[Enhanced neuroticism in relation to cardiovascular risk factors in men].

Chronic stress is well known as a cardiovascular risk factor in men. There are few reports referred to the relationship between cardiovascular risk factors and level of neuroticism in the psychological examination. We analysed 77 healthy men in mean age of 33.3 +/- 7.4 years being at risk of chronic professional stress. Level of neuroticism was examined by Geras scale and expressed as: low (1-4 sten)--group 1, mean (5-6 sten)--group 2 and high (7-10 sten)--group 3 level of neuroticism. The common biochemical, clinical and environmental cardiovascular risk factors were examined. In the whole group of 77 probands we identified 24 (31%) (group 2) and 10 (13%) (group 3) persons with mean and high level of neuroticism. In the group 2 we find the highest cholesterol (222.7 +/- 39.4 mg/dl), LDL cholesterol (147 +/- 35.1 mg/dl, triglicerydes (144.9 +/- 93.8 mg/dl) and apolipoprotein B (1.11 +/- 0.31 g/l) levels statistically higher than in the other group of neuroticism. In group 3 we find the highest glucose (96.5 +/- 7.4 mg/dl) and fibrinogen (353.8 +/- 39.2 mg/dl) levels. The worst results of the environmental inquiry were obtained in the group 2 (obesity (BMI > 30 kg/m2) in 25%, hypertension in 12.5%, nicotinism in 54%, low physical activity in 79% and positive to cardiovascular disease family history in 91%). Because we found correlation mostly in the group of mean level of neuroticism these results must be confirmed in the larger group of probands using other psychological tests.

[Long-term evaluation results for adequacy of continuous ambulatory peritoneal dialysis in patients with diabetes].

The aim of the study was to analyze CAPD adequacy using own scoring including three groups of parameters: "Adequest" parameters, clinical and biochemical indices. The studied population comprised 17 patients treated with CAPD: 12 DN patients, in this number 6 female and 6 male, aged 44 +/- 10.9 years, treated with CAPD for 47 +/- 10.5 months, observed for 38.5 +/- 12.8 months were included in group 1 (studied group). 105 studies were performed in this group. Group 2 (control) consisted of 5 non-diabetic patients, including 2 female and 3 male, aged 51 +/- 13.6 years, treated with CAPD for 51 +/- 20.2 months, observed for 42.6 +/- 15.2 months. 47 studies were performed in group 2. On the base of own scoring group 1 was divided into two subgroups: 1A including 8 persons dialyzed adequately and 1NA consisting of 4 persons dialyzed inadequately. All patients from group 2 were dialyzed adequately. No significant differences concerning "Adequest" scoring as well as clinical and biochemical scorings were found between subgroup 1NA and group 2. The three kinds of scoring were significantly lower in subgroup 1NA in comparison to other groups of patients. The high level of consistence expressed as percent coefficients between particular scorings was stated in groups dialyzed adequately. This consistence was lower in group 1NA. The results of Spearman correlation test regarding three kinds of scoring were not as clear as consistence assessment using percent coefficients. The results of the study let to conclude that DN patients can reach similar level of CAPD adequacy to non-diabetics. High level of consistence between three kinds of scoring confirms the usefulness of own method of adequacy assessment. However the analysis of CAPD adequacy should not be based on single group of parameters.

[Problems with therapy for renovascular hypertension in Takayasu's arteritis--on the basis of two cases].

Takayasu's arteritis is non-specific vasculitis, affecting aorta and its main branches. Renal involvement is usually manifested by renovascular hypertension followed by ischemic nephropathy. Early start of immunosuppressive treatment can substantially improve prognosis and delay progress of dangerous organic complications. On the basis of two cases of Takayasu's arteritis with renal involvement, we paid the attention to progressive nature of disease with renovascular hypertension, requiring corrective operation of renal arteries, despite intensive immunosuppressive treatment.

[Bone turnover markers in diabetics treated with continuous ambulatory peritoneal dialysis].

Low turnover renal osteodystrophy (ROD) is an important problem in patients on CAPD. The aim of the study was to evaluate in CAPD patients the frequency of low turnover ROD with special interest to diabetic nephropathy (DN) as well as estimation the clinical usefulness of some biochemical bone turnover markers in monitoring this complication. The study was performed in group of 54 patients: 28 on CAPD (15 M, 13 F) and 26 on HD (14 M, 12 F). There were 20/28 patients with DN in CAPD group. The observation lasted 12 months. Every 3rd month levels of iPTH, PICP, PINP, ICTP, Ca, P, total AP in serum were determined. Correlation between these biochemical markers of ROD and causes of renal failure, sex, age of patients were analyzed statistically. In CAPD patients significantly lower values of iPTH were found in comparison with HD patients (p < 0.01). In patients with DN levels of iPTH were significantly lower (p < 0.001) than in patients with other causes of renal failure (respectively: iPTH 142 +/- 81 vs 403 +/- 128 pg/ml). In patients with DN treated with CAPD levels of iPTH were significantly lower in men than in women and in younger men with DN (< 50 years). The similar results were found in levels of PINP as a marker of bone formation. Close correlation between PINP and iPTH (r = 0.80) was also found. In summary we showed that low turnover ROD is the predominant bone lesion in CAPD patients. Men with DN < 50 age are specially predisposed to the low turnover ROD. Monitoring of iPTH and PINP levels as sensitive markers of low turnover ROD might be useful in assessment of bone turnover rate in CAPD patients.

[Influence of trimetazidine on echocardiography parameters and free radical stress index in coronary artery disease and end-stage renal failure patients treated by hemodialysis: preliminary communication].

The aim of the study was the assessment of the influence of trimetazidine (TMZ) on echocardiography parameters and oxidative stress markers in hemodialyzed patients with coronary artery disease. The studies were carried out in 9 patients group before and after 3 months of TMZ treatment in doses 3 x 20 mg. Additionally we measured MDA concentration in the blood, as a oxidative stress marker, before and after 2 and 3 months of TMZ treatment. At the start of the study impairment of relaxation as a sign of diastolic dysfunction was detected. This abnormality persisted up to the end of the study. After 3 months of TMZ treatment we observed elevation of ejection fraction from 59.17 +/- 5.99 to 65.33 +/- 8.7% (p = 0.05). Also occurred statistically significant decrease in MDA level after 2 (0.674 +/- 0.029 vs 0.627 +/- 0.034; p < 0.001) and 3 (0.674 +/- 0.029 vs 0.567 +/- 0.063 mumol/L; p = 0.001) months of TMZ treatment. Significant improvement of the left ventricular function and decrease of MDA levels in the blood might be due to intracellular mechanisms of TMZ activity. There were no side effects of the drug in hemodialyzed patients.

[Cardiovascular complications versus adequacy and peritoneal transport characteristics in CAPD patients].

The aim of the study was a comparative analysis of cardiovascular status and basic parameters of adequacy and peritoneal transport in CAPD patients. 15 persons treated with CAPD over 2 years were included in the study. Cardiovascular status at the beginning and at the end of observation was assessed using the own scoring. Patients were divided into 3 groups of cardiovascular disease (CVD) progression: group I--without CDV progression, group II--with moderate CVD progression and group III--with severe CVD progression. The biochemical factors of CVD risk at the beginning and the end of observation were also measured. The adequacy and peritoneal transport parameters (Kt/V, CrCl and D/P creatinine) were calculated. Group I was characterized by the highest average Kt/V value as well as percent of Kt/V studies > or = 2.0. These values were significantly lower in group II and III. The average CrCl value was highest in group I and lowest in group II. The relatively high, significant reverse correlation between Kt/V and CVD scoring at the end of observation was revealed. Lower but significant correlation between serum albumin and CVD scoring at the end of observation was also established. The analysis of the link between CVD progression and changes of transport characteristics in studied population revealed the highest peritoneal transport in group III from the beginning of observation. The undesirable change of peritoneal transport characteristics was stated in group II and III. The results of our study let us to conclude that in observed population only high values of adequacy parameters reduced the risk of CVD progression. Kt/V seems to be better predictor of CVD progression than CrCl. The risk of CVD in CAPD patients is probably higher due to increase of peritoneal permeability.

Peritonitis in continuous ambulatory peritoneal dialysis: twenty years of center's experience.

Twenty years of our experiences in diagnosis and treatment of peritonitis in patients treated with continuous ambulatory peritoneal dialysis are presented. Special attention was put on reduction of frequency and efficacy of treatment improvement of this complication. Influence of peritonitis on peritoneal transport and dialysis adequacy was also analyzed. There were neither persistent changes of peritoneal membrane permeability nor worsening of dialysis adequacy following peritonitis.

[Diagnostic and therapeutic problems in primary antiphospholipid syndrome].

The purpose of the paper is to estimate clinical problems of the primary antiphospholipid syndrome in a woman presenting recurrent fetal losses, thrombocytopenia, thrombo-embolic disorders, presence of lupus anticoagulant factor and high titer of antiphospholipid antibodies in serum. Clinical anamnesis, physical examination and laboratory findings including kidney biopsy excluded the possibility of SLE as a cause of this syndrome. In the perspective of the next pregnancy prophylaxis against pregnancy complication is discussed.

[Screening for renovascular hypertension in own material (1986-1998)].

In 370 patients (pts) with hypertension(HT) in years 1986-1998 (168F + 202M, mean age 46 yrs) screening value of the following tests was evaluated: standard initial angioscintigraphy DTPA 99mTc(SA) in all pts(1-st screening group), significance of clinical suspicion on renovascular hypertension (RVHT) in the group of 74 pts (II-nd screening group). Captopril tests: renin captopril test(RCT) and isotopic captopril test (ICT) were performed in all 370 pts. Classical renal angiography as a reference test for renal artery stenosis (RAS) was performed in all pts suspected for RVHT on the basis of clinical anamnesis and or positive results of captopril tests. Results were as follows. Initial SA being abnormal in the whole group, appeared to be more significant for RAS only in the case of profound one side renal ischemia (GFR lower than 30% of total GFR). Resistance to three antihypertensive drugs, diastolic blood pressure > 120 mmHg and sudden onset of Ht, found in all 74 pts from the II-nd group, were the most significant clinical symptoms of RVHT, because critical RAS was found in 41, that is 55% of pts from the II-nd group. At least one positive CT was found in 37 from 42 pts with critical RAS in angiography with RTC being more sensitive and ICT more specific for hemodynamically significant RAS. The following screening protocol for RVHT was presented and discussed: precise clinical anamnesis followed by angiography or captopril tests according to the severity of clinical symptoms, aim of the study as well as accessibility and laboratory reproducibility of the captopril tests.

[Significance of clinical anamnesis in the preliminary diagnosis of renovascular hypertension].

Differences in the occurrence of renovascular hypertension (RVH) in the population of patients with arterial hypertension related to different preselection criteria induced us to analyse practical usefulness of clinical anamnesis for RVH. In 130 patients with arterial hypertension (72 women, 58 men, average age 48 years) and clinical suspicion on RVH, the initial renal angioscintigraphy (with 99m Tc-DTPA) was abnormal, demonstrating impairment of glomerular filtration rate. In the whole group captopril angioscintigraphy test (CAT) as well as captopril renin test (CRT) were simultaneously performed according to own protocol. In 24 cases highly suspected for RVH on the ground of clinical anamnesis the critical stenosis of renal artery (> 75% of the lumen) was found in the arteriography. The most significant signs of RVH in clinical anamnesis were the following: the diastolic blood pressure > 120 mm Hg (92% cases), resistance to 3 hypertensive drugs (92%), sudden onset and rapidly progressive course of hypertension (88%) and advanced hypertension retinopathy (79%). On the ground of these results, detailed clinical anamnesis is recommended as a cheap and easy preselection test for RVH in the hypertensive population.

[Assessment of active renin instead of plasma renin activity in the renin captopril test].

In the study usefulness of active renin (AR) measurement instead of plasma renin activity (PRA) in the diagnosis of reno-vascular hypertension(RVH) was evaluated. 42 patients with arterial hypertension (17F, 25M; mean age 45 + 12 years) and suspicion on RVH on the ground of the abnormal result of the preliminary renal angioscintigraphy were qualified to the study. Angioscintigraphic (SCT) and simultaneously renin captopril test (RCT) followed by renal arteriography or Doppler ultrasonography of renal arteries were performed in every case Haemodynamic significant stenosis of renal artery was revealed in 5 cases. The results of the examination in this group were as follows: in 3 patients positive RCT with PRA, in 4 patients positive RCT with R and in 2 patients positive SCT were found. The false positive results of RCT using PRA were obtained in 5 patients and only in 2 patients if using AR. There were no false positive results of SCT. The introduction of AR instead of PRA in RCT resulted in simplification and shortening of the diagnostic procedure. Furthermore, this modification increased detection of hemodynamically significant stenosis of renal artery from 2 cases revealed with SCT and 3 cases with PRAin RCT to 4 cases with AR. The successful preliminary results let us to introduce this modification of RCT to routine out-patients screening for RVH.
