[Effectiveness of Nao'an Capsule on stroke prevention among high risk population in Nanhui, Shanghai].

OBJECTIVE: To evaluate the effectiveness of stroke prevention among high risk population, using Nao'an Capsules. METHODS: Participants were selected from 696,558 residents in Nanhui, using county of Shanghai city. Individuals aged 35 years old and over with at least one risk factor exposure to stroke, received cerebral vascular hemodynamic examination. 18,271 cases meeting the criteria of cerebral vascular hemodynamic indexes accumulative score below 70 points were defined as individuals with high-risk and targets to receive intervention. According to the willingness of the participants, 10,313 cases received Nao'an Capsules for intensive intervention based on general intervention measures compared to 7958 cases only receiving general intervention. After the implementation of intervention, incidence and mortality rates of stroke and the effectiveness of three-year intervention were studied. RESULTS: Incidence of stroke in Nao'an Capsules group was significantly lower than that of the general intervention (P < 0.01) with 53.8% in males and 58.4% in females. The relative risk (RR) in two gender groups were 0.46 (0.33 - 0.64) and 0.39 (0.30 - 0.50) respectively. Multiple logistic regression analysis indicated that the history of hypertension, accumulative score of cerebral vascular hemodynamic indexes, age, gender and Nao'an Capsules intervention were the variables selected into the equation and significantly related to stroke. Among the variables, Nao'an Capsule was the strongest factor with a RR of 0.41. CONCLUSION: Incidence of stroke in Nao'an Capsule intervention group was significantly lower than that in the general intervention group after 3 years of intervention, suggesting that Nao'an Capsule intervention was the strongest factor affecting stroke occurrence in individuals at high-risk.

[Resection and reparation of heel with malignant melanoma].

OBJECTIVE: To investigate the surgical resection and reparation of heel with malignant melanoma. METHODS: Eight patients with malignant melanoma were treated from May 2001 to December 2003. The patients included 5 males and 3 females, and their ages ranged from 28 to 56 years. All lesions were located in the heel and were proved by pathological examination. According to Breslow classification, there were 2 cases of Grade I, 5 cases of Grade I, and 1 case of Grade II. Local extensive resection was performed in all cases. Lateral pedal skin flap, plantar medial artery island skin flap, and retrograde skin flap supplied by sural nutrition blood vessel were respectively applied in the reparation according to the size of heel soft tissue defect. The treatment with interferon was delivered before and after the operation. RESULTS: The surgical reparation was successful in all 8 cases. The postoperative follow-up was conducted from 18 months to 4 years. All patients remained alive and no tumor recurrence was observed. Considering the recovery of the function and sense, the best result was acquired with plantar medial artery island skin flap and lateral pedal skin flap, good with retrograde skin flap supplied by sural nutrition blood vessel. CONCLUSION: Local extensive resection is essential for the heel with malignant melanoma. Reparative reconstruction should be made on negative operative margin. Satisfactory clinical outcome is achieved by using lateral pedal skin flap, plantar medial artery island skin flap, and retrograde skin flap supplied by sural nutrition blood vessel.

[Purification of Al13 species in polyaluminum chloride (PAC) by column chromatography and the character of the fractions].

The polynuclear keggin species, Al12AlO4(OH)24(H2O)12(7+) (Al13), was separated and purified from polyaluminum chloride (PAC) by column chromatography method. The three fractions were characterized by Al-Ferron timed complexation spectrophotometry, TEM and ZATESIZER tester. Jar tests were performed to test the coagulation efficiency of the three fractions in treating synthetic or actual water samples. The results show that the sample purified by the column chromatography method influenced by the size of the species. The bigger molecules were eluted first, then the smaller ones. Al13 would be obtained by taking out the middle fraction. Compared with the other two fractions, the third fraction gives the best results for turbidity, humic acid and color removal, and achieves the highest charge-neutralizing ability. Al13 species is a higher positive-charged species and can be applied in a wide pH range, and it is much more steady in hydrolyzing process and most effective polymeric Al species in water and wastewater treatment.

[Pathologic diagnosis of endometrial carcinoma in curettage specimens in women under forty years of age].

OBJECTIVE: To investigate the diagnostic pathological features of endometrial carcinomas present in women under 40 years of age (by curettage samples). METHODS: A retrospective analysis was performed on 20 cases of endometrial carcinomas in women under 40 years of age. RESULTS: The patients included 18 endometrioid adenocarcinoma cases, one adenosquamous carcinoma case, and one papillary serous carcinoma case. The morphological features of the endometrial adenocarcinoaare loss of polarity (orientation) of the endometrial glands. The tumor cells have large round vesicular nuclei, prominent nucleoli and coarse chromatin. In addition, endometrial stroma was instead of by the fibrous and/or granulation tissue type stroma, usually with inflammatory response. Majority cases of endometrioid adenocarcinomas had superficial invasion of myometrium and no lymph node metastasis. The adenosquamous case metastasized to the ovaries and the papillary serous carcinoma case metastasized to the pelvic lymph nodes. CONCLUSIONS: Endometrial carcinomas which are highly differentiated endometrioid adenocarcinomas occur frequently in women under 40 years of age. An important differential diagnosis of the endometrioid carcinoma in a curettage specimen is to differentiate it from atypical endometrial hyperplasia and atypical polypoid adenomyoma.

[Study on the growth and spectral characteristics of Cr3+: Al2 (WO4)3 crystal].

Cr3+:Al2(WO4)3 Crystal was grown from the Na2WO4 solvent system by using the TSSG method. The absorption spectrum shows that there are two strong absorption wideband peaks and one we ak absorption peak belonging to Cr3+ ion in the crystal. The peak values of two strong absorption wideband peaks are at 422 and 595 nm, respectively. The values of two weak absorption peaks are at 684 and 663 nm, respectively. The fluorescence spectrum shows that one emission wideband peak ranging from 650 to 820 nm coexists with one stronger emission peak at 680 nm. The peak value of emission wideband is at 740 nm. The crystal field and Racah parameters were calculated, where Dq/B = 2.420 indicates that it has an intermediary crystal field. The result shows that Cr3+:Al2(WO4)3 crystal is promising for tunable laser because it not only has the spectral characteristics required for tunable laser crystal but also has the good physicochemical properties.

[The effects of an NGF isolated and purified from venom of Naja naja atra on GAP-43 in spinal cord dorsal horn of cats subjected to partial rhizotomy].

OBJECTIVE: To investigate the effects of a nerve growth factor (NGF) isolated and purified from the venom of Naja naja atra on GAP-43 in spinal cord dorsal horn of cats subjected to partial rhizotomy. METHODS: A total of 45 adult male cats were subjected to unilateral rhizotomy of L1-L5 and L7-S2 dorsal roots, with the intact L6 as spared dorsal root. Then the cats were equally divided into the NGF low-dosage group (1 microg/kg), NGF high-dosage group (2 microg/kg), normal saline control group, and were allowed to survive 3 days, 6 days, 12 days respectively. Immunohistochemical technique was used to detect GAP-43 of L6 segments of spinal cord. RESULTS: After the operation, GAP-43 positive products were mainly distributed in the nerve terminals of spinal cord I, II, III. Expression of GAP-43 positive products was detected at 3 days and 6 days after the operation; the nerve terminals of regeneration increased and reached the highest level on the 12th day; there was significant difference between the control group and the treatment groups; the regeneration level of the NGF-high-dosage group was significantly (P < 0.01) higher than that of the other groups. CONCLUSION: NGF can effectively induce the synthesis of neuronal structural protein in the process of recovery of spinal cord damage and enhance the spared root's collateral sprouting; the density of positive nerve terminals is related with NGF dosage.

[Effect of penetration enhancers on the transdermal penetration of sinomenine liposome patch].

OBJECTIVE: To study the effect of penetration enhancers on the transdermal penetration of sinomenine liposome patch. METHODS: We firstly established the experimental method of transdermal penetration, and the methodology study was carried out at the same time. Furthermore, we added 5% Azone, 10% Oleic acid, 1% PG + 4% Azone, 2% PG + 8% Oleic acid into the patch. The cumulative penetration amounts were determined during the test of transdermal penetration and the part lagged in the skin was determined after the test. RESULTS: The cumulative penetration amounts released from the patch increased, while the part lagged in the skin decreased at the same time. CONCLUSIONS: The results indicate that penetration enhancers can facilitate drug through corneum into dermis, but can't enhance the drug' s accumulation in the corneum. It is concluded that penetration enhancers can destroy the two-molecule structure of liposomes and cause drug leakage, so penetration enhancers should not be added into liposome patch of part functions.

[Preparation of humanized anti-digoxin scFv and diabody].

AIM: To prepare humanized anti-digoxin(Dig) antibodies(single chain Fv, scFv) from a human phage antibody library and construct anti-Dig diabody vector. METHODS: A human phage antibody library was panned against immobilized Dig for four times, and the specificity of the selected scFv expression clones was identified by ELISA. Positive clones against Dig were analyzed by DNA fingerprint and sequencing. A clone which had high affinity to Dig was selected to construct a diabody vector. Diabody was secreted from E. coli by IPTG induction, and the specificity was also identified by ELISA. RESULTS: After four rounds of panning, four specific humanized anti-Dig antibodies(scFv) were obtained. DNA fingerprint and sequencing analysis proved that they had different Ab-encoding genes. The V(L) of the 4 expression clones belonged to lambda subgroup 1 and V(H) belonged to 3 and 4 subgroups, respectively. A scFv clone was picked out to construct diabody vector. The prepared humanized diabody reacted with Dig specifically. CONCLUSION: Humanized anti-Dig antibodies(scFv) have been got by using phage display. A diabody has been obtained from scFv. These humanized anti-Dig antibodies may be used for the diagnosis and therapy of Dig toxication.

[Fluid shear stress regulates secretion of tissue-type plasminogen activator in human endothelial progenitor cells].

OBJECTIVE: In order to investigate the role of shear stress in the regulation of endothelial function, we assessed here effects of shear stress on tissue-type plasminogen activator in human endothelial progenitor cells (EPCs). METHODS: The peripheral blood mononuclear cells were separated from healthy adult and inducted into EPCs, which were identified by double staining for the fluorescent labeled acetylated-LDL and lectin. EPCs were seeded on the small diameter artificial vessels, and then divided into four different experimental groups including stationary group, low-flow shear stress group (5 dyn/cm(2)), medium-flow shear stress group (15 dyn/cm(2)) and high-flow shear stress group (25 dyn/cm(2)). The levels of t-PA in EPC culture medium at 0 hour, 5 hours, 10 hours, 15 hours, 20 hours and 25 hours after culture were measured by enzyme-linked immunosorbent assay. RESULTS: The peripheral blood mononuclear cells differentiated into EPCs after induction, which were positively labeled by fluorescent acetylated-LDL and lectin. Shear stress enhanced production of the t-PA by EPCs, which was paralleled to levels and times of shear stress. CONCLUSIONS: Shear stress increases t-PA secretion by human EPCs, suggesting that shear stress not only regulates vascular endothelial function but also participates in the pathogenesis of arteriosclerosis.

[Lyophilization for platelet preservation].

To explore a new lyophilized preservation methods for human platelets, platelets were pre-treated with aldehyde, human albumin or trehalose was added to the system of condensed cooling as protectant to stabilize the structure of platelets. The optimal resuspending buffer was also selected in the study. The morphological changes of platelets were observed by using electron microscopy after lyophilization, and the expression of membrane proteins on platelets was detected also after lyophilization. The results indicated that the recovery rate of platelets treated with aldehyde was generally more than 60%. Aggregative ability was reduced a little than the platelet untreated. 5% of human albumin had an advantage over 40 mmol/L of trehalose in respect of the preservation effect. In the way of keeping aggregative ability, PPP was obviously better than PBS. The results of electron microscopy displayed that organelles including mitochondria and excreted granules could be observed distinctly. Whereas, expression of membrane proteins of platelet treated with aldehyde was evidently dropped as compared with those of the fresh platelet. In conclusion, aldehyde as a novel protective agent, has excellent effects on lyophilization of platelets and is worthy to be further studied.

[Clinical analysis of 1,400 cases of coronary artery angiography without heparin].

OBJECTIVE: To evaluate the feasibility and safety of coronary artery angiography via femoral artery approach without heparin. METHODS: Totally 1 400 consecutive patients undergoing coronary artery angiography with heparin were analyzed retrospectively in comparison with the data of 354 patients for coronary artery angiography without heparin. RESULTS: Success in selective coronary artery angiography was achieved in 99.3% of the patients in the non-heparin group, with the mean operation time of 17.9+/-11.3 min ranging from 8 min to 1 hour. Angiography identified coronary artery stenosis in 72.2% of the patients including 49.8% with multivessel involvement. Subcutaneous hematoma occurred in 25 (1.8%) of the patients, and 1 (0.07%) patient developed arterial and venous fistula and pseudoaneurysm at the puncture site, but other complications (retroperitoneal hematoma, acute myocardial infarction, stroke and peripheral artery thrombotic events) occurred neither during nor after the procedure. In the heparin group, 18 (5.1%) patients developed subcutaneous hematoma and 1 (0.2%) had arterial and venous fistula and pseudoaneurysm at the puncture site after angiography. CONCLUSION: Coronary artery angiography without heparin is both safe and feasible with only very low risk of complications.
