[Effect of diuretic, azosemide (SK-110), in combination with antibiotic, cephaloridine, on kidney].

This study was performed to compare the acute-renal toxicity of azosemide (SK-110) or furosemide (FM) treatment in combination with cephaloridine (CER). 1) Initially, the acute-renal toxicity of CER was studied after a subcutaneous administration at dose levels of 500, 1000, 1500 and 2000 mg/kg. The values of BUN, creatinine and relative kidney weights were increased in rats given CER at dose levels above 1500 mg/kg. consequently, it was considered that the threshold of CER with regard to acute-renal toxicity was 1000 mg/kg. 2) Secondarily, the acute-renal toxicity of SK-110 or FM were studied after oral administration of 40, 80, 160 or 320 mg/kg doses alone or in combination with CER. Acute-renal toxicity was evident in the 320 mg/kg-treated rats after both SK-110 and FM alone, as revealed by analysis of a number of parameters, i.e., BUN, serum creatinine, urine sediment and composition and pathological data composing both kidney weights and studies at the microscope level. However, the increase of BUN and relative kidney weights values, elevated numbers of epithelial cells in the urine and necrosis observed in the uriniferous-tubular epithelium on histopathological examination of kidneys were noted in rats given at dose levels of only 80 mg/kg with SK-110 + CER, whereas they were seen in animals treated with FM even at the lowest applied dose of 40 mg/kg in combination with CER. In conclusion, this study showed that the minimum dose of SK-110 or FM in combination with CER which causes acute-renal toxicity were, respectively, 80 mg/kg or less than 40 mg/kg.

[Effect of azosemide (SK-110) and its metabolites on mouse liver].

The effect of azosemide (SK-110), and its metabolites, 5-(2'-amino-4'-chloro-5'-sulfamoylphenyl)-tetrazole (M-1), 2-thiophenecarboxylic acid (TC), on mouse liver was investigated using biochemical and pathological parameters as indices of hepatotoxicity. The effects were compared with that of furosemide (FM) administration. A single dose of each compound was administered orally, or intraperitoneally, while multiple oral dosing was carried out once daily for a week. The results are summarized as follows: 1) SK-110 did not produce hepatotoxicity even after a single p.o. dose as high as 5000 mg/kg, a single i.p. dose of 400 mg/kg, or multiple p.o. doses of 700 mg/kg/day. 2) M-1 also did not produce hepatotoxicity even after a single p.o. dose of 4000 mg/kg or multiple p.o. doses of 550 mg/kg/day. TC did not exert hepatotoxicity after a single i.p. dose of 150 mg/kg or multiple p.o. doses of 250 mg/kg/day, but did produce hepatotoxicity after a single p.o. dose of more than 1000 mg/kg. However, it was presumed that, in vivo, TC formed as a metabolite of administered SK-110 would hardly produce hepatotoxicity. 3) FM produced hepatotoxicity after a single p.o. dose of more than 800 mg/kg, or a single i.p. dose of more than 200 mg/kg, but not after multiple p.o. doses of 700 mg/kg/day. Based on these findings, it was concluded that, in contrast to FM, SK-110 had no hepatotoxic effect on the mouse liver.

Electron microscopic study of cells in vitreous of guinea pig.

The fine structure of the cells in the posterior segment of the vitreous body of three Hartley strain guinea pigs was investigated by electron microscopy. A total of fifty-four cells were observed. Fifty-three of the 54 cells had specific granules which manifested homogeneous or heterogeneous appearance and were considered as hyalocytes. A difference in cell activity was demonstrated among these cells. Only one cell of the 53 suggested phagocytosis. Another cell was discharging lipopigment. This suggests that hyalocyte granules are gradually degraded, becoming of more complicated structure, and are discharged into extracellular space. One of the 54 cells had cytoplasmic filaments and was not considered as a hyalocyte.

Vitiligo with an inflammatory erythema in Vogt-Koyanagi-Harada disease: demonstration of filamentous masses and amyloid deposits.

A 35-year-old Japanese woman developed vitiligo with an inflammatory erythema on the face two years after the onset of bilateral uveitis and dysacusis. Histological studies of the lesions revealed the loss of pigment and dense mononuclear cell infiltration around the blood vessels. At the dermo-epidermal junction area, a few eosinophilic masses were found. Electron microscopy disclosed that the masses were filamentous masses (colloid bodies) and amyloid. After the oral administration of prednisolone, the erythema subsided, and the vitiligo showed some repigmentation.

Mechanisms of allograft rejection of corneal endothelium.

The local intraocular graft-vs.-host (GVH) reaction, involving the destruction of the corneal endothelial cells of the rabbit host by sensitized donor lymphoid cells, has been used to study the mechanism of corneal allograft rejection. Pretreatment of donor cells with a specific mouse monoclonal hybridoma anti-T cell antibody and complement suppresses the destructive reaction, suggesting that a cellular-immune mechanism is primarily involved. Pretreatment of donor cells with mitomycin-C completely abolishes the local GVH reaction, indicating that the effector lymphocytes must undergo mitosis within the eye before they can engage in target cell destruction. Finally, studies of the local GVH reaction in irradiated leukopenic recipients or in preinflamed rabbit eyes suggest that host leukocytes may contribute nonspecifically to enhance the destructive process. These studies show that the local ocular GVH reaction may provide a useful model for the study of the mechanisms involved in the rejection of corneal allografts.

Orbital leiomyoma accompanied by orbital pseudotumor.

We examined a 53-year-old man who had diplopia and proptosis of the right eye. Computed axial tomography showed a mass extending from the retrobulbar space to the apex and the lower portion of the right orbit. The tumor removed by the first operation from the retrobulbar space was diagnosed as leiomyoma. The mass palpated through the tissues of the lower right eyelid in the initial examination enlarged 7 months after the first operation. The tumor specimen subsequently obtained from this portion histologically revealed inflammatory pseudotumor. Therefore it was concluded that this patient had a leiomyoma accompanied by an inflammatory pseudotumor ipsilaterally.

Secretory component of IgA: a marker for differentiation of ocular epithelium.

Secretory component (SC) was studied by indirect immunofluorescence of the ocular surface epithelium. We find that conjunctival epithelium produces this component, and that it is absent in the corneal epithelium. Conjunctival epithelium loses its SC staining within 1 to 2 days as it grows over a denuded corneal stroma. This implies a very rapid turn-off of the SC gene and rapid export of the gene product previously formed. However, conjunctival flap epithelium does not change its characteristic structure or functions. Vascularization of corneas resurfaced by conjunctival epithelium usually leads to a rapid reversal of both morphological and biochemical characteristics in the surface epithelium, as judged by the reappearance of goblet cells and positive staining for SC. Vascularization of normal corneas leaves the epithelium unchanged, so that neither goblet cells nor SC appear. Thus metaplasia of conjunctival to corneal epithelium is incomplete, permitting reversion to type under certain conditions.

[Shoulder movements during abduction and adduction of quadriplegic patients (author's transl)].

The relationship between glenohumeral and scapular movements during abduction and adduction of the limb in the scapular plane were studied in twenty normal subjects and nine quadriplegic patients by analysis of roentgenograms and enveloped electromyograms with electrogoniometer. In normal subjects the ratio of glenohumeral to scapulothoracic movement was 4.2 : 1 for the range 0-30 degrees and 1.7 : 1 for the range 30-150 degrees for abduction, and 1.9 : 1 for the range 150-30 degrees and 1.7 : 1 for the range 30-0 degrees for adduction. In comparing quadriplegic patients with normal subjects, outstanding medial shift of the scapula was observed in early phase of abduction, elevation, and upward rotation of the scapula which were limited through whole abduction. Distances of the shift of the scapula in quadriplegic patients were the same as those in normal subjects in the final phase of adduction. In enveloped electromyograms of normal subjects, each activity of the six sampled muscles, increased or decreased gradually during abduction and adduction in similar relationships. The upper fiber of the trapezius demonstrated the highest activity through abduction and adduction. During abduction and adduction the supraspinatus showed undulated wave patterns and had the plateau at 80-140 degrees. The activity of the supraspinatus was higher than that of the deltoideus statistically for the range 0-40 degrees for abduction. In each quadriplegic patient the patterns of muscle activities during abduction and adduction were characteristically peculiar and did not indicate any constant relationship findings in normal subjects, irrespective of the functional level.
