enterobacter sp., Bacteroides sp. and Peptococcus sp. isolated from obstetrical and gynecological infections with relatively high frequencies area 0.1, less than or equal to 0.05, 0.2, 3.13, 1.56 micrograms/ml, respectively, with an inoculation of 10(6) cells/ml. 2. When 1 g of CMX is administered by intravenous drip infusion for 1 hour, the maximum concentrations in various tissues of female genital organs were as follows: 14.2 and 13.2 micrograms/g in ovary and oviduct, respectively, at 1.20 hours after the start of administration, and 16.9 and 26.3 micrograms/g in corpus uteri and cervix uteri, respectively, after 1 hour. As for the transfer to the exudate in the pelvic dead cavity, the peak concentration was 15.6 micrograms/ml after 2.13 hours. 3. In the clinical studies, CMX was given to 258 cases with female genital organ infections and others. As for the clinical effects, with exclusion of 3 cases in which other antibiotics are concomitantly used, responses were excellent in 76 cases, good in 162 cases and poor in 17 cases, among 255 cases in total. The efficacy rate was 93.3%. The efficacy rates by diseases were 97.1% (68/70) for intrauterine infections, 88.8% (79/89) for intrapelvic infections, 98.4% (62/63) for adnexitis, and 100% (23/23) for infections of external genital organs. As for the clinical effects on causative bacteria, the efficacy rates were 100% (19/19) for single infections due to Gram-positive bacteria, 94.8% (55/58) for single infections due to Gram-negative bacteria, and 88.2% (15/17) for single infections due to anaerobic bacteria. And its efficacy rates were 89.6% (69/77) for mixed infection cases. Side effects were observed in 2 cases (0.8%); 1 case with eruption, and 1 case with diarrhea and vomiting. As for abnormal laboratory findings, lower white blood cell count was observed in 2 cases and elevation of the values regarding hepatic functions in 9 cases. All cases were returned to the normal after the completion of the administration. Cefmenoxime showed a satisfactory clinical efficacy and a potent bacteriological effect in treatment of the infections in the field of obstetrics and gynecology, and it has been concluded that cefmenoxime will be useful addition to the antibiotics for the therapy of these infections.

Local therapy of carcinoma of the uterine cervix: part I.

One-hundred-fifteen patients with cervical cancer were treated by placing bleomycin, carbazilquinone (Carboquone), and 5-fluorouracil suppositories in contact with cancer lesions located in the vaginal portion of the uterus or by inserting them into the cervical canal, twice a week, one to 14 times. The cancer lesions disappeared form 17 to 33 patients with Stage 0 lesions and 9 of 42 with Stage IA lesions. The remaining lesions were principally found in the deep portion of the cervical glands and tended to decrease the more often the suppositories were applied. This led to the presumption that if this local therapy had been carried on longer it would have eradicated even the remaining lesions from the patients with Stage 0 and IA lesions. The optimal per administration dose of bleomycin appeared to be 30 mg. The use of Witepsol (Dynamit Nobel Aktiengesellschaft, F.R.G.) suppository base was accompanied by frequent adverse reactions to it, which hindered the local therapy from being repeated over long periods of time. An equal amount mixture of hydroxypropyl cellulose and carbomer (Carbopol 934) as the suppository base was accompanied by relatively less adverse reactions, which permitted the prolonged local therapy with the suppositories. A trace of bleomycin was also found in the ovaries, but it did not cause microscopically evident histologic changes, nor did it affect the menstrual cycle. If early cancer can be treated completely by local therapy, it will be useful for the younger patient with early cervical cancer.

Cellular immunity in normal pregnancy and abortion: subpopulations of T lymphocytes bearing Fc receptors for IgG and IgM.

Studies on the change of peripheral T and B lymphocytes and T cells bearing Fc receptors for IgG (IgG.FcR+.T lymphocytes) and IgM (IgM.FcR+.T lymphocytes) in normal pregnant women and patients with threatened abortion were performed by using rosette-formation tests. There was no significant difference in the proportion of T and B lymphocytes between pregnant and nonpregnant women. The percentage change of T cells of the patients in threatened abortion with good prognosis was significantly decreased and that of B cells of the patients in threatened abortion was significantly increased compared with that of normal pregnant women. The percentage of IgG.FcR+.T lymphocytes in the T lymphocytes increased in the various stages of pregnant and postpartum women as compared with that in nonpregnant women, and in the case of the patients with threatened abortion it also increased significantly from that of normal pregnant women. On the contrary, the percentage of IgM.FcR+.T in the T lymphocytes decreased in normal pregnant and postpartum women.

Cellular immunity in pregnancy: subpopulations of T lymphocytes bearing Fc receptors for IgG and IgM in pregnant women.

Studies on the change of peripheral T and B lymphocytes and T cells bearing Fc receptors for IgG and IgM in pregnant women were performed by using rosette-formation tests. There was no significant difference in the proportion of T and B lymphocytes between pregnant and non-pregnant women. The percentage of T cells bearing Fc receptors for IgG in the T lymphocytes which are considered to have suppressive activity increased in the various stages of pregnancy and post-partum as compared with that in non-pregnant women. On the contrary, the percentage of T cells bearing Fc receptors for IgM in the T lymphocytes which have a helper function decreased in pregnant and post-partum women. The results of this investigation suggest that the depression of cell-mediated immunity during pregnancy depends on the qualitative change of T lymphocytes, i.e. increased suppressor and decreased helper T lymphocyte subpopulations.

Viral placentitis - a case report.

A case of viral placentitis, in a 20 year-old pregnant woman suffering from chronic glomerulonephritis, was presented. In light microscopic findings of the placenta, many necrotic foci with intranuclear inclusion - bearing cells revealed in chorionic villi and by electron microscopy numerous immature and mature virus particles, probably herpes simplex were proved in intranuclear inclusion-bearing cells.

A case of hemorrhagic cyst of the pancreas resembling the cystic endometriosis.

A 47-year-old Japanese woman with a history of epigastric pain and a recent episode of acute pancreatitis (back pain, nausea, and vomiting) and anemia was found to have a pancreatic cyst of the tail on CT-scan and ultrasonography. Especially, ultrasonography revealed the papillary solid lesion in the cyst. With the tentative diagnosis of a cystic neoplasm, distal pancreatectomy was performed. Histological examination of sections showed massive hemorrhage, surrounded fibrous connective tissue, and numerous macrophages with hemosiderin deposits; these histological findings resembled cystic endometriosis. The clinicopathological features and pathogenesis of the pancreatic endometrial cyst are discussed.
