[The physical capacity of men with ischaemic heart disease after cardiac rehabilitation].

The aim of the study was the evaluation of physical capacity of cardiovascular system and the ability to perform permitted exercises in a group of 114 patients who underwent pharmacological or surgical treatment of ischaemic heart disease and subsequently were submitted to 12 month lasting cardiac rehabilitation. Rehabilitation programme comprised hospital, sanatorium and ambulatory periods. The control group comprised 29 patients without changes in coronary arteries. All patients underwent treadmill test, according to Bruce protocol. The efficacy of rehabilitation was estimated in the particular groups regarding to the method of treatment. 143 men, were enrolled in the study in following groups: A--patients with the history of myocardial infarction (MI) treated pharmacologically (n = 29) B--patients with the history of MI after surgical treatment (n = 29) C--patients without MI after surgical treatment (n = 28) D--patients without MI treated pharmacologically (n = 28) K--control group (n = 29). All men in groups A, B, C completed cardiac rehabilitation programme. The highest level of physical capacity was observed in group of operated (CABG) patients. Patients treated pharmacologically reached significantly lower level of physical capacity.

[Bone mineral density in patients with irreversible renal failure treated with peritoneal dialysis].

Frequency of osteoporosis in patients on dialysis varies in different studies depending of method and measured region of bone mineral density (BMD). The aim of the study was to evaluate the frequency of disturbances of BMD in patients on peritoneal dialysis (PD). We studied 100 patients: 56 on PD and 44 on HD. (49F, 51 M.age mean 54 +/- 29) on dialysis for 6 to 104 months. BMD was measured using dual energy X-ray densitometry (DEXA) in L1-L4 segment of the vertebral column, femoral neck and forearm. The results of the BMD were presented as T-score in the standard deviation of the mean peak bone mass according to WHO criteria. Depending on the region of interest (ROI), osteoporosis was diagnosed in 20% to 70% of women and in 4% to 35% of men. In females the lowest values of BMD were found in forearm (T-score = -3.16 SD), in males it was femoral neck (T-score = -2.87 SD). In these ROI's the lowest values of BMD in both genders were found in patients with the level of iPTH higher than 300 pg/ml. We did not find any significant difference in BMD between patients on PD and HD. Values of bone mineral density were not related to the duration of dialysotherapy and cause of irreversible renal failure. On the results of our study it has been concluded that, enhancement of disturbances of BMD in patients on PD is related to the region of interest, gender of the patient and functional status of parathyroid glands.

Identification of fecundity gene (FecB) carriers using microsatellite markers and its effect on sheep weight.

Abstract. Altogether 115 animals representing 5 genetic groups: 3 purebred Booroola Merino, Corriedale and Olkuska, and 2 Booroola crossbreds were included in the studies. In total 6 alleles from 97 bp to 119 bp in microsatellite OarAE101, and 5 alleles from 162 bp to 174 bp in BM1329 were identified. The marker of FecB gene presence seems to be an allele of 97 bp in the case of microsatellite OarAE 101 and 162 bp in the case of BM1329. Significant differences FecB carriers (Booroola-Corriedale) and non-carriers (Corriedale) in birth weight and at weaning at 100 days (males and females from twins) as well as weight gain during the first 28 days and 100 days were found. Purebred lambs showed higher values of the investigated traits.

[Bone mineral density in dialysis patients: the optimal region of interest depending on parathormone levels].

Frequency of osteoporosis in patients on dialysis with respect to the optimal Region of Interest (ROI) has not been established. Frequency of this complication varies in different studies, especially because of no agreement according to the optimal ROI. We studied 71 patients: 32 on CAPD and 39 on HD (31F, 40m. age mean 56 +/- 29) on dialysis for 3 to 81 months. BMD was measured using dual energy X-ray densitometry (DXA) in L1-L4 segment of the vertebral column, femoral neck and forearm. The results of the BMD were presented as T-score in the standard deviation of the mean peak bone mass according to WHO criteria. Males and females were evaluated separately. Depending on the ROI, osteoporosis was diagnosed in 20 to 68% of women and in 2 to 40% of men. In females the most sensitive region of interest appeared to be forearm, in males it was proximal part of the femur. In these ROI's the lowest values of bone mineral density in both genders were found in patients with the level of iPTH higher than 200 pg/ml. We did not find any significant difference in BMD between patients on CAPD and HD. Values of bone mineral density were not related to the duration of dialysis therapy. On the results of our study it has been concluded, that optimal region of interest for BMD evaluation in dialysed patients depends on gender of the patient and functional status of parathyroid glands but is not related to either the method or duration of dialysis therapy.

[The evaluation of the coronary artery sufficiency in myocardial infarction patients after cardiac surgery and preventive therapy].

During 5 years period (1989-1993) the authors investigated a group of 40 patients with coronary artery disease after myocardial infarction treated in the Silesian Center of Cardiology in Katowice. 20 patients were operated on (CABG), 20 were medically-treated. It was evaluated the history, physical status, stress-test, echocardiography and 24-hours ECG. Stress test was estimated according to Mark's test. In the echocardiographic examination it was observed wall motion score index (WMSI) and the left ventricular abnormal contraction area (AA). In the operated group it was noticed higher physical ability and no influence of CABG on left ventricular contractability.

[Bone turnover markers in diabetics treated with continuous ambulatory peritoneal dialysis].

Low turnover renal osteodystrophy (ROD) is an important problem in patients on CAPD. The aim of the study was to evaluate in CAPD patients the frequency of low turnover ROD with special interest to diabetic nephropathy (DN) as well as estimation the clinical usefulness of some biochemical bone turnover markers in monitoring this complication. The study was performed in group of 54 patients: 28 on CAPD (15 M, 13 F) and 26 on HD (14 M, 12 F). There were 20/28 patients with DN in CAPD group. The observation lasted 12 months. Every 3rd month levels of iPTH, PICP, PINP, ICTP, Ca, P, total AP in serum were determined. Correlation between these biochemical markers of ROD and causes of renal failure, sex, age of patients were analyzed statistically. In CAPD patients significantly lower values of iPTH were found in comparison with HD patients (p < 0.01). In patients with DN levels of iPTH were significantly lower (p < 0.001) than in patients with other causes of renal failure (respectively: iPTH 142 +/- 81 vs 403 +/- 128 pg/ml). In patients with DN treated with CAPD levels of iPTH were significantly lower in men than in women and in younger men with DN (< 50 years). The similar results were found in levels of PINP as a marker of bone formation. Close correlation between PINP and iPTH (r = 0.80) was also found. In summary we showed that low turnover ROD is the predominant bone lesion in CAPD patients. Men with DN < 50 age are specially predisposed to the low turnover ROD. Monitoring of iPTH and PINP levels as sensitive markers of low turnover ROD might be useful in assessment of bone turnover rate in CAPD patients.

[Influence of alphacalcidole on selected markers of the bone turnover in hemodialysed patients with secondary hyperparathyroidism].

In 19 chronically hemodialysed patients (11M + 8F, mean age 52 +/- 15 yrs) with secondary hyperparathyroidism (sHP), selected markers of bone turnover in the course of pulsative treatment with alphacalcidol were evaluated. Alphakalcidol was given 3 times per week after hemodialysis. Every 3-rd month of therapy we measured serum concentration of procollagen type I carboxy-terminal propeptide PICP as a marker of bone formation as well as carboxy-terminal telopeptide type I collagen ICTP and iPTH as markers of bone resorption. The results were as follows. During 12 months of controlled treatment of sHP with alphacalcidol, statistically significant decrease of serum iPTH (from 393 +/- 145 pg/ml to 246 +/- 172 pg/ml) with simultaneous increase of serum PICP (from 235 +/- 168 mg/L to 414 +/- 168 mg/L) were obtained. Serum ICTP levels, although significantly increased in comparison to normal values, did not change under influence of alphacalcidol therapy. The mean weakly dosis of alphacalcidol of 2.27-2.39 mg successfully suppressed process of bone resorption evaluated by iPTH giving predominance to bone formation expressed by PICP. Serum levels of ICTP appeared to be of no value in monitoring treatment efficacy of alphacalcidol in hemodialysed patients with secondary hyperparathyroidism.

[Advances of clinical diagnosis of osteodystrophy complicated by chronic renal failure].

Renal osteodystrophy complicated chronic renal failure presents many clinical problems because of multiple pathogenetic factors, non specific clinical symptoms as well as not precisely enough methods evaluating progress and treatment efficacy of this complications. The paper is a review of such approved markers of bone turnover as intact PTH, bone specific alkaline phosphatase, calcium and phosphorus, bone biopsy and DEXA, as well as new bone turnover markers. Special references are referred to procollagen type I carboxyterminal (PICP) and aminoterminal (PINP), osteocalcin as new markers of bone formation process and to tartare resistant acid phosphatase pyridynoline, deoxypyridynoline and carboxyterminal telopeptide type I collagen (ICTP) as new markers of bone resorption process.

[The circulatory system in children with cutaneous forms of scleroderma. The results of routine as well as 24-hour ECG and physical performance test].

Thirty-four children suffering from so-called cutaneous forms of scleroderma were studied (physical examination, routine and 24-hour ECG, and physical performance test). Physical development was significantly impaired in 5 cases. Congenital heart malformations with left-to-right shunt were detected in 3 children. These children underwent surgery with good results. Heart murmur was found during physical examination in 14 cases. In 30 subjects routine ECG was described as abnormal or doubtful. Abnormal Holter monitoring results were found in 13 children. The authors were not able to detect inferior physical performance test results in the studied children as compared to the control group (when the children suffering from congenital heart malformations and resting tachycardia were excluded from the study). It is suggested that children suffering from so-called cutaneous forms of scleroderma should remain under constant cardiological care.

[Changes of pituitary hormone concentration during vasomotor disorders, in blood serum of women during the early phase after surgical castration].

The behaviour of the concentration of the selected pituitary hormones during hot flushes at women after a surgical castration was evaluated. The inquiry concerned 42 women 46.7 +/- 3.4 years old, with a regular menstruation till the hysterectomy and the adnexectomy due to sexual organs diseases. The inquiry contained a determination, by means of radioimmunological methods, of the concentration of the hormones: LH, FSH, PRL, HGH, TSH in the blood serum taken at women 48 hours before the operation, on the 8th day after the intervention and 5 min after the hot flushes apparition. At the same time, the finger skin temperature and the pulse rate was taken. The rise in the finger skin temperature by 1.2 degrees C and the mean rise in the pulse rate by 11.3 beats/min during hot flushes were noticed. An important increase of the LH and HGH concentration during hot flushes was also observed.

The effects of occupational exposure to hydrocarbons on some immune parameters of workers of the phenol division of a petrochemical plant.

The subjects of investigation included 34 workers at the Phenol Division of the Mazovian Refining and Petrochemical Plants and 18 persons working in administrative positions; the purpose was to elucidate some functions of the immune system cells. The lymphocyte subpopulations (CD3, CD4, CD8) and NK cells were evaluated using monoclonal antibodies and the immunofluorescence method. To search for the functions of lymphocytes and monocytes such as the in vitro production of interleukin-1 (Il-1) and interleukin-2 (Il-2), macrophage inhibitory factor generation (MIF), a cytotoxicity test and T cell proliferation in autologous mixed lymphocyte reaction (AMLR) were also examined. We have shown that workers from the Phenol Division exhibited pronounced deviations in the tested parameters as compared to the administration workers. We found a decrease in CD3 lymphocytes and in Il-1 production. After analysis of the individual results we selected three persons from the Phenol Division who showed abnormal values in four or more parameters (decreased CD4:CD8 ratio, abnormal values of lymphocyte subpopulations, impaired lymphocyte functions in functional tests). These cellular-immunity control tests are promising methods for studies of the biological effects of environmental and/or occupational pollution to toxic derivates of petroleum.

Immunomodulatory effect of naloxone in duodenal ulcer patients.

The effect of a single dose of the opiate receptor antagonist naloxone (NAL) on some immune parameters in chronic duodenal ulcer patients and healthy controls was investigated. Twelve hospitalized men, aged 19-23 years (nine duodenal ulcer patients and three controls) were given NAL intravenously at a dose of 0.03 mg/kg. Blood samples were drawn before and 30 and 150 min after NAL injection. Results obtained in duodenal ulcer patients and healthy subjects revealed the same direction of changes despite a rather wide scatter. Baseline NK cell activity was lower in duodenal ulcer patients than in healthy persons. In approximately one fourth of patients the changes in individual parameters observed following NAL injection were slight. Numbers of total lymphocytes, Th, Ts, NK cells, monocytes and Th/Ts ratio did not significantly change after NAL administration. T-lymphocyte counts moderately decreased at 30 min after NAL injection. NAL did not affect spontaneous IL-2 receptor expression and it moderately increased PHA-induced IL-2 receptor expression in most of the investigated persons. IL-2 generation and NK cell activity slightly, but significantly, increased at 30 min following NAL injection. NAL markedly inhibited lymphocyte proliferation in an AMLR test 30 min after its administration. Most of the investigated parameters returned to their initial levels after 150 min following NAL administration. The studies showed that not only endorphins and enkephalins may have an immunomodulatory action, but NAL, their antagonist, may also affect some functions of the immune system in humans, although its action is transient.
