Hypocretin (orexin) enhances neuron activity and cell synchrony in developing mouse GFP-expressing locus coeruleus.

The noradrenergic neurons of the locus coeruleus (LC) play an important role in modulating arousal and selective attention. A similar function has been attributed to the hypocretin neurons of the hypothalamus which maintain a strong synaptic projection to the LC. As the LC can be difficult to detect in the embryonic and neonatal mouse brain, we used a new transgenic mouse with strong GFP expression in the LC under the regulation of a mouse prion promoter. GFP colocalized with immunoreactive tyrosine hydroxylase in sections and dispersed cultures of the LC, allowing visualization and whole cell or single-unit recording from the LC in early stages of cellular development. GFP expression in the LC had no apparent effect on cellular physiology, including resting membrane potential, input resistance, spike threshold, depolarization-induced spike frequency increase, current-voltage relations, or hypocretin responses. In slices of the mature mouse and rat LC, hypocretin-1 and -2 increased spike frequency, with hypocretin-1 being an order of magnitude more potent. In the postnatal day (P) 0-2 developing mouse slice during a developmental period when spikes could be elicited in some cells, other developing LC neurons showed rhythmic, subthreshold oscillations (approximately 1 Hz) in membrane potential (2.9-7.4 mV amplitude); others were arrhythmic. Hypocretin-1 depolarized the membrane potential, resulting in the appearance of spikes in developing LC cells that showed no spikes under control conditions. In the presence of TTX and glutamate receptor antagonists, hypocretin-1-mediated inward currents were blocked by substitution of choline-Cl for NaCl, suggesting an excitatory mechanism based on an inward cation current. Hypocretin-1 initiated strong regular membrane voltage oscillations in arrhythmic immature neurons. Hypocretin increased the temporal synchrony of action potentials studied with dual-cell recording in P1-P5 mouse LC slices, consistent with the view that synchrony of LC output, associated with improved cognitive performance, may be increased by hypocretin. Together these data suggest that the hypothalamus, via hypocretin projections, may therefore be in a position to enhance arousal and modulate plasticity in higher brain centres through the developing LC.

The amino acid residues asparagine 354 and isoleucine 372 of human farnesoid X receptor confer the receptor with high sensitivity to chenodeoxycholate.

The critical steps in bile acid metabolism have remarkable differences between humans and mice. It is known that human cholesterol 7 alpha-hydroxylase, the enzyme catalyzing the rate-limiting step of bile acid synthesis, is more sensitive to bile acid suppression. In addition, hepatic bile acid export in humans is more dependent on the bile salt export pump (BSEP). To explore the molecular basis for these species differences, we analyzed the function of the ligand-binding domain (LBD) of human and murine farnesoid X receptor (FXR), a nuclear receptor for bile acids. We observed a strong interspecies difference in bile acid-mediated FXR function; in the coactivator association assay, chenodeoxycholate (CDCA) activated human FXR-LBD with 10-fold higher affinity and 3-fold higher maximum response than murine FXR-LBD. Consistently, in HepG2 cells human FXR-LBD increased reporter expression more robustly in the presence of CDCA. The basis for these differences was investigated by preparing chimeric receptors and by site-directed mutagenesis. Remarkably, the double replacements of Lys(366) and Val(384) in murine FXR (corresponding to Asn(354) and Ile(372) in human FXR) with Asn(366) and Ile(384) explained the difference in both potency and maximum activation; compared with the wild-type murine FXR-LBD, the double mutant gained 8-fold affinity and more than 250% maximum response to CDCA in vitro. This mutant also increased reporter expression to an extent comparable with that of human FXR-LBD in HepG2 cells. These results demonstrate that Asn(354) and Ile(372) are critically important for FXR function and that murine FXR can be "humanized" by substituting with the two corresponding residues of human FXR. Consistent with the difference in FXR-LBD transactivation, CDCA induced endogenous expression of human BSEP by 10-12-fold and murine BSEP by 2-3-fold in primary hepatocytes. This study not only provides the identification of critical residues for FXR function but may also explain the species difference in bile acids/cholesterol metabolism.

Prostate-specific expression of the diphtheria toxin A chain (DT-A): studies of inducibility and specificity of expression of prostate-specific antigen promoter-driven DT-A adenoviral-mediated gene transfer.

Diphtheria toxin (DT) is a potent inhibitor of protein synthesis. As little asa single molecule of DT can result in cell-cycle independent cell death. This profound potency has led to difficulties in the development of DT as a suicide gene in cancer gene therapy, because toxicity appears to be related primarily to the fidelity of basal gene expression and the yield of viral titer. We evaluated the feasibility of prostate-specific DT gene therapy by cloning the catalytic domain (A chain) of DT under the control of the prostate-specific antigen (PSA) promoter, the PSA promoter and enhancer, or the cytomegalovirus promoter. The data on expression of DT from the plasmid constructs demonstrate that the basal level of DT gene expression determines the toxicity. To better test the potential therapeutic efficacy of DT suicide gene therapy, we first developed a DT-resistant adenoviral packaging line (293DTR). This allowed us to manufacture a relatively high titer adenoviral vector encoding the DT-A gene under the control of the PSA promoter and enhancer (Ad5PSE-DT-A) as well as an attenuated DT-A virus (Ad5PSE-tox176). In vitro studies showed that our viral constructs preferentially kill PSA-positive prostate cancer cells in the presence of exogenous androgen (R1881). In vivo studies showed that the nu/nu mice with PSA-positive cancer cell LNCaP xenograft treated with wild-type DT-A virus had a rapid regression of tumors and survived over a year without tumor progression, whereas the attenuated DT-A virus restricted tumor growth for only 1 month. The same constructs had no significant effect on the non-PSA-secreting cell line DU-145. These encouraging results suggest that DT-A viral gene transfer may ultimately have a therapeutic role in the treatment of advanced human prostate cancer.

Development of a recombinant HSP110-HER-2/neu vaccine using the chaperoning properties of HSP110.

Several studies have shown that when purified from a tumor, certain heat shock proteins (HSPs) can function as effective vaccines against the same tumor by virtue of their ability to bind tumor-specific peptides. However, only a small fraction of the associated peptides would be expected to be immunogenic, in addition to which, the clinical application of this vaccine requires the availability of a surgical specimen of sufficient quantity for purification of the HSP. The present study describes a new approach for the development of natural HSP vaccines that do not have these limitations. This approach uses a recombinant HSP that is noncovalently bound to a recombinant tumor protein antigen by heat shock. HSP110 has been selected for this purpose, because it has been shown to be a highly efficient molecular chaperone in binding to large protein substrates. We show that a "natural chaperone complex" between HSP110 and the intracellular domain (ICD) of human epidermal growth factor receptor 2 protein (HER-2)/neu is formed by heat shock. This HSP110-ICD vaccine elicited both CD8(+) and CD4(+) T-cell responses against ICD as determined by an antigen-specific IFN-gamma production in an enzyme-linked immunospot assay (ELISPOT). In vivo depletion studies revealed that the CD8(+) T-cell response was independent of CD4(+) T-cell help. The HSP110-ICD complex also significantly enhanced ICD-specific antibody responses relative to that seen with ICD alone. No CD8(+) T cell or antibody response was detected against HSP110. The use of recombinant HSP110 to form natural chaperone complexes with large protein antigens represents a new and powerful approach for the design of protein-targeted cancer vaccines.

Drosophila Gp150 is required for early ommatidial development through modulation of Notch signaling.

Cellular signaling activities must be tightly regulated for proper cell fate control and tissue morphogenesis. Here we report that the Drosophila leucine-rich repeat transmembrane glycoprotein Gp150 is required for viability, fertility and development of the eye, wing and sensory organs. In the eye, Gp150 plays a critical role in regulating early ommatidial formation. Gp150 is highly expressed in cells of the morphogenetic furrow (MF) region, where it accumulates exclusively in intracellular vesicles in an endocytosis-independent manner. Loss of gp150 function causes defects in the refinement of photoreceptor R8 cells and recruitment of other cells, which leads to the formation of aberrant ommatidia. Genetic analyses suggest that Gp150 functions to modulate Notch signaling. Consistent with this notion, Gp150 is co-localized with Delta in intracellular vesicles in cells within the MF region and loss of gp150 function causes accumulation of intracellular Delta protein. Therefore, Gp150 might function in intracellular vesicles to modulate Delta-Notch signaling for cell fate control and tissue morphogenesis.

Cytochrome P450 3A4-mediated oxidative conversion of a cyano to an amide group in the metabolism of pinacidil.

The conversion of nitriles to amides is generally considered to be a hydrolytic process that does not involve redox chemistry. We demonstrate here that cytochrome P450 (CYP) is responsible for the conversion of the cyano group of pinacidil to the corresponding amide. The reaction in human liver microsomes was NADPH-dependent and was nearly completely inhibited by an anti-CYP3A4 antibody. Incubations of pinacidil with recombinant CYP enzymes confirm that CYP3A4 is the principal catalyst of this reaction. The kinetics of pinacidil amide formation by CYP3A4 yielded an apparent K(m) of 452 +/- 33 microM and k(cat) of 0.108 min(-1) (k(cat)/K(m) = 0.238 mM(-1).min(-1)). Incubation of pinacidil with CYP3A4 in the presence of (18)O(2) or H(2)(18)O showed that the amide carbonyl oxygen derived exclusively from molecular oxygen. The CYP3A4-mediated reaction also was supported by hydrogen peroxide when incubations were carried out in the absence of cytochrome P450 reductase and NADPH. The reaction can be explained by a nucleophilic attack of a deprotonated ferric peroxide intermediate (Fe(3+)-O-O(-)) on the carbon atom of the -C triple bond N triple bond to form an Enz-Fe(III)-O-O-C(=NH)R intermediate, followed by cleavage of the O-O bond to give pinacidil amide. This nucleophilic addition of an Fe(3+)-O-O(-) intermediate to a -C=N pi-bond in a P450 system resembles the analogous reaction catalyzed by the nitric oxide synthases.

Circulating erythroblasts in maternal blood are not elevated before onset of preterm labor.

OBJECTIVE: Preterm labor has recently been reported to be associated with an increased release of cell free fetal deoxyribonucleic acid (DNA) into the maternal circulation. We have previously observed increases in both fetal cell traffic and cell free fetal DNA in preeclamptic pregnancies. In this study, we investigated whether fetal cell traffic is also disturbed in pregnancies with preterm labor. METHODS: In a case-control study, we examined 47 pregnancies complicated by preterm contractions that occurred between 20 and 34 weeks' gestation and an equal number of matched controls. Erythroblasts were enriched for by magnetic cell sorting and enumerated. These values were then correlated with subsequent pregnancy outcome. RESULTS: In the study group 16 patients delivered prematurely (subgroup A). The other 31 (subgroup B) delivered at term, as did all those in the control group. No significant difference was noted in erythroblast numbers between either one of the subgroups and the controls. CONCLUSION: Contrary to the reported increased levels of free fetal DNA in maternal serum, erythroblasts in maternal blood are not elevated significantly in pregnancies with threatened premature labor or in those that deliver preterm.

Overweight is an independent risk factor for cardiovascular disease in Chinese populations.

In the last decade of the 20th century, cardiovascular disease was the leading cause of death in China, accounting for one-third of the total deaths. In comparison with western populations, the mean body weight or body mass index (BMI) of the Chinese population was lower, but showed an increasing trend. Whether the variation within lower levels of BMI or waist circumference was associated with other risk factors of cardiovascular disease, and whether they contribute independently to the risk of cardiovascular disease in the Chinese population, was investigated in this study. In keeping with a uniform study design, in each of 14 study populations at different geographical locations and with different characteristics, the incidence rates of stroke, coronary heart disease (CHD) and the causes of death were monitored in approximately/= 100000 residents from 1991 to 1995 using the MONICA procedure. Risk factors were surveyed in a random cluster sample of 1000 subjects (35-59 years of age) from each population under surveillance using internationally standardized methods and a centralized system to ensure quality control. Among the risk factors, body weight, height, and waist and hip circumferences were measured. Cross-sectional stratified analyses were used to analyse the relationship of BMI (kg m(-2)) or waist circumference to other metabolic risk factors. Ten cohorts among the 14 study populations with 24734 participants were surveyed from 1982 to 1985 as a baseline for further study and were followed-up for 9 years taking the events of stroke, CHD and different causes of death as end-points. Cox regression models were used to explore the association of BMI with the relative risks of stroke, CHD and total death. The survey in 14 random samples with a total number of 19 741 subjects showed that the mean BMI (20.8-25.1) and waist circumference (67.8-86.7 cm) were much lower than those of western populations. There was, however, variation in the anthropometric measurements among populations within China. Thus, rates of overweight varied from 2.7% to 48.1% and obesity from 0% to 9.5% on the basis of the World Health Organization (WHO) classification, but these values were lower than those found in western populations. Data from the 10 cohort samples compared with baseline data in the early 1980s showed that the mean BMIs increased significantly in eight populations during the early 1990s with the differences ranging from 0.5 to 2.5 kg m(-2). Despite the lower level of BMI and the lower rate of overweight, cross-sectional analyses showed that the prevalence of hypertension, high fasting serum glucose, high serum total cholesterol and low high-density lipoprotein cholesterol (HDL-C) and their clustering were all raised with increases in BMI or waist circumference. The prospective cohort study showed that the BMI was one of the independent risk factors for stroke and CHD in Chinese populations. Hence, in a Chinese population characterized by lower levels of BMI and great variability in rates of overweight, variation of BMI was significantly related to the prevalence of other metabolic risk factors and their clustering. Overweight was one of the independent risk factors for stroke and CHD, both at population and individual levels. Given the increasing trends of BMI in the last 10 years, during the period of economic transition there is a need to encourage the population to adopt healthy dietary habits and to increase their physical activity. Health education and health promotion are important for the prevention and non-pharmacological therapy of cardiovascular disease in China.

Identification and characterization of testis- and epididymis-specific genes: cystatin SC and cystatin TE-1.

Differential display-reverse transcriptase-polymerase chain reaction was used to examine Sertoli cell gene expression. As a result, two new members of the mouse cystatin multigene family were isolated and named cystatin SC (cystatin-related gene expressed in Sertoli cells) and cystatin TE-1 (cystatin-related gene highly expressed in testis and epididymis). The full-length cDNA sequence of cystatin SC contains an open reading frame that encodes a putative signal peptide of 20 amino acids and a mature protein of 110 amino acids, whereas that of cystatin TE-1 encodes a 128 amino acid protein with a predicted signal peptide of 21 amino acids. Both of the deduced amino acid sequences contain four highly conserved cysteine residues in precise alignment with other cystatin family members. The derived cystatin SC and TE-1 amino acid sequences lack some of the specific, highly conserved motifs believed to be necessary for cysteine proteinase inhibition activity. Northern blot analysis revealed that cystatin SC mRNA was detected only in the testis, whereas the cystatin TE-1 gene was highly expressed in testis and epididymis with very low expression in ovary and prostate. In situ hybridization showed that cystatin SC mRNA was localized mainly to Sertoli cells with an obvious stage-dependent expression, and that cystatin TE-1 mRNA was predominantly expressed in Sertoli cells without apparent stage-dependent expression. Cystatin TE-1 mRNA, as displayed by in situ hybridization, was expressed only in the epithelial cells of the proximal caput region of the epididymis. The unusual amino acid sequence and highly restricted expression suggests that cystatins SC and TE-1 play a very specialized role in the testis and epididymis.

A fine physical map of the rice chromosome 4.

As part of an international effort to completely sequence the rice genome, we have produced a fine bacterial artificial chromosome (BAC)-based physical map of the Oryza sativa japonica Nipponbare chromosome 4 through an integration of 114 sequenced BAC clones from a taxonomically related subspecies O. sativa indica Guangluai 4 and 182 RFLP and 407 expressed sequence tag (EST) markers with the fingerprinted data of the Nipponbare genome. The map consists of 11 contigs with a total length of 34.5 Mb covering 94% of the estimated chromosome size (36.8 Mb). BAC clones corresponding to telomeres, as well as to the centromere position, were determined by BAC-pachytene chromosome fluorescence in situ hybridization (FISH). This gave rise to an estimated length ratio of 5.13 for the long arm and 2.9 for the short arm (on the basis of the physical map), which indicates that the short arm is a highly condensed one. The FISH analysis and physical mapping also showed that the short arm and the pericentromeric region of the long arm are rich in heterochromatin, which occupied 45% of the chromosome, indicating that this chromosome is likely very difficult to sequence. To our knowledge, this map provides the first example of a rapid and reliable physical mapping on the basis of the integration of the data from two taxonomically related subspecies.

Spatially oscillating activity and microbial succession of mercury-reducing biofilms in a technical-scale bioremediation system.

Mercury-contaminated chemical wastewater of a mercury cell chloralkali plant was cleaned on site by a technical-scale bioremediation system. Microbial mercury reduction of soluble Hg(II) to precipitating Hg(0) decreased the mercury load of the wastewater during its flow through the bioremediation system by up to 99%. The system consisted of a packed-bed bioreactor, where most of the wastewater's mercury load was retained, and an activated carbon filter, where residual mercury was removed from the bioreactor effluent by both physical adsorption and biological reduction. In response to the oscillation of the mercury concentration in the bioreactor inflow, the zone of maximum mercury reduction oscillated regularly between the lower and the upper bioreactor horizons or the carbon filter. At low mercury concentrations, maximum mercury reduction occurred near the inflow at the bottom of the bioreactor. At high concentrations, the zone of maximum activity moved to the upper horizons. The composition of the bioreactor and carbon filter biofilms was investigated by 16S-23S ribosomal DNA intergenic spacer polymorphism analysis. Analysis of spatial biofilm variation showed an increasing microbial diversity along a gradient of decreasing mercury concentrations. Temporal analysis of the bioreactor community revealed a stable abundance of two prevalent strains and a succession of several invading mercury-resistant strains which was driven by the selection pressure of high mercury concentrations. In the activated carbon filter, a lower selection pressure permitted a steady increase in diversity during 240 days of operation and the establishment of one mercury-sensitive invader.

Species diversity improves the efficiency of mercury-reducing biofilms under changing environmental conditions.

Six mercury-resistant environmental proteobacterial isolates and one genetically modified mercury-resistant Pseudomonas putida strain were analyzed for physiological traits of adaptive relevance in an environment of packed-bed bioreactors designed for the decontamination of mercury-polluted chlor-alkali wastewater. The strains displayed characteristic differences in each trait (i.e., biofilm formation capability, growth rate in mercury contaminated wastewaters, and mercury reduction efficiency). Subsequently, they were immobilized either as a monoculture or as a mixed culture on porous carrier material in packed-bed bioreactors through which different batches of filter-sterilized industrial chlor-alkali wastewater were pumped. In monospecies bioreactors, the mercury retention efficiency was sensitive to rapidly increasing mercury concentrations in the wastewater. Mixed culture biofilms displayed a high mercury retention efficiency that was not affected by rapid increases in mercury or continuously high mercury concentrations. The dynamic in the community composition of the mixed culture bioreactors was determined by ribosomal intergenic spacer polymorphism analysis. Mercury-mediated selective pressure decreased the number of prevalent strains. Microbial diversity was completely restored after easing of the selective pressure. Microbial diversity provides a reservoir of strains with complementary ecological niches that results in a superior bioreactor performance under changing environmental conditions.

Hyposmotic membrane stretch potentiated muscarinic receptor agonist-induced depolarization of membrane potential in guinea-pig gastric myocytes.

AIM: To investigate the relationship between hyposmotic membrane stretch and muscarinic receptor agonist-induced depolarization of membrane potential in antral gastric circular myocytes of guinea-pig. METHODS: Using whole cell patch-clamp technique recorded membrane potential and current in single gastric myocytes isolated by collagenase. RESULTS: Hyposmotic membrane stretch hyperpolarized membrane potential from -60.0mV+/-1.0mV to -67.9mV+/-1.0mV. TEA (10 mmol/L), a nonselective potassium channel blocker significantly inhibited hyposmotic membrane stretch-induced hyperpolarization. After KCl in the pipette and NaCl in the external solution were replaced by CsCl to block the potassium current, hyposmotic membrane stretch depolarized the membrane potential from -60.0 mV+/-1.0mV to -44.8 mV+/-2.3mV (P<0.05), and atropine (1 micromol/L) inhibited the depolarization of the membrane potential. Muscarinic receptor agonist Carbachol depolarized membrane potential from -60.0mV+/-1.0mV to -50.3 mV+/-0.3mV (P<0.05) and hyposmotic membrane stretch potentiated the depolarization. Carbachol induced muscarinic current (I( cch )) was greatly increased by hyposmotic membrane stretch. CONCLUSION: Hyposmotic membrane stretch potentiated muscarinic receptor agonist-induced depolarization of membrane potential, which is related to hyposmotic membrane stretch-induced increase of muscarinic current.

Recombinant human Flt3 ligand exerts both direct and indirect effects on hematopoiesis.

OBJECTIVE: To investigate the direct effects of the Flt3 ligand (FL) on hematopoiesis, such as the stimulation of the formation of hematopoietic colonies and the proliferation of dendritic cells, as well as the indirect stimulation of hematopoiesis, especially via the proliferation of endothelial cells. METHODS: Mononuclear cells from human cord blood were plated in methylcellulose medium containing different cytokines to induce hematopoietic colony formation. Dendritic cells (DCs) were induced from the mononuclear cells with a cytokine cocktail with or without recombinant human soluble FL (rhFL; 100 ng/ml). The Flt3 receptors on the surface of a human microvascular endothelial cell line (ECV) were analyzed by flow cytometry. The proliferation of ECV stimulated by rhFL was measured with the microculture tetrazolium assay. The levels of FL, IL-6, IL-8, G-CSF and GM-CSF in the supernatant of ECV cultures were measured by enzyme linked immunoabsorbent assay (ELISA). RESULTS: rhFL stimulates colony formation from cord blood when used as a sole stimulant. FL in combination with other cytokines increased colony formation significantly. The number of DCs was approximately 2.5 times higher when rhFL was used. rhFL stimulates the proliferation of ECV on which Flt3 receptors are expressed. Furthermore, ECV secretes FL, IL-6, IL-8, G-CSF and GM-CSF, which were augmented by tumor necrosis factor-alpha and rhFL. CONCLUSIONS: rhFL enhances hematopoietic colony formation and DC proliferation from human cord blood cells. FL not only stimulates the proliferation of ECV, but is also secreted by ECV. FL may exert direct and indirect effects on hematopoiesis.

The Chediak-Higashi protein interacts with SNARE complex and signal transduction proteins.

BACKGROUND: Chediak-Higashi syndrome (CHS) is an inherited immunodeficiency disease characterized by giant lysosomes and impaired leukocyte degranulation. CHS results from mutations in the lysosomal trafficking regulator (LYST) gene, which encodes a 425-kD cytoplasmic protein of unknown function. The goal of this study was to identify proteins that interact with LYST as a first step in understanding how LYST modulates lysosomal exocytosis. MATERIALS AND METHODS: Fourteen cDNA fragments, covering the entire coding domain of LYST, were used as baits to screen five human cDNA libraries by a yeast two-hybrid method, modified to allow screening in the activation and the binding domain, three selectable markers, and more stringent confirmation procedures. Five of the interactions were confirmed by an in vitro binding assay. RESULTS: Twenty-one proteins that interact with LYST were identified in yeast two-hybrid screens. Four interactions, confirmed directly, were with proteins important in vesicular transport and signal transduction (the SNARE-complex protein HRS, 14-3-3, and casein kinase II). CONCLUSIONS: On the basis of protein interactions, LYST appears to function as an adapter protein that may juxtapose proteins that mediate intracellular membrane fusion reactions. The pathologic manifestations observed in CHS patients and in mice with the homologous mutation beige suggest that understanding the role of LYST may be relevant to the treatment of not only CHS but also of diseases such as asthma, urticaria, and lupus, as well as to the molecular dissection of the CHS-associated cancer predisposition.

[Prevalence of overweight and obesity in Chinese middle-aged populations: Current status and trend of development].

OBJECTIVE: To understand the current status on prevalence of overweight and obesity in Chinese middle-aged population, and to quantify the magnitude of the trends of development in the past two decades. METHODS: Body mass index (BMI) was measured on 15 389 middle-aged men and women from 15 natural populations all over China with different geographical, economical (urban/rural) and occupational status. Overweight was defined as BMI >/= 25 while obesity as BMI >/= 30. RESULTS: 1) The rates of prevalence on overweight and obesity varied dramatically between populations. 2) In general, the prevalence rates were higher in the northern areas, in urban areas, and in women. 3) The current problem of obesity was related to 'pre-obese' stage, with less than 10% in most populations. 4) The prevalence of overweight and obesity started to show significantly increase from early 80's to early 90's but more prominant in the late 90's. CONCLUSION: Prevention and intervention of overweight and obesity are in urgent need in the Chinese populations, especially in those economically fast developing areas.

[A prospective study on body mass index and mortality].

OBJECTIVE: To assess the relationship between body mass index (BMI, weight in kilograms divided by the square of height in meters) and both all-causes mortality and mortality from specific cause. METHODS: Cardiovascular disease (CVD) risk factors were studied in 1982 - 1985 on more than 30 thousands participants aged 35 - 59 from 10 Chinese populations. 30 560 participants (15 723 for men, 148 837 for women) without known myocardial infraction, stroke or cancer was followed from 1999 to 2000. All-causes of death were documented. RESULTS: Three thousand two hundred and twelve death cases occurred during follow-up of average 15.2 years, including 676 CVD (coronary heart disease and stroke) deaths, 1 281 cancer deaths and 1 255 deaths for other reasons. Cox proportional hazards model adjusting age and gender showed that the relative risks of all causes of death in groups of low BMI (BMI < 18.5), normal BMI (BMI from 18.5 to 23.9), overweight (BMI from 24 to 27.9) and obesity (BMI >/= 28) which were defined according to the strata of BMI for Chinese were 1.21 (95% CI: 1.10 - 1.33), 1.00, 0.91 (95% CI: 0.82 - 1.01) and 1.12 (95% CI: 0.93 - 1.37), respectively ("U" shaped relation). The relative risk of low BMI group (RR = 1.01, 95% CI: 0.84 - 1.21) was not significant different and the relative risk of obesity significantly increased (RR = 1.36, 95% CI: 1.04 - 1.80) while the lowest relative risk was in normal BMI group. The relative risks increased for CVD death, but decreased for cancer death with increased levels of BMI and a "U" shaped relationship was found between BMI groups and mortality for other reasons, which remained after excluding the early death and smokers. CONCLUSION: BMI in normal level was not only related to low risk of all causes of death, but also with relative low risk of CVD, cancer and other deaths. Data were important to public health.

[Relative risk of death and life expectancy in low cardiovascular risk population].

OBJECTIVE: To assess the relationship between low cardiovascular disease (CVD) risk and the mortalities of coronary heart disease (CHD), stroke, cancer and all causes as well as life expectancy in Chinese population. METHODS: CVD risk factors were surveyed in 1982 approximately 1985 for 30 thousand male and female participants aged 35 approximately 59 from 10 Chinese population groups including 3 groups of factory workers, 6 groups of farmers and 1 group of fishermen according a uniform protocol with standardized methods. Follow-up was carried out up to 1999 approximately 2000 and the causes of death were all documented. Low cardiovascular risk was defined as: SBP/DBP < 120/80 mm Hg without anti-hypertensive medication 2 weeks before the baseline survey, serum total cholesterol < 5.17 mmol/L, BMI > 18.5 but < 24 kg/m(2) and no smoking at baseline survey. Mortality rates of CHD and stroke, cancer and all causes the participants with low CVD risk were compared with the rest of the cohort and difference in life expectancy was estimated. RESULTS: Out of the whole cohort of 30 560 participants, 24 900 with complete data were available for the present analysis. Among these, 4 542 participants (18.2%) were defined as low CVD risk group, including 959 (7.7%) men and 3 583 (28.9%) women. During a follow-up of 15.2 years on average, 243 deaths including 6 CHD, 15 stroke and 117 cancer deaths took place in low risk group, while 2 383 deaths including 182 CHD, 333 stroke, and 934 cancer deaths occurred in rest of the cohort. Cox proportional hazards model adjusted for age showed that the relative risks of death of CHD, stroke, cancer and all causes in low risk group were 0.51 (95% CI: 0.19 approximately 1.37), 0.30 (95% CI: 0.11 approximately 0.81), 0.86 (95% CI: 0.62 approximately 1.19) and 0.77 (95% CI: 0.62 approximately 0.96) for men and 0.11 (95% CI: 0.03 approximately 0.45), 0.31 (95% CI: 0.17 approximately 0.58), 0.84 (95% CI: 0.66 approximately 1.10) and 0.67 (95% CI: 0.56 approximately 0.79) for women, respectively. Life expectancy was 2.6 years longer for men and 4.0 years longer for women in the low risk group. CONCLUSION: The mortality rates of CVD and all causes were lower and life expectancy was longer in the participants with low CVD risk. Keeping low risk profile may not only prevent from having CVD but also extend life expectancy.
