One homozygous cell "Wa" for HLA-D alleles, associated with HLA-Bw22J (Japanese specific variant of HLA-Bw22), was found in a Japanese family. The HLA-D antigenic specificity defined by this cell has been tentatively named LD-Wa. Phenotype frequency of LD-Wa in the Japanese population was 0.16 (13/81). HLA-D typing for LD-Wa was performed in patients with the Vogt-Koyanagi-Harada syndrome (Harada's disease) with which Bw22J is strongly associated (15/35, 42.9%; corrected P less than 0.02). LD-Wa was presented in 22 of 33 patients (33.7%; P less than 0.0000003). The association of LD-Wa with Harada's disease was stronger than that of Bw22J, and this implies that there might be one of the disease susceptibility gene loci affecting the development of this disease more closely linked to an HLA-D gene locus than the HLA-B locus.

Between 1969 and 1991 a total of 136 patients (119 men, 17 women) underwent bronchoplastic procedures for lung cancer. A bronchoplastic procedure with angioplasty was performed in 37 patients. Ages ranged from 30 to 79 years (mean 62 years). The histologic type of cancer was squamous cell carcinoma in 97 patients (70.0%). The 30-day mortality was 5.1% (7 patients), and morbidity was 30.1% (41 patients). The most common complications were bronchopleural fistula in nine (6.6%), stricture or stenosis in eight (5.9%) and atelectasis in eight patients. Local recurrence occurred in nine (6.6%) patients. The overall 5-year survival for patients undergoing bronchoplastic procedures was 37.1%: 60.1% for patients with stage I disease (n = 41), 31.7% for stage II (n = 17), and 29.7% for stage IIIA (n = 66). We conclude that bronchoplastic procedures are effective therapy for selected patients with lung cancer.
PURPOSE: To determine the pathophysiological etiology of superior limbic keratoconjunctivitis (SLK), we compared the superior limbic conjunctivae of SLK patients and normal controls. METHODS: Frozen sections of conjunctival specimens from five SLK patients and two controls were examined by immunohistochemical methods. Transforming growth factor (TGF)-beta2, integrin beta1, and tenascin (TN) were chosen for analysis because their expression is known to be affected by mechanical stress or injury. The staining pattern was observed by confocal laser scanning microscopy. RESULTS: Prominent positive TGF-beta2 staining on the surface region and heterogeneous staining in the suprabasal region were observed in the SLK specimens. TN expression was markedly up-regulated in the subepithelial stroma. In addition, suprabasal expression of integrin beta1 was observed. CONCLUSIONS: Up-regulation of TGF-beta2 and TN suggested that an increased amount of mechanical stress existed in the conjunctivae of the SLK patients. In addition, deposition of TN and suprabasal expression of integrin beta1 suggested that chronic minor injury contributed to the pathogenesis of SLK.
PURPOSE: To investigate the correlation between monocyte chemotactic protein-1 (MCP-1) levels in the vitreous and clinical findings in eyes with proliferative diabetic retinopathy (PDR). METHODS: We assayed MCP-1 levels by ELISA in vitreous samples of 88 consecutive patients with PDR (52 eyes) and macular holes or idiopathic epimacular membrane (controls, 36 eyes). RESULTS: The level of MCP-1 in the vitreous was 2,097.5 +/- 1,099.4 pg/ml (mean +/- SD) in PDR, and 504.3 +/- 405.6 pg/ml in the controls. In PDR eyes, multivariate regression analysis revealed a significant association between MCP-1 levels in the vitreous and the degree of proliferative membrane, and a significant negative association between MCP-1 levels and the extent of preoperative retinal photocoagulation. CONCLUSION: The results suggest that MCP-1 may play a role in the development of the proliferative phase of PDR.
