Modification of the Abbé flap for reconstruction of the vermilion tubercle and Cupid's bow in cleft lip patients.

PURPOSE: This article reports on the modification of the Abbé flap for correction of mild tightness of the corrected cleft lip deformity. PATIENTS AND METHODS: Twelve patients with a moderately tight lip deformity, from 16 to 35 years old, underwent this procedure. Nine had a unilateral deformity and 3 a bilateral deformity. The flap, which was taken from the central portion of the lower lip vermilion, was designed to repair the vermilion tubercle and the Cupid's bow. A tiny portion of skin was included to facilitate closure of the donor site. It was inserted into the center of the upper lip and the pedicle was divided 1 week after operation. RESULTS: Each patient showed a more natural contour of the vermilion tubercle and the Cupid's bow. The scarring of the donor site was inconspicuous. CONCLUSIONS: Use of modified Abbé flap to reconstruct the contour of the vermilion tubercle and the Cupid's bow makes the upper lip look more natural.

Dietary casein phosphopeptides prevent bone loss in aged ovariectomized rats.

The effect of dietary Ca-bound casein phosphopeptides (CaCPP) on the bones of aged ovariectomized (OVX) rats was studied as a model for post-menopausal bone loss. Three groups of ovariectomized rats were fed a control diet or one of two experimental diets, and one group of sham-operated rats (SHAM) was fed the control diet. The experimental diets contained 0.5% Ca and 0.4% P. In one diet, CaCPP was the sole source of calcium and provided 62.5% of dietary phosphorus (CaCPP diet). In the other, Ca-free CPP provided 100% of dietary phosphorus (Ca-free CPP diet). In the control diet, CaCO3 and KH2PO4 were used. During a 17-wk feeding period, there was little change in femoral bone mineral densities (BMD) of ovariectomized rats fed CaCPP and Ca-free CPP, or in the SHAM rats fed the control diet, whereas the bone mineral densities in the control ovariectomized rats decreased with time. Some of the segmental bone mineral densities of the excised femurs from the rats fed CaCPP were significantly higher than those from the control ovariectomized rats, but the values of the Ca-free CPP group were similar to those of the control ovariectomized rats. In the Ca-free CPP group, the discrepancy in bone mineral densities obtained between in vivo results and excised specimens might have been the result of a loss in bone mass due to their significant loss in body weight. There were no significant differences in serum inorganic phosphorus, alkaline phosphatase activity, osteocalcin or 1 alpha-25-dihydroxycholecalciferol concentrations among the ovariectomized groups. In the CaCPP and Ca-free CPP groups, urinary phosphorus excretion decreased and urinary calcium excretion increased significantly with time. The inhibitory effect on bone loss in aged ovariectomized rats could be due to the effects of dietary CaCPP on phosphorus and calcium metabolism.

Use of cantilever iliac bone grafts for reconstruction of cleft lip-associated nasal deformities.

PURPOSE: The purpose of this study was to describe the clinical and radiographic observations on cantilever iliac bone grafts for reconstruction of cleft lip-associated nasal deformities. MATERIALS AND METHODS: Cantilever iliac bone grafts were performed on 14 patients with a severely deformed cleft lip-associated nose using the open rhinoplasty technique. An approximately 6-cm length of iliac bone was tightly inserted into a subperiosteal pocket over the nasal bones, and the nasal tip was elevated by the distal end of the graft. The clinical follow-up ranged from 7 months to 3 years. RESULTS: All patients were judged to have satisfactory results. The grafted bone decreased slightly in size during the first 2 to 3 months, and irregularities in contour became rounded. During the same period, bony union with the underlying nasal bones was observed in all cases. After about 6 months, further changes did not occur in the grafts. CONCLUSION: This type of bone graft can be used for additional structural support and to achieve the desired nasal projection and profile. Augmenting the nasal bridge creates the illusion of a narrower nose. The key to success of the operation seems to be the proper fixation of the grafted bone to the underlying nasal bones.

Columella lengthening using a cartilage graft in the bilateral cleft lip-associated nose: choice of cartilage according to age.

PURPOSE: This article describes the technique of columellar lengthening using a cartilaginous strut in patients with a severely deformed bilateral cleft lip-associated nose. MATERIALS AND METHODS: When the upper lip is not deficient, and especially when resection of lip scar tissue is indicated, the Millard forked flap technique is recommended. Advancement of the prolabium into the columella for lengthening, combined with an Abbé flap for upper lip reconstruction, is indicated when a deficient upper lip is unable to provide adequate donor tissue. A cartilaginous strut is inserted behind the forked flap or the advanced prolabium. According to the age of the patient, septal cartilage, costal cartilage, or ear cartilage is selected. Ten patients with a severely deformed bilateral cleft lip-associated nose underwent these procedures. RESULTS: In each case, the columella was lengthened satisfactorily. In four patients, the scar became hypertrophic at the base of the columella and scar revision was performed secondarily. CONCLUSIONS: A cartilaginous strut is the key to avoiding the tendency toward retraction or thickening of the lengthened columella. It gives a slight lift to the tip, provides more definition, and improves the columellar contour.

Early correction of the nose in unilateral cleft lip patients using an open method: a 10-year review.

PURPOSE: To review a 10-year follow-up of cases with rhinoplasty for a cleft lip-associated nose deformity done during the preschool years. MATERIALS AND METHODS: Sixteen patients from 16 to 19 years of age were evaluated with two indices: the nasal index and the lobule portion of the columella index. RESULTS: Several years after surgery the results appeared to be reasonably satisfactory. However, as the patients approached their adolescent growth spurt at 15 years of age, undesirable features became obvious. Each patient showed a strikingly bulbous nose. The nasal index of male patients ranged from 68.8% to 82.7% (mean, 75.4%) and that of female patients ranged from 72.7% to 85.0% (mean, 79.2%). The mean value of the nasal index in the Japanese male and female is 58.5% +/- 1.5% and 59.0% +/- 1.0%, respectively. The lobule portion index of male patients ranged from 57.1% to 72.0% (mean, 65.1%) and that of female patients ranged from 62.5% to 82.0% (mean, 74.2%). The lobule portion of the columella constitutes 33% of the total columellar length on average. CONCLUSIONS: A possible cause of the undesirable deformities is use of an open surgical method. A second possible cause is mobilization and suspension of the alar cartilages. These undesirable features may occur only in Oriental noses.

Correction of the cleft nasal deformity with an L-shaped iliac bone graft.

We describe our technique for correcting a cleft lip nasal deformity using L-shaped iliac bone grafts to achieve additional structural support and the desired nasal projection and profile. Augmenting the nasal bridge creates the illusion of a narrower nose. Further, the columellar portion of the L-shaped graft provides stabilization, eliminating the "see-saw" effect of the bridge graft on the fulcrum of the bony bridge, which can lead to a depressed tip and loss of the root indentation. Clinical and radiographic observation has revealed that this method of correction has consistently produced satisfactory results.

[The interrelation of serum lysozyme level and cytoplasmic lysozyme level].

By means of the immunocytochemical method, the level of cytoplasmic lysozyme in leukocytes from healthy volunteers (n = 50) and from patients with uremia (n = 50), leukocytosis (n = 50), various forms of leukemia (n = 36) and myelodysplastic syndrome (MDS) (n = 7) were analysed, and compared with that of simultaneously assayed serum lysozyme. Both the cytoplasmic and serum levels of lysozyme in uremia and leukocytosis were significantly higher than normal subjects (p < 0.001). No correlation, however, was found between their cytoplasmic and serum levels of lysozyme. Morphological analysis for various kinds of leukemia and MDS indicated that myelocytic and monocytic cells became highly positive for lysozyme staining with maturation, and that lymphocytes, leukemic myeloblasts and monoblasts were negative. The cytoplasmic and serum lysozyme levels of leukemias or MDS having a number of lysozyme-positive cells were elevated as compared with those of normal individuals. Among them acute myelocytic leukemia (FAB M4) revealed an excellent correlation between the lysozyme levels in cytoplasm and in serum. The rest whose serum lysozyme level tend to be lower than the cytoplasmic one gave poor correlation. Thus, serum lysozyme level is not fully reflected by the cytoplasmic level. The dual determination of cytoplasmic and serum lysozyme is suggested to be helpful on estimating leukemia types, the degree of cellular maturation and total cell mass, and might also provide a valuable tool for prediction of prognosis for these disorders.

Experimental study on growth of epiphysial plate: free graft in rabbits.

The growth potential of a free graft of an epiphysial plate was investigated in rabbits. Two epiphysial plate grafts were harvested from each iliac crest. One was grafted to the head (onto bone) and the other to the ear (onto cartilage). Both of the epiphysial plates enlarged to a maximum height of 1.4 cm and became similar to iliac crests. Enchondral ossification was observed up till approximately 28 weeks of age. We conclude that an epiphysial plate has growth potential after free heterotopic transplantation.

Mandibular lengthening by gradual distraction: analysis using accurate skull replicas.

Bone lengthening in the upper and lower extremities by gradual distraction has become an accepted procedure. We have used an extraoral device to lengthen the mandible in four patients with unilateral mandibular hypoplasia. Using an accurate skull replica, the proposed corticotomy line, intended direction of lengthening, and appropriate position for the screws were determined. Following distraction, a significant increase in the dimensions of the affected mandible was obtained in each case. In this series, accurate skull replicas proved very useful for defining the anatomy, for surgical simulation and for pre- and postoperative evaluation.

Experimental study of prefabricated flaps using vein grafts.

Prefabrication of a flap may represent a new alternative, not limited by natural vascular territories, for creating donor sites. Experimental procedures in which an A-V shunt results in sufficient flap neovascularization have been reported. This study was undertaken to investigate how neovascularization from the recipient bed affects survival of this type of flap. An 8-cm x 10-cm prefabricated skin flap, nourished by the left epigastric vein, was constructed in a rabbit abdomen. In half the models, a silicone sheet was placed under the flap, to impede neovascularization from the bed. The flaps of both experimental groups showed excellent viability, and no statistically significant difference in survival rate was recognized. This flap was found to be independent of the vascularity of the underlying bed.

[The sensitivity and specificity of various peroxidase stains--the difference between DAB and 3AC stainings].

FAB Cooperative Group has cited 3 or higher percentage in the peroxidase (PO) activity as one of criteria for the diagnosis of acute myeloid leukemia. We have previously reported in two cases of acute myelomonocytic leukemia (M4) that there was a great difference between 3,3'-diaminobenzidine (DAB) and 3-amino 9-ethyl carbazole (3AC) as substrates for PO reaction. In order to confirm the previous result, we extended the cases and compared the PO activities in blood films taken from several leukemic subjects, which were fixed with various kinds of fixatives, using DAB and 3AC as substrates. Their peroxidase activities were also determined through electron microscopy (EM-PO). There were no differences among fixatives and substrates in staining normal granulocytic cells (neutrophils and monocytes) and cells in 13 cases of leukemia, except for two cases, one with M4 and the other with relapse of M2. These showed the highest PO activity with 3AC staining and 10% formaldehyde acetone buffer for fixation. Besides, all types manifested the highest positivity in EM-PO, except for the relapse of M2 that showed a higher positivity in the peroxidase stain by 3AC staining, 10% formaldehyde acetone buffer fixation than the EM-PO. Unlike other leukemic blasts PO reaction products of blast cells of the two cases showed a scattered distribution of microscopic granules. These findings suggested the difference, which was seen in the previous reports, of the PO stain between two substrates might be due not only to sensitivity for staining but to the presence of some heterogeneity such as isoenzyme in the myeloperoxidase.

Binder's syndrome: peculiarities in Japanese patients.

We have seen 4 Japanese patients with Binder's syndrome or maxillonasal dysostosis. The diagnosis of Binder's syndrome was very difficult, based on facial appearances alone. Although the flattened nasal tip was quite unusual in Japanese individuals, the midface retrusion and flattened nasal bridge would be considered within normal limits. Consequently, the diagnosis of Binder's syndrome is only rarely made in Japanese persons. The peculiarities of this syndrome in Japanese patients are discussed and our surgical methods are shown.

Heterotopic gastrointestinal cyst of the oral cavity.

We report a heterotopic gastrointestinal cyst of the oral cavity. Heterotopic islands of gastrointestinal mucosa have been found in the esophagus, the small bowel, and particularly in Meckel's diverticulum. However, their occurrence in the head and neck is extremely rare. Previously reported cases and the theories of embryogenesis are reviewed.

[A case of acute mixed leukemia with expression of J5 and GPIIb/IIIa complex by ABC method].

Identification of the lineage and maturation stage of the leukemic cells is essential for the diagnosis, treatment and followup of acute leukemia. Staining of blood smear by avidin-biotin-peroxidase complex method, which requires no special equipments, enables to observe both morphology and surface antigens at the same time. Simultaneous expression of a B-lymphocyte antigen J5 and a platelet antigen GP IIb/IIIa complex on the same blasts was demonstrated by this technique in the present case.
