The Na(+)/H(+) exchanger is a major pH regulator in GABAergic presynaptic nerve terminals synapsing onto rat CA3 pyramidal neurons.

The effects of pH(i) on GABAergic miniature inhibitory postsynaptic currents (mIPSCs) were studied in mechanically dissociated CA3 pyramidal neurons, by use of ammonium prepulse and whole-cell patch-clamp techniques, under the voltage-clamp condition. NH(4)Cl itself, which is expected to alkalinize pH(i), increased GABAergic mIPSC frequency in a concentration-dependent manner. In contrast, NH(4)Cl decreased mIPSC frequency, either in the presence of 200 microm Cd(2+) or in Ca(2+)-free external solution, suggesting that intraterminal alkalosis decreased GABAergic mIPSC frequency while [NH4(+)] itself may activate Ca(2+) channels by depolarizing the terminal. On the other hand, GABAergic mIPSC frequency was greatly increased immediately after NH(4)Cl removal, a condition expected to acidify pH(i), and recovered to the control level within 2 min after NH(4)Cl removal. This explosive increase in mIPSC frequency observed after NH(4)Cl removal was completely eliminated after depletion of Ca(2+) stores with 1 microm thapsigargin in the Ca(2+)-free external solution, suggesting that acidification increases in intraterminal Ca(2+) concentration via both extracellular Ca(2+) influx and Ca(2+) release from the stores. However, the acidification-induced increase in mIPSC frequency had not recovered by 10 min after NH(4)Cl removal either in the Na(+)-free external solution or in the presence of 10 microm 5-(N-ethyl-N-isopropyl)-amiloride (EIPA), a specific Na(+)/H(+) exchanger (NHE) blocker. The present results suggest that NHEs are major intraterminal pH regulators on GABAergic presynaptic nerve terminals, and that the NHE-mediated regulation of pH(i) under normal physiological or pathological conditions might play an important role in the neuronal excitability by increasing inhibitory tones.

[Occupational stress and job burnout in doctors].

OBJECTIVE: To investigate the status of job burnout in doctors and its relationship with occupational stress. METHODS: A total of 561 doctors from three provincial hospitals were randomly selected. The Maslach Burnout Inventory-General Survey (MBI-GS) was used to identify job burnout. The occupation stress inventory revised edition (OSI-R) was used to evaluate the level of occupational stress. RESULTS: Surgeon and doctors working in the internal medicine wards scored significantly higher in job burnout than their colleagues (P < 0.05). The 30-40 years of age group scored highest in exhaustion. The score of professional efficacy decreased with age and increased with educational levels. Role overload, responsibility, physical environment, reaction and self-care were major predictors for exhaustion. Role insufficiency, role overload and responsibility were major predictors for cynicism. Role insufficiency, social support and rational/cognitive were major predictors for professional efficacy. CONCLUSION: Maintaining moderate professional duty and responsibility, clearly defining job requirements, enriching leisure activities, and improving self-care ability are important measures to preventing job burnout.

[Evaluating training programs on occupational health and safety: questionnaire development].

OBJECTIVE: To develop a questionnaire to evaluate the quality of training programs on occupational health and safety. METHODS: A questionnaire comprising five subscales and 21 items was developed. The reliability and validity of the questionnaire was tested. Final validation of the questionnaire was undertaken in 700 workers in an oil refining company. RESULTS: The Cronbach's alpha coefficients of the five subscales ranged from 0.6194 to 0.6611. The subscale-scale Pearson correlation coefficients ranged from 0.568 to 0.834 . The theta coefficients of the five subscales were greater than 0.7. The factor loadings of the five subscales in the principal component analysis ranged from 0.731 to 0.855. Use of the questionnaire in the 700 workers produced a good discriminability, with excellent, good, fair and poor comprising 22.2%, 31.2%, 32.4% and 14.1 respectively. Given the fact that 18.7% of workers had never been trained and 29.7% of workers got one-off training only, the training program scored an average of 57.2. CONCLUSION: The questionnaire is suitable to be used in evaluating the quality of training programs on occupational health and safety. The oil refining company needs to improve training for their workers on occupational health and safety.

[Appraisal of occupational stress in different gender, age, work duration, educational level and marital status groups].

OBJECTIVE: This study was conducted to assess occupational stress in different gender, age, work duration, educational level and marital status group. METHODS: A test of occupational stress in different gender, age, work duration, educational level and marital status group, was carried out with revised occupational stress inventory (OSI-R) for 4278 participants. RESULTS: The results of gender show that there are heavier occupational role, stronger interpersonal and physical strain in male than that in female, and the differences are statistically significant (P < 0.01). The score of recreation in the male is higher than that in female, but the score of self-care in the female is higher than that in male, and the differences are statistically significant (P < 0.01). Difference in the scores of occupational role, personal resource among various age groups is significant (P < 0.01). Vocational, interpersonal strain scores among various age groups is significant (P < 0.05). The results of educational level analyses suggest that the difference in the scores of occupational stress and strain among various educational levels show statistically significant (P < 0.05), whereas there are no statistic significance of coping resources among the groups (P > 0.05). CONCLUSION: The occupational stress so as to improve the work ability of different groups. Different measure should be taken to reduce the occupational stress so as to improve the work ability of different groups.

[Study on the occupational stress norm and it's application for the marketing group, public service/safety group and production laborer group].

OBJECTIVE: A study of the occupational stress norm and it's application for the marketing group, public service/safety group and production laborer group. METHODS: In this study, cross-sectional study method is used, and a synthetic way of sorting and randomized sampling is adopted to deal with research targets (36 marketing group, 331 public service/safety group, 903 production laborer group). RESULTS: Descriptive statistics for OSI-R scale scores for the marketing group, public service/safety group and production laborer group were modulated. Scale raw score to T-score conversion tables derived from the OSI-R normative sample for marketing group public service/safety group and production laborer group were established. OSI-R profile from for marketing group, public service/safety group and production laborer group were established. For the ORQ and PSQ scales, scores at or above 70 indicate a strong levels of maladaptive stress and strain. Score in the range of 60 to 69 suggest middle levels of maladaptive stress and strain. Score in the range of 40 to 59 indicate normal levels of stress and strain. Score below 40 indicate a relative absence of occupational stress and strain. For the PRQ scales, score below 30 indicate a significant lack of coping resources. Score in the range of 30 to 39 suggest middle deficits in coping resources. Score in the range of 40 to 59 indicate average coping resources. Scores at or above 60 indicate a strong levels of coping resources. CONCLUSION: The authors combined subjective and objective environment match model of occupational stress. Different intervention measure should be take to reduce the occupational stress so as to improve the work ability.

Phase I/II enzyme gene polymorphisms and esophageal cancer risk: a meta-analysis of the literature.

AIM: Phase I/II enzymes metabolize environmental carcin-ogens and several functional polymorphisms have been reported in their encoding genes. Although their significance with regard to esophageal carcinogenicity has been examined epidemiologically, it remains controversial. The present systematic review of the literature was performed to clarify associations. METHODS: Eligible studies were case-control or cohort studies published until September 2004 that were written in any language. From PubMed and a manual review of reference lists in relevant review articles, we obtained 16 studies related to the CYP1A1 Ile-Val substitution in exon 7, CYP1A1 MspI polymorphisms, CYP2E1 RsaI polymorphisms, GSTM1 null type, GSTT1 null type and GSTP1 Ile104Val. All were of case-control design. Summary statistics were odds ratios (ORs) comparing heterozygous-, homozygous-non-wild type or these two in combination with the homozygous wild type, or the null type with the non-null type for GSTM1 and GSTT1. A random effect model was used to estimate the summary ORs. A meta-regression analysis was applied to explore sources of heterogeneity. RESULTS: Individuals with the Ile-Val substitution in CYP1A1 exon 7 had increased esophageal cancer risk, with ORs (95%CI) compared with Ile/Ile of 1.37 (1.09-1.71), 2.52 (1.62-3.91) and 1.44 (1.17-1.78) for Ile-Val, Val/Val genotype and the combined group. No significant association was found between esophageal cancer risk and the other genetic parameters. CONCLUSION: A significant association exists between the CYP1A1 Ile-Val polymorphism and risk of esophageal cancer. Polymorphisms that increase the internal exposure to activated carcinogens may increase the risk of esophageal cancer.

A randomized controlled trial of laparoscopic versus open cholecystectomy in patients with cirrhotic portal hypertension.

AIM: To evaluate the characters, risks and benefits of laparoscopic cholecystectomy (LC) in cirrhotic portal hypertension (CPH) patients. METHODS: Altogether 80 patients with symptomatic gallbladder disease and CPH, including 41 Child class A, 32 Child class B and 7 Child class C, were randomly divided into open cholecystectomy (OC) group (38 patients) and LC group (42 patients). The cohorts were well-matched for number, age, sex, Child classification and types of disease. Data of the two groups were collected and analyzed. RESULTS: In LC group, LC was successfully performed in 36 cases, and 2 patients were converted to OC for difficulty in managing bleeding under laparoscope and dense adhesion of Calot's triangle. The rate of conversion was 5.3%. The surgical duration was 62.6+/-15.2 min. The operative blood loss was 75.5+/-15.5 mL. The time to resume diet was 18.3+/-6.5 h. Seven postoperative complications occurred in five patients (13.2%). All patients were dismissed after an average of 4.6+/-2.4 d. In OC group, the operation time was 60.5+/-17.5 min. The operative blood loss was 112.5+/-23.5 mL. The time to resume diet was 44.2+/-10.5 h. Fifteen postoperative complications occurred in 12 patients (30.0%). All patients were dismissed after an average of 7.5+/-3.5 d. There was no significant difference in operation time between OC and LC group. But LC offered several advantages over OC, including fewer blood loss and lower postoperative complication rate, shorter time to resume diet and shorter length of hospitalization in patients with CPH. CONCLUSION: Though LC for patients with CPH is difficult, it is feasible, relatively safe, and superior to OC. It is important to know the technical characters of the operation, and pay more attention to the meticulous perioperative managements.

[Study on relationship between fatigue and work ability in chemistry workers].

OBJECTIVE: Explore the relationship between fatigue and work ability in 976 chemistry workers. METHODS: A test of fatigue and work ability was carried out with fatigue scale and work ability index (WAI) for 976 workers, other influence factors of the work ability (such as work environment, labor load, job factors) were investigated with questionnaire. RESULTS: (1) The frequency of fatigue of the unmarried workers was significantly lower than that of the married workers and other marital status workers, while the score of WAI of the unmarried was significantly higher than that of those( P < 0.05); (2) the frequency of fatigue of the mental workers was significantly lower than that of the mixed physical and mental workers, while the score of WAI of the mental workers was significantly higher than that of physical workers and mixed physical and mental workers ( P < 0.05); (3) compared with the workers free of fatigue, the other workers had lower WAI scores; (4) the fatigue score correlated negatively to the WAI score (r = -0.499, P < 0.01); (5) Cumulative odds model analysis showed that after controlling the other risk factors, fatigue was an important risk factor of work ability (OR = 4.005). CONCLUSIONS: Fatigue has affected work ability in chemistry workers, the frequency of fatigue is higher, the score of WAI is lower.

[Study on relationship between coping resources and strain in mental workers].

OBJECTIVE: To study the effects of mental workers' coping resources on their strain. METHODS: Strain (vocational strain, psychological strain, interpersonal strain, physical strain) and coping resources (recreation, self-care, social support, rational coping) were measured on 3379 mental workers with personal strain questionnaire (PSQ) and personal resources questionnaire (PRQ). RESULTS: There existed significantly negative correlations between PRQ and PSQ (P < 0.01). In PRQ, social support (SS) and rational coping (RC) had the closest relationships with PSQ. In the items of SS, those related to feeling support had more remission on strain. In the items of RC, those related to reasonable arrangement of time and anti-interference had more remission on strain. In the items of recreation (RE), those related to self-determination had more effects on the remission of strain, while indulging in watching TV could cause the strength of strain, and there was a positive correlation between watching TV and strain (P < 0.01). In the items of self-care (SC), sufficient sleeping had the most effects on the remission of strain in the all 40 items of PRQ, while not being immersed in contemplation had a positive correlation with strain (P < 0.01). CONCLUSION: Enhancing coping resources, especially sufficient sleeping, feeling support and reasonable arrangement of time, were the important measures for the remission of mental worker' strain.

[The relationship between the concentration of TGF-beta1 in serum and its gene polymorphisms in CWP].

OBJECTIVE: To study the relationship between the concentration of TGF-beta1, in serum and its gene polymorphisms in CWP. METHODS: 101 cases of coal worker's pneumoconiosis (CWP)were selected. And the peripheral vein blood samples were collected from them. The concentration of TGF-beta1 of their serum were detected with the TGF-beta1 ELISA kit. And their TGF-beta1 gene -509 site polymorphisms were analyzed with the polymerse chain reaction-restriction fragment length polymorphisms (PCR-RFLP) techniques. RESULTS: The mean value of TGF-beta1, concentration in serum of T/T, C/T and C/C gene type group were 90 ml, 60 ml and 55.960 +/- 28.833 ng/ ml respectively. Significant differences were observed within the three groups (P<0.05). The differences of the mean value of TGF-beta1, concentration in serum in various work years groups, age groups and in the groups with different stage of pneumoconiosis were not significant (P > 0.05). CONCLUSION: The correlative relationship between the concentration of transforming growth factor I (TGF-beta1) in serum and its gene -509 site polymorphisms might be occurring.

[Analysis on the incidence and relevant risk factors of campus violence among college students].

OBJECTIVE: To study the incidence and risk factors of campus violence and to provide evidence for preventing campus violence among college students. METHODS: 5300 college students in two universities in a province of China were selected to participate in the study and were interviewed with questionnaires. Logistic regression was used for data analysis. RESULTS: (1) In total, 3910 useable questionnaires were gathered to show a response rate of 73.77%. 17.98% of the college students reported they had ever experienced campus violence in the last one year. 29.60% of the male students experienced campus violence so as 7.27% of the female students. The incidence of violence among male students was significantly higher than those of female students (chi2 = 329.89, P = 0.000). (2) Among male students who were victims of campus violence, the incidence rates of threat or blackmail, emotional abuse, physical assault, verbal sexual harassment, sexual assault were 18.03%, 13.97%, 10.77%, 0.85%, 0.48% respectively. Among female students who were victims of campus violence, the incidence rates were 3.64%, 5.84%, 1.38%, 1.33%, 1.13% respectively. The main source of the violence was from their schoolmates. (3) 10.40% of the male students reported they were perpetrators of campus violence in the last year, while 1.47% of the female students reported so. Schoolmates were the main subjects of their aggressive behaviors. (4) Among the college students who were victims of campus violence, logistic regression analysis revealed that smoking, drinking alcohol, frequently getting computer online were important risk factors. The OR values were 1.48, 2.96, 1.66 respectively. Among college students who were perpetrators of campus violence, the OR values were 2.92, 1.88, 2.09 respectively. CONCLUSION: Campus violence among college students was serious, suggesting that intervention measures should be taken.

[Non-recurrent laryngeal nerve during thyroid surgery: a report of 4 cases and literature review].

OBJECTIVE: To investigate clinical significance of non-recurrent laryngeal nerve. METHODS: Clinical data from 4 cases of non-recurrent laryngeal nerve and related literature review was made to acknowledge its incidence, variant types and matters concerned during thyroid surgery. RESULTS: Seven hundred and nineteen recurrent laryngeal nerves were exposed during 2156 thyroid operations in Xiangya Hospital, from which 4 were confirmed to hold non-recurrent laryngeal nerves (0.56%). Two cases were found on the right side and the other 2 on the left. Among the 4 cases, 3 (patients) were found to have non-recurrent laryngeal nerves during reoperation because of voice horse after the first operation. The other one was recognized during the first operation. CONCLUSIONS: Non-recurrent laryngeal nerve, a rare anomaly, is very vulnerable during thyroid surgery. Knowing the related knowledge of the non-recurrent laryngeal nerve and its types is helpful to avoid its damage during thyroid surgery.

[Norm study of occupational stress on the professionals in Chengdu and Chongqing area].

OBJECTIVE: To establish the norm of occupational stress on the professionals in Chengdu and Chongqing area. METHODS: According to the "Occupation Sorting Canon in the People's Republic of China", the professionals of the above-mentioned area were sorted out and randomly stratified. The normative data were derived from a sample of 2 064 participants. The modified Occupation Stress Inventory Revised Edition (OSI-R) was used to test the targets' occupational stress level (including stressors, strain and coping resources). RESULTS: (1) In these professionals, the sex ratio was about 1:1, average age (36.4 +/- 9.4) years, average length of service (15.9 +/- 10.0) years, 63.07% people were college educated and over 80% married. (2) Descriptive statistics for OSI-R scale scores for the total normative sample, for gender samples and for occupational group samples were modulated. (3) The gender norm showed that there were heavier role overload and stronger occupational strains in males than those in females (P < 0.01). The score of personal resources in the males was not different from that in females (P > 0.05). (4) In terms of different occupations, the scores of occupational role and personal strain among various groups indicated significant differences (P < 0.01). The score of personal resources did not manifest difference (P > 0.05). (5) The major factors influencing OSI-R were sex, occupation and length of service. CONCLUSION: The norm established can basically represent the occupational stress on professionals in Chengdu and Chongqing area.

[The scale and application of the norm of occupational stress on the professionals in Chengdu and Chongqing area].

OBJECTIVE: To establish the scale of the norm of occupational stress on the professionals and put it into practice. METHODS: T scores were linear transformations of raw scores, derived to have a mean of 50 and a standard deviation of 10. The scale standard of the norm was formulated in line with the principle of normal distribution. RESULTS: (1) For the occupational role questionnaire (ORQ) and personal strain questionnaire (PSQ) scales, high scores suggested significant levels of occupational stress and psychological strain, respectively. T scores >/= 70 indicated a strong probability of maladaptive stress, debilitating strain, or both. T scores in 60 approximately 69 suggested mild levels of maladaptive stress and strain, and in 40 approximately 59 were within one standard deviation of the mean and should be interpreted as being within normal range. T scores < 40 indicated a relative absence of occupational stress or psychological strain. For the personal resources questionnaire (PRQ) scales, high scores indicated highly developed coping resources. T scores < 30 indicated a significant lack of coping resources. T scores in 30 approximately 39 suggested mild deficits in coping skills, and in 40 approximately 59 indicated average coping resources, where as higher scores (i.e., >/= 60) indicated increasingly strong coping resources. (2) This study provided raw score to T-score conversion tables for each OSI-R scale for the total normative sample as well as for gender, and several occupational groups, including professional engineer, professional health care, economic business, financial business, law, education and news. OSI-R profile forms for total normative samples, gender and occupation were also offered according to the conversion tables. CONCLUSION: The norm of occupational stress can be used as screening tool, organizational/occupational assessment, guide to occupational choice and intervention measures.

Use of endoscopic naso-pancreatic drainage in the treatment of severe acute pancreatitis.

AIM: To review the experience on the use of endoscopic nasopancreatic drainage (ENPD) in the treatment of severe acute pancreatitis (SAP). METHODS: Since March 1998, under the regular management of SAP with non-operative method, ENPD has been randomly used in 14 patients. The average age of the patients was 41.3+/-15.9 (years), with 8 males and 6 females. The time from onset to admission was 32.9+/-22.8 (hours). 8 cases were found to have gallbladder stone. The daily output of pancreatic fluid was measured. The body temperature, heart rate, WBC count, blood glucose, blood calcium, PaO(2), blood and urine levels of amylase were detected on the fifth day and compared with their respective data on the first day. Therapeutic results and hospitalization times were recorded. RESULTS: The time of drainage was 7.3+/-4.0 days. The daily drainage outputs of the first five days were 236.4+/-176.6, 287.1+/-164.7, 284.6+/-216.4, 435.0+/-357.8 and 377.8+/-223.8 ml, respectively. The decreases in body temperature, heart rate, WBC counts, blood and urine levels of amylase and the increase in PaO(2) were significant on the fifth day when compared with those on the first day. Infection of pancreatic necrosis was found in one patient and controlled by anti-infectives. 6 out of 8 patients with gallbladder stone were operated during hospital stay. All patients were cured and discharged and the average hospital stay was 28.1+/-11.6 days. CONCLUSION: ENPD is an effective method for the drainage of pancreatic fluid and might have an important role in the treatment of SAP. Further observation, comparison and summary by this method are worthy to be considered.

Helicobacter species ribosomal DNA recovered from the liver tissue of chinese patients with primary hepatocellular carcinoma.

Using polymerase chain reaction (PCR), we examined 16S ribosomal DNA (rDNA) of Helicobacter species in liver tissue specimens obtained from 15 patients with hepatocellular carcinoma. Sixty percent (9 of 15) of these specimens were found to be positive for Helicobacter species. Four 16S rDNA fragments from positive PCR samples were directly sequenced. By sequence comparison, all were found to be 99% identical to the 16S rRNA of Helicobacter pylori.

[Influence of dosage on cell biocompatibility of hydroxyapatite/tricalcium phosphate].

OBJECTIVE: To investigate the influence of different dose levels of hydroxyapatite/tricalcium phosphate (HA/TCP) on the proliferation and alkaline phosphatase (ALP) activity of rabbit osteoblasts. METHODS: Three different dose levels of HA/TCP (10%, 40%, 70%) were co-cultivated with rabbit osteoblasts respectively. The proliferation and ALP expression capacity of osteoblasts were examined with MTT method and enzyme histochemistry once every 24 hours until 5 days. Three control groups of other materials were treated and examined in the same way: rabbit osteoblasts as normal control; polyvinylchloride as positive control; titanium alloy as negative control. RESULTS: There was remarkable time-effect relationship in the proliferation of osteoblasts. Ten percent HA/TCP did not affect osteoblasts growth while 40% HA/TCP could slow the cell growth rate down though time-effect relationship still existed. The proliferation of osteoblasts stagnated when co-cultivated with 70% HA/TCP. On the other hand, 10% HA/TCP could cause reversible damage on ALP activity of osteoblasts, whereas when the dose was 40%, and the cultivation lasted 6 days the damage was irreversible. Three different dose levels of titanium alloy (10%, 40%, 70%) had no effect on the proliferation or ALP activity of osteoblasts. CONCLUSION: Dosage is an important factor affecting the biocompatibility evaluation of biomaterial. It suggests that dose choosing should be more specified upon each individual biomaterial. It also indicates that ALP may be a good supplementary index of the cell compatibility of material.

Prevention of cancer cachexia by pyrrolidine dithiocarbamate (PDTC) in colon 26 tumor-bearing mice.

BACKGROUND: The precise mechanism of cancer cachexia is not fully elucidated. This study was aimed to assess the effect of pyrrolidine dithiocarbamate (PDTC, an inhibitor of NFkappaB) on interleukin (IL)-6 synthesis and cachexia in colon 26 tumor-bearing mice. METHODS: Murine colon 26 adenocarcinoma cells were inoculated subcutaneously in male BALB/c mice to induce cachexia. Saline and various doses of PDTC (10, 50, or 100 mg/kg/d) were given intraperitoneally daily from 7 days after tumor inoculation to killing. Body weight, food intake, and tumor volume were monitored daily. Serum and tumor tissue levels of IL-6, serum biochemical indicator, and activity of NFkappaB in tumor tissue were investigated in all mice. RESULTS: Significant tissue wasting was observed in all tumor-bearing mice. By day 16, carcass weights of untreated tumor-bearing mice were about 71.3% of healthy controls (p < .01), and the weights of gastrocnemius muscle and epididymal fat were lowered by 42.4% and 70.4% (p < .01), respectively. Furthermore, tumor-bearing caused a significant decrease of serum albumin, glucose, and triglyceride (p < .01) and increase of IL-6 (p < .01) in serum and tumor tissues. Administration of PDTC dose dependently inhibited the NFkappaB activation in tumor tissues, inhibited IL-6 synthesis of the tumor cells, and attenuated the wasting of carcass weight, gastrocnemius muscle, and epididymal fat. Tumor growth was inhibited by PDTC with 100 mg/kg (p < .05). No differences of food intake were found between groups (p > .05). CONCLUSIONS: These results suggest that PDTC, an inhibitor of NFkappaB, can attenuate the development of cachexia in colon 26 tumor-bearing mice through inhibition of IL-6 synthesis regulated by NFkappaB.
