An analysis on follow-up results of photoreactive keratectomy for treatment of myopia.

A prospective study of excimer laser photorefractive keratectomy (PRK) was performed with the aim of correcting a range of myopic errors between -1.00 and -10.00 diopters on 422 myopic eyes that were followed for more than one year. At postoperative one year, 91.4% of the eyes had uncorrected visual acuity of 20/20 or better, and 97.4% were within +/- 1.00 diopter of the desired emmetropia, and the corneal haze disappeared in most of the eyes. Elevated intraocular pressures were observed in 20.4% of the eyes. The decrease of one line of the best corrected visual acuity occurred in 7.6% and of two lines in 0.5% of the eyes. We think that the excimer laser PRK is a very predictable, safe, stable, and effective method to correct myopia up to -10.00 D, and longer follow-up will be necessary.

Excimer laser photorefractive keratectomy for myopia in China. A report of 750 eyes with a 6-month follow-up.

A total of 750 six-month follow-up records of myopia treated with excimer laser photorefractive keratectomy (PRK) were analyzed. By 6 months, in group 1 (up to -6.00D, 587 eyes), uncorrected visual acuity was improved to 20/20 or better in 86.7% of treated eyes and 95.4% fell into the range of refraction of +/- 1.00D. In group 2 (-6.25D to -11.00D, 163 eyes), 74.5% of treated eyes had uncorrected visual acuity of 20/20 or better and 89.4% of the eyes were within +/- 1.00D. The designed predictability of the two groups was the same (P > 0.1). The refractive stability from 3 to 6 months between the two groups was not statistically different, and no severe complications were observed in these two groups. The excimer laser PRK for correcting myopia up to -11.00D appears to be effective, predictable, stable and safe in this study. The ideal outcome may result from the precise ablation quality of excimer laser, its computerized surgical manipulation and an appropriate postoperative management including detection of refractive indices by corneal topography and modulation by steroid.

[The effectiveness of combined insulin and sulfonylurea in treating non-insulin dependent diabetic patients].

The effectiveness of combined therapy with insulin and sulfonylurea in non-insulin dependent diabetes mellitus (NIDDM) patients has, for decades, been a debated issue. In order to probe this question, we studied 33 diabetes mellitus patients aged 40 years and over, having a history of this disease for more than 5 years, treated with one of the sulfonylureas at maximal dosage for three weeks but without effect. These 33 cases were divided into three groups randomly: group A: treated with glurenorm and insulin. B insulin and C glurenorm. The treatment lasted 4 months. During the course of the trial, estimation of the free-insulin, C-peptide, blood glucose etc. was carried out three times i.e. one before the trial and then 2, 4 months after the trial. The data were dealed with U-test and comparison was made before and after the trial and between the three groups. It is shown that combined therapy with sulfonylurea and insulin is effective in NIDDM patients, especially in those with secondary failure of beta-cell function and the combined therapy may be used in NIDDM patients during the transitional period from treatment with sulfonylurea alone to traditional insulin cure.

[The clinical significance of detecting minimal residual disease in the bone marrow of patients with B-cell neoplasm].

Clonal IgH gene rearrangement as a gene marker of B-cell malignancies, a new method of polymerase chain reaction amplification in conjunction with single-strand conformational polymorphism (PCR-SSCP) analysis was performed on the stored bone marrow slide specimens from patients with B cell neoplasm for detecting minimal residual disease (MRD). The method was sensitive to 1-2 malignant B-cells in 1,000 normal bone marrow cells. In untreated patients with B-NHL, the incidence of MRD in bone marrow specimens was 68.4% (26/38). Patients with MRD in bone marrow had a shorter survival time than those without (P < 0.05). In patients with B-ALL in complete clinical remission, the incidence of MRD in bone marrow was 71.4% (15/21). There was a tendency of relapse in patients with MRD in bone marrow but persistent complete remission in patients without. The results showed that detection of MRD in bone marrow may provide some important information for assessing prognosis and predicting relapse in patients with B-cell malignancy.

Inhibition of angiotensin-converting enzyme and attenuation of myocardial fibrosis by lisinopril in rats receiving angiotensin II.

High-density angiotensin-converting enzyme (ACE) binding is present in heart valve leaflets and the fibrous tissue that appears in the rat myocardium after either chronic administration of angiotensin II (AngII) or after myocardial infarction. This suggests that connective tissue ACE is independent of circulating AngII and that ACE may be an integral component of normal and pathologic tissue repair. To address this possibility the present study was undertaken. We sought to determine whether the ACE inhibitor lisinopril would attenuate fibrous tissue ACE binding and fibrous tissue formation in the myocardium of rats receiving AngII. Three experimental groups were examined: untreated, age-matched controls; rats receiving subcutaneous AngII (150 ng/min) by minipump for 2 weeks; and rats receiving AngII plus oral lisinopril (20 mg/kg/day) for 2 weeks. Cardiac ACE binding density was localized and quantified by in vitro autoradiography with [125I]-labeled 351A, a tyrosyl derivative of lisinopril, while fibrosis was identified by light microscopy in serial sections stained with picrosirius red. Hematoxylin and eosin and anti-fibronectin antibody were used to identify cardiac myocyte necrosis. Immunohistochemical labeling with alpha-smooth muscle actin was used to identify myofibroblasts.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of Sichuan herba Epimedii on the concentration of plasma middle molecular substances and sulfhydryl group of "yang-deficiency" model animal].

The effects of tonifying the kidney and strengthening the "Yang" produced by the extracts of Chinese herbs Epimedium wushanense and E. pubescens were studied in this paper. The results showed that both two herbs could decrease the concentration of plasma middle molecular substances and increase the concentration of plasma sulfhydryl group of "Yang-deficiency" model mice, thus suggesting that the above-said tonifying and strengthening effects of Herba Epimedii may result from its effects on middle molecular substances and sulfhydryl group, so as to strengthen the body resistance and eliminate the invading pathogenic factors.

Analysis of plasma and urinary tea polyphenols in human subjects.

Tea has been shown to inhibit chemically induced tumorigenesis in many animal models, but the effects of tea consumption on human carcinogenesis are not conclusive. In order to develop biomarkers for tea consumption, we developed methods for the analysis of tea polyphenols in human plasma and urine samples using HPLC with the coulochem electrode array detection system. (-)-Epigallocatechin-3-gallate (EGCG), (-)-epigallocatechin (EGC), (-)-epicatechin-3-gallate (ECG), and (-)-epicatechin (EC) are the major polyphenols in green tea. Most of the tea polyphenols were in their conjugated forms in the plasma and urine. The samples were incubated with a mixture of beta-glucuronidase and sulfatase to generate the free form of tea polyphenols. After extraction into ethyl acetate and separation by reversed-phase chromatography, EGCG, EGC, and EC were identified on the basis of their retention times and electrochemical characteristics. Due to the high selectivity of the detection mode, interference was minimized. Good quantitative relationships were established for a large concentration range of tea polyphenols. The limits of detection for EGCG, EGC, ECG, and EC were from 0.5 to 1.5 ng/ml of plasma or urine sample. After ingestion of 1.2 g of decaffeinated green tea in warm water, the plasma samples collected at 1 h from 4 human volunteers contained 46-268 ng/ml of EGCG, 82-206 ng/ml of EGC, and 48-80 ng/ml of EC. ECG was not detected in plasma samples. The maximum urinary excretion of EGC and EC occurred at 3-6 h.(ABSTRACT TRUNCATED AT 250 WORDS)

Prospective multimodality treatment of SCLC--experience during the past 18 years.

Lung cancer has been one of the leading malignancies in China. Since 1989 it has ranked first among common malignancies in the male population, and second in females. In 1975, a multimodality treatment study group for lung cancer was organized in our institute. The group has since expanded to 15 institutions in the Beijing and Tianjin area during the past four years. The present communication is based on our experiences and data, and tries to explain the treatment strategy of our study group.

Local regulation of extracellular matrix structure.

The extracellular matrix (ECM) is composed of various collagens, glycosaminoglycans, and elastin bathed by a tissue fluid found throughout the interstitial space. It is this substratum in which fibroblasts and macrophages normally reside, where fibroblast phenotypic transformation occurs, and into which inflammatory cells migrate when called upon during tissue repair. Many diseases, expressed in an organ-specific manner, require organ-specific ECM remodeling. Regulation of fibrillary type I collagen synthesis, whose disproportionate (relative to degradation) accumulation is characteristic of the tissue fibrosis that adversely alters organ function, is therefore of considerable importance. Emerging evidence implicates angiotensin converting enzyme (ACE), found in fibroblast-like cells, and ACE-related peptides, angiotensin II and bradykinin, in serving important regulatory functions that influence wound healing and thereby ECM structure in health and disease. The heart and its collagen matrix have been targeted for discussion in this brief review.

[The detection of p53 gene and HPV16 oncogene in cervical carcinoma].

Special primers of HPV16 E6 and E7 ORFs as well as p53 exon 7 were used with PCR, PCR-SSCP technique to study 35 cervical carcinoma specimens. The results were as follows: (1) HPV16 E6 and E7 DNA were found in 25/35 specimens (71.42%). However, only 11/35 (31.42%) had E6, E7 ORFs Simultaneously; 3/35 (8.57%) had E6 ORFs and 11/35 (31.42%) had E7 ORFs, indicating that the distribution of E6 E7 genes were heterogeneous in cervical carcinoma. (2) No mutation and loss of heterogeneity of p53 exon 7 was found in any of the 35 specimens, indicating that the alteration of p53 exon 7 in cervical carcinoma was uncommon.

Effects of acute and delayed effects of prior chronic cocaine administration on regional rates of cerebral protein synthesis in rats.

Single or repeated treatments with cocaine (15 mg/kg, i.p.) in rats modify rates of local cerebral protein synthesis (ICPSleu) measured with the [1-14C]leucine method. A single dose of cocaine to naive rats reduced ICPSleu by about 10% throughout the brain; the most statistically significant reduction was in the nucleus accumbens, shell portion (P = .0003). A comparable dose of cocaine administered acutely after 1 wk of daily cocaine injections had no effects on ICPSleu. Delayed effects of prior chronic cocaine treatment were studied in experiments in which one rat of each pair received injections with saline for 8 days and the other cocaine, and on the 15th day ICPSleu was measured. In these experiments delayed effects of the chronic cocaine treatment were observed; in the cocaine-treated rats ICPSleu was significantly increased in selective brain regions, i.e., prefrontal and primary olfactory cortex (P < .006). These results suggest that acute effects of a single dose of cocaine and residual effects of chronic cocaine treatment on ICPSleu are distinctly different and occur in different regions of the brain.

Angiotensin converting enzyme and kininase-II-like activities in cultured valvular interstitial cells of the rat heart.

OBJECTIVE: The function of angiotensin converting enzyme (ACE) at cell sites of high collagen turnover, such as heart valves, is uncertain. The aim of this study was to assess ACE and kininase-II-like activities and collagen turnover in cultured valvular interstitial cells of the adult rat heart. METHODS: The valvular interstitial cell phenotype was determined by immunolabelling (rhodamine phalloidin, desmin, and Griffonia simplicifolia lectin), and the presence of ACE mRNA and protein was confirmed by reverse transcriptase-polymerase chain reaction analysis, ACE monoclonal antibody and in vitro autoradiography, respectively. ACE and kininase-II-like activities in valvular interstitial cells were analysed by high performance liquid chromatography. Angiotensin II (AT1) and bradykinin receptors in valvular interstitial cell membranes were examined by western immunoblotting and binding assay. Type I collagen and collagenase in valvular interstitial cell culture media were determined by ELISA and zymography, respectively. Type I collagen mRNA expression in cultured valvular interstitial cells was determined by northern blot analysis and in situ hybridisation. RESULTS: In intact valvular interstitial cells or their cell membrane we found: (1) actin microfilaments, but not desmin or lectin labelling; (2) ACE mRNA expression and binding activity; (3) conversion of angiotensin I to angiotensin II, which was completely inhibited by 50 microM lisinopril, while kinase-II-like activity exceeded ACE activity and was not inhibited by lisinopril; (4) AT1 and bradykinin receptors in valvular interstitial cell membrane preparations; (5) type I collagen mRNA expression and collagenase activity; and (6) angiotensin II induced increase in type I collagen synthesis and mRNA expression. CONCLUSIONS: Cultured valvular interstitial cells represent a nonendothelial, non-smooth-muscle cell type that expresses mRNA for ACE and type I collagen. ACE and kininase-II-like activities in valvular interstitial cells may be involved in the regulation of peptides that influence collagen turnover. Angiotensin II stimulates type I collagen synthesis and mRNA expression in these cells.

Evaluation of a tubular mesh and a spiral stent woven from titanium nickel alloy for prostatic obstruction in patients at high risk for surgery. Report of 62 cases.

The use of two urethral stents woven from titanium nickel alloy (TiNi) in the form of a tubular mesh or a spiral stent (both made in China) is described. They were implanted in 62 patients with prostatic outflow obstruction. All patients were considered contraindicated for surgery. They were divided into a spiral stent group (group 1) treated between March 1992 and May 1993, comprising 35 cases, and a tubular mesh group (group 2) treated between October 1993 and December 1994, comprising 31 cases, including four failures in group 1. Thirty-three out of the 35 patients in group 1 were treated successfully. Good results were achieved in eight cases (22.8%) and significant improvements occurred in 24 (68.5%), giving a total effective rate of 91.3%, with a follow-up of 11 to 27 months. Fourteen stents were removed within 6 months after the insertion and six more were removed within 12 months. The mean effective time of the stent in situ was 10.8 months. All 31 cases in the mesh group were treated successfully. Dramatically good effects were obtained in 28 cases (over 90%) and distinct improvements were achieved in two, giving a total effective rate of over 96.5%, with a follow-up of 6 to 14 months (mean 10.5 months). Cystoscopy was carried out in 12 patients after 6 months following the insertion. The major part of the meshes became covered by urothelium. Compared with the spiral, the mesh makes it possible to insert a prosthesis with a larger diameter and anti-pressure. The spiral, however, can be used as a temporary alternative for the relief of prostatic obstruction. A tubular mesh can work well for the relief of prostatic obstruction and remain in situ without causing major problems for at least 1 year.
