Inflammation due to surgical glove powders in the rabbit eye.

Inflammatory anterior chamber reactions secondary to contamination by talc, absorbable dusting powder (ADP), and a combination of these powders were studied in rabbits. These common surgical glove powders were introduced into the anterior chambers of 30 animals, and structural changes were observed on days 1 and 3, and at weeks 1, 4, 8, and 12. Our findings indicate that the combination of talc and ADP causes a substantially more acute inflammatory reaction than either agent used individually. Chronic granulomatous inflammation was more severe in the group that received ADP plus talc than in the group that received talc only.

Frozen section of the optic nerve in retinoblastoma surgery.

The usefulness of the frozen section examination to determine the extension of retinoblastoma into the optic nerve is discussed. Frozen sections performed at the time of surgery in seven retinoblastoma patients revealed the presence of tumor at the resection margin of the optic nerve in two cases.

Ocular involvement in sinus histiocytosis with massive lymphadenopathy.

Sinus histiocytosis with massive lymphadenopathy is a distinct clinical entity, with ophthalmic involvement in 10% of patients. Orbital as well as eyelid lesions have been described as part of the extra-lymph node involvement of this disorder. We recently examined a young boy with sinus histiocytosis with a bulbar conjunctival mass and chronic dacryocystitis.

Incidence of granulomatous lesions in postmortem conjunctival biopsy specimens.

We examined 58 postmortem conjunctival biopsy specimens from 30 patients without sarcoidosis. The specimens were excised randomly from the lower fornices. Histopathologic examination showed chronic inflammation, epithelial hyperplasia, subepithelial foreign bodies, inclusion cysts, and ectopic lacrimal glands. Although aggregates of foreign body giant cells were observed in three specimens (5.2%), no distinct, noncaseating granuloma formation was seen.

Retinoblastoma with anterior chamber extension.

Anterior chamber retinoblastoma is a rare clinical entity. The authors have reviewed the records of 1500 patients with retinoblastoma to determine the incidence and prognostic ramifications. Of 30 patients with anterior chamber involvement, 15 were noted on initial examination and 15 during observation after therapy. Despite aggressive multimodality treatment, total response was never achieved and all eyes were eventually lost. Results of pathologic examination showed ciliary body invasion, which was held accountable for the poor response to therapy. Anterior chamber retinoblastoma is a poor prognostic sign which cannot be controlled effectively with current techniques and should be considered an indication for enucleation without delay.

Comparison of aqueous humour and serum zinc levels in humans.

Zinc concentrations in aqueous humour and serum were determined by means of atomic absorption spectrophotometry in 11 patients with cataract. The zinc content of the aqueous humour was found to be considerably lower than the corresponding serum values. From this finding it is suggested that aqueous humour does not serve as a likely route of delivery of zinc to the cornea.

Sodium hyaluronate (Healon) in the surgical repair of Descemet's membrane detachment.

Two cases of Descemet's membrane detachment are presented. In both cases, sodium hyaluronate (Healon) was successfully used to repair the detachment surgically. The viscoelastic property of sodium hyaluronate allows it to move Descemet's membrane easily into proper position and maintain it there post-operatively. Sodium hyaluronate left in the anterior chamber can cause an increase in intraocular pressure and prophylactic measures, such as diamox and timoptic should be used in these cases.

Conjunctival biopsy in sarcoidosis.

In a prospective study of systemic sarcoidosis, 28 patients underwent 55 conjunctival biopsies. The incidences of positive granulomatous lesions in proven and suspected sarcoidosis cases were 71.4% (ten of 14 patients) and 28.5% (four of 14 patients), respectively. Thus, random, bilateral conjunctival biopsies should be performed as soon as possible in all cases of presumed sarcoidosis.

Tumor seeding in ocular fine needle aspiration biopsy.

The purpose of this study was to investigate the occurrence of tumor cell seeding in needle tracks following fine needle aspiration biopsy of intraocular tumors. Sixteen biopsies were performed in four enucleated globes (3 with retinoblastoma and 1 with choroidal melanoma). The biopsies were done immediately after surgery and the needle tracks from the pars plana region were serially sectioned for histological examination. Clusters of tumor cells were seen within six of the 11 needle tracks that could be histologically identified.

Clostridium septicum panophthalmitis with systemic complications.

A fulminant case of endophthalmitis due to Clostridium septicum is described. The patient presented with spontaneous gas gangrene panophthalmitis, with early visual loss and an air bubble in the anterior chamber. Death ensued, and necropsy revealed changes consistent with severe arterosclerotic cardiovascular disease, a relationship not uncommon in patients with clostridium sepsis. This association as well as the histopathology of the globe are discussed.

Ectopic lacrimal gland tissue of the orbit and sclerosing dacryoadenitis.

A 28-year-old man with unilateral proptosis was found to have sclerosing dacryoadenitis and ectopic lacrimal gland tissue of the orbit. The unusual combination simulated invasive adenocarcinoma of the lacrimal gland both clinically and histopathologically. Based on clinical and experimental observations, we propose that the acinar gland atrophy and fibrous replacement of the lacrimal gland and of the heterotopic tissue was the result of a primary developmental abnormality associated with obstruction to normal gland outflow. Physician awareness that ectopic lacrimal gland tissue in the orbit can be affected by sclerosing dacryoadenitis is probably the most critical factor in establishing the correct diagnosis.

Corneal argyrosis: histologic, ultrastructural and microanalytic study.

The clinical, histopathological, ultrastructural and microanalytic features of a case of iatrogenic ocular argyrosis are reported. The case, in a 63-year-old woman with long-standing herpetic keratitis, was unusual in that it occurred after apparently only one treatment with eyedrops consisting of a 1% solution of "mild silver protein" (Argyrol). Another unusual feature was that the silver deposited in the cornea was mainly conjugated to sulfur; selenium, which commonly demonstrates a high affinity for silver in tissues, was detectable in only trace amounts.

Serum zinc levels in retinitis pigmentosa.

Serum zinc and copper levels were analyzed in 26 retinitis pigmentosa patients and compared with serum zinc levels of a control group. Serum zinc levels were documented to be decreased with a mean concentration of 0.735 +/- 0.167 micrograms/ml (p less than 0.001). Serum copper levels, on the other hand, were found to be increased in retinitis pigmentosa patients with a mean value of 1.261 +/- 0.348 micrograms/ml (p less than 0.001).

Papillomas of lacrimal drainage system: a clinicopathologic study.

This paper presents a clinicopathologic review of Schneiderian papillomas in the lacrimal drainage system, nasal cavity, and paranasal sinuses. The embryologic, histopathologic, and clinical aspects of these lesions are discussed. A more practical classification for the papillomas of the lacrimal drainage system, offering a better understanding of their histogenesis and an easy follow-up system, is proposed.

A case of prostate carcinoma with bilateral orbital metastases and the review of the literature.

A 79-year-old man presented with bilateral periorbital swelling of unknown duration. Diagnostic workup, including orbital and prostate biopsies processed with conventional hematoxylin-eosin and immunoperoxidase stains, revealed that the prostate was the origin of the orbital tumors. Our review of the literature revealed 17 previously reported cases of metastatic prostate carcinoma to the orbit. Salient clinical and histopathologic features of these cases are summarized. To the best of our knowledge, this is the first biopsy proven case of bilateral orbital metastasis of prostate carcinoma.
